Pig brain contains cholecystokinin octapeptide and several cholecystokinin desoctapeptides.

A sequential method employing methanol extraction of the COOH-terminal fragment of cholecystokinin (CCK) from pig brain followed by HCl extraction of the more basic CCK peptides was used as the first step in purification of these peptides. Recovery was monitored with two different assays, one directed to the COOH terminus of CCK and the other to the NH2 terminus. The amino acid content and sequence were determined for each of five peptides after purification. The only peptide containing COOH-terminal immunoreactivity was CCK-octapeptide (CCK8). The other four peptides did not contain CCK8 and had lost one or two additional amino acids, perhaps as a consequence of the action of carboxypeptidases. These peptides were shown to be CCK33-desnonapeptide, CCK39-desnonapeptide and -desdecapeptide, and a large molecular weight precursor, CCK58-desnonapeptide, containing 19 amino acids (Ala-Val-Gln-Lys-Val-Asp-Gly-Glu-Ser-Arg-Ala-His-Leu-Gly-Ala-Leu-Leu-Ala-Arg) NH2-terminal to CCK39. The three NH2-terminal fragments of CCK58, CCK39, and CCK33 were about equally prominent. The brain, unlike the gut, appears to cleave CCK8 rapidly from a precursor peptide but to process the NH2-terminal portions of the molecule more slowly and incompletely.

[A new method of ovarian transfer in patients with cervical cancer].

As a general treatment for women with invasive carcinoma of the cervix with pelvic lymph node metastasis and/or invasion into the cardinal ligament, a radical operation has been necessary followed by pelvic radiation. However, the irradiation leads to complete cessation of function of ovaries which were retained intact at the radical operation. In order to avoid the influence of irradiation on the ovary, a new operation has been considered; this consists of the transfer of the ovary (1) to the breast or (2) to the lateral upper abdomen. Both sites for transplantation are entirely removed from the pelvic radiation field. The procedure in the former is that the ovary with its own vessels, A. and V. ovarica separated from the retroperitoneum by at least 20 cm, is transferred into the breast through subcutaneous tissue and is followed by fixation into the breast adipose tissue through semicircular incision on areola mammae. In the latter technique, however, a sufficient length of ovarian vessels separated from retroperitoneum is mostly 10 cm or less in order to transfer the ovary to the upper lateral abdomen and fix it on the fascia of this site. A total of 51 patients, in which the age ranged from 24 to 45 years old, have received the ovarian transfer at the radical operation; 31 patients by the former technique and 20 by the latter. After the operation, angiography showed that the circulation of the transplanted ovary was sufficient. Monthly cyclic change in size of the transplanted ovary (CCOS) was evident after operation. This change was related to BBT and levels of serum hormones (FSH, LH, E2 and P4). The frequency of occurrence of the evident CCOS was 20 out of 27 cases (74%) in the former technique and 18 out of 18 (100%) in the latter technique. Radiation doses measured on both the transplanted locations at pelvic irradiation were low enough to avoid disfunction in the transplanted ovary.

A case of granulomatous renal sarcoidosis with a dramatic response to corticosteroid and urokinase therapy.

A case of renal granulomatous sarcoidosis that presented with chronic renal failure (CRF) is described. Renal biopsy specimens revealed typical features of sarcoidosis in light microscopy and immunofluorescence microscopy examinations. The absence of bilateral hilar lymphadenopathy (BHL) was a distinctly unusual feature of sarcoidosis although uveitis and rectal granuloma were observed during the clinical course. A dramatic response occurred on corticosteroid and urokinase therapy, characterized by a fall of serum creatinine levels.

[Studies on non-surgical therapy of tubal pregnancy].

8 cases of tubal pregnancy in the pre-abortional stage were detected by B-scope examination. The administration of MTX (total doses: 60-300mg) resulted in complete remission in 7 cases; the other received tubectomy because of the progression of abortion. The dose of MTX used to obtain remission is responsible for the HCG titer of urine before the treatment; 60-150mg of MTX was sufficient in cases which showed a titer below 1,000iu/l, although 75-300mg was necessary in cases over 4,000iu/l. The patency of tubes after the regimen was confirmed in 4 out of 5 cases with hystero-salpingography. Serious side effects were not observed in the present study in spite of the great efficacy; transient suppression of liver has been observed in 2 cases. Two patients had normal intra-uterine pregnancy subsequent to the treatment; one had a matured full-term female neonate and the other received D & C in 9 gestational weeks. These results suggest that the MTX regimen is practical for the treatment of patients with tubal pregnancy. This is characterized by unimpaired fertility following the treatment, although the detection of patients in the pre-abortional stage is absolutely necessary.

Post-translational processing of cholecystokinin in pig brain and gut.

A sequential extraction method employing methanol extraction of the COOH-terminal fragments of cholecystokinin (CCK) from pig tissues followed by HCl extraction of intact CCK and its NH2-terminal fragments is described. Radioimmunoassay of extracts and their fractionation by Sephadex chromatography and HPLC demonstrate that the distributions of COOH-terminal and NH2-terminal immunoreactivities among various regions of brain are similar and independent of the concentrations in individual regions. The distribution in gut differs from that in brain. Greatest concentrations of CCK immunoreactivity are located in cortical tissue in the brain and in duodenal mucosa in gut. Both brain and gut contain CCK octapeptide (CCK8) and an NH2-terminal fragment that is likely to be desoctapeptide-CCK33. Intact CCK33 is extractable from gut but not from brain. Brain contains another NH2-terminal immunoreactive molecule lacking COOH-terminal immunoreactivity that may be a peptide with a COOH-terminal extension, as has been described for gastrin, or one that may not be derived from a CCK33-like precursor. This peptide is much less prominent in gut, or may be nonexistent there. The failure to find CCK33 in the brain and the presence in the brain of this as-yet-uncharacterized NH2-terminal peptide raises the question as to whether the differences between neuronal and mucosal tissues are a consequence of differences in post-translational processing or in the DNA templates.

Vitamin D metabolism in pregnant rabbits: differences between the maternal and fetal response to administration of large amounts of vitamin D3.

Maternal and fetal metabolism of vitamin D was examined in term pregnant rabbits fed a normal diet and in those supplemented with a large amount of vitamin D3. Term pregnant rabbits (27--30 days of gestation) fed the normal diet showed lower levels of plasma calcium, 25-hydroxyvitamin D3 (250HD3), and 24,25-dihydroxyvitamin D3 [24,25-(OH)2D3] and higher plasma 1 alpha, 25-dihydroxyvitamin D3 [1 alpha, 25-(OH)2D3] levels than age-matched nonpregnant female rabbits. Kidney homogenates from pregnant rabbits produced mainly 1 alpha 25-(OH)2D3, while those from nonpregnant animals produced 24,25-(OH)2D3 primarily. Plasma concentrations of calcium and phosphorus were significantly higher in fetuses than in mothers. Plasma levels of 250HD3 and 24,25-(OH)2D3 in fetuses were almost identical to those in mothers, whereas 1 alpha,25-(OH)2D3 levels in plasma were significantly higher in mothers than in their fetuses. A daily administration of 650 nmol vitamin D3 for 3 days to term pregnant rabbits caused a significant increase in calcium, phosphorus, 25OHD3, and 24,25-(OH)2D3 in maternal plasma, and in 25OHD3 and 24,25-(OH)2D3, but not calcium and phosphorus in fetal plasma. Treatment with large amounts of vitamin D3 also induced a marked suppression of 1 alpha-hydroxylase activity and a concomitant increase of 24-hydroxylase activity in the maternal but not in the fetal kidney. Plasma concentrations of 1 alpha,25-(OH)2D3 were not affected by treatment with large amounts of vitamin D3 in either the fetuses or the mothers. These results clearly indicate that the renal 25OHD3 metabolism in the fetus is regulated independently of that in the mother.

Simultaneous detection of thyroglobulin (Tg), thyroxine (T4), and Triiodothyronine (T3) in nontoxic thyroid tumors by the immunoperoxidase method.

In 97 nontoxic thyroid tumors, detection of thyroxine(T4) and triiodothyronine(T3) was attempted by the immunoperoxidase method. T4 was demonstrated in 58 tumors (59.8%) and T3 in 76 (78.4%). The feasibility of biosynthesis of T4 and T3 by such tumors was thus established. In 65 of the tumors, we applied the immunostaining method to serial or semiserial sections to study the correlation among the localizations of thyroglobulin(Tg), T4, and T3. The localization of T4 agreed relatively well with that of Tg, and part of the Tg-positive structure frequently revealed simultaneous positive staining for T4. The localization of T3, however, did not always correspond with that of T4 or Tg. T4 and T3 with localizations identical to that of Tg may be considered to be bound to Tg, but the mode of existence of T3 without correspondence in localization to Tg remains unknown.

[The propensity to malignant changes in heterozygous moles].

There are at least two mechanisms of origin for complete mole, each with different genetic implications; 1) the fertilization of an empty egg by a haploid sperm and its subsequent duplication (homozygous mole), 2) the fertilization of such an egg by two haploid sperms, which produces an heterozygous constitution of allelic genes (heterozygous mole). The present study was undertaken to investigate whether the difference of origin would determine whether the moles underwent malignant transformation or not. By using of chromosomal, enzymatic and HLA polymorphisms, 35 moles were confirmed to be androgenetic in origin. Among them fifteen homozygous and 4 heterozygous moles were identified. The persistency of high HCG titration of the urine in the mother was observed in all 4 cases from the latter (4/4-100%), though the percentage of the development of sequelae in the former was low (1/15-6.7%). The diagnosis of destructive mole was established in the uterine lesion of 2 heterozygous moles by pathological examination. These two patients had lung metastases. A high incidence of sequelae after the expulsion of heterozygous moles suggests that the heterozygous constitution of allelic genes plays an important role in the process of malignant transformation of trophoblasts.

Cytomorphologic and immunocytochemical studies of chlamydial infections in cervical smears.

A total of 872 cells in 183 Papanicolaou-stained cervical smears morphologically suspected of harboring chlamydial infections were cytologically investigated in an attempt to differentiate the morphologic features of chlamydial infection from those of mucus vacuoles or bacterial infection. The observed inclusions were classified according to their morphologic appearance and their staining by a Chlamydia-specific peroxidase-antiperoxidase stain and by the periodic acid-Schiff technique. Immunoperoxidase-positive inclusions were detected in 201 cells from 13 cases; 200 (99.5%) of these cells contained "nebular" inclusions while 1 cell (0.5%) contained multiple inclusions with homogeneous central target formation. These findings suggest that nebular-type inclusions may be the key morphologic finding for the identification of chlamydial infection and that the application of an immunoperoxidase staining technique on the destained Papanicolaou preparation may be useful for the diagnosis of equivocal inclusions.
