A case of Ewing's sarcoma with intracranial metastasis is reported with special reference to positron emission tomography (PET) evaluation of cerebral hemocirculation and metabolism after brain irradiation.
The present study was designed to investigate the differences in serum levels of sex steroids, bone metabolism, and bone mineral density (BMD) between 22 premenopausal subjects and 21 age- and body size-matched postmenopausal subjects. Dual photon absorptiometry (DPA) demonstrated total body BMD significantly decreased in postmenopausal subjects. It was also found that not only spinal BMD as assessed by DPA, but also total third lumbar BMD and third lumbar trabecular BMD as measured by quantitative computed tomography (QCT) significantly decreased in these subjects. In postmenopausal subjects, the urinary-hydroxyproline/creatinine ratio increased significantly, which suggested that bone resorption had been stimulated, and serum levels of alkaline phosphatase (ALPase) and osteocalcin also significantly increased, which suggested a stimulation of bone formation. Consequently, the reduction in BMD was thought to result from a negative balance of bone resorption versus bone formation, or a high turnover of bone metabolism. Androgens were not confirmed to participate in these changes in bone metabolism, but the reduction in serum levels of estradiol, as a major factor, and the reduction in serum levels of estrone, as a participating factor, were assumed to result in an increase in bone resorption in excess of bone formation.
Two-staged intact canal wall tympanoplasty is a common operation for treatment of middle ear cholesteatoma. MRI provides better tissue differentiation of the middle ear and/or mastoid, which often become occupied with soft density tissue after the first operation. If MRI was able to detect the presence of a recurrent or residual cholesteatoma with sufficient sensitivity and specificity, this may facilitate a decrease in the number of second-look procedures. This study compared MRI findings to surgical findings at second-look surgery and calculated the correlation rates between the two sets of findings. Thirty ears having undergone intact canal wall tympanoplasty for cholesteatoma at the initial operation were examined by MRI prior to the second look. Otoscopic findings of the tympanic membrane were nonsuspect in all cases. The true positive rate was 11/30 (37%) and the true negative rate was 10/30 (33%), leading to a radiosurgical correlation of 70%, whereas the false positive rate was 6/30 (20%) and the false negative rate was 3/30 (10%). This indicates that 30% of the MRI findings were incorrect. Therefore, at the present time, MRI does not appear as a likely replacement for second-look surgery in cases of intact canal wall tympanoplasty.
To investigate prognostic indicators for gastric adenocarcinomas in advanced stage, we have carried out immunohistochemical determination of thymidylate synthase (TS) in carcinoma tissues. Sixty seven cases of curatively resected gastric carcinoma, under 70 years old at the time of surgery, who had been in the same grade of invasion and lymph nodal involvements (pT3/pN2, Stage IIIB), and had underwent postoperative 5FU-related chemotherapy for a long period were extracted from the files of our hospital. Formalin-fixed and paraffin-embedded specimens were stained with an antibody for human TS (polyclonal). A positive signal was always compared with some types of inflammatory cells in the same slide because of their constantly strong signal. Normal control tissue used was human bone marrow containing blastic cells. Both staining intensity and the fraction of positively stained carcinoma cells were examined in each case, and then the results were summarized with respect to fraction of positively stained carcinoma cells. The present study indicates that the TS-negative group had better survival than the positive group (p < 0.05), the 5 year survival rates being 42.3% and 25.1%, respectively, which suggests that the frequency of TS-positive carcinoma cells could also be a prognostic indicator.
