Two cases of Hashimoto's thyroiditis with pupillary disturbances.

Two cases of Hashimoto's thyroiditis with pupillary disturbances are reported. Case 1 was a 58-year-old housewife, displaying pandysautonomia. Case 2 was a 65-year-old housewife, suffering from chronic polyneuropathy and polymyopathy. Both patients showed similar pupillary disturbances, suggesting both sympathetic and parasympathetic postganglionic denervation of the pupil. Hashimoto's thyroiditis may have caused the pupillary disturbances in association with generalized polyneuropathy or pandysautonomia.

Pupillary abnormalities in schizophrenic patients during long-term administration of psychotropic drugs: dissociation between light and near vision reactions.

Pupillographic studies were made of the reactions to light and near vision in 12 schizophrenic patients under long-term administration of psychotropic drugs. The results showed a significant reduction in the light reaction, while the near vision reaction was preserved. The pupillographic study revealed not only reduction in amplitude of the light reaction but also changes in dynamic aspects of the reaction, i.e., prolonged latency time, shortened constriction time, and half redilatation time after the light stimulus. The mechanisms underlying the dissociation between light reaction and near vision reaction induced by long-term administration of psychotropic drugs are obscure, but both the peripheral and central actions of these drugs may be involved.

[Clinical experiences and experimental studies of false sensing with demand pacemakers (author's transl)].

False triggering with a ventricular triggered pacemaker and false inhibition with a ventricular inhibited pacemaker were observed in two patients with complete heart block. These arrhythmias were considered to be attributable to false sensing with a demand pacemaker. Both in vitro and in vivo studies were performed to elucidate the mechanism of these false sensings, and the false sensings were reproduced in a series of experiments. Loose contact of the tip of electrodes to myocardial tissue, refractory period, the distance between two electrodes, and the after potential and its dv/dt of pacemaker impulses are considered to affect the performance of demand pacers.

Mesenteric flap in free jejunal transfers: a versatile technique for head and neck reconstruction.

BACKGROUND: The technique of free jejunal transfers has been widely used for pharyngoesophageal reconstruction. However, secondary infectious and fistulous complications may develop in extensive and preoperatively high-dose irradiated cases. METHODS: A generous jejunomesenteric composite graft was harvested and the mesenterium was effectively applied for head and neck reconstruction. RESULTS: The mesenteric flap connected with the revascularized jejunum was used as means for obliterating dead space in the head and neck region, as a cover for the cover for the cervical vessels, as a vascularized bolstering for the vascular and enteral anastomoses, or as a vascularized bed for skin grafting when the skin flaps are deficient. Only one small fistula occurred in 10 patients who underwent the mesenteric flap. CONCLUSIONS: This versatile technique is expected to reduce the incidence of postoperative complications in pharyngoesophageal reconstruction using free jejunal transfers.

Combined V figure-shaped scapular osteocutaneous and latissimus dorsi myocutaneous flap for composite mandibular reconstruction.

BACKGROUND: As early as 1981, the scapular crest was reported as a versatile donor site of vascularized bone flap. In our institute, much attention has been paid to applying a compound flap based on the subscapular vascular system for composite mandibular reconstruction. METHODS: The combined V figure-shaped scapular osteocutaneous and latissimus dorsi myocutaneous flap has been used for primary or secondary reconstruction of the mandible, intraoral mucosa and/or external skin following major ablation of the malignant tumors in seven patients. RESULTS: There was one total flap failure. Satisfactory results were obtained in the remaining patients. CONCLUSIONS: The V figure-shaped scapular bone flap supplied by the angular branch provides a long rotation arc of vascular pedicle, a long bony segment of more than 14 cm, and a good contour of mental protuberance. The combination of the scapular osteocutaneous flap with latissimus dorsi myocutaneous flap allows reliable reconstruction of massive and three-dimensional composite mandibular defects.
