[Experimental and clinical evaluation of latamoxef in newborn and premature infants].

Latamoxef (LMOX) was used in the treatment and prophylaxis of infections in neonates and immature infants. The following results were obtained. Mean serum concentrations (bioassay) 30 minutes after a single intravenous injection of about 20 mg/kg of LMOX were 49.9 mcg/ml in neonates and 47.3 mcg/ml in immature infants aged 0--3 days, 54.1 mcg/ml in neonates and 60.6 mcg/ml in immature infants aged 4--7 days, 48.9 mcg/ml in neonates and 46.7 mcg/ml in immature infants aged 8--28 days and 62.1 mcg/ml in immature infants aged over 29 days. Six-hour values were 24.1 mcg/ml, 22.5 mcg/ml, 15.9 mcg/ml, 27.2 mcg/ml, 12.9 mcg/ml, 19.1 mcg/ml and 12.8 mcg/ml, respectively. Mean serum concentration half-lives were 6.70 hours in neonates and 8.16 hours in immature infants aged 0--3 days, 3.68 hours in neonates and 5.83 hours in immature infants aged 4--7 days, 3.06 hours in neonates and 4.47 hours in immature infants aged 8--28 days and 2.59 hours in immature infants aged over 29 days. Adequate clinical efficacy can be expected by the intravenous injection of LMOX in doses of 20 mg/kg 1--2 times daily, in neonates and immature infants aged 0--3 days, 20 mg/kg 2--3 times daily, in neonates and immature infants aged 4--7 days and 20 mg/kg 3 times daily, in neonates and immature infants aged 8--28 days. The clinical efficacy of LMOX was good in 5 cases of sepsis (including suspected cases), 5 cases of urinary tract infection, 2 cases of respiratory tract infection and 6 cases of intrauterine infection (including suspected cases). Only a case of respiratory tract infections due to P. aeruginosa was thought to be ineffective. Bleeding tendency was noted in 3 cases, which results from secondary vitamin K deficiency should be checked carefully during the administration of LMOX.

Clinical experience of Al2O3 ceramics as a surgical implant.

The authors have treated 75 patients with various orthopaedic disorders including trauma, degenerative joint disease and tumours with ceramic surgical implants. A short account of the experience gained, together with some illustrative clinical cases is presented. Supplementary laboratory experiments have shown that Al2O3 ceramic material produces minimal changes of boundary reaction with the bone and the material produced no adverse effect upon organ cultures of developing chick femora and tibiae.

Ultrastructural localization of acid phosphatase activity in the large intestinal principal cells of postnatal rats.

A study was made of ultrastructural localization of acid phosphatase activity in the large intestinal mucosa of postnatal and adult rats. In rats 1 to 5 days old, phosphatase activity was observed on the microvillous membrane, the lateral cell membrane, the Golgi apparatus, the GERL of Novikoff (1963), the tubulo-vacuolar system, and on the supranuclear vacuole of the principal cells of the cecum and the ascending colon. In 30-day-old adult rats acid phosphatase activity, was noted on the GERL and the lysosomes of the principal cells in the cecum and the ascending colon. In all rats, young and adult, acid phosphatase activity was found only on the lysosomes and the GERL of the principal cells of the descending colon. Therefore, it was concluded that the acid phosphatase-containing tubulo-vacuolar system and the supranuclear vacuole in the surface principal cells of the proximal part of the large intestine of suckling rats may possess a strong phagolysosomal function as well as having an absorption capacity the same as ileal cells of suckling rats (Ono and Satoh 1981).

Abortifacient effect and uterine cervix-dilating action of 16, 16-dimethyl trans delta 2 PGE1 methyl ester (ONO 802) in the form of a vaginal suppository (a randomized, double-blind, controlled study in the second trimester of pregnancy).

A newly synthesized 16, 16-dimethyl trans delta 2 PGE1 methyl ester (ONO 802) was clinically applied in the form of a vaginal suppository for therapeutic abortion of second trimester pregnancies. Its effects were studied in a double-blind test comparing it with an inactive placebo suppository. The study was conducted at 12 Japanese university hospitals. The number of patients was 125 in total, i.e. 63 receiving ONO 802 vaginal suppository (containing 1.0 mg ONO 802) and 62 receiving inactive placebo suppository. ONO 802 was more effective than placebo with a success rate of 87% (complete (71%) and incomplete (16%) abortions). The onset of uterine contractions was observed in 154.3+/-18.1 min. and the onset of uterine bleeding in 323.6+/-41.0 min. The expulsion of the fetus and placenta was observed in 955.4+/-97.0 and 961.6+/-97.0 min., respectively. The cervix dilating effect of ONO 802 was observed in 63.5% of the patients at 3 hours after the start of administration. Nausea, vomiting, abdominal cramps, diarrhea and pyrexia were noticed. However, all these side effects were transient and mild, requiring no treatment. No abnormality was observed in the puerperal course, duration of uterine bleeding or onset of subsequent menstruation following the therapeutic abortion. Therefore, the present study demonstrated that ONO 802 vaginal suppository was an effective and valuable drug for therapeutic abortion of second trimester pregnancies.

Antitumor effect of human fibroblast interferon on the growth of human melanoma cells implanted in nude mice.

The antitumor effect of human fibroblast interferon (HuIFN-beta) on the growth of human melanoma cells (HMV-1) in vitro and in vivo was examined. In in vitro experiments, HMV-1 cells were highly sensitive to HuIFN-beta compared to other human cells. The cell growth in nude mice was also suppressed by daily administration of HuIFN-beta, depending on the dosage and administration route. The most prominent effect was obtained by intratumoral injection and a lesser therapeutic effect was achieved by subcutaneous injection around the inoculated tumor. Intraperitoneal injection of HuIFN-beta was unsuccessful. Pharmacokinetic studies of HuIFN-beta in nude mouse indicated that the three administration routes gave similar blood plasma levels and decline curves, suggesting that there is no correlation between the plasma HuIFN-beta level and the therapeutic efficacy. However, a relatively high interferon titer was detected for a long time after intratumoral injection of HuIFN-beta. Because of the species specificity of interferon and the use of the nude mouse system lacking T-cell function, the antitumor effect observed in the present study might reflect an anticellular activity of HuIFN-beta retained in the tumor region.

Autoxidation of oxymyoglobin. A nucleophilic displacement mechanism.

In the presence of a salt in an excess amount, the oxidation of native MbO2 was enhanced considerably above the normal autoxidation in buffr alone with the formation of the corresponding MetMb . anion complex. This anion-induced oxidation of MbO2 was measured for each salt at some 30 different values of pH in 0.1 M buffer at 25 degrees C. The anions examined were SCN-, F-, OCN-, N3- and CN-. The resulting pH dependence shows that the reaction involves two types of the displacement processes of O2- from MbO2 by the anion, i.e. those with and without proton assistance. The Brønsted plots for the rate constants versus the pKa values of the conjugate acids of anions indicate that the displacing oxidation of MbO2 proceeds by way of a nucleophilic attack of anions on the iron center and that both H2O and OH- can react with native MbO2 as the most common nucleophiles in vivo. These findings lead to a view that the proton-catalyzed nucleophilic displacement of O2- from MbO2 by an entering water molecule, or SN2 mechanism with proton assistance, is the basis for most of the autoxidation reaction under normal conditions.

Myeloscopy, with special reference to blood flow changes in the cauda equina during Lasègue's test.

Myeloscopy has been undertaken in a series of 208 patients without any serious complications. Observations have been made of changes in blood flow in the vessels accompanying the cauda equina during Lasègue's test and in some pathological states. Direct observation of the nucleus pulposus has been achieved and the possibility of the removal of herniated disc material by use of the myeloscope is suggested.

Ultrastructural localization of acid phosphatase activity in the small intestinal absorptive cells of postnatal rats.

The ultrastructural localization of acid phosphatase activity was investigated in ultrathin (0.05 micrometer) and semithin (0.5 micrometer) sections of the small intestinal epithelial cells of postnatal rats. Until around the 15th day of neonatal life acid phosphatase activity in the duodenal and jejunal epithelial cells was observed on the microvillous membrane, the membrane of the tubulo-vacuolar system, the lateral cell membrane, the lysosomes, the Golgi apparatus and the GERL of Novikoff (1963). After about the 15th neonatal day, the tubulo-vacuolar system enzyme disappeared form both cells. Acid phosphatase activity then became localized on the microvillous membrane, the lateral cell membrane, the lysosomes, the Golgi apparatus, and the GERL, as in adult rats. During the suckling period, acid phosphatase in the ileal cells could be seen on the microvillous membrane, the lateral cell membrane, the Golgi apparatus, the GERL, the membrane of tubulo-vacuolar system and the supranuclear vacuole. At weaning, however, the tubulo-vacuolar system and the supranuclear vacuole enzyme disappeared, and only the lysosomes and the GERL of these cells showed acid phosphatase activity, as in the adult rat. It was concluded that the acid-phosphatase-containing tubulo-vacuolar system and the supranuclear vacuole in the epithelial cells of the distal intestine of suckling rats may possess a strong phagolysosomal function as well as having an absorptive capacity.

[Studies of vesico-urethral function by urethral pressure profile during drug treatment (author's transl)].

The urethral closure pressure profile (UPP) examination was performed routinely together with the administration of alpha-blockers. By the measurement of UPP, we can understand whether the organic changes at the bladder neck have already occurred or not. Further, by calculating the decreasing rate of the pressure at the bladder neck (BNP) and the maximum urethral closure pressure (MP), we can determine which nerve component is affected. Particularly, when we want to detect the affected nerve component in a mild disorder of vesico-urethral function, the measurement of UPP is quite useful as one of the screening urodynamic tests.
