[Multiple organ failure following open heart surgery in adults].

The cases multiple organ failure (MOF) which occurred post-operatively following open heart surgery in adults were studied on a pre and post-operative hemodynamics. Post-operative low cardiac output syndrome (LOS) was thought to result in decreased renal and hepatic perfusion with subsequent ischemic damage to these organs. Therefore, to management MOF, we have to prevent post-operative LOS, for example, administration of cathecholamine and vasodilator, and early clinical application of IABP and ventricular assist device (VAD). In a post-operative hemodynamics, the cases of severe right ventricular failure tend to have more protracted MOF. It was though that MOF could be minimized with shortened cardiopulmonary bypass duration, and early application of assisted circulation.

[Combined operation for patients with ischemic heart disease and thoracic aortic aneurysm].

Myocardial infarction is one of the chief causes of operative mortality after surgical treatment of aortic aneurysm. For this reason, coronary bypass grafting combined with aneurysmectomy has been recommended by some surgeons. We advise routine preoperative cardiac catheterization in all patients with clinical indications of coronary artery disease who are scheduled for aneurysmectomy. For patients with operative indication of coronary bypass grafting, we would think that patients with unstable coronary artery disease and symptomatic thoracic aneurysm might have both lesions safely repaired as a combined operative procedure.

[Clinical experience for ventricular assist device].

We began applying of our VAD system clinically in 1982 and have subsequently obtained good results. The first case treated with the VAD returned to normal life. In a total of 70 cases of clinical applications this system there has been instance of apparatus dysfunction. Confirming the reliability and safety promised by the results of biological experiments. Here, we report the results of clinical application of the system in a total of 61 cases between August, 1985 and March, 1989. The following conclusions were reached: 1) Satisfactory weaning and survival rates were obtained i.e. 18 cases out of 61 cases (30%) survived over one month and 13 cases (21%) are currently surviving, even in elderly patients of 60 years of age or older. 2) The weaning rate and survival rates were good after approximately 1 week of support. Detailed evaluation of the recovery of the heart and functions of other major organs has led to the conclusion that the results can be improved further when the weaning period is carefully determined. 3) With regard to surgical factors, good findings were obtained when the artificial heart-lung bypass time was less than 6 hours. The VAD can be used in the early stages without vainly attempting weaning from the artificial heart-lung bypass. 4) There were no mechanical problems with the hardware of the blood pump drive assembly, etc., or noteworthy side effects.

[Investigation of cases of acute arterial thrombosis of the lower extremities].

Thirty-six cases of acute arterial thrombosis treated in our department were investigated. The lesion was limited to the legs. The basic disease were 33 cases of arteriosclerosis obliterans, 2 of thromboangiitis obliterans and one cases of popliteal sclerotic aneurysm. The limb salvaged rate was 65% and the mortality was 22%. According to the findings of the extremities, they were classified into 4 groups. As for the results, this classification was useful to determine the prognosis of the legs. According to the clinical experience of myonephropathic-metabolic syndrome in (MNMS) 7 cases, staged revascularization was considered as one of the preventive treatments to avoid MNMS. The thrombolytic continuous perfusion therapy was supportive measure for these cases. This procedure was useful and available for limb salvage.
