Relapsing polychondritis associated with mixed cryoglobulinemia.

The case of a patient with mixed-type cryoglobulinemia and cutaneous necrotizing vasculitis, who later suffered from relapsing polychondritis affecting the auricle and the nose, is described. To the best of our knowledge, this combination is unique.

Differences in the activity of adenosine deaminase and of purine nucleoside phosphorylase and in the sensitivity to deoxypurine nucleosides between subpopulations of mouse thymocytes.

The activities of adenosine deaminase (ADA) and of purine nucleoside phosphorylase (PNP) were measured in thymocyte subpopulations separated by peanut agglutinin (PNA), in unseparated thymocytes, in lymph node and in spleen cells. The PNA+ thymocyte subpopulation exhibited the highest ADA activity of all cells studied. The lowest PNP activity was found in the PNA- subpopulation of thymocytes. PNA+ cells, moreover, exhibited a more intensive DNA synthesis than the PNA- cells, and a greater sensitivity to deoxyadenosine toxicity in the presence of erythro-9-(2-hydroxy-3-nonyl)-adenine (EHNA). The two thymocyte subpopulations exhibited a similar sensitivity to deoxyguanosine (dG) toxicity.

Pre-T cell lineage of hand-mirror cells in acute lymphoblastic leukemia.

Acute lymphoblastic leukemia was observed in a 64-year-old male patient with 50-80% hand-mirror cells in the peripheral blood and in the bone marrow. Immunologic surface marker tests indicated that the cells were non-T non-B lymphocytes; however, the addition of thymosin (fraction V) to the bone marrow cells induced the capability of mounting a positive local xenogeneic graft-versus-host reaction (GVHR), demonstrating the pre-T-cell lineage of the hand-mirror cells in this patient.

Macrophage migration inhibition assay as a means of detecting lymphocyte sensitization to leukemic myeloblasts in neutropenic patients.

The migration inhibiting factor (MIF) test was performed on 27 patients with chronic neutropenia using acute myeloblastic leukemia (AML) blasts and normal lymphocytes as antigens. Eight patients reacted positively to AML blasts, and the natural history of their disease differed from that of the remaining neutropenic patients: two of the eight patients developed AML and one showed evidence of preleukemic changes. Recurrent severe infections were common and one patient in this group developed systemic lupus erythematosus. The MIF test toward AML blasts may be regarded as a warning sign of a preleukemic state among neutropenic patients.

Human lymphocytes-macrophages interaction: a good prognostic sign in malignant lymphoma.

The lymphocyte surface markers as well as the lymphocytes-macrophage interaction were examined in 12 malignant lymphoma patients in the peripheral blood, bone marrow and/or lymph node biopsy or in the pleural effusion (1 patient). In 5 of them a lymphocytes-macrophage interaction ("immunological island") was found: these 5 patients responded well to chemotherapy and their clinical course was satisfactory. The patients in whom a low mouse rosettes percentage was found were in a poorer clinical condition as compared to those with a high mouse rosettes percentage. The lymphocyte-macrophage interactions are probably due to immunological activation of the patient by the malignant antigen and seemed to be a good prognostic sign.

Continuous high-dose intravenous desferrioxamine treatment for iron overload.

We describe a patient with heavy posttransfusion iron overload, to whom continuous high-dose i.v. desferrioxamine was administered shortly before his death. The administration of nearly 500 g of this agent was followed by urinary excretion of 5.4 g of iron. The chelator therapy lost its efficiency in the last of five courses and the patient eventually died as a result of cardiac and hepatic damage related to iron overload. The theoretical and practical implications of these observations are discussed.

Activity of adenosine deaminase and of purine nucleoside phosphorylase in peripheral lymphocytes from patients with acquired immunological disorders.

The activity of adenosine deaminase (ADA) and of purine nucleoside phosphorylase (PNP) was determined in the peripheral lymphocytes of patients with diseases associated with acquired partial dysfunction of the immune response. Increase ADA activity was found in patients with Waldenstrom's macroglobulinemia and in some patients with non-Hodgkin's lymphoma. Increased PNP activity was found in patients with non Hodgkin's lymphoma whereas decreased PNP activity was fund in patients with connective tissue disorders. The alternations found in ADA and PNP activities probably reflect changes in the lymphocyte subpopulations and do not seem to have an etiological role in the pathogenesis of the disturbed immune response.

Hemochromatosis in a renal transplant recipient.

A 47 year old kidney transplant recipient who died from liver failure caused by hemochromatosis, is described. The diagnosis was established by post mortem examination. The question whether these findings are an extreme form of the common pathological changes seen in the liver in other transplant recipients, or were related to infectious hepatitis or were due to the use of immunosuppressive therapy, remains unanswered.

Viral hepatitis with extreme hyperbilirubinemia, massive hemolysis and encephalopathy in a patient with a new G6PD variant.

Extreme hyperbilirubinemia, massive hemolysis and hepatic encephalopathy were reported in a G6PD-deficient patient with viral hepatitis. Investigation of the G6PD properties revealed that this patient's enzyme represented a hitherto undescribed variant (Bnei Brak), characterized by heat stability and a unique triphasic pH-related enzyme activity curve. It is possible that the marked hemolytic process in this patient was enhanced by a further decrease in the activity of the already deficient enzyme during acidosis.
