Regulation of the insulin-like growth factor system by insulin in burn patients.

The aim of the present investigation was to determine whether there is a net uptake of insulin-like growth factor I (IGF-I) or IGF-binding proteins (IGFBPs) by the leg after burn injury and to elucidate the regulatory role of insulin exerted on this system under in vivo conditions in burn patients. Studies were performed on nine patients after burn injury (approximately 60% body surface area). Each patient was studied twice during a continuous infusion of a carbohydrate-rich enteral diet. Blood was collected simultaneously from the femoral artery and vein for the measurement of various elements of the IGF system after 7 days of enteral diet alone (basal period) and after 7 days of the enteral diet plus the infusion of insulin (insulin period). Data from these patients were compared to values in age-matched fed healthy volunteers. During the basal period, burn patients demonstrated a significant reduction in the venous concentration of IGF-I and an increase in both IGFBP-1 and -2 compared to control values. Insulin produced a significant 15% increase in the IGF-I concentration in burn patients, but decreased the circulating levels of IGFBP-1 by 50%. The IGF-I and IGFBP-1 concentrations at the end of the insulin period were still significantly different from those in control subjects. Burn patients also exhibited a marked reduction in intact IGFBP-3 and the acid-labile subunit under basal conditions, and these alterations were not reversed by insulin. Under basal conditions, all burn patients had a positive arterio-venous (A-V) difference for IGF-I across the leg. The A-V difference was increased 50% in response to insulin. The net uptake of IGF-I by the leg was 2.4 micrograms/min under basal conditions, and as leg blood flow also tended to increase in response to insulin, IGF-I uptake was elevated more than 3-fold during the insulin period. No A-V difference across the leg was detected for IGFBP-1, -2, or -3 in burn patients. In conclusion, burn injury in humans produces dramatic and sustained alterations in various components of the IGF system that persist despite adequate nutritional support. Our data indicate the presence of a net uptake of IGF-I by the leg in burn patients that may serve to counteract the catabolic state.

Regional cerebral blood flow measured with 99mTc-HMPAO based on two-dimensional 133Xe study in ischemic brain.

Two-dimensional regional cerebral blood flow (rCBF) was measured using the xenon-133 inhalation method, and used to calibrate the rCBF measured by single photon emission computed tomography (SPECT) in the non-affected middle cerebral artery (MCA) area. The value of the rCBF in the target area could then be measured by SPECT and compared between patients. Technetium-99m-d,l-hexamethyl-propylene-amine-oxime and technetium-99m-diethylenetriaminepenta-acetic acid-human serum albumin D were used to measure rCBF and regional cerebral blood volume (rCBV), respectively, in the ischemic area of 16 patients who underwent surgical revascularization in the chronic stage. No significant increase in rCBF was obtained after the bypass surgery. The delta rCBF and delta CBF/CBV ratio correlated well with the extent of vascularization in the MCA area on postoperative angiograms. Patients with a low CBF/CBV ratio on preoperative SPECT scans tended to show favorable postoperative vascularization and rCBF in the MCA area. Since the angiographic findings and the delta rCBF or delta CBF/CBV were correlated well, this simple and non-invasive method could be used for the analysis of rCBF in a group of patients.

Relation of green tea consumption to serum lipids and lipoproteins in Japanese men.

Animal experiments have shown a hypocholesterolemic effect of green tea extracts. Only few epidemiological studies have addressed the relation between green tea consumption and serum total cholesterol (TC) and low density lipoprotein cholesterol (LDL-C). This paper examined the relation of green tea consumption to serum lipids and lipoproteins in 2,062 male self-defense officials in Japan, aged 49-55 years, who received a preretirement health examination at three hospitals of the Self-Defense Forces in 1991-1992. A self-administered questionnaire queried the consumption of green tea and a limited number of other dietary items as well as lifestyle characteristics. After adjustment for body mass index, waist-hip ratio, smoking, alcohol use, exercise, rank, and hospital, green tea consumption was inversely associated with serum levels of TC and LDL-C, but not with either high density lipoprotein cholesterol or triglycerides. Rice consumption was positively correlated with green tea consumption, and also was associated independently with lower levels of TC and LDL-C. Additional adjustment for rice, however, did not change the results. Daily drinking of 10 cups of green tea was associated with differences of 6.2 mg/dl in TC (95% confidence interval [CI] 0.4-12.1) and 6.2 mg/dl in LDL-C (95% CI 0.7-11.7). These findings of association of green tea with blood cholesterol hint at a possible causal relationship, which requires confirmation by further studies in humans using different methods.

[Normalization of hypercholesterolemia in a female stroke patient after switching from enteral tube feeding to oral feeding].

A 70-year-old female was admitted to a general hospital in a rural area due to left putamenal cerebral hemorrhage in December 1994. She had right hemiplegia and was totally aphasic. In May 1995, she was moved to Tokyo where her son lives, and she was admitted to Tokyo Metropolitan Geriatric Hospital in order to prepare a home care system. her family's support (serving her favorite dishes) allowed enteral tube feeding to be halted. After one month she could absorb enough energy to maintain her serum albumin level. The total calories ingested orally was comparable to that of enteral feeding but the fat composition was 62% of that of enteral feeding (fat was 19.6% and 31.7% of the total calories in the two diets, respectively). Her cholesterol level decreased from 286 mg/dl to 197 mg/dl. Nutrient-balanced tube feeding is useful, but may disturb lipid metabolism in patients used to having vegetable-rich diets.

Mycotic aneurysm of the internal iliac artery caused by Klebsiella pneumoniae.

Mycotic aneurysms are rarely caused by Klebsiella species. We describe a male patient with diabetes mellitus and alcoholism who developed a mycotic aneurysm of the right internal iliac artery complicated by the formation of a false aneurysm. Klebsiella pneumoniae was cultured from arterial blood. An extra-anatomic bypass (left external iliac artery to right common femoral artery) was performed with a successful excision of the aneurysm.

Activation of prefrontal and posterior superior temporal areas in visual calculation.

Regional cerebral blood flow was measured with H2(15)O-positron emission tomography during the performance of visually presented calculation tasks to clarify cortical areas involved in arithmetic operations. The findings on a calculation task were compared with those on a covert reading task and with those of a fixation control task. Region of interest-based analysis showed that the left prefrontal area was activated in visual calculation relative to covert reading and the posterior superior temporal gyrus including part of Wernicke's area was activated in calculation relative to fixation control. The role of the prefrontal area in computation (arithmetic operations) and that of the posterior superior temporal gyrus in both comprehension of the stimuli and computation were suggested.

[MRSA-induced prosthetic valve endocarditis complicated by synthetic graft infection that did not respond to repeated vancomycin administration but improved after surgery].

We performed AVR and OMC in a 55-year-old male with ASR complicated by MS and ASO. Postoperatively, an ulcer formed on the left lower extremity, resulting in methicillin-resistant Staphylococcus aureus (MRSA) infection. Axillo-bilateral-femoral artery bypass was performed using synthetic grafts. However, infection developed in the anastomosis site, leading to MRSA sepsis. Since MRSA is highly susceptible to vancomycin (VCM), this drug was administered at a dose of 1.5 g/day. With negative conversion of MRSA, the infected wound healed, and the general status improved. However, after discontinuation of VCM administration, sepsis recurred. VCM administration was resumed with prolongation of the administration period and an increase in the dose. Drug administration was discontinued 3 times after 2-4 weeks each, but recurrence was observed each time. During this period, AR due to prosthetic valve endocarditis (PVE) developed. AVR and MVR were performed again, and the synthetic graft was left in place. However, after discontinuation of postoperative VCM administration, infection of the synthetic graft was also demonstrated. The graft was removed, and revascularization was performed by another route. The patient improved after these procedures. When MRSA-induced PVE or synthetic graft infection develops, the infectious foreign material should be completely removed at an early stage before progression of tissue destruction or ulcer formation even if antibiotics are effective.

Age-related changes in the numbers of mammotrophs, somatotrophs and mammosomatotrophs in the anterior pituitary gland of female rats: a flow cytometric study.

Age-related changes in the numbers of mammotrophs, somatotrophs, mammosomatotrophs, and the cells of other types in the anterior pituitary of female Wistar rat were measured by flow cytometry. The mammotrophs increased with age, and the somatotrophs decreased with senescence. The mammosomatotrophs increased remarkably in senescent rats, and these cells of the rats older than 21 months were about 10 times more than those of 3, and 12-13 months old rats. This result indicates that the stability of gene expression in cell differentiation is reduced in the aging process of the anterior pituitary.

Subarachnoid haemorrhage induced proliferation of leptomeningeal cells and deposition of extracellular matrices in the arachnoid granulations and subarachnoid space. Immunhistochemical study.

Subarachnoid haemorrhage (SAH) often leads to subarachnoid fibrosis and resultant normal pressure hydrocephalus; however, how subarachnoid fibrosis occurs is unknown. We examined the changes within arachnoid granulations (AGs) and the subarachnoid space (SAS) chronologically at the parasagittal region obtained from patients with SAH at autopsy and made comparison with controls by immunostaining for cytokeratin, specific marker for leptomeningeal cells and by the elastica Masson-Goldner methods. Within a week some AGs were torn, and many inflammatory cells filled the AGs and SAS. Cytokeratin positive cells were scarce. During the next two weeks cytokeratin positive cells increased. After three weeks, AGs and SAS were filled by dense deposits of extracellular matrices surrounded by multiple layers of leptomeningeal cells.

Mechanism of cell detachment from temperature-modulated, hydrophilic-hydrophobic polymer surfaces.

Poly(N-isopropylacrylamide) (PIPAAm), exhibiting a lower critical solution temperature (LCST) at 25 degrees C in physiological phosphate buffered saline solution (pH 7.4) and at 32 degrees C in pure water, was grafted onto the surfaces of commercial polystyrene cell culture dishes. This PIPAAm-grafted surface exhibited hydrophobic surface properties at temperatures over the LCST and hydrophilic surface properties below the LCST. Endothelial cells and hepatocytes attached and proliferated on PIPAAm-grafted surfaces at 37 degrees C, above the LCST. The cultured cells were readily detached from these surfaces by lowering the incubation temperature without the usual damage associated with trypsinization. In this case, the optimum temperature for cell detachment was 10 degrees C for hepatocytes and 20 degrees C for endothelial cells. Cell detachment was partially inhibited by sodium azide treatment, suggesting that cell metabolism directly affects cell detachment. Morphological changes of the adherent cells during cell detachment experiments indicated further involvement of active cellular metabolic processes. Cells detached from hydrophobic-hydrophilic PIPAAm surfaces not only via reduced cell-surface interactions caused by the spontaneous hydration of grafted PIPAAm chains, but also by active cell morphological changes which were a function of cell metabolism.

Temperature-modulated platelet and lymphocyte interactions with poly(N-isopropylacrylamide)-grafted surfaces.

Temperature-responsive semitelechelic poly(N-isopropylacrylamide) (PIPAAm) bearing a carboxyl end group has been chemically immobilized on aminated polystyrene particle surfaces via condensation reaction. PIPAAm-grafted particles were uniformly suspended in aqueous media at lower temperatures. With increasing temperature, PIPAAm-grafted particles aggregated and precipitated. Such reversible changes in particle colloidal behaviour was correlated to temperature-modulated hydrophilic/hydrophobic changes of particle surfaces modified by PIPAAm hydration/dehydration with temperature changes. Interactions between platelets and PIPAAm-grafted surfaces were studied by monitoring cytoplasmic free Ca2+ concentration ([Ca2+]i) changes in platelets using intracellularly-trapped Ca2+ indicator dye, Fura 2, at various temperatures. Although changes in [Ca2+]i in platelets in contact with PIPAAm-grafted particles were not observed below the critical temperature of PIPAAm, significant changes in [Ca2+]i in platelets were induced by contact with particles above this critical temperature. Furthermore, temperature-modulated cell adsorption/desorption control by PIPAAm-grafted particles was investigated using a particle aggregation assay in the presence of lymphocytes. Below the critical temperature of PIPAAm, mixed suspensions were completely homogeneous due to minimal interaction between lymphocytes and hydrated particles. In contrast, aggregated precipitates were observed by increasing the suspension temperature above the critical temperature of PIPAAm resulting from strong hydrophobic interactions between particles with lymphocytes. These precipitates are reversibly resuspended in cold buffer. The feasibility of cell activation/inactivation or cell attachment/detachment control by temperature-modulated surface changes is attractive for suspension cell culture and drug delivery at targeted sites in vivo.

Hypofrontality does not occur with 6-hydroxydopamine lesions of the medial prefrontal cortex in rat brain.

This study examined the effect of lesions of dopamine (DA) nerve terminals the medial prefrontal cortex on local cerebral glucose utilization (LCGU) and dopamine metabolism in the rat brain. Bilateral 6-hydroxydopamine lesions were stereotaxically placed in the medial prefrontal cortex. Twenty-eight days after the lesion, concentrations of DA and its metabolites, 3,4-dihydroxyphenylacetic acid (DOPAC) and homovanillic acid (HVA), were determined in eight brain regions with a high-performance liquid chromatographic assay. LCGU was assessed by [14C]2-deoxy-D-glucose autoradiography. The lesion produced a striking reduction in DA (to 6% of the control value), and a moderate reduction in DOPAC and HVA in the medial prefrontal cortex. The ratio of DOPAC to DA in the medial prefrontal cortex was significantly elevated in the 6-OHDA lesioned animals. In contrast to DA depletion, LCGU in the medial prefrontal cortex of the lesioned rats was unaltered when compared with the control. These findings suggest that decreased energy metabolism in the frontal cortex, i.e., hypofrontality, does not occur with decreased DA innervation of that site.

Effect of 6-hydroxydopamine lesions of the medial prefrontal cortex of the rat on performance in a reaction time task.

Rats were trained to keep pressing a panel and then release it within a time limit after a light cue. Reaction time was measured from the light cue to the release of the panel. In 'regular' preparatory interval (PI) sessions, the rat had to wait (keep pressing the panel) for a light cue during an interval fixed in a session. In 'irregular' PI sessions, which followed regular PI sessions, intervals until a light cue presentation changed randomly from trial to trial in a session. Rats lesioned by 6-hydroxydopamine infusion into their medial prefrontal cortices showed shorter and more stable reaction times in irregular PI sessions than sham-operated control rats. Cross-over patterns of reaction time of the lesioned rats were not as clear as those of the control rats. Impairment of mnemonic predictive function might underlie the reaction time features in irregular PI sessions in the lesioned rats.

Relationship of cigarette smoking, alcohol use, and dietary habits with sigmoid colon adenomas.

The relationship between cigarette smoking, alcohol use, and dietary factors and the risk of adenomas of the sigmoid colon was examined in male self-defense officials who received a preretirement health examination at three Self-Defense Forces hospitals in Japan. In the comparison between 228 patients with sigmoid adenomas and 1484 control subjects with normal colonoscopy findings (> or = 60 cm from the anus), a clear dose-response relationship was observed between cigarette smoking and risk of adenoma. After adjustment for rank, body mass index, alcohol use, and physical activity as well as for hospital and survey season, the odds ratios (and 95% confidence intervals (CIs)) for the categories of 0, 1 to 399, 400 to 799, and 800 or more cigarette-years were 1.0, 2.1 (1.2 to 3.5), 2.8 (1.8 to 4.3), and 3.5 (2.1 to 5.8), respectively. Current alcohol drinkers tended to have an increased risk, but without a dose-response relation. Among four types of alcoholic beverages (shochu, sake, beer, and whiskey), only whiskey showed a weak association with risk of adenoma. None of the 13 dietary items studied (including meat and rice consumption) was measurably associated with adenoma risk. The present findings provide additional evidence that cigarette smoking is a risk factor for colon adenomas. It is inconclusive regarding alcohol intake's association with adenoma risk.

Insulin-like growth factor-I and insulin reduce leucine flux and oxidation in conscious tumor necrosis factor-infused dogs.

BACKGROUND: We have tested the hypothesis that insulin-like growth factor-I (IGF-I) or insulin can prevent the protein catabolic effects of tumor necrosis factor (TNF). METHODS: After a 2-hour basal period TNF was infused (prime, 2.5 micrograms.kg-1; constant, 31.25 ng.kg-1.min-1) for 4 hours into conscious dogs to create the catabolic state. After 2 hours of TNF infusion either recombinant human IGF-I (n = 5) or recombinant human insulin (n = 5) was infused for an additional 2 hours. A third group (n = 5) received TNF alone for 4 hours. RESULTS: TNF infusion caused an increase in both glucose production, reflected by [6,6-d2]glucose tracer data, and net protein catabolism, reflected by both [1-13C]leucine and [15N2]urea tracer methods. IGF-I and insulin both significantly reduced the rates of appearance of leucine and leucine oxidation to a similar extent, resulting in the significant decrease in net protein catabolism. CONCLUSIONS: IGF-I and insulin can ameliorate the catabolic effects of TNF on protein and glucose metabolism equally effectively, although more IGF-I is required on a molar basis.

Physical entrapment of adriamycin in AB block copolymer micelles.

The entrapment of Adriamycin (ADR) in micelles composed of AB block copolymers (poly(ethylene oxide-co-beta-benzyl L-aspartate) (PEO-PBLA)) was investigated. The loading process involved transfer of ADR and PEO-PBLA into an aqueous milieu from dimethylformamide (DMF) through a dialysis procedure. Evidence for the physical entrapment of ADR in the polymeric micelles was derived from fluorescence spectroscopy and gel permeation chromatography (GPC). The total fluorescence intensity of ADR was low, suggesting that the drug was self-associated in the micelles. In addition, quenching experiments, using a water-soluble quencher (iodide (I-)), showed that the fluorescence of ADR present in micellar solutions was largely unaffected by I-, whereas the fluorescence of free ADR was readily quenched. From Stern-Volmer plots, quenching constants (KSV) of 2.2 and 17 M-1 were determined for ADR in micellar solutions and free ADR, respectively. As a result of the entrapment of ADR in the micelles, ADR binds only slightly serum albumin as evidenced by GPC. In contrast, ADR readily binds serum albumin in aqueous solutions. The findings suggest that ADR is stably entrapped in PEO-PBLA micelles. ADR entrapment in polymeric micelles is expected to affect markedly the pharmacokinetics of ADR.
