[Deformity of the cardiac structures in patients with mitral valve prolapse: the relationship with flat chest].

In view of the close relationship of mitral valve prolapse (MVP) and chest deformity, seven patients of MVP associated with flat chest were echocardiographically evaluated, and the results were compared with those of 43 normal control subjects, 29 cases with flat chest, and patients with MVP either of the anterior (25 cases) or posterior (21 cases) leaflet. MVP associated with flat chest was observed in the anterior leaflet in all cases. MVP was related to the dislocation of the two mitral leaflets caused by the displaced posterior mitral ring observed by two-dimensional echocardiography. The transition type was observed among the cases with or without dislocation of the mitral ring in cases with flat chest.

Membrane lipid environment of carp brain and liver mitochondrial monoamine oxidase.

Delipidation of carp liver mitochondria by treatment with methyl ethyl ketone (MEK) or Triton X-100 and then perchlorate greatly reduced MAO activities. Treatment with only Triton X-100 resulted in less reduction in activity. The Km values of the remaining activities were similar regardless of these treatments. The sensitivities towards clorgyline and l-deprenyl of the remaining activity in the Triton X-100-treated residue and the phospholipase C-treated carp brain mitochondria were found to be unchanged, but those of the activity remaining in the MEK-treated residue were similarly decreased. No evidence was obtained suggesting conversion of carp MAO to either MAO-A or MAO-B by the modification of the mitochondrial lipid environment by the treatments employed.

[Echocardiographic manifestations of postinfarction perforation with extensive dissection of the interventricular septum: report of an autopsied case].

This is a report of the echocardiographic features of postinfarction perforation associated with dissection of the interventricular septum (IVS) in a 61-year-old woman. She had acute myocardial infarction and was admitted to a nearby hospital, and later admitted to our CCU because hemodynamic deterioration. On admission, she was somnolent and hypotensive (90/64 mmHg), with sinus tachycardia, marked cyanosis, and peripheral edema. On auscultation there were a harsh holosystolic murmur over the LLSB and moist rales in both lung fields. An ECG revealed extensive anterior and inferior infarctions. Catheterization data confirmed O2 step-up in the right ventricle. Two-dimensional echocardiograms demonstrated extensive dissection of the IVS characterized by an echo-free lumen extending from the apex to the cardiac base in the long-axis and partly reaching the left ventricular free wall in the short-axis with the concomitant paradoxical movement of the IVS. Autopsy disclosed marked hemorrhagic infarction and extensive dissection of the IVS forming a lumen (5 X 5.5 X 2 cm3) corresponding exactly to the echocardiographic free space. In addition, a shunt between the right and left ventricles was confirmed by the presence of two perforations near the apex on the right and left sides of the IVS, whose diameters were 6 and 10 mm, respectively.

A new type of mitochondrial monoamine oxidase distinct from type-A and type-B.

The characteristics of mitochondrial monoamine oxidase (MAO) in carp liver were studied with MAO inhibitors and substrates. This enzyme was thermolabile, but was stabilized in the presence of bovine serum albumin. With clorgyline and deprenyl, single-sigmoidal curves for inhibition of the activity towards tyramine or 5-hydroxytryptamine were obtained; the sensitivities to the two inhibitors were identical. The activity towards beta-phenylethylamine was not completely inhibited by clorgyline or deprenyl, but the remaining activity was inhibited by semicarbazide and the inhibition curves by either clorgyline or deprenyl and semicarbazide were also identical to the curves with the other two substrates. These results suggest that carp liver mitochondria contain "classical" MAO and a clorgyline- and deprenyl-resistant amine oxidase and that the classical MAO does not seem to be MAO-A or MAO-B, which are present in mitochondria of most mammalian tissues.

Inhibition of carp liver mitochondrial monoamine oxidase by some commonly-used detergents.

The inhibitory effects of some detergents commonly used in biochemical research on carp liver mitochondrial monoamine oxidase were examined. Sodium dodecylsulfate, octyl-beta-D-glucopyranoside, sodium cholate and Triton X-100 at relatively low concentrations caused strong dose-dependent inhibition of the activity towards tyramine, but digitonin caused only weak inhibition. Sodium dodecylsulfate, octyl-beta-D-glucopyranoside and sodium cholate caused almost complete inhibition of activity in the concentration ranges tested. The extent of inhibition by Triton X-100 was greater after preincubation at 37 degrees C for 30 min than that without preincubation, but with or without preincubation, the inhibition was not substrate-selective and was not complete at a relatively high concentration (2%) of Triton X-100. Without preincubation, the mode of inhibition by Triton X-100 was competitive and reversible with respect to the oxidations of 5-HT, tyramine and PEA, but after preincubation (37 degrees C for 30 min), it became noncompetitive and irreversible, depending on the concentration of detergent used. These findings suggest that it had different actions on the enzyme depending on preincubation. Triton X-100 also slightly changed enzyme sensitivity towards clorgyline and deprenyl, regardless of the preincubation time or the substrate used. Some possible mechanisms of the inhibitory effect of Triton X-100 are discussed.

Treatment of the femoral shaft fracture with a curved heat-treated COP clover-leaf nail.

The commonly used straight intramedullary nail has certain limitations and disadvantages. Following radiographic and metallurgical studies we have developed a curved heat-treated clover-leaf nail using a newly developed precipitation hardening stainless steel (COP). The nail is bent with a slot on the convex side and the radius of curvature is 115 cm which corresponds to the anatomical antero-lateral convexity of the human femur. The nail has been satisfactory in clinical use since 1971 and has provided not only more stable internal fixation but also allowed fixation of fractures beyond the middle third of the shaft.

A case of bilateral primary posterior dislocation of the shoulder.

We present a case of bilateral primary dislocation of the shoulder, which was treated operatively on the left and conservatively on the right side. Primary posterior dislocation of the shoulder is rarely seen and bilateral case is extremely rare. For the diagnosis it is essential to have a knowledge of this dislocation. As to roentgenographic examination, the axillary projection is most helpful but can't be routinely taken because of severe pain and limited abduction of the shoulder. So special roentgenographic techniques are necessary.

[Anatomical and biomechanical considerations for medullary nailing (author's transl)].

By using X-ray films of 390 adult femurs and 15 cadaveric femurs of Japanese origin, and by measuring successive cross-sections the authors have made morphological and statistical analyses of the shape of the femoral medullary canal. These studies suggest new dimensions for the diameter of the reamer and the amount of contact area between nail and inner cortex. Furthermore it was found that the physiological antecurvature of the femur on the lateral view does not correspond with the prebending of the OA-nail. The authors made some mechanical experiments on four types of nails which are widely used today. As a consequence of these studies, the authors developed a new elastic nail for trial (Okayama-nail). This new type of nail greatly increases the area of contact with the inner femoral cortex. Obviously this nail satisfies better all of Kuentscher's originally mentioned requirements an intramedullary nail.

COP, a new alloy for surgical implants.

Today Vitallium is used for surgical implants. It is a casting alloy which, with advances in casting technology, is also used commercially for making instruments of fairly complex shape. Because of its expense, however, it is not widely used in Japan. Instead, a series of 18-8 Mo alloys are used in Japan even though of insufficient strength. Used over a long period of time in the body, especially for the purpose of preserving structual functions as part of the human skeleton, it often corrodes, resulting in either abnormalities in tissue cells or, because of its insufficient strength, danger of bending and breaking with aging. In spite of a marked advance in fracture treatment, we have hardly any suitable materials for making instruments appropriate to the internal fixation of fractures in Japan. We, therefore, conducted various experiments to develop an alloy with sufficient corrosive resistance and strength that could be formed into a complex shape to take the place of Vitallium alloy, finally succeeding in developing an alloy we call "COP". The characteristic properties of COP may be summarized as follows: 1. The main components are 20% Cr, 20% Ni, 20% Co and 4% Mo aside from 0.2% P. 2. As it contains "P", it shows a marked age-hardening. In its molten state its machinability is excellent, and later it can readily be hardened by heat-treatment. 3. It has not only a marked yield point and tensile strength but also has toughness in elongation and reduction of area, showing a strength which surpasses Vitallium. 4. Its corrosive resistance is great. 5. Its cost is far cheaper than Vitallium.

Use of the halo-pelvic apparatus for treatment of fracture-dislocations of the thoracic and lumbar spines accompanied by paraplegia.

Twenty-five cases of paraplegia due to fracture-dislocations of the thoracic and lumbar vertebrae were treated using the halo-pelvic apparatus. Thirteen of them only needed the apparatus, two underwent laminectomy, and ten had posterior spinal fusion in addition. The advantages of this method are that gentle reduction can be achieved without severe complications, and that posterior spinal fusion can be performed with this apparatus on, thus enabling an early start to the subsequent exercise program and minimal absence from work.

A new nail-plate for treatment of fracture of the neck of the femur.

Operative treatment of fractures of the neck and trochanter of the femur does not always produce a satisfactory result. This is usually due to biomechanical problems with the available internal fixation methods. We studied the anatomy of the neck of the femur by roentgenograms and sectional specimens from 70 cadavers. In addition, various nail-plates were subjected to buckling tests and, by simultaneously attaching a strain-gauge, stress distribution was calculated. The results of these preliminary studies were then used to design a new nail-plate better than those available at present. Testing of this new nail-plate confirmed that it had a strength equal to that of the Holt nail-plate (the strongest of the available nail-plates).
