Leukapheresis reduces early mortality in patients with acute myeloid leukemia with high white cell counts but does not improve long- term survival.

PURPOSE: Current published data on therapeutic leukapheresis in hyperleucocytic AML does not define the impact on survival from this procedure. Between 1992 and 1999 we saw 146 patients with newly-diagnosed AML (APL excluded) and an initial WBC count > 50 x 10(9)/L of whom 71 underwent leukapheresis at the discretion of their treating doctors. We compared outcome (early mortality, CR, and overall survival) rates in the patients who were and were not pheresed. After accounting for covariates relevant to these outcomes, including age, performance status, and cytogenetics, there was evidence (p = .006) that pheresis reduced 2-week mortality rate and a suggestion (p = .06) that this resulted in a higher CR rate. However there was no evidence that pheresis lengthened longer-term or overall survival; if anything the suggestion was the converse (p = .06). These data may reflect the fact that the patients chosen to have pheresis were prognostically unfavorable as defined by variables that were not captured in our data set, since the alternative explanation i.e. that pheresis per se shortens overall survival seems less likely. Whether the above justifies the use of pheresis in the absence of evidence from a randomized trial is doubtful, but it seems likely that any long-term benefit to be derived from this procedure must await further advances in anti-leukemia therapy.

Isolation and characterization of a zebrafish homologue of the cone rod homeobox gene.

PURPOSE: To isolate and characterize a zebrafish CRX: homologue. Mammalian CRX: genes are expressed specifically in photoreceptors and pinealocytes, regulate photoreceptor gene expression, are necessary for normal photoreceptor differentiation, and when mutated cause a variety of photoreceptor degenerations. METHODS: A zebrafish retinal cDNA library was screened with a human CRX cDNA probe. Radiation hybrid mapping, Northern blot analysis, in situ hybridization, and transient transfection studies were performed using standard methods. RESULTS: Based on amino acid sequence comparisons, zebrafish crx shows 50% identity with human CRX, and 85% identity in the homeodomain. A phylogenetic analysis indicates that zebrafish crx is most closely related to the mammalian Crx proteins, and more distantly related to the Otx proteins. Zebrafish crx maps between 49.6 and 54.5 cm from the top of linkage group LG05C, a map position consistent with the location of the mouse and human CRX genes. Northern blot analysis and in situ hybridization indicate that zebrafish crx is expressed in the retina and pineal gland. In adult zebrafish, crx is expressed by both rods and cones in the outer nuclear layer, and in cells in the outer zone of the inner nuclear layer, in the region occupied by bipolar cells. Similar to mammalian Crx, zebrafish crx interacts with neural retinal leucine zipper (Nrl) to activate, although weakly, rhodopsin promoter activity. CONCLUSIONS: Based on molecular phylogeny, chromosomal location, expression pattern, and ability to activate rhodopsin promoter activity in transient transfection assays, zebrafish crx appears to be an orthologue and functional homologue of mammalian CRX:

[Development and assessment of real cardiac motion simulation phantom].

To evaluate cardiac images obtained by multi-slice CT, assessments of the temporal resolution and reconstruction algorithm are essential. For these purposes, we have designed and developed a cardiac motion phantom. Using suitable driver sequences, the phantom allowed for modeling of real left ventricular motion, which was characterized by fast pumping, slow pumping, fast filling and slow filling. Our phantom seems to be useful for use in the fundamental assessments and researches of cardiac CT.

Sympathetic skin response: a new test to diagnose erectile dysfunction.

AIM: Electrophysiological monitoring of the activity of the penile sympathetic skin responses (PSSR) in healthy men and patients with erectile dysfunction (ED). METHODS: PSSR were recorded from the skin of penis with disk electrodes at the time of electric stimulation of left median nerves. RESULTS: PSSR were recorded from all the healthy men and almost all the patients. In healthy men the latency of P0, the latency of N1, the duration of N1 and the amplitude of N1 were 1,249 +/- 111 ms, 2,239 +/- 286 ms, 1,832 +/- 505 ms and 470 microV (median), respectively. In ED patients the latency of P0, the latency of N1, the duration of N1 and the amplitude of N1 were 1,467 +/- 183 ms (P < 0.01), 2,561 +/- 453 ms (P < 0.05), 2,560 +/- 861 ms ( P < 0. 01) and 91 microV ( P < 0.01), respectively. The normal latency of P0 was less than 1,471 ms. The normal amplitude of N1 was more than 235 microV. According to this normal value, of 20 patients 11 showed longer latency of P0, and 14 showed lower amplitude of N1 as compared with those of normal subjects. CONCLUSION: PSSR can be used as an electrophysiological method in assisting the diagnosis of ED.

[Analysis of toxic organic compounds in MGP-Wastewater and investigation of Wastewater's general characteristics].

A full-scale investigation and analysis of MGP(Manufacture Gas from Petroleum)-Wastewater was carried out in this study. It's the first time that this paper publicizes the wastewater's general data, pretreatment method of GC/MS analysis and compositions of toxic organic compounds. The concentrtion of wastewater's CODCg, BOD5, phenolic, N-NH4+ and extractive-organic is: 400 mg.L(-1)-600 mg.L-1, 60 mg.L(-1)-80 mg.L-1, 20 mg.L(-1)-30 mg.L-1, 60 mg.L(-1)-80 mg.L-1 and 170 mg.L(-1)-200 mg.L-1 respectively. There are 79 kinds of aromatic compounds were detected by GC/MS, the total concentration of influent and effluent is 140.79 mg.L-1 and 129.11 mg.L-1 respectively. The data approved that MGP-wastewater is toxic and difficult for bio-treatment.

[Detection of monosomy 7 or 7q- in cases of myelodysplastic syndrome].

OBJECTIVE: To investigate the value of interphase fluorescence in situ hybridization(FISH) in the detection of monosomy 7 (-7)or deletion of long arm of chromosome 7(7q-) in the cases of myelodysplastic syndrome (MDS). METHODS: Forty-six cases of MDS and 10 normal controls were studied simultaneously by conventional karyotype analysis and interphase FISH technique using SpectrumRed directly labeled DNA specific probe for 7q32. Two hundred interphase cells were analyzed for each case and the cells with one red hybridization spot<7% were regarded as positive. RESULTS: Three cases displayed -7/7q- by conventional cytogenetics(CC) and were confirmed by interphase FISH. Six cases in 43 cases who did not show -7/7q- by CC displayed -7/7q- by interphase FISH. CONCLUSION: Interphase FISH is very useful for the detection of -7 or 7q- in MDS and it is more sensitive than CC.

The vascular biology of the glycoprotein Ib-IX-V complex.

It has long been recognized that binding of von Willebrand factor (vWf) by the platelet membrane glycoprotein (GP) Ib-IX-V complex initiates the cascade of events leading to thrombosis and haemostasis. In the last several years, however, it has become apparent that the GP Ib-IX-V complex plays a much broader role in vascular biology, a recognition in part due to the identification of novel adhesive ligands for GP Ib-IX-V such as P-selectin and Mac-1. Binding of vWf by the GP Ib-IX-V complex is also important in formation of the developing thrombus. Platelets can utilize the GP Ib-IX-V complex and PSGL-1 to roll on activated endothelium via surface-expressed endothelial P-selectin. The GP Ib-IX-V complex can bind the I domain of Mac-1, a binding interaction potentially relevant to transmigration of macrophages through mural thrombus, a process required for vessel remodeling post angioplasty. This review will focus on recent advances in our understanding of the structure and function of this important platelet receptor, with particular emphasis on insights made within the last two years.

[Studies on inhibiting activities of five antitumour drugs to human cancer cell in vitro with MTT assay].

The inhibiting activities of five antitumour drugs including "An ke su" to human cancer cell lines coming from 19 patients were measured in vitro with MTT assay, which can provide proper antitumor agents for clinical chemotherapy. The evaluation rate of medicine sensitivity is 89.47%, which is same as the result of clinical therapy. This method indicates the response of tumour cells to "An Ke Su" and other agents. Being simple, sensitive and rapid, it proves to be valuable in clinical therapy.

Basaloid squamous cell carcinoma of the esophagus: an immunohistochemical study of 8 cases.

OBJECTIVE: To explore the biological features of basaloid squamous cell carcinoma (BSC) of the esophagus. METHODS: Cytokeratins (CK4, CK18 and CK19), epithelial membrane antigen (EMA), carcino embryo antigen (CEA), alpha-smooth muscle antigen (alpha-SMA), S-100, laminin (LN), collagen IV (Col-IV), neural-specific enolase (NSE), proliferating cell nuclear antigen (PCNA) and p53 antibodies were used to detect the corresponding antigen expression in 8 cases of BSC with ABC immunohistochemical methods. RESULTS: Two kinds of BSC cell components have different responses to the above antibodies. For basaloid cells (BCs), 7 cases were positive for CK19, and were negative for the other 4 epithelial antibodies CK4, CK18, CEA and EMA. BCs of 4 cases were positive to the muscular antibodies alpha-SMA and S-100, and the hyaline degeneration in the tumor nests was positive for LN and Col-IV. BCs had a high index of PCNA, with an average level of 54%. For squamous cells (SCs), 7 cases were positive for the epithelial antigen CK4, CEA and EMA, but were negative for CK19, alpha-SMA and S-100. The index of PCNA of SC was low, with an average level of 25%. CONCLUSION: BSC of the esophagus is a high-malignancy tumor which is of multi-oriented differentiation. BCs represent basal cells which have the tendency of myoepithelial differentiation and have strong proliferation ability, whereas SCs represent typical squamous cell differentiation.

[Detection of -5/5q- chromosome abnormality in myelodysplastic syndromes by interphase fluorescence in situ hybridization].

OBJECTIVE: To evaluate the value of interphase fluorescence in situ hybridization(FISH) in the detection of acquired complete and partial deletion of chromosome 5(-5/5q-) in myelodysplastic syndromes (MDS). METHODS: Marrow cells from 48 MDS patients and 10 normal controls were studied by interphase FISH using SpectrumRed directly labelled DNA specific probe for 5q31, 200 to 300 cells were scored for each case and cells with a red hybridization signal > 6% were defined as -5/5q- positive. RESULTS: In 48 MDS patients, 13 were positive for interphase FISH, of whom, 7 were positive and 6 were negative for conventional cytogenetics (CC). The difference in the percentage of positive cells detected by the two methods was statistically significant (P < 0.05). CONCLUSION: Interphase FISH is more sensitive than CC for the detection of -5/5q- in MDS.

Arsenic trioxide induces multiple myeloma cell apoptosis via disruption of mitochondrial transmembrane potentials and activation of caspase-3.

OBJECTIVE: To investigate the response of multiple myeloma (MM) cells to arsenic trioxide (As2O3) and their possible mechanisms. METHODS: Two MM-derived cell lines RPMI8226 and U266 cells were used as in vitro models. Cell apoptosis was assessed by morphology, flow cytometry, and DNA gel electrophoresis. Mitochondrial transmembrane potentials (delta psi m) were evaluated by measuring cellular Rhodamine 123 staining intensity. Protein expression was analyzed using Western blot. RESULTS: Zero point one to 0.5 mumol/L As2O3 inhibited cell proliferation and 2.0 mumol/L As2O3 induced cell apoptosis, while 1.0 mumol/L As2O3 inhibited proliferation with a weak degree of apoptosis induction in RPMI8226 and U266 cell lines. As2O3-induced apoptosis was accompanied by mitochondrial transmembrane potentials (delta psi m) collapse and caspase-3 activation in the presence of intact membrane. Glutathione depleter buthionine sulfoximine enhanced, while disulfide bond-reducing agent dithiothreitol partially antagonized As2O3-induced delta psi m collapse and apoptosis in MM cells. All-trans retinoic acid (ATRA) could also induce apoptosis in RPMI8226 cells, but it did not show any cooperative effects with As2O3. CONCLUSION: As2O3 exerts apoptosis-inducing and growth-inhibiting effects on MM cells, and mitochondrium is a pivotal and common target of As2O3 for apoptosis induction.

Optimal timing of coronary stenting in unstable angina patients.

OBJECTIVE: To assess the efficacy and safety of intracoronary stenting in the acute phase of unstable angina pectoris (UAP). METHODS: Fifty-five patients with UAP were randomized to early (Group I, n = 29) and delayed interventional treatment (Group II, n = 26). Coronary angiography and stenting were performed within 48 hours in Group I and 7-10 days later in Group II. Procedural success rate, time interval from admission to angina relief and duration of hospitalization were recorded. Cardiac events within 30 days were observed as well. RESULTS: Clinical characteristics and angiographic features were similar between the two groups. There was no significant difference in the procedural success rate (93% versus 96%), but the cardiac event rate within 30 days was significantly lower in Group I than in Group II (0% versus 9.2%, P < 0.05). The time interval from admission to angina relief (4.4 +/- 3.1 days versus 5.7 +/- 2.9 days) and the duration of hospitalization (8.8 +/- 3.2 days versus 13.5 +/- 3.1 days) were significantly reduced in Group I (both P < 0.05). CONCLUSIONS: Intracoronary stent implantation is effective and safe in the acute phase of UAP. Early percutaneous coronary intervention results in rapid improvement in symptomatology and a shorter hospitalization. Its long-term effect has to be confirmed in a future randomized study.

Confirmation of susceptibility gene loci on chromosome 1 in northern China Han families with type 2 diabetes.

OBJECTIVE: To confirm previous effort to identify type 2 diabetes susceptibility genes in a Northern Chinese population by conducting a new genome scan with both an increased number of type 2 diabetes families and a new set of microsatellite markers within the previously localized regions. METHODS: A genome scan method was applied. After multiplexed PCR, electrophoreses, genescan and genotyping analysis, we obtained size information for all loci, and then a further study was done by both parametric and non-parametric linkage analysis to investigate the P values and Z values of these loci. RESULTS: We surveyed 34 microsatellite markers which distributed within 5 regions along chromosome 1, and a total of 12,000 genotypes were screened. Evidence of linkage with diabetes was identified for 8 of the 34 loci. All P values of the 8 loci were lower than 0.05, and the highest Z value was 2.17. A very interesting finding is that all 5 markers at the p- terminal 1p36.3-1p36.23 region, spanning a long range of 16.9 cM, were identified to have a low P value of less than 0.05, which suggests that this region may contain multiple susceptibility genes. Regions 4 and 5 also confirmed the previous findings, and we narrowed these two regions to a 2.7 cM and 2.5 cM regions, respectively. CONCLUSIONS: We further confirmed the results gained in the previous genome-wide scan using an increased number of NIDDM families and a new set of microsatellite markers lying within the initially localized regions. The fact that all 5 loci at the p- terminal region displayed a low P value of less than 0.05 suggests that more than 1 susceptibility gene may reside in this region.

Heart rate variability and its response to thyroxine replacement therapy in patients with hypothyroidism.

OBJECTIVE: To analyze heart rate variability (HRV) and its changes with thyroxine treatment in patients with hypothyroidism. METHODS: HRV was analyzed using 24-hour electrocardiographic recording in 38 patients with hypothyroidism and 21 normal controls. The changes in HRV were evaluated for the 18 hypothyroid patients after 3 months of thyroxine therapy. RESULTS: The time domain measurements of HRV in hypothyroid patients were much lower than those in the control group. As to HRV frequency domain, the high frequency power was significantly higher, but the ratio of low frequency power to frequency power for hypothyroid patients was lower than in the controls. These abnormal changes of HRV measurements in hypothyroid patients were improved after treatment with thyroxine and were associated with recovery of serum concentrations of FT3 and FT4. CONCLUSIONS: Patients with hypothyroidism often have autonomic neuropathies with a higher level of vagal tone. These abnormalities could be partly improved by thyoxine therapy.

Adeno-associated virus vector-mediated triple gene transfer of dopamine synthetic enzymes.

OBJECTIVE: To explore triple gene transfer of dopamine synthetic enzymes with separate adeno-associated virus (AAV) vectors. METHODS: The genes for dopamine synthetic enzymes, tyrosine hydroxylase (TH), aromatic L-amino acid decarboxylase (AADC), and GTP cyclohydrolase I (GCH, an enzyme critical for tetrahydrobiopterin synthesis) were contransduced into 293 cells with separate AAV vectors. Expressions of TH, AADC and GCH were detected by Western blot analysis. Intracellular dopamine level was assayed by high-performance liquid chromatography. RESULTS: TH, AADC and GCH were effectively coexpressed in transduced cells with three separate AAV vectors, AAV-TH, AAV-AADC and AAV-GCH. Furthermore, the coexpression resulted in an effectively spontaneous dopamine production in cotransduced cells. CONCLUSION: The triple transduction of TH, AADC and GCH genes with separate AAV vectors is effective, which might be important to gene therapy for Parkinson's disease.

[The combination of cyclosporin A and androgen in the treatment of chronic aplastic anemia].

OBJECTIVE: To explore the therapeutic effectiveness of combination of cyclosporin A (CsA) and androgen in the treatment of chronic aplastic anemia (CAA). METHOD: Androgen alone or combined with CsA for the treatment of CAA was compared by a randomized controlled clinical trial. RESULT: The efficacy of androgen combined with CsA (87.9%) was higher than that of androgen alone (57.1%). Therapeutic effectiveness of the combination treatment between the patients with positive and negative peripheral blood mononuclear cells (PBMNCs) inhibiting normal colony formation unit-granulocyte and macrophage (CFU-GM) test showed a significant difference (P < 0.005). CONCLUSION: Androgen combined with CsA had a much better efficacy than that of androgen alone in the treatment of CAA. The patients' PBMNCs inhibiting normal CFU-GM growth test can be used as an index of the treatment outcome. The side effects of the combination therapy are low and tolerable.

[The optimal concentration of drinking water in eastern Guangdong].

OBJECTIVE: The objectives were to explore the optimal concentration of drinking water in Eastern Guangdong and to provide information for the health administrative department to set up the criteria for fluoride concentration. METHODS: Totally 1237 9-17 and 35-44 years old residents in Eastern Guangdong who drinking water containing 0.1-2.5 mg/L fluoride were recruited to investigate the occurrences of dental caries and dental fluorosis. Their background information and perceived appearances of teeth were also recorded. RESULTS: The major factor influencing dental fluorosis was fluoride concentration in drinking water. Dental caries decreased when the fluoride concentration increased but dental fluorosis also increased at the same time. It was showed by calculation that the Dental Fluorosis Index would be 0.4 and 0.6 when the fluoride concentration was 0.57 mg/L and 0.7 mg/L respectively. The acceptation to the severity of dental fluorosis among the study subjects was in accordance with the Dean's criteria. CONCLUSION: It suggests that the optimal fluoride concentration in Eastern Guangdong is 0.57 mg/L and should not exceed the upper limit of 0.7 mg/L.
