Elastolytic strains of Prevotella intermedia were isolated from pus samples of adult periodontal lesions. Elastase was found to associate with envelope, and it could be solubilized with guanidine-HCl. The enzyme was purified to homogeneity by sequential procedures including ion-exchange chromatography, gel filtration, and hydrophobic interaction chromatography. This elastase was a serine protease, and its mass was 31 kDa. It hydrolyzed elastin powder, but collagen and azodye-conjugated proteins were not degraded by this enzyme. Both synthetic substrates for human pancreatic (glutaryl-L-alanyl-L-alanyl-L-prolyl-L-leucine p-nitroanilide) and leukocyte elastase (methoxy succinyl-L-alanyl-alanyl-L-prolyl-L-valine p-nitroanilide) were hydrolyzed.
The tsBN7 cell line, one of the mutant lines temperature sensitive for growth which have been isolated from the BHK21 cell line, was found to die by apoptosis following a shift to the nonpermissive temperature. The induced apoptosis was inhibited by a protein synthesis inhibitor, cycloheximide, but not by the bcl-2-encoded protein. By DNA-mediated gene transfer, we cloned a cDNA that complements the tsBN7 mutation. It encodes a novel hydrophobic protein, designated DAD1, which is well conserved (100% identical amino acids between humans and hamsters). By comparing the base sequences of the parental BHK21 and tsBN7 DAD1 cDNAs, we found that the DAD1-encoding gene is mutated in tsBN7 cells. The DAD1 protein disappeared in tsBN7 cells following a shift to the nonpermissive temperature, suggesting that loss of the DAD1 protein triggers apoptosis.
By use of site-specific antibodies against synthetic oligopeptides, we examined the localizations of the gap junction proteins connexin 32 (Cx32) and connexin 26 (Cx26) in rat and guinea pig liver. Double-labeling immunofluorescence microscopy revealed that in guinea pig liver both proteins were spread throughout the liver lobules and seemed to localize together within the same gap junction plaque. In rat liver, co-localization of both Cx32 and Cx26 in the same plaques was also suggested in periportal zones. Quick-freeze, deep-etch immunoelectron microscopy showed that immunolabeling of isolated guinea pig liver gap junction plaques with either Cx32 or Cx26 antiserum yielded complete and dense antibody decoration of the cytoplasmic surface of the plaques. In isolated rat liver plaques, the cytoplasmic surfaces were densely decorated with Cx32 antiserum, whereas Cx26 labeling yielded diffuse decoration with variable intensity of the plaques. In both species we did not observe any focal or patchy clusters of the labeling in any plaques examined. Double-labeling immunoelectron microscopy confirmed that both Cx32 and Cx26 are co-localized in the same gap junction plaques. These results suggest that in hepatocytes expressing both Cx32 and Cx26, both types of gap junction proteins are not segregated but intermingle randomly within the same plaques.
The reactivity of factor G mediated coagulation pathway in limulus amebocyte lysate which is triggered by (1-->3)-beta-D-glucans is thought to depend on the structure of the glucans, especially on the ultrastructure: triple helix, single helix and random coil. We used Sonifilan (SPG) and grifolan (GRN) as parent compounds to compare the reactivities of these three conformers. Under a neutral condition, alkaline treated SPG (SPG-OH, single helix) and polycarboxylated SPG (PC-SPG, random coil) showed significantly stronger reactivity than untreated SPG (triple helix). After the alkaline treatment, all three conformers showed comparable reactivities. It is suggested that the pretreatment of the glucan preparations by sodium hydroxide is quite important to compare quantitatively the reactivity of the glucans by limulus test, and comparing the data of untreated and alkaline treated glucans would provide information about their conformations. Using this approach, it was found that after heat treatment at around 150 degrees C, the conformation of GRN was changed to rich in the triple helix, and that following sodium hydroxide treatment and dialysis of GRN, the conformation of GRN was changed to single helix rich conformer. About half of the single helix conformer was gradually changed to triple helix conformer over one week at 4 degrees C.
Noninvasive ambulatory blood pressure and heart rate monitoring were used to investigate cardiovascular dysfunction in patients with human T-lymphotropic virus type-I (HTLV-I)-associated myelopathy (HAM). The subjects were 23 patients with HAM, and 23 sex- and age-matched normotensive healthy volunteers (controls). Circadian rhythms of blood pressure and heart rate were present in both the HAM patients and controls. Amplitudes and the 24-hr mean systolic and diastolic blood pressures were significantly lower in the patients than in the controls. The 24 hr mean heart rate was significantly higher in the patients, the difference being particularly marked during the night. Differences in the acrophases of the systolic and diastolic blood pressures and heart rate between patients and controls were small but still significant. These results suggest that subclinical cardiovascular autonomic dysfunction is present in HAM patients.
The health effects of human T-lymphotropic virus type I (HTLV-I) were studied in HTLV-I endemic areas in Japan during the period from September 1990 to October 1991. The study subjects were 2,847 men living in the southern part of Kyushu and engaged in light work. Ages ranged from 19 to 79 years (47.8 +/- 7.2; mean +/- standard deviation). The prevalence of each disturbance was analyzed with a logistic model. The disturbances in which the prevalence showed a significant association with HTLV-I were old tuberculosis, ulcerative colitis and ossification of the posterior longitudinal ligament of the cervical spine (OPLL). The results of the present study suggest that HTLV-I may be a risk factor for these diseases and may indicate the necessity for a synthetic study on HTLV-I.
We report a rare case of thoracic epidural angiolipoma in a 38-year-old female with clinical, radiological, and histological findings including magnetic resonance imaging. Magnetic resonance imaging can provide preoperative diagnosis of spinal angiolipoma, and complete removal can achieve a successful outcome. The possible embryogenetic origin is, however, not proved.
The utility of gadolinium (Gd)-enhanced magnetic resonance (MR) angiography in the planning of supratentorial glioma surgery was evaluated. MR angiograms of the arterial and venous anatomy visualized the anatomical relationship of the main arteries to the tumor, especially on three-dimensional images. Combination with data from surface anatomical images allowed the tumor location to be determined preoperatively. Gd-enhanced MR angiography can be used non-invasively prior to tumor surgery to ascertain the spatial relationship of the vessels to the tumor mass.
We analyzed morphologically autoimmune disease in MRL/MpJ mice bearing both the Yaa and lpr genes (MRL-lpr,Yaa mice), and compared it with that in MRL/MpJ-lpr/lpr (MRL-lpr) mice, in order to examine the effect of the Yaa gene on lpr-induced tissue-specific immunopathologies. MRL-lpr,Yaa male mice developed glomerulonephritis more rapidly than did MRL-lpr males. The glomerular damage in MRL-lpr,Yaa males, as evaluated by histologic and immunofluorescent methods, was significantly greater than that in age-matched MRL-lpr males. In contrast, no differences in the development of vasculitis and arthritis were noted between the two groups. Pathological examination of the dead mice revealed a similar incidence of lethal glomerulonephritis in the two groups. Lymphoid hyperplasia in the spleen consisted predominantly of unusual T cells (B220+, Thy-1+, CD4-, CD8-) in the two groups, and an increased number of B cells was not found in MRL-lpr,Yaa mice. The histological nature of the autoimmune diseases was similar in MRL-lpr,Yaa and MRL-lpr males. These results indicate that the Yaa gene accelerates the development of glomerulonephritis but not that of vasculitis or arthritis, suggesting that the mechanisms underlying the initiation of glomerulonephritis are different from those leading to vasculitis or arthritis in MRL-lpr mice.
A 58-year-old female with renal failure underwent anastomosis of the left internal mammary artery to the left anterior descending branch of coronary artery without extracorporeal circulation (ECC) and cardiac arrest. Anesthesia was maintained smoothly with modified NLA under normothermia. Without use of ECC, which often disturbs water and electrolyte metabolism, the patient's physical status remained quite stable. Though the left anterior descending artery was clamped for 20 minutes during the anastomosis, there was no complication, including perioperative myocardial infarction. For high risk patients with ECC such as patients with renal failure, respiratory failure and cerebrovascular diseases, this method is useful for the coronary artery bypass surgery.
A 65-year-old man was admitted because of bleeding from gastric ulcer. A massive deposition of AL lambda type amyloid proteins in the gastric lesion was found. There were a few Bence-Jones (BJ) proteins in the urine, but serum M proteins and bone lesions were not found. Bone marrow puncture showed no dysplastic change of plasma cells. Moreover, there were no amyloid proteins deposited anywhere in the digestive tract examined, except for the stomach, nor were they found in the bone marrow, prostata, liver or kidney. Gastric amyloidosis was diagnosed. Gastrectomy was performed because of uncontrollable bleeding. Sixteen months later, bone swelling occurred in the sternoclavicular joint and the 3rd rib. At that time, plasma cells with dysplasia and increasing levels of urine BJ protein were revealed. BJ type multiple myeloma was finally diagnosed. For remission induction, cyclo-VAMP chemotherapy was given in 3 courses. Decreased levels of urine BJ protein, diminished bone lesion, normalization of bone marrow and fewer amyloid deposits were seen, and partial response was obtained. The patient is well with no signs of recurrence evident 42 months after treatment. This is an interesting case of initial gastric amyloidosis that developed into BJ type multiple myeloma and was effectively treated with cyclo-VAMP chemotherapy.
A 65-year-old man with advanced prostate cancer was treated with a luteinizing hormone-releasing hormone (LH-RH) analogue. Four days after the initial injection of 3.6 mg of goserelin acetate, severe dyspnea developed due to worsening pleuritis carcinomatosa, which was considered as a flare-up. Chest drainage was required to save his life. Therefore, we emphasize the importance of treatment to prevent tumor flare during LH-RH analogue therapy.
The influence of age and sex on the threshold dose of mercury in Minamata disease was studied by dose-response analysis based on mercury concentrations in hair obtained mainly from adults living near the Agano River at the beginning of Niigata Minamata disease outbreak in 1965. The subjects were 174 male and 694 female inhabitants of polluted areas including 55 males and 66 females officially recognized as Minamata disease patients. Because of the large sex differences in numbers and age distributions, the data were analyzed separately for males and females. While revealing no sex difference in the threshold dose, hockey-stick regression analysis indicated an effect of age on threshold dose in females. Logistic regression analysis showed no interaction between age and hair mercury concentration. These results suggest that, in adults, neither sex nor age exert a considerable effect on the threshold dose of mercury in Minamata disease.
A 59-year-old man with coronary artery disease and arteriosclerosis obliterans of left lower extremity underwent anastomosis of left internal thoracic artery to left anterior descending artery with cardiopulmonary bypass of aortic perfusion and left femoro-popliteal bypass with saphenous vein graft. On the first postoperative day, urinary output decreased and then stopped. The transesophageal echocardiography and angiography revealed the Stanford A type acute aortic dissection. Immediately the resection of the ascending aorta including the intimal tear, which was found on the site of the previous aortic perfusion, and the reconstruction of the ascending aorta with the prosthetic graft was performed. After the reperfusion of left femoral artery, which was used as the route of the arterial perfusion during cardiopulmonary bypass, serum potassium level increased gradually and at last the heart was arrested. Hemodialysis with draining from inferior vena cava produced the stability of hemodynamics, but on the next day he died of low cardiac output syndrome. We presented the case with the acute aortic dissection after open heart surgery, which was one of the rare complications in aortic perfusion of cardiopulmonary bypass and emphasized the possibility of occurrence of myonephropathic-metabolic syndrome after the repair of acute aortic dissection with limb ischemia.
A 67-year-old man was diagnosed as non-Hodgkin lymphoma (diffuse, mixed cell type, B cell) with clinical stage IV in March 1989. He had been treated many times with salvage chemotherapy on admission including CHOP-etoposide and ProMACE-Cyta BOM. Recurrence, however, occurred in two or three months after any therapy. He had a therapeutic history of chronic daily administration of etoposide. In November 1991, recurrence was found in the peripheral lymph node and the colon. A new regimen of oral administration of etoposide, 50 mg/day four times a week, was employed. Two months later, the peripheral lymph node swelling disappeared. Seven months later, the patient showed no signs of recurrence by Ga-scintigraphy. This therapy is being continued. It is suggested that anti-neoplastic activity of etoposide is dependent on the schedule of administration. This regimen is suggested to be effective in the treatment of patients with refractory malignant lymphoma.
Flow injection-inductively coupled plasma mass spectrometry has been evaluated for determining the distribution profile of trace elements along a single strand of hair. Hair was cut into several mm long sections from follicle to the distal end. Each section was solubilized in a capped 1.5-mL polypropylene tube with small volume of nitric acid (typically 50 microL) at room temperature. After dilution an aliquot (50 microL) was introduced into the mass spectrometer by flow injection. The limit of determination was typically 5-50 pg with 5-10% precision (CV), depending on the element examined; this corresponds to sub-microgram/g concentrations of these elements in hair segments. Recent exposure and intake history of individuals to thallium or mercury could be reconstructed by this system.
Thirty five-year-old man required reoperation, twelve years after radical operation of total anomalous pulmonary venous connection (Darling IIb type), because of the clinically apparent pulmonary venous obstruction. The stenosis of ASD was revealed by cineangiocardiography and echocardiography. The fibrous thickening of atrial septal wall and stenosis of ASD were found at the operation. Operating procedure consisted of the enlargement of ASD and cut-back of the atrial septal wall. Postoperative study showed no evidence of stenosis of pulmonary venous return. The patient was discharged well and returned to his job. Late postoperative pulmonary venous obstruction is of particular concern. Usually it occurs within the first few postoperative months. We reported this case because it is rare that pulmonary venous obstruction occurred twelve years after radical operation.
