[Clinical analysis of 25 cases of leiomyoma of the vagina].

OBJECTIVE: To study the histologic source, clinical features and treatment methods of leiomyoma of the vagina. METHODS: From January of 1988 to January of 1999, 25 patients with leiomyoma of the vagina were retrospectively analyzed. RESULTS: The clinical features of leiomyoma of the vagina were slow in growth and solitary in number. Leiomyoma of the vagina can be recurrence and sarcomatous change. The symptoms of leiomyoma of the vagina depended on the size and location of the leiomyoma. Treatment consisted of surgical excision by vagina. CONCLUSIONS: Leiomyoma of the vagina is a rare condition. Whenever such a tumor is detected, it has to be removed immediately to prevent further growing and sarcomatous change in the future.

[Study of mechano-sensitive K+ channel in cerebral microvascular endothelial cells from rats].

This study was conducted to gain an understanding of the electrophysiologic characteristics of cerebral microvascular endothelial cells. The stress response of the potassium ion channel in cerebral microvascular endothelial cells was explored. We developed a model of the opening shear stress effect device and used it for calculation of shear stress. The cultured cerebral microvascular endothelial cells from rats were seeded on glass slide of 1 cm x 1 cm. K+ ion channel current was recorded by Patch-Clamp amplifier of Axonpatch 200A type and whole cell Patch-Clamp technique. The inward rectifier potassium current of cerebral microvessel endothelial cells has been elicited by flow stimulus of 1.1 dynes/cm2 shear stress, and there is good relationship between the current and voltage changing. The various stress responses of the cerebral vascular endothelial cells are related to the stress sensitive potassium ion channels of cellular membrane.

[The effects of high dose atropine on function of isolated diaphragmatic preparation of rats with omethoate poisoning].

OBJECTIVE: The study was designed to examine the influence of high dose atropine on respiratory muscle of rat isolated phrenic diaphragm. METHODS: Rats were divided into 2 groups: (1) a toxic group administered with 2LD(50) omethoate, SC. (2) a contrast group with 0.9% NS, SC. The function of isolated rat phrenic diaphragm was observed by MS-302 analysis instrument physiologically and pharmacologically. RESULTS: In both groups the function of isolated rat phrenic diaphragm was significantly reduced with atropine dose increasing gradually to 0.6 mg (P < 0.01). When the atropine dose amounted to 0.8 mg. The function of isolated rat phrenic diaphragm almost disappeared in the picture. When the atropine in the diaphragm was weed out, the function of isolated rat phrenic diaphragm recovered completely in 50 minutes. CONCLUSION: It is suggested that the cause of respiratory muscle paralysis may be related to high dose of atropine.

[bcl-2 and p53 protein expressions in the malignant transformation of ovarian endometriosis].

OBJECTIVE: To study the impact of bcl-2 and p53 protein expressions on the malignant transformation of ovarian endometriosis. METHODS: Immunohistochemical technique was used to detect the bcl-2 and p53 protein expressions in: endometriosis (Group I, 22 patients), atypical hyperplasia of ectopic ovarian endometrial glands (Group II, 14 patients) and endometriosis with malignant change (Group III, 22 patients). RESULTS: The positive bcl-2 protein expression rate of Group III was apparently higher than that of Group I (P < 0.05); The positive bcl-2 protein expression rate of Group II was clearly higher than that of Group I(P < 0.01). The positive p53 protein expression rate of Group III was higher than that of Group II (P < 0.01) and Group I (P < 0.01); In Group III, the positive p53 protein expression rate was higher in the malignant transformation area than in the endometriosis area (P < 0.05); In Group III, the positive bcl-2 protein expression rate in the endometriosis area was higher than that of Group I (P < 0.05); In Group II, the positive bcl-2 protein expression rate in the endometriosis area was higher than that of Group I (P < 0.01); However, the bcl-2 and p53 protein expressions were not related to the menopause, stage of disease or the survival (P > 0.05), nor were they related to the malignant transformation area in Group III (P > 0.05). CONCLUSION: It is possible that the overexpressions of bcl-2 protein or p53 protein play an important role in the malignant transformation of ovarian endometriosis. The bcl-2 protein (+) or p53 protein (+) endometriosis may signify a precancerous lesion.

Clinical and experimental study on facial paralysis in temporal bone fracture.

OBJECTIVE: To study the main prognostic factors and significance of facial nerve decompression for facial paralysis in temporal bone fracture. METHODS: The main relative prognostic factors of 64 patients with facial paralysis were analyzed. An experimental model of facial paralysis was made. The expansion rates of facial nerve in the facial canal opening group and the facial canal non-opening group were measured and observed under electron microscope. RESULTS: The main factors affecting the prognosis were facial nerve decompression and selection of surgery time. The expansion rate of facial nerve in the facial canal opening group was significantly higher than that of the facial canal non-opening group (t=7.53, P<0.01). The injury degree of the nerve fiber in the facial canal non-opening group was severe. CONCLUSIONS: Early facial nerve decompression is beneficial to restoration of the facial nerve function.

Clinical retrospective and comparative study on diaphragm injuries in 46 cases.

OBJECTIVE: To explore a way of guiding diagnosis and treatment of blunt and penetrating diaphragm injuries. METHODS: According to injury violence, 46 chest trauma patients with diaphragm rupture were divided into two groups: a blunt injury group and a penetrating injury group. The injury condition and trauma scores between the two groups were compared and analyzed. RESULTS: The incidence of blunt diaphragm injuries was lower than that of penetrating injuries (1.78% vs 8.53%, P <0.05). In the blunt injury group most patients had multiple injuries. Penetrating injuries developed more quickly than blunt injuries, and resulted in hemorrhagic shock in the early period. Trauma scores showed that there was no significant difference in the Revised Trauma Score (RTS), the Injury Severity Score (ISS) and thoracic Abbreviated Injury Scale (AIS) between the two groups (P<0.05), but the blunt injury group had lower Glasgow Coma Scale (GCS) and abdominal AIS than the penetrating group (P<0.0 5). CONCLUSIONS: Blunt and penetrating diaphragm injuries have different clinical characteristics. So they should be dealt with differently to reduce the incidence of complication and improve prognosis.

[A study on the epidemiological characteristics and the preventive measures of rabies in Shanghai].

OBJECTIVE: To analyze the epidemiological characteristics and the preventive measures of rabies in Shanghai from 1950 to 2000. METHODS: Suspected cases of rabies were investigated epidemiologically and collection of samples to isolate the virus. RESULTS: The most highest morbidity of rabies was 1.98/100,000 in 1950. There had been neither case of rabies, nor the rabies virus positive identified in dogs between 1958 and 1988. The morbidity of rabies was 0 - 0.03/100,000 from 1989 to 2000. According to the data of 20 reported cases from 1985 to Sept. 2000, the mean incubation was 66.37 days, except one case of 14 years. The mean course of diseases was 4.55 days. Seventy percent of the cases did not receive any preventive measure. A hundred and ninety-four persons who were bitten by rabies virus positive dogs were timely treated and all of them survived. In recent years, there has been more than 50,000 persons bitten by dogs every year in Shanghai. The rate of rabies vaccine immunization was over 95%. The rate of anti-rabies serum immunization was over 20%. CONCLUSION: Rabies is a preventable disease even when persons were bitten by dogs with rabies virus positive. The important measure is to treat the wound timely, giving vaccine and anti-rabies serum injection. According to the regulation of dog control in Shanghai, the number of dogs be kept at the lowest level, immunizing all the registration dogs, and destroying the none-registration dogs are the most essential measures to be taken.

[Pediatric phacoemulsification with Acrysof intraocular lens implantation].

OBJECTIVE: To investigate the operative effects of pediatric phacoemulsification with Acrysof intraocular lens (IOL) implantation. METHODS: Among 153 eyes of 120 children with cataract aged 3 -- 12 years old, phacoemulsification with Acrysof IOL implantation was performed on 76 eyes of 61 children, and cataract extracapsular extraction with polymethylmethacrylate (PMMA) IOL implantation was performed on 77 eyes of 59 children. The visual acuity, percentages of various complications and rate of neodymium: YAG (Nd: YAG) capsulotomy were followed up for 6 -- 18 months. RESULTS: There were significant differences in the percentages of various complications and rate of neodymium: YAG (Nd: YAG) capsulotomy between the Acrysof group and the PMMA group (P < 0.01). The Acrysof group was associated with less posterior capsular opacification (6.6%) than the PMMA group (80.5%). The YAG capsulotomy rate was 2.6% for the Acrysof group, and 64.9% for the PMMA group. After operation 100% and 90.6% patients were relieved from blindness in the Acrysof group and the PMMA group respectively, the difference being not statistically significant (P > 0.05). 72.6% patients in the Acrysof group and 51.5% in the PMMA group obtained corrected visual acuity greater-than-or-equal 0.3, and the difference was statistically significant (P < 0.05). CONCLUSIONS: Phacoemulsification with Acrysof intraocular lens implantation is effective in pediatric cataract surgery. Most of the patients obtain remarkable improvement in visual acuity with minimal complications and YAG capsulotomy rate after operation. It is necessary to perform further randomized and prospective studies to evaluate the long-term postoperative effects of Acrysof IOLs used in pediatric cataract surgery.

[Clinical and histopathologic analyses of after cataract in rabbit eyes having undergone three different cataract surgeries].

OBJECTIVE: To discuss the surgical method which can efficiently prevent the occurrence of after cataract. METHODS: Eighteen experimental rabbits were randomly divided into 3 groups. Group A was treated with phacoemulsification, group B with phacoemulsification combined with posterior continuous curvilinear capsulorhexis (PCCC) and Group C phacoemulsification combined with PCCC and anterior vitrectomy. Post-operatively, all the surgical eyes were checked with slit lamp microscope and ophthalmoscope. At the post-operative 3 months, histopathologic examinations were performed. RESULTS: There were no post-operative complications, such as retinal detachment, cystoid macular edema, etc. There were Elschnig's pearls and fibers in the peripheral area of the posterior capsule, and the amount was about the same in the 3 groups. There were Elschnig's pearls and fibers at the central area in group A. The fibrous membrane grew from the edge of the PCCC in group B. But in group C, the central area of the posterior capsule was clear, and there was no obvious pathological changes. CONCLUSION: Phacoemulsification combined with PCCC and anterior vitrectomy is a safe cataract surgical method which can effectively prevent the formation of after cataract.

[Studies on antiviral flavonoids in yinqiaosan powder].

OBJECTIVE: To study the chemical constituents in Yinqiaosan Powder that have antiviral action on influenza. METHOD: Isolation of the constituents by different kinds of column chromatography and elucidation of their structures by chemical and spectral methods. RESULTS: Seven flavonoids were obtained, and six of them were identified as linarin, acacetin, hesperidin, isoquiritigenin, isoquiritin and hyperoside respectively. CONCLUSION: Linarin and acacetin were isolated from Yinqiaosan Powder for the first time.

[Cloning, sequencing of beagle E-selectin genomic DNA and comparison of it with other species].

OBJECTIVE: Selectins are a group of members of Cell Adhesion Molecular family; they play very important roles in inflammation and neovascularization. The aim of this study is to clone, sequence Beagle E-selectin genomic DNA and make homogeneous analyses to compare it with those of other animals. METHODS: Twelve fragments of genomic DNA of Beagle E-selectin were obtained by PCR, and the 12 fragments were cloned in T-vector. At last, the genomic DNA sequences of Beagle E-selectin were obtained through gene sequencing, assembling and base calling, and the gene structure was analyzed. RESULTS: The sequence of Beagle E-selectin genomic DNA that contained the whole open reading box was acgured. The results of homogeneous analyses showed a great extent of E-selectin gene homology among human, porcine and dog. CONCLUSION: The demonstration of a large extent of the gene homology has substantiated in this connection the important roles of E-selectin in transplantation immunity and neoplasm development.

[Quantitative detection of flow indices in tubal flow via pulsed Doppler ultrasound, in vitro].

OBJECTIVE: This paper deals with the method and feasibility of quantitative detection of flow indices via commercially available Pulsed Doppler Ultrasound. METHODS: The indices, such as Reynolds number, relative spectral broadening (RSB), laminar shear stress and Reynolds normal stress (RNS) were selected and evaluated. Detection was done temporally and spatially, upstream and downstream of the stenosis. The data were evaluated qualitatively and quantitatively. RESULTS: The roughest turbulence was met at 1/2R sample volume, 1.0 cm downstream of the stenosis. CONCLUSION: The methodology in this study is proved to be feasible and may have good prospects in clinical use.

[The preliminary research of maxillary sinus cancer on oncogene level].

OBJECTIVE: To determine the oncogene c-myc, bcl-2' product expression in the maxillary sinus cancer and to investigate its clinical significance. METHOD: In 46 maxillary sinus cancer and 56 normal nasal mucous membrane, immunochemical method was adopted to determine oncogene c-myc and bcl-2' product. RESULT: In the normal nasal mucous membrane, the positive rates of oncogene c-myc and bcl-2' product were 0; in the maxillary sinus cancer, the positive rates of c-myc and bcl-2' product were 73.91 (34/46), 60.87 (28/46) respectively. The positive rate of c-myc' product in the relapse group(91.67%, 11/12) was significantly higher than that in the non-relapse group(67.65%, 23/34), P < 0.01; the positive rate of bcl-2' product in the relapse group(50%, 6/12) was significantly lower than that in the non-relapse group(64.71%, 22/34), P < 0.05; the positive rate of c-myc' product in the stage IV (88.89%, 8/9) was significantly higher than that in the stage II (63.64%, 7/11), P < 0.05; the positive rate of bcl-2' product in the stage II (72.73%, 8/11) was significantly higher than that in the stage IV (55.56%, 5/9), P < 0.05; the positive rate of bcl-2' product was in the negative correlation to the pathological grade of primary carcinoma. CONCLUSION: The oncogene c-myc and bcl-2 take part in the genesis and advancing course of maxillary sinus cancer. Their products were the important oncogology indexes, which might be useful to determine illness condition and predict relapse.

[The application of mastoid cavity plombage in open method tympanoplasty].

OBJECTIVE: The role mastoid cavity plombage played in restoring physiological function of external meatus in open method tympanoplasty was studied. METHOD: 85 cases of cholesteatoma otitis media were performed open method tympanoplasty with the broken bits of their own mastoid process cortex, homological costal cartilage or U shape periosteum connective tissue valve. And plombage was performed at the same time. The reconstruction of ossicular chain (type 0 tympanoplasty): 10 ears did not undergo reconstruction due to increase ment of bone conduction threshold or were in preparation for the second phase operation; 40 ears underwent type III tympanoplasty; 35 ears underwent type IV tympanoplasty. RESULT: The total improvement rate of hearing was 76%, among which 80% were performed type III tympanoplasty and 71.4% underwent type IV tympanoplasty. After operation, the time waited till dry-ears was 19.8 +/- 65.31 days and the dry-ears rate was 97.6% (83/85). Six months after operation, 89.4% (76/85) had smooth external meatus and fine self-cleaning function and no recurrence was found. CONCLUSION: As to cholesteatoma otitis media that could not be performed closed method tympanoplasty, U shape periosteum connective tissue valve and mastoid process cortex were used. Mastoid cavity plombage with homological costal cartilage could effectively overcome the shortcomings of destruction of physiological structure of external meatus caused by open method tympanoplasty, the self-cleaning function of skins of external meatus was remained and the life quality of patients after operation was increased.

[Introduction of Vitreoscilla hemoglobin gene in a recombinant E. coli for PHB production].

Vitreoscilla hemoglobin gene (vgb) was introduced into the recombinant Escherichia coli VG1(pTU14) for production of poly-beta-hydroxybutyrate (PHB), in order to enhance the oxygen uptake of strain from molecular level and to solve the problem of oxygen limitation in process of fermentation. The carbon monoxide difference spectra analysis of Vitreoscilla hemoglobin (VHb) showed that vgb could be successfully expressed in VG1 (pTU14), and this expression was under the regulation of dissolved oxygen (DO). The higher amounts of VHb could be got correspond to the lower level of DO. Because of the introduction of vgb, cell growth and PHB accumulation of VG1 (pTU14) were all strongly promoted during the process of batch culture.

[Purification and some properties of D-hydantoinase produced by Pseudomonas 2262].

A D-hydantoinase produced by Pseudomonas 2262 was purified to electrophoretic homogeneity by the steps of thermal treatment, (NH4)2SO4 fractionation and column chromatography with Q-Sepharose fast flow, phenyl-Sepharose fast flow and Superose 12. Purification of about 60 fold was achieved with an overall yield of 16%. The relative molecular mass of the native enzyme is 109 kD and that of subunit is 53.7 kD by the analysis of Native and SDS-PAGE as well as gel filtration respectively. Some properties of the enzyme such as the sensitivity to thiol reagent and the effects of metal ions, for instance inhibited by Zn2+ and activited by Mn2+, Mg2+ are identical to dihydropyrimidinase. The optimum temperature and pH for enzymatic catalysis are 70 degrees C and 8.0 respectively. The enzyme activity is stable under 60 degrees C and in the pH range of 6-10. The N-terminal sequence for 10 amino acid residues is MDKLIKNGTI.

[Determination of T lymphocytes and trace elements in spleen from rats infected with Toxoplasma gondii].

OBJECTIVE: To determine the level of five trace elements(Fe2+, Cu2+, Zn2+, Ca2+, Mg2+) in the spleen and changes of T lymphocyte and its subtype variations in peripheral blood from the rats infected with Toxoplasma gondii. METHODS: Twenty rats were randomly and equally divided into two groups: control group and experiment group. Each rat in the experiment group received an i.p. injection of 2 ml normal saline containing 1.5 x 10(6) tachyzoites of T. gondii. On the 64th day after injection of T. gondii, the changes in T lymphocytes (TL) and their subgroups, the helper T lymphocytes (Th) and the suppressor T lymphocytes(Ts) in the peripheral blood of the rats with T. gondii were determined by the assay of the lymphocytes labeled with intercellular acid alpha-naphthyl acetate esterase. All the rats were killed and the atomic absorption method were used for detecting the level of trace elements in the spleen tissue. RESULTS: The number of TL and Th in experiment group was significantly lower than that of control (P < 0.01, P < 0.01). The ratio of Th/Ts showed a significant difference between the two groups. The level of Fe2+, Cu2+ in experiment group was significantly reduced (P < 0.01). The amount of Mg2+ in infected rats was higher than that of the control(P < 0.01). No statistical difference in the content of Zn2+, Ca2+ was found between the two groups. CONCLUSION: T. gondii infection might cause the changes in the TL and Th in peripheral blood and the changes in trace elements in spleen of the rats.

[Randomized controlled trial for the effect of amrinone and aprotinin on proinflammatory cytokine release in patients with prosthetic valve replacement during perioperative period].

OBJECTIVE: To explore the effect of amrinone and aprotinin on whole-body inflammatory response in the patients with prosthetic valve replacement during perioperative period. METHODS: 24 patients undergoing prosthetic valve replacement were randomized to control group (group A, n = 8), aprotinin group (group B, n = 8) and amrinone combined with aprotinin group (group C, n = 8). In the aprotinin group, 3 x 10(6) of aprotinin was added to the priming solution of the extracorporeal circulation (ECC). In the amrinone combined with aprotinin group 3 x 10(6) of aprotinin was added to the priming solution of the ECC and amrinone began with a bolus of 1 mg/kg followed by a maintenance infusion of 8 micrograms/(kg.min). The control group received an equivalent prime volume without aprotinin. Venous blood samples were drawn before the operation, at the end of ECC, 1 hour after the end of ECC, and one day after the operation respectively. Enzyme-linked immunosorbent assay techniques were used to measure each of the cytokines. RESULTS: Before ECC, there were no differences of the levels of IL-6 and IL-8 among groups (P > 0.05). After ECC, the levels of IL-6 and IL-8 increased significantly in all groups (P < 0.05). The levels on day one after the operation were still higher than those before the operation in all groups (except the level of IL-8 in group C), but no statistical significance was observed. (P > 0.05). At 1 hour after the end of ECC, the level of IL-6 in group B was lower than that in group A, and the level of IL-6 in group C was lower than that in group B, but there was no statistically significant difference (P > 0.05); At the end of ECC, the level of IL-8 in group B was lower than that in group A and the level of IL-8 in group C was lower than that in group B, but no significant difference was noted (P > 0.05). It was also observed that the level of IL-8 was lower in group C than group A or B at 1 hour after the end of ECC. CONCLUSION: Although amrinone and aprotinin have antiinflammatory activity, but pump prime only aprotinin or aprotinin combined with amrinone may fall in preventing proinflammatory cytokine release (IL-6, IL-8) completely in patients with prosthetic valve replacement during ECC perioperative period.
