[Ruptured ICPC junction aneurysm revealing angiographically only small bulging (author's transl)].

Two cases revealed only small bulging at ICPC junction on carotid angiogram after subarachnoid hemorrhage were reported. One case was treated conservatively and other was operated. The diagnosis and operative indication of these cases were discussed. The authors think that all of these cases, whether these bulging are classified angiographically as aneurysm or junctional dilatation, should be operated positively when CT scan shows high density area due to subarachnoid hemorrhage around them and there are no other findings inducing subarachnoid hemorrhage on angiograms.

[Computed tomography in ruptured intracranial aneurysm--sequential changes of the high density area in the basal cistern (author's transl)].

Using the findings of CT scanning in cases of subarachnoid hemorrhage, we have undertaken an investigation of the relationship between the degree of consciousness disturbance at the time of subarachnoid hemorrhage and the degree of high density area within the basal cistern within the 1st day from onset, and the sequential changes of the high density areas. The study included 23 cases of initial attacks of cerebral aneurysm rupture in which 2 or more CT scans were obtained prior to surgery, or in non-operated cases, within 2 weeks of onset. The severity of the attacks were classified into 3 groups: minor attack, cases of subarachnoid hemorrhage without loss of the consciousness; moderate attack, those with loss of consciousness extending for less than 1 hour; and major attack, those with loss of consciousness extending for more than 1 hour. Since a correlation was found between the CT number of high density areas in the basal cistern within the first 24 hours from onset and the severity of the attack, the classification of the severity of the attack is thought to correlate with the volume of the subarachnoid hemorrhage. Disappearance of the high density areas in the basal cistern occurred after about 2 days in the minor, about 4 days in the moderate, and 8--10 days in the major attack cases, but in the group of patients suffering recurrent attacks and in which death occurred without a return of consciousness, the high density areas remained until death. We conclude that by considering the CT number of the high density areas obtained by CT scanning in the basal cistern in cases of subarachnoid hemorrhage, the degree of bleeding can be determined, and the incidence of infarction due to vasospasm can be anticipated.

Antitumor activities of newly synthesized 5-carbamoyl-1 H-imidazol-4yl 1-adamantanecarboxylate and 5-carbamoyl-1H-imidazol-4yl piperonylate.

In synthetic studies on the chemical modification of the nucleoside antibiotic bredinin, two new derivatives, 5-carbamoyl-1H-imidazol-4-yl 1-adamantanecarboxylate and 5-carbamoyl-1H-imidazol-4-yl piperonylate, were found to possess a potent antitumor activity in several experimental tumor systems, even though bredinin itself shows only in vitro cytotoxicity and thus lacks therapeutic effectiveness. These two derivatives of bredinin exhibited antitumor activity against a wide variety of tumors, including leukemias L1210 and P388, Lewis lung carcinoma, B16 melanoma, Colon 26 and 38 adenocarcinomas. Ehrlich carcinoma, and Sarcoma 180. It is noteworthy that these agents showed good therapeutic effects not only against ascitic types of tumors but also against a number of slow-growing solid tumor lines, particularly the ascitic and solid forms of Ehrlich carcinoma. At their optimal doses, both compounds effected a complete cure of all or most of the mice treated. Although the mechanisms of action of these compounds remain unknown, they are able to suppress in vivo tumor growth, presumably by being slowly anabolized in vivo to an active form and inhibiting purine de novo synthesis as bredinin does.

Estimation of urinary kininogenase activity using bovine serum low molecular weight kininogen.

Estimation of urinary kininogenase activity by radioimmunoassay of generated kinin was studied. Bovine serum low molecular weight kininogen was proved not to cross-react with kallidin antibody and also bradykinin antibody. This kininogen was used as substrate measuring urinary kininogenase activity. Separation of released kinin from the kininogen was not required in the present method. Urinary kallikrein activity was found to be significantly decreased in essential hypertension, in chronic glomerulonephritis and in patients who had received renal transplantation. On the contrary, an increase in urinary kallikrein was found in primary aldosteronism and in Bartter's syndrome. The present method was very useful for measuring kininogenase activity.

Acute right-sided hemorrhagic colitis associated with oral administration of ampicillin.

Among 56 cases who presented to Kanto-Teishin Hospital complaining of bloody diarrhea or considerable hematochezia of acute onset, 8 cases (14.3%) were considered due to colitis associated with oral ampicillin therapy. The bloody diarrhea, often with abdominal cramps, began 2-7 days after starting the treatment. The dosage of ampicillin taken ranged from 2.0 to 4.5 g. Early total colonoscopy and biopsy revealed marked mucosal hemorrhage with minimal or no inflammatory changes mainly in the right colon. Rectum and sigmoid colon are completely normal except in one case. Symptoms rapidly resolved after the endoscopy. At follow-up colonoscopy, performed 4-12 days later, the mucosal changes had cleared completely. There was no evidence to support a hypersensitivity reaction of the colonic mucosa to ampicillin. We believe that right-sided hemorrhagic colitis is one of the common forms of colitis associated with ampicillin. Its differentiation from other kinds of acute colitis and the importance of early total colonoscopy are discussed.

Aneurysm surgery above the eighth decade.

A series of nine patients over 70 years of age with saccular aneurysms, operated on by direct surgery, is reviewed in order to discuss aneurysm surgery in older patients. The median age of the patients was 74 years, and the median interval from haemorrhage to surgery was 18 days. Operative mortality was 11%, and morbidity was 33%. Because of arteriosclerosis round the aneurysm neck and parent artery, special considerations for surgical treatment on the older patients are required.

Enhanced efflux of actinomycin D, vincristine, and vinblastine in adriamycin-resistant subline of P388 leukemia.

In vitro cross-resistance of adriamycin-resistant subline of P388 leukemia to non-anthracycline agents-actinomycin D, vincristine and vinblastine was elucidated. Decreased uptake of these 3 drugs was observed with the resistant cells. In addition, 2,4-dinitrophenol, a metabolic inhibitor, greatly enhanced the uptake of these drugs by the sensitive and resistant cell lines, particularly by the latter, abolishing a difference in drug uptake between the 2 which was observed under the normal condition. These results suggest that adriamycin-resistant cells are endowed with an enhanced capacity for outward transport of not only anthracycline antibiotics, but chemically and pharmacologically dissimilar agents such as the aforementioned agents.

Comparison of efficacy of amoxapine and imipramine in a multi-clinic double-blind study using the WHO schedule for a standard assessment of patients with depressive disorders.

A multi-clinic double-blind controlled study on amoxapine in comparison with imipramine, using the WHO Schedule for a Standard Assessment of Patients with Depressive Disorders, was performed and the data were analyzed with 111 patients. The assessment of severity of illness and overall improvement indicated clearly the superiority of the antidepressive effect of amoxapine to that of imipramine. The onset of antidepressive effect of amoxapine was clearly more rapid than that of imipramine, and in more than half of the patients in the amoxapine group the improvement was seen within four days following the drug administration. Amoxapine was superior to imipramine in terms of safety and usefulness. The side-effects due to amoxapine appeared less frequently and were less serious than with imipramine. The difference between amoxapine and imipramine was especially remarkable for hypotensive effect. The antidepressive effect of amoxaphine was superior to that of imipramine for almost all symptoms and signs. Amoxapine displayed an especially remarkable effect on psychomotor retardation, depressive feeling, anxiety and tension, somatic complaints and sleep disturbance.

Change in plasma renin activity by cold storage of plasma in normal subjects and patients with essential hypertension and primary aldosteronism.

The time courses of change in renin activity after cold storage of human plasma at -5 degrees C and pH 7.4 were examined in 5 normal subjects, 6 patients with essential hypertension and one female patient with primary aldosteronism before and after extirpation of the adrenal tumor. In the 5 normal subjects and 6 essential hypertensives, the gradual increase in plasma renin activity was observed until 10 days of cold storage. The same result was obtained in the case of primary aldosteronism. However, there was no increase in renin activity despite of cold storage for 10 days in plasma which was sampled from this patient 45 days after operation. These data indicate that a period of 4 days for cryoactivation of human plasma renin as has been reported by Sealey et al. is not sufficient to accomplish activation of renin by cold storage.

Effect of triamterene on urinary excretion of immunoreactive prostaglandin E in essential hypertension.

The effect of triamterene on urinary excretion of prostaglandin E was studied, and the results were compared with those obtained with spironolactone. Urinary excretion of immunoreactive prostaglandin E was measured radioimmunologically. Triamterene was administered in a dose of 100 mg/day for 8 days to 7 patients with essential hypertension. Following the administration of triamterene, urinary prostaglandin E tended to increase. However, the increment was not significant. The lack of significant increase in urinary prostaglandin E excretion during the administration of triamterene contrasted with our previous finding with spironolactone, in which a significant increase in prostaglandin E excretion was observed on the first day of spironolactone administration. Urinary Na excretion and urinary Na/K ratio were significantly increased and urine volume also tended to increase following the administration of triamterene. Plasma renin activity and plasma aldosterone concentration were increased in all cases. However, there was no significant correlation between these parameters and urinary prostaglandin E. These results suggest that the effect of triamterene on renal prostaglandin E synthesis is different from that of spironolactone and that the change in urinary prostaglandin E after the administration of triamterene is not the reflection of the change in the renin-angiotensin-aldosterone system.
