We investigated the presence of a platelet-activating factor-like substance in blister fluid obtained from patients with bullous pemphigoid. Human platelet aggregation activity was present in blister fluids obtained from 4 out of 6 patients with bullous pemphigoid and in blister fluids obtained from 3 cases with contact dermatitis. Platelet aggregation activity of pemphigoid blister fluids was inhibited by pretreatment with lyso-PAF, which is a precursor metabolite of platelet activating factor. The activity of blister fluids obtained from patients suffering from contact dermatitis was not inhibited by precursor. Blister fluid obtained from burn lesions and blisters of normal skin induced in the suction revealed no platelet aggregation activity. These results suggest that platelet-activating factor or a similar substance is present in the blister fluid obtained from suffered bullous pemphigoid.
Using N-isopropyl-(I-123)p-iodo-amphetamine (I-123 IMP) and gallium-67 citrate, it was possible to detect human malignant melanomas. I-123 IMP imaging was helpful in detecting melanomas occurring in the legs and arms, and the 4 hour image was better than the 1 hour image. Moreover, while gallium scintigraphy more clearly detects human malignant melanoma, I-123 IMP allows for the detection of human malignant melanoma within 1 hour after administration.
An estimation of the prevalence of epidermolysis bullosa was carried out in Japan. A questionnaire was sent by mail to the departments of dermatology and pediatrics in 644 main hospitals throughout Japan requesting confirmation of the number of epidermolysis bullosa cases experienced. The response rate was 63.0% or 406 hospitals. Based on the secondary questionnaire survey, we confirmed 393 cases of epidermolysis bullosa, and then divided them into the five following groups: "simplex" type 182 (48.2%), "junctional" type 23 (6.0%), "dominant dystrophic" type 72 (18.7%), "recessive dystrophic" type 109 (28.2%), unclassified 7 (1.8%). Using the respondent rate by geographic region and clinical department, the estimated prevalence of each type of epidermolysis bullosa was calculated as follows: "simplex" type 340-470 (0.29-0.40 per 100,000 population), "junctional" type 18-24 (0.015-0.020), "dominant dystrophic" type 130-180 (0.11-0.15) and "recessive dystrophic" type 180-250 (0.15-0.21). The total number of epidermolysis bullosa patients in Japan was estimated as being more than 670 in 1983.
The skin is the major site on anaphylaxis, and cutaneous mast cells have an important role in its reactions. The isolation and purification of rat cutaneous mast cells are described here. Rat abdominal skin was digested with collagenase and hyaluronidase, and centrifuged with Percoll. The buoyant density of cutaneous mast cells was high, and relatively pure mast cells were obtained. The purity of cutaneous mast cells was 74% +/- 2.4% before and 50.0% +/- 6.4% after Percoll density centrifugation; peritoneal mast cells revealed 5.8% +/- 1.3% purity before and 61.0% +/-10.6% purity after the same procedure. The isolated cutaneous cells released 21.3% +/- 3.8% histamine and the peritoneal mast cells released 55.5% +/- 3.8% histamine upon stimulation with 10 micrograms/ml compound 48/80. These findings suggest that there are functional subsets of connective tissue mast cells.
A survey of 150 cases with sporotrichosis seen at the Dermatological Clinic of Kurume University Hospital from February 1962 to October 1986 was reported. The proportion of cases with sporotrichosis to the total number of outpatients was 0.17%. Greater percentage of cases fell into the less than 10 years old or more than 40 years old groups. The male to female ratio was 1:1.46, and 38 cases occurred in farmers. Geographic distribution was remarkable, especially around the Chikugo and Yabe river. Sixty-four cases showed the cutaneous lymphangitic type and 85 cases the localized cutaneous type and one case atypical type. The face and upper extremities were the most affected. The sporotrichin test was positive in 117 of 131 cases. The causative organism was demonstrated in tissue sections in 69% of the cases.
Substance P is known to be a potent histamine liberator for mast cells. The influence of antianaphylactic agents, disodium cromoglycate (DSCG), ketotifen, and tranilast was studied on substance-P and compound 48/80-induced histamine release from rat peritoneal mast cells. Substance-P induced histamine release was inhibited by these agents, while compound 48/80-induced histamine release was not inhibited by tranilast. Our findings suggest that these antianaphylactic agents are assumed to be effective for cutaneous diseases which might be concerned with substance P and histamine.
Epidermolysis bullosa (EB) simplex is a congenital disease that has blister formation following minor mechanical trauma to the skin. However, the least amount of information concerning the pathogenesis is known in this condition. One possibility is that there are structural abnormalities in epidermal cell membranes. In the present study, we examined the binding of lectins to epidermal cell membranes by ABC method. Biopsy skin specimens were obtained from patients with simplex, junctional and dystrophic forms of EB, and normal volunteers. In the case of EB simplex, the cell membranes of keratinocytes in the basal and spinous cell layers did not bind to soybean agglutinin in specimens from blister edges or mechanical traumatized areas. No differences were found in binding pattern between the other forms of EB and normal controls. This results suggest the presence of structural abnormalities in epidermis of EB simplex.
An immunohistochemical technique for the detection of S-100 protein, neuron specific enolase (NSE), carcinoembryonic antigen (CEA) and muramidase (lysozyme) was applied to a case of the granular cell tumour. S-100 protein was detected both in the nuclei and cytoplasma of the granular cells, and NSE was weakly positive in their cytoplasms. CEA and lysozyme were negative in the tumour cells. Our results supports the concept that granular cell tumours are derived from Schwann cells.

The presence of neuron-specific enolase (NSE) and S-100 protein, which are nerve specific proteins, was immunohistochemically investigated on the cutaneous nerves. NSE was found in the axons of the cutaneous nerve bundles and the terminal axons in the Meissner and Pacinian corpuscles of the normal human and macaque skin. S-100 protein was found in the Schwann cells, lamellar cells of the Meissner corpuscles, and inner core cells of the Pacinian corpuscles. After denervation of the ulnar nerve on macaque, NSE on axons of the cutaneous nerves and Meissner and Pacinian corpuscles was completely disappeared in the 5th digit. However, S-100 protein was still maintained in the Schwann cells and Meissner and Pacinian corpuscles in the same digit. From these results, we conclude that the comparative immunohistochemical staining of NSE and S-100 protein is simple and reliable method to demonstrate the cutaneous nerves in normal and pathological conditions.
We present a case of junctional epidermolysis bullosa (EB) associated with urethral tract involvement. He was a 27-year-old man who had symblepharon of the right eye, hoarseness and urethral stricture in addition to generalized blister formation. Ultrastructural examination of a biopsy skin specimen demonstrated the characteristic morphology of junctional epidermolysis, a form of EB that has not been previously associated with urethral stricture.

The fluorescence polarization method was applied to measure the intracellular fluidity of fractionated guinea pig keratinocytes. Guinea pig epidermal cell suspension was obtained by treatment with EDTA and trypsin, and was separated into high, intermediate, and low density fractions using Percoll density gradient centrifugation. Morphological observation and cytofluorometric analysis of DNA content in the fractionated epidermal cells showed that the high, intermediate, and low density fractions were basal, spinous, and granular cell-rich fractions, respectively. Intracellular fluorescence polarization of each fraction was determined by a polarization spectrofluorometer (Hitachi MPF-4, prototype) with fluorescein diacetate. The P-values were calculated for high, intermediate, and low density fractions as 0.192 +/- 0.021, 0.172 +/- 0.019, and 0.147 +/- 0.012, respectively. Since low P-values indicate a high degree of fluidity, the results indicate that intracellular fluidity of keratinocytes is lower in basal cells and higher in granular cells. Dye-binding experiments showed that fluorescein-binding proteins were not detected in the soluble fraction of the epidermal cells. The present findings suggest that intracellular fluidity of the guinea pig keratinocyte increases during the process of its differentiation.
