Evaluation of the progress and prognosis of adult respiratory distress syndrome. Simple respiratory physiologic measurement.

In our study of 14 patients with adult respiratory distress syndrome (ARDS), we measured A-aDO2, VD/VT, arterial-to-end tidal PCO2 ), effective dynamic compliance, and pulmonary vascular resistance on a daily basis. At the onset of ARDS, all patients showed bilateral interstitial edema on the chest X-ray films, P(A-a)O2 of more than 500 mm Hg, marked decrease in effective dynamic compliance, a moderate increase in VD/VT, and a normal value of a-etPCO2. Pulmonary vascular resistance was low. After seven days, all of those who subsequently died and developed persistent elevation of P(A-a)O2, significant increase in VD/VT, a-etPCO2 and pulmonary vascular resistance, and significant decrease in effective dynamic compliance compared to the values at the onset of ARDS. Those abnormalities diverged significantly from the findings in those who survived. By evaluating sequential changes of those parameters, we might be able to predict an accurate prognosis of ARDS.

Differences in urinary trichloroethylene metabolites of animals.

Differences in urinary excretion of trichloroethylene were studied in rabbits, rats and mice. Trichloretylene (1 m mole/kg) was injected intra-peritoneally, then urinary trichloroacetic acid and trichloroethanol glucuronide were measured. The results were: 1. The ratio of total excretion of trichloroethylene metabolites to the administered trichloroethylene decreased in the order of mice, rats and rabbits. 2. The ratio of total trichloroethanol to trichloroacetic acid in urine decreased in the order of rabbits (69.2), mice (12.8) and rats (2.3). The high ratio in rabbits was due to the extremely small amount of trichloroacetic acid in the urine. 3. Differences in these two urinary metabolites in the three kinds of animals and in human subjects were discussed.

Determination of 15N-ammonia by chemical ionization mass spectrometry.

The tracing of ammonium nitrogen is important in hepatic diseases, especially in liver cirrhosis with hyperammonemia. In this paper, a new convenient method is described for determining stable isotope 15N-labeled ammonia using the Conway apparatus and chemical ionization mass spectrometry. The minimum quantity of ammonia necessary for reliable measure of 15N content was 0.1 muMol. The average error was 5.3%. In clinical application, 4 ml of blood after oral administration of 1 g of 15NH4Cl was sufficient for measurement.

Observations on hepatic superficial lymph flow.

Hepatic superficial lymph flow was studied by injecting patent blue V solution into the liver under peritoneoscopy. This procedure enabled detailed observations of the direction of hepatic lymph flow in superificial lymphatic vessels and migration of lymph from these lymphatic vessels to lymphatic vessels of the gallbladder. Increase in this flow was observed in patients with increased liver fibrosis and lobular distortion.

[Treatment of Diphyllobothrium latum and Taenia saginata infections with aminosidine (author's transl)].

Recently, it has been known that the aminosidine has marked anthelmintic efficacy against tapeworm. In this investigation, aminosidine was used for treating 14 cases with Ciphyllobothrium latum infection and 5 cases with Taenia saginata infection. Aminosidine was administered orally in a single dose of 50 mg/kg, followed by a purge after the treatment. Fourteen patients with D. latum infection and 5 patients with T. saginata infection expelled long strobila in all cases. Although only 7 scolices of 18 worms of D. latum were found in the stool and no scolex of T. saginata was found, follow-up examination for a long period showed no evidence of remaining infection with one exception of D. latum. Mild nausea, vomiting and abdominal pain were observed in only one of 19 cases given aminosidine. But in the other 18 cases, no side effects were encountered. It was concluded that aminosidine is safe, effective therapeutic agent for the treatment of cestodiasis in man.

The initial positive component of the scalp-recorded somatosensory evoked potential in normal subjects and in patients with neurological disorders.

The initial positive component of the scalp-recorded somatosensory evoked potential to median nerve stimulation was studied in 12 patients with sensory impairment for all modalities of the unilateral extremities due to lesions at or above or below the thalamic level, taking the potentials obtained from eight normal subjects as control. In normal subjects, this potential could be easily obtained in ear reference recordings with a peak latency of 12--15 msec. This finding and positive polarity of this potential indicate that the earlobe could be active for this potential. The wide distribution of this potential was different from the subsequent negative-positive diphasic components. In patients with lesions at or above the thalamic level the p15 potential was of normal configuration and latency to stimulation of the affected side, whilst in patients with lesions in the brain stem or in the cervical cord it could not be obtained to stimualtion of the affected side. These findings and short latency of the p15 potential suggest that it may be the result of activity of the medial lemniscal systems from the medulla to the thalamus.

Fine structure of the myotendinous junction of lathyritic rat muscle with special reference to connectin, a muscle elastic protein.

The fine structure of the myotendinous junction of the skeletal muscle of lathyritic rats caused by beta-aminopropionitrile was investigated. In the junction there are finger-like processes of muscle fibers, in which thin filaments were extended from the last Z lines of myofibrils and attached to the sarcolemma of the processes. By the heavy meromyosin decoration technique, these thin filaments were identified as actin filaments. In the lathyritic muscle, the thin filaments were markedly fewer in number and distributed sparsely in the sarcoplasm. The content of connectin, an elastic protein, which is localized in myofibrils and also in sarcolemma was significantly decreased in the lathyritic muscle. A possible relationship between the changes in the fine structure of the myotendinous junction and in the connectin contents is discussed.
