Absorption, distribution, metabolism and excretion of [carbonyl-14C]mosapride citrate after repeated oral administration in rats.

Absorption, distribution, metabolism and excretion of mosapride citrate ((+/-)-4-amino-5-chloro-2-ethoxy-N [[4-(4-fluorobenzyl)-2-morpholinyl]methyl]benzamide citrate, AS-4370, CAS 112885-42-4), a novel gastrokinetic agent, were studied with 14C-labeled compound in male rats during and after 21 consecutive daily oral administration at a dose of 10 mg/kg/d. After the first administration, plasma radioactivity concentrations were essentially equal to those in the single dose experiment, including the maximum concentration (Cmax; 1130 ng eq./ml) at 1 h. Plasma concentrations at 1 h after each administration were virtually constant in the range of 770-1350 ng eq./ml for 21 days. On the other hand, the plasma concentration at 24 h gradually increased for 6-7 days to about 120 ng eq./ml and became substantially constant, suggesting that apparent steady state was achieved by around 7 consecutive daily administration. Plasma concentration reached Cmax of 1230 ng eq./ml at 0.5 h after the 21st administration and decreased biphasically with half-lives of 3.4 h (t1/2 alpha) and 14.9 h (t1/2 beta). t1/2 alpha was virtually similar but t1/2 beta was about 2 times longer compared with single dose experiment. About 40% and 55% of dose radioactivity were excreted in urine and feces, respectively, for 21 days and radioactivity was almost completely excreted within 168 h after the last administration. The analysis by thin layer chromatography elucidated that composition of radioactive metabolites in plasma and urine after repeated administration was similar to that in the single dose study.(ABSTRACT TRUNCATED AT 250 WORDS)

Identification of the site of translational frameshifting required for production of the transposase encoded by insertion sequence IS 1.

Previous genetic analyses indicated that translational frameshifting in the--1 direction occurs within the run of six adenines in the sequence 5'-TTAAAAAACTC-3' at nucleotide positions 305-315 in IS 1, where the two out-of-phase reading frames insA and B'-insB overlap, to produce transposase with a polypeptide segment Leu-Lys-Lys-Leu at residues 84-87. IS 1 mutants with a 1 bp insertion, which encode mutant transposases with an amino acid substitution within the polypeptide segment at residues 84-87, did not efficiently mediate cointegration, except for an IS 1 mutant which encodes a mutant transposase with a Leu-Arg-Lys-Leu segment instead of Leu-Lys-Lys-Leu. An IS 1 mutant with the DNA segment 5'-CTTAAAAACTC-3' at positions 305-315 carrying the termination codon TAA in the B'-insB reading frame could still mediate cointegration, indicating that codon AAA for Lys corresponding to second, third and fourth positions in the run of adenines is the site of frameshifting. The beta-galactosidase activity specified by several IS 1-lacZ fusion plasmids, in which B'-insB is in-frame with lacZ, showed that the region 292-377 is sufficient for frameshifting. The protein produced by frameshifting from the IS 1-lacZ plasmid in fact contained the polypeptide segment Leu-Lys-Lys-Leu encoded by the DNA segment 5'-TTAAAAAACTC-3', indicating that--1 frameshifting does occur within the run of adenines.

DNA sequences required for translational frameshifting in production of the transposase encoded by IS1.

The transposase encoded by insertion sequence IS1 is produced from two out-of-phase reading frames (insA and B'-insB) by translational frameshifting, which occurs within a run of six adenines in the -1 direction. To determine the sequence essential for frameshifting, substitution mutations were introduced within the region containing the run of adenines and were examined for their effects on frameshifting. Substitutions at each of three (2nd, 3rd and 4th) adenine residues in the run, which are recognized by tRNA(Lys) reading insA, caused serious defects in frameshifting, showing that the three adenine residues are essential for frameshifting. The effects of substitution mutations introduced in the region flanking the run of adenines and in the secondary structures located downstream were, however, small, indicating that such a region and structures are not essential for frameshifting. Deletion of a region containing the termination codon of insA caused a decrease in beta-galactosidase activity specified by the lacZ fusion plasmid in frame with B'-insB. Exchange of the wild-type termination codon of insA for a different one or introduction of an additional termination codon in the region upstream of the native termination codon caused an increase in beta-galactosidase activity, indicating that the termination codon in insA affects the efficiency of frameshifting.

Transient neonatal distal renal tubular acidosis with secondary hyperparathyroidism.

We describe a neonate with distal renal tubular acidosis with secondary hyperparathyroidism manifesting as hyperchloraemia, hypercalcaemia, elevated serum parathyroid hormone (PTH) and life-threatening metabolic acidosis. He exhibited general weakness, tachypnoea, dry skin and weight loss. Urinary excretion of titratable acid and ammonium was decreased. Daily alkali (2.5 mEq/kg body weight) was required to maintain a normal plasma bicarbonate (HCO3-). With alkali therapy, the fractional excretion of HCO3- was below 5%. Serum calcium and PTH were restored to normal promptly on initiation of alkali therapy. After 5 months of alkali therapy, normal growth and urine acidifying ability were restored and alkali therapy was discontinued. The acidification defect in this patient was transient. We consider this patient to be consistent with Lightwood's syndrome of "transient infantile renal tubular acidosis".

Kinesin family in murine central nervous system.

In neuronal axons, various kinds of membranous components are transported along microtubules bidirectionally. However, only two kinds of mechanochemical motor proteins, kinesin and brain dynein, had been identified as transporters of membranous organelles in mammalian neurons. Recently, a series of genes that encode proteins closely related to kinesin heavy chain were identified in several organisms including Schizosaccharomyces pombe, Aspergillus niddulans, Saccharomyces cerevisiae, Caenorhabditus elegans, and Drosophila. Most of these members of the kinesin family are implicated in mechanisms of mitosis or meiosis. To address the mechanism of intracellular organelle transport at a molecular level, we have cloned and characterized five different members (KIF1-5), that encode the microtubule-associated motor domain homologous to kinesin heavy chain, in murine brain tissue. Homology analysis of amino acid sequence indicated that KIF1 and KIF5 are murine counterparts of unc104 and kinesin heavy chain, respectively, while KIF2, KIF3, and KIF4 are as yet unidentified new species. Complete amino acid sequence of KIF3 revealed that KIF3 consists of NH2-terminal motor domain, central alpha-helical rod domain, and COOH-terminal globular domain. Complete amino acid sequence of KIF2 revealed that KIF2 consists of NH2-terminal globular domain, central motor domain, and COOH-terminal alpha-helical rod domain. This is the first identification of the kinesin-related protein which has its motor domain at the central part in its primary structure. Northern blot analysis revealed that KIF1, KIF3, and KIF5 are expressed almost exclusively in murine brain, whereas KIF2 and KIF4 are expressed in brain as well as in other tissues. All these members of the kinesin family are expressed in the same type of neurons, and thus each one of them may transport its specific organelle in the murine central nervous system.

Effect of fenbufen on the pharmacokinetics of sparfloxacin in rats.

The effects of fenbufen on the serum concentrations and penetration into the brain and CSF of sparfloxacin (AT-4140), a new quinolone antibacterial agent, were investigated in rats. At designated times after a bolus iv dose of sparfloxacin 10 mg/kg with or without fenbufen 20 mg/kg, arterial blood, CSF and whole brain were simultaneously collected from each rat. Sparfloxacin concentrations were assayed by HPLC. Serum concentration of sparfloxacin declined bi-exponentially with time and was not changed by coadministered fenbufen. Binding sparfloxacin to serum protein slightly decreased after the coadministration. No elevation of sparfloxacin concentrations was observed in either brain or CSF after coadministration with fenbufen except for only a few time-points. The pharmacokinetic analysis based on the physiological model indicated that fenbufen did not affect the permeability across the blood-brain or blood-CSF barrier. These results suggest that fenbufen may be unlikely to affect the pharmacokinetics, involving the entry into the central nervous system, of sparfloxacin.

A new series of natural antifungals that inhibit P450 lanosterol C-14 demethylase. II. Mode of action.

From a Penicillium sp. we identified a new series of antifungals having a tetrahydropyran skeleton with an alkenyl side chain. We elucidated the mode of action of Ro 09-1470, the most active compound of the series. Treatment of Candida albicans with the compound caused an accumulation of C-14 methyl intermediates of ergosterol at concentrations of which no significant interference with the biosyntheses of other macromolecules and respiration was observed. P450 lanosterol C-14 demethylase (P450(14DM)) activity was inhibited and furthermore, the binding of Ro 09-1470 to the heme of the enzyme was demonstrated by a difference spectrum. We conclude that Ro 09-1470 is the first natural antifungal that inhibits the P450(14DM) of fungi.

[Study on local recurrence in patients undergoing curative surgical resection of lung cancer].

Clinical features of recurrence in patients of lung cancer undergoing curative surgical resection were examined with special reference to the local recurrence. Subjects were 308 patients, consisting of 160 adenocarcinomas, 121 squamous cell carcinomas, 15 large cell carcinomas, 12 small cell carcinomas. They underwent curative resection in our department between 1973 and 1987. Local recurrence developed in 54 patients (18%). There was no significant difference in the incidence of local recurrence among the four histological types of carcinoma. According to the pathological stage, the incidence was 9% in stage I, 15% in stage II, and 35% in stages IIIA+IIIB. The local recurrence was subdivided into the following patterns: 1) lymphatic metastases to the hilar, mediastinal, or supraclavicular sites, 2) recurrence at the surgical margin, 3) malignant pleuritis or pericarditis, 4) so-called "endobronchial metastasis". The incidence of recurrence according to the patterns was 15%, 2%, 2% and 1%, respectively. The incidence of recurrence due to lymphatic metastases was correlated significantly with the pN factor but not with the pT factor. Patients with central type lung cancer showed a significantly higher incidence of lymphatic recurrence than patients with the peripheral type. Patients having postoperative radiotherapy to prevent local recurrence showed a lower incidence of lymphatic recurrence than patients having no radiotherapy. In conclusion, lymphatic recurrence was the most frequent pattern in local recurrence after curative resection of lung cancer, and therefore improvements in the operative procedure of lymphatic dissection, as well as in postoperative adjuvant therapy including radiotherapy will be urgently required for the purpose of reducing local recurrence.

[Investigation of glutathione-S-transferase isozymes in human lenses].

Glutathione-S-Transferase (GST) isozymes in human lenses were investigated by the immunoenzymatic method using antibody against human liver GST2 and GST3 after polyacrylamide gel isoelectric focusing (western blotting method). Also, rocket immunoelectrophoresis was carried out for the detection of GST1. It was found that GST1 in lens also showed a polymorphism. The GST2 was not found in human clear lenses as well cataractous lenses. In addition, crossreacting materials against both antisera of GST2 and 3 were detected in the pH 6-7 area of the gel in all human lenses.

[The advantages of laser assist in parenchyma-sparing pulmonary resection].

The advantages of laser irradiation were investigated retrospectively, using a total of 89 parenchyma-sparing pulmonary resection (PSPR) for localized lung lesions which included 42 conventional wedge resection (CWR), 8 PSPR using electrocoagulator (PE) and 39 PSPR using CO2 laser or Nd:YAG laser (PCYL). The major axes of the lesions in PNYL (25.4 +/- 10 mm) were significantly (p = 0.0387) longer than that in CWR (20.3 +/- 11.3 mm). The amount of bleeding during operation was similar in three groups. The mean value of the amount of postoperative exudation from the thoracic drains in PCYL (615 ml) was only 60 ml more than that in CWR. Bloody sputum was observed for 4.8 days in PCYL and 5.0 days in CWR, whereas 7.1 days in PE. Air leak was observed 33.3% in PCYL, 37.5% in PE, whereas 22.5% in CWR, that continued for 1.7 days (mean value) in PCYL, 1.6 days in PE and 2.0 days in CWR. Decrease (differences between preoperative and postoperative lung functions/volume of the lesions) in FVC and FEV1.0 in PCYL (mean values = 221, 158) and PE (174, 135) were less than that in CWR (339, 204). Laser assist in PSPR revealed advantages in hemostasis comparing with PE, and in parenchyma-sparing comparing with CWR. And no clear difference between PCYL and CWR in bleeding during operation, the amount of postoperative exudation and postoperative air leak. CO2 laser is reported that has higher potential in vaporization or cutting but lower ability in hemostasis comparing with Nd:YAG laser, so authors would express that Nd:YAG laser assist is most safe and effective in parenchyma-sparing lung resection.

Monoclonal antibodies immunoprecipitating omega-conotoxin-sensitive calcium channel molecules recognize two novel proteins localized in the nervous system.

Two monoclonal antibodies raised against brain synaptic membranes immunoprecipitated significant fractions of the brain omega-conotoxin receptor (probable omega-conotoxin-sensitive calcium channels) solubilized with digitonin. These antibodies recognized different proteins of 36 kDa and 28 kDa, respectively. Immunoblot analysis of fractions obtained by sucrose gradient centrifugation suggested that these two proteins were not subunits of the omega-conotoxin receptor but were bound to it. These proteins were found to be conserved at least from an amphibian to mammals, and to be present in the nervous system and adrenal medulla among the tissues examined.

In vitro and in vivo evaluation of Ro 09-1428, a new parenteral cephalosporin with high antipseudomonal activity.

Ro 09-1428, a new parenteral cephalosporin with a catechol moiety attached at position 7 of the cephalosporin ring, showed high in vitro activity against Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis, Proteus vulgaris, and Streptococcus pyogenes, with MICs for 90% of strains tested (MIC90s) of less than or equal to 0.39 micrograms/ml. Morganella morganii, Providencia rettgeri, Citrobacter freundii, Haemophilus influenzae, Staphylococcus aureus, and Streptococcus pneumoniae were inhibited with MIC90s of less than or equal to 3.13 micrograms/ml. Serratia marcescens was less susceptible to Ro 09-1428, with a MIC90 of 25 micrograms/ml. The most distinctive feature of Ro 09-1428 was its potent activity against Pseudomonas aeruginosa and Acinetobacter calcoaceticus, with MIC90s of 0.39 and 6.25 micrograms/ml, respectively. Most of the ceftazidime-resistant strains of P. aeruginosa, E. cloacae, and C. freundii were inhibited by Ro 09-1428, while those of S. marcescens were resistant at a concentration of 12.5 micrograms/ml. Ro 09-1428 was more active than ceftazidime against staphylococci. PBP 3 was the most sensitive target in E. coli and P. aeruginosa. The response to ferric iron in growth medium suggests that Ro 09-1428 may be taken up by transport mechanisms similar to those of other catechol cephalosporins. In accordance with its in vitro activity, Ro 09-1428 activity was equal to or greater than ceftazidime activity in efficacy against experimental septicemias in mice. The results indicate that Ro 09-1428 is a broad-spectrum cephalosporin with advantages over ceftazidime in its activity against P. aeruginosa, staphylococci, and ceftazidime-resistant strains of C. freundii and E. cloacae.

Synthesis of [carbonyl-14C]sparfloxacin.

A new antimicrobial quinolone, sparfloxacin (5-amino-1-cyclopropyl-7- (cis-3,5-dimethyl-1-piperazinyl)-6,8-difluoro-1,4-dihydro-4-oxoquinoline -3- carboxylic acid, AT-4140; CAS 110871-86-8), was labeled by 14C for studies of disposition and metabolism. Ethyl pentafluoro[carbonyl-14C]benzoylacetate (I) was reacted with ethyl orthoformate, cyclopropylamine and then potassium tert-butoxide to give a quinolone intermediate (IV). A benzylamino derivative (V) obtained by condensation of IV and benzylamine was subject to catalytic hydrogenolysis and hydrolyzed to give the carboxyl derivative (VII), which was condensed with cis-2,6-dimethylpiperazine to form [carbonyl-14C]sparfloxacin. The average yield of 3 preparations was 41.5% and specific activities were 310.8-366.3 MBq (8.4-9.9 mCi)/mmol. Both chemical and radiochemical purities were greater than 99%.

[Localization of glutathione-S-transferase in transparent and cataractous human lenses].

The localization of glutathione-S-transferase (GST) activity was investigated in 15 cataractous lenses obtained by intracapsular extraction of senile cataract. Additionally, 8 clear lenses, obtained from donor eyes in cases of corneal transplantation or of traumatic lens luxation, were used as controls. The lenses were divided into the central and peripheral portions with a trephine. The activity of GST was quantitated in each portion according to the method described by Habig. GST activity in clear lenses was significantly higher in the peripheral portion than in the central portion and in the former it was significantly higher in clear lenses than in cataractous ones. The ratio of GST activity between central and peripheral portions (C/P) was significantly higher in cataractous lenses than in clear ones. These findings suggested that the scavenging system of the human clear lens is more active in the equatorial portion than in the central portion. Nevertheless the activity of GST in human cataractous lenses decreased more significantly in the peripheral portion than in the central portion. These findings indicated that the human cataract usually develop from equatorial portion of lens.

[Results of noncurative resection of primary lung cancer].

Out of 1099 cases of primary lung cancer patients who underwent surgical treatment in our department from 1952 to 1988, 374 cases were divided as noncurative by the post surgical examination. We divided the causes of noncurative resection as follows; I) absolute noncurative resection 1) tumor rest in a) lymph nodes (Ly rest), b) chest wall or diaphragma (Ch + Dia rest), c) bronchial stump (Bro rest) 2) intra-pulmonary metastasis (pm), 3) dissemination or pleuritis carcinomatosa (Dis + pl) II) relative noncurative 1) incomplete dissection of lymph nodes (Ro) 2) group 2b lymph node metastasis (2b Ly). Ly rest was the commonnest (103 cases 27.5%) in noncurative resection and Ch + Dia rest (94 cases, 25.1%) and p.m. (54 cases 14.4%) followed thereafter. Two thirds of the noncurative cases were classified into these 3 groups, and the rest of the causes were Dis + pl, Bro rest. RO and 2b Ly groups. p.m. and Ly rest groups were the most common causes of noncurative resection in adenocarcinoma cases whereas Ly rest and Ch + Dia rest groups were common in squamous cell carcinoma cases. Five year survival rate of the total noncurative cases was 12.1% although those of Dis + pl groups was 22.9% and those of p.m. group was 18.8%. Five year survival rate of the other groups were around 10%. A slightly better survival rate was observed when relative noncurative resection was performed in RO (19.7%) and 2b Ly (18.7%) groups.

Direct radioimmunoassay for haloperidol in human serum.

A direct radioimmunoassay for the accurate determination of haloperidol in human serum has been developed. Based on recent information about the metabolism of haloperidol, a new haloperidol hapten, in which a (3-carboxypropionyl)methylamino group was attached as a bridge in the place of fluorine atom, was synthesized and coupled to bovine serum albumin through the bridge to provide a new immunogen. Guinea pigs were used for the immunization. Since the antisera obtained by the new immunogen still cross reacted greater than 10% with reduced haloperidol, the immunological tolerance to reduced haloperidol was induced by administration of a copolymer of D-glutamic acid and D-lysine linked with reduced haloperidol. This gave an antiserum in guinea pigs which was highly specific for unchanged haloperidol with negligible cross reactivity (less than or equal to 1.0%) to any haloperidol metabolites including the newly found ones. With the newly developed antiserum and [3H]haloperidol, serum haloperidol levels can be determined over the concentration range from 0.3 to 20 ng/mL, using 0.1 mL of human serum, without an extraction procedure.

Elevation of the placental glutathione S-transferase form (GST-pi) in tumor tissues and the levels in sera of patients with cancer.

The content of glutathione S-transferase placental form (GST-pi) in colon and esophageal cancer tissues was determined by single radial immunodiffusion or activity inhibition tests. Average levels in colon adenomas and carcinomas were 163 +/- 38 and 143 +/- 30 micrograms/g, respectively, about 6-fold higher than the value of 25 +/- 4 micrograms/g observed for normal colonic mucosa. The content in esophageal cancer was similarly increased at 240 +/- 160 micrograms/g, about 6-fold higher than the level found in normal mucosa, 42 +/- 21 micrograms/g. The content was significantly higher in highly differentiated carcinoma (331 +/- 138 micrograms/g) than in moderately (205 +/- 123) or poorly (125 +/- 61) differentiated carcinomas, suggesting that GST-pi content seems to be related to the degree of differentiation of esophageal cancer. GST-pi was also expressed in cell lines derived from various cancers, including IMR 32, TE-9, and Ca Ski cells. The results thus indicate that GST-pi may be a useful marker for a wide range of cancers. An enzyme-linked immunosorbent assay developed to determine serum GST-pi content is described. Using this method GST-pi could be accurately measured in the range between 0.7 and 150 ng/ml, without interference of other isoenzymes. The serum GST-pi content was 1.3 +/- 1.2 ng/ml in 35 healthy controls and values of over 3.7 ng/ml (control mean + 2 SD) were found in patients with cancers of the stomach (10 of 23 cases), esophagus (26/43), bile duct (3/9), and colon (3/9), and in some leukemic cases. Although elevation of the serum content was not so often as that of tissue content, the fact that higher serum values of patients with esophageal cancer often reverted to the normal range after surgical removal of the cancer suggested a direct derivation of serum GST-pi from tumor tissues. Thus, follow-up of elevated serum GST-pi levels may be useful for monitoring cancer patients during the course of treatment.
