[Magnetic resonance imaging in agenesis of the corpus callosum].

The recent use of computed tomography (CT) scan is providing an easy diagnoses of agenesis of corpus callosum which had been difficult to diagnose only by clinical signs and symptoms. Since, more neuropsychological studies on agenesis of corpus callosum are being done, clinical details of agenesis of corpus callosum are being clarified. We examined magnetic resonance imaging (MRI) on 3 patients who were suspected to have agenesis of corpus callosum by CT scan. And we studied the usefulness of MRI in agenesis of corpus callosum. Case 1. We studied a thirty-two-year-old right-handed man with a one-year history of amyotrophic lateral sclerosis. He had no callosal disconnection syndrome. CT scan on horizontal view showed dilated third ventricle, the dilatation of the occipital horns and wide separation of the lateral ventricles. On coronal view, CT scan showed the absence of the corpus callosum, the concave mesial borders of the lateral ventricle, the dilatation of the third ventricle and its abnormal dorsal extent(bat-wing appearance). On sagittal MRI (the inversion recovery technique), the corpus callosum was absent and the convolutions and sulci were arranged radically centering the third ventricle (radial distribution). But the anterior commissure was preserved. Case 2. We studied a fourty-one-year-old right-handed man with a seven-year history of hyper ventilation syndrome. He had no callosal disconnection syndrome. CT scan showed the similar findings as those of case 1. On sagittal MRI (the inversion recovery technique), the corpus callosum was absent, and the convolutions and sulci showed radial distribution. But the anterior commissure was preserved.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effects of krestin and p-aminobenzoic-N-xyloside sodium salt on activities of drug-metabolizing enzymes and glutathione-related enzymes in rat liver].

Effects of krestin (PSK) and p-aminobenzoic-N-xyloside sodium salt (K-247) (both products of Kreha Chemical Co., Japan) on activities of drug-metabolizing enzymes and glutathione (GSH)-related enzymes were investigated in rat liver. When PSK was administered at a dose of 100 mg/kg body weight, ip every day for 7 or 14 days, the action of UDP-glucuronyltransferase (UDP-GT) on o-aminophenol (o-GT) and those of GSH S-transferase (GST) on both 1, 2-dichloro-4-nitrobenzene ( DCNB ) and 1-chloro-2, 4-dinitrobenzene (CDNB) slightly increased together with increased activities of GSH-peroxidase on both H2O2 and cumene hydroperoxide, were as GSH levels were decreased. When PSK or K-247 was administered at 1% in diet for 4 or 8 weeks, o-GT activity and GST activities with both substrates increased on K-247 feeding, while GST activity for CDNB decreased on PSK and K-247 feedings. These changes were statistically significant but very small. The content of P-450 and the activity of gamma-glutamyl transpeptidase changed little in any administration schedules mentioned above.

[Immunopharmacological actions of 6-amidino-2-naphthyl-4-guanidinobenzoate (FUT-175). 2. Effect of FUT-175 on immunological reactions in man].

FUT-175 is a new anti-complemental drug which strongly inhibits complement-mediated allergic reactions in animals. It was reported that FUT-175 does not affect both antibody formation and host defense to bacterial infection in mice. The present study was undertaken to examine the effects of FUT-175 on various immunological reactions in humans. FUT-175 dose-dependently decreased the antigen-induced anaphylactic histamine release from leukocytes of atopic patients. However, i.d. treatment of FUT-175 neither inhibited antigen- or compound 48/80-induced immediate type skin reactions in atopic patients nor antigen-induced early, late or delayed type skin reactions in Candida-sensitive patients. FUT-175 also did not inhibit the PPD-mediated delayed type skin reaction in healthy subjects. FUT-175 at a dose of 10(-4)M but not at 10(-8) to 10(-5)M significantly decreased the proliferation of human atopic peripheral blood lymphocytes (PBL) caused by mite antigen, concanavalin A or pokeweed mitogen. 51Cr release from human PBL was slightly enhanced by FUT-175 at a dose range of 10(-6) to 10(-4)M. FUT-175 did not change the number of SIg receptors or C3 receptors on human B cells. FUT-175 hardly affected the nitroblue tetrazolium reduction test and Escherichia coli-mediated chemotaxis in human neutrophils. These results strongly suggest that FUT-175 does not affect immunological functions and host defense in humans.

Clinical results of cardiopulmonary bypass with selective cerebral perfusion for aneurysm of the ascending aorta and the aortic arch.

The purpose of this report is to describe our experience using cardiopulmonary bypass with selective cerebral perfusion in 10 patients whose aneurysms required cross-clamping the aorta and brachiocephalic vessels. Cerebral perfusion was carried out with individual roller pumps. Flow rate was 11.2 +/- 4.8 ml per kilogram of body weight per minute to the right axillary artery, 8.5 +/- 1.0 to the right common carotid artery, 7.0 +/- 2.0 to the left common carotid artery, and 3.9 to the left subclavian artery. All the patients came off bypass smoothly, and only 3 required small amounts of inotropic agents postoperatively. One patient with mycotic aneurysm died in the fourth postoperative week of massive bleeding due to disruption of the suture line. No cerebral complications were experienced. The operative results of this series suggest the usefulness of cardiopulmonary bypass with selective cerebral perfusion, and reasonably favorable outlook for patients with aneurysm involving the aortic arch and the ascending aorta.

[Evaluation of mitral valve, subvalvular structures and valvular flexibility in mitral stenosis by two-dimensional echocardiography].

UNLABELLED: In order to identify the determinants for surgical procedures in mitral stenosis, we evaluated two-dimensional echocardiographic findings of the mitral valve and subvalvular structures in 35 patients undergoing open mitral commissurotomy (OMC) or valve replacement (MVR). As indices of a degree of subvalvular shortening and valvular flexibility, the distance between the mitral ring and the tip of the anterior mitral leaflet was measured by the LV long-axis view by in both midsystole (S) and early diastole (D). As a possible major determinant for MVR, a degree of valvular calcification (C) was semi-quantatively scored according to the extent of abnormally strong echo density. In nine of 11 patients undergoing MVR, a main reason for selecting MVR was a marked thickening or shortening of subvalvular structures. In patients in whom OMC was feasible, the degree of improvement of the mitral valve area (delta MVA) was assessed by the pre- and post-operative mitral valve areas (MVA) measured on the LV short-axis view, which were averaged 0.15 and 1.38 cm2, respectively. RESULTS: The index C was significantly higher in cases with MVR than those with OMC (9.2 +/- 2.6 vs 4.7 +/- 2.3 points, p less than 0.001), although there was a significant overlap between these two groups and index C did not correlate with delta MVA in the OMC patients. Similarly, the value S was significantly smaller in patients undergoing MVR than those undergoing OMC (1.2 +/- 0.4 vs 0.7 +/- 0.2 cm, p less than 0.001), though S did not correlate with delta MVA. On the other hand, the index of valve flexibility D--S was smaller in patients undergoing MVR (0.5 +/- 0.3 vs 0.8 +/- 0.3 cm, p less than 0.05) and correlated well with delta MVA (delta MVA = 0.699 x (D--S)+0.007, R = 0.678, p less than 0.02) in patients undergoing OMC. Furthermore, in all patients undergoing OMC with D--S greater than or equal to 0.8 cm, delta MVA was above 0.5 cm2, contrasting with delta MVA of 0.5 cm2 or less in 6 of 7 patients with D--S less than 0.7 cm. Using these indices, surgical procedures were successfully predicted in another 7 prospectively studied patients and predicted delta MVA in 4 patients was quite comparable with actual delta MVA. It was concluded that measurements of S and D by two-dimensional echocardiography are useful, 1) to predict patients requiring MVR and 2) to predict patients with inadequate delta MVA in whom OMC is surgically feasible.
