[Recent intractable bacterial infections in otolaryngology].

Antibiotic treatment has resulted in a significant change in the etiology and character of bacterial infections in otolaryngology as well as in another fields. Classic signs and symptoms may be masked. Administration of penicillin can be still effective in the treatment of usual bacterial infections, as it is effective against group A beta-hemolytic streptococci and most anaerobes except those that produce beta-lactamase. Administration of antibiotics, such as penicillin with clindamycin or chloramphenicol to combat both aerobic and anaerobic bacteria, coupled with surgical drainage is essential in management of deep neck infections. In this communication, several important bacterial diseases are described. Care should be taken to prevent airway obstruction, especially in deep neck infections and acute epiglottitis.

[The evaluation of hyposensitization with house dust in patients with nasal allergy to house dust-mite].

We evaluated the clinical and pathophysiological efficacy of hyposensitization with house dust extracts to perennial nasal allergy. Fifty-one patients with allergic perennial rhinitis completed 5 months to 15 years with a mean of 2.4 years course of hyposensitization treatment with house dust extracts, and analyzed clinical symptoms, nasal provocation testing, nasal hypersensitivity to histamine, mast cells and eosinophils in nasal scraping, specific IgG and IgG4 antibody in serum, and blood eosinophils and basophils in relation to therapeutic duration. The improvement in nasal symptoms was found in 74.1% in adults and 66.7% in child and the continuation of hyposensitization with house dust extracts for a half to one year showed significant improvement of clinical symptoms. In objective examination, nasal provocation testing, nasal hypersensitivity to histamine, the number of mast cells and eosinophils in nasal scraping, and blood basophils showed significant improvement at 6 to 12 months the same as clinical symptoms. These findings suggest that the continuation of hyposensitization with house dust extracts is recommendable as a basic treatment against perennial nasal allergy.

[The evaluation of hyposensitization with sugi pollen extracts in patients with nasal allergy to Japanese sugi pollen].

We evaluated the clinical and pathophysiological efficacy of hyposensitization with Japanese sugi pollen extracts to sugi pollinosis. Twenty patients with Japanese cedar pollinosis received hyposensitization with Japanese cedar pollen extracts for one to 15 years with a mean of 9.5 years were compared with 18 patients without hyposensitization. The improvement of nasal symptoms was not found in patients with hyposensitization of sugi pollen extracts compared with patients with non-hyposensitization, but the number of mast cells in nasal scraping, nasal hypersensitivity to histamine, and specific sugi IgG4 antibody in serum was improved significantly compared with non-hyposensitization group. These findings suggest that the continuation of hyposensitization with sugi pollen extracts is recommendable as a basic treatment against seasonal nasal allergy.

Secretion of endogenous 16-kDa beta-galactoside-binding lectin from vitamin A-pretreated chick embryonic cultured skin.

Localization of endogenous beta-galactoside-binding 16-kDa lectin and its gene expression pattern were investigated during differentiation of chick embryonic skin in vivo and in vitro and were compared with those of a 14-kDa lectin. By light microscopy, immunostaining of the 16-kDa lectin was weak in the undifferentiated epidermis, while it became intense in the keratinized epidermis, particularly in the intermediate cells. Essentially the same staining pattern was observed both in vivo and in vitro. The gene expression pattern was consistent with these immunohistochemical observations. These results were similar to those of the 14-kDa lectin [1]. On the other hand, in the vitamin A-pretreated cultured skin, mucous metaplasia of the epidermis was induced and marked changes in localization of these two isolectins were observed. The 16-kDa lectin expression was increased in the epidermis, especially in the superficial cells, and the gene expression was detected in all layers of the epidermis. However, 14-kDa lectin expression was markedly reduced in the metaplastic epidermis. In the keratinized epidermis, detailed localization of these lectins under the electron microscope was almost the same. Both of them were located primarily along the plasma membrane, in the intercellular space, and in the desmosomes. In the mucous metaplastic epidermis, however, the localization of the 16-kDa lectin was completely different from that of the 14-kDa lectin; e.g., the former was detected in the mucous granules and on the microvilli of the superficial cells, while the latter was scarcely observed in the epidermis. The present results revealed that (1) the 16-kDa lectin as well as 14-kDa lectin were expressed during the epidermal differentiation and (2) much of the 16-kDa lectin was produced and secreted from the vitamin A-pretreated cultured epidermis, while 14-kDa lectin disappeared. In the view of these facts, it is suggested that (1) both 14- and 16-kDa lectins may play an important role in differentiation of the skin and (2) the gene expression of 14- and 16-kDa lectins is regulated independently.

Arachidonic acid metabolism by isolated and cultured middle ear epithelial cells from the chinchilla.

Thin-layer chromatography was used to examine the metabolism of arachidonic acid and prostaglandins (PGs) in freshly isolated and cultured middle ear epithelial cells from the chinchilla. The freshly isolated cells converted arachidonic acid predominantly to PGE2, while those cells grown in culture for 10 days acquired the ability to convert arachidonic acid to 6-keto-PGF1 alpha, PGD2, and PGE2. Incubation of the isolated cells and primary cultures with acetylsalicylic acid and indomethacin inhibited the formation of these PGs. These findings suggest that studies on the factors regulating arachidonic acid metabolism in middle ear epithelium may help to explain the role of eicosanoids in middle ear secretions, particularly in relation to the pathophysiology of otitis media.

Biochemical characterization of mucous glycoproteins secreted by in vitro chinchilla middle ear epithelial cells.

A method for middle ear epithelial (CMEE) cell culture with active mucus secretory function has been successfully developed, using the chinchilla as an animal model. CMEE cells were dissociated by protease digestion from the middle ear mucosa. The CMEE cells grown in primary culture incorporated [3H]glucosamine into a glycoconjugate after its release into medium. This substance was characterized biochemically as mucin, although the production of mucin by the cells required growth on a substratum of collagen gel. These cultures provide an excellent model for studying factors that regulate synthesis and secretion of glycoproteins in CMEE cells.

Quantification of interleukin-1 in nasal polyps from patients with chronic sinusitis.

We have measured interleukin-1 alpha (IL-1 alpha) and beta levels in nasal polyps (NP) from patients with chronic sinusitis (CS) in order to determine their significance in the pathogenesis of NP. NP in ten cases (five male and five female; age range, 17-63 years) were removed to separate mononuclear (M) and polymorphonuclear (PMN) fractions through Ficoll-Paque separation media. ELISA measurements of the M fraction showed that the mean value of IL-1 beta was 17.8 pg/ml per gram, which was significantly higher than that of IL-1 alpha (7.04 pg/ml per gram, P < 0.01, Wilcoxon test). In the PMN fraction, the mean value of IL-1 alpha was 8.79 pg/ml per gram and that of IL-beta was 7.85 pg/ml per gram, which was not significantly different. Mononuclear leukocytes, particularly activated monocytes, seem to be a major source of IL-1 in the NP taken from patients with CS.

Clinical and pathophysiological evaluation of tranilast in patients with pollinosis: the effects of pre-seasonal treatment.

Tranilast is an orally effective anti-allergic agent evaluated for clinical activity in prevention of symptoms of allergic rhinitis. It was investigated in a randomized, pre-seasonal, and in-seasonal treatment of tranilast to test the clinical efficacy and pathophysiological changes on the onset of nasal symptoms in season. Thirty-eight patients with a history of Sugi pollinosis and positive allergic tests were treated with tranilast 300 mg daily 6 to 7 weeks before the onset of the pollen season and continuously during the season or no medication including tranilast until the onset of clinical symptoms in season. The number of sneezing and the grade of stuffiness at the onset of pollen season were inhibited significantly in pre-seasonal treatment. The number of mast cells and eosinophils in season increased compared with off-season and the threshold of nasal hypersensitivity in season decreased compared with off-season in inseasonal treatment, but not in pre-seasonal treatment. This study indicates that tranilast may have the prophylactic effect for Sugi pollinosis.

Effects of nasal allergen challenge on dynamic viscoelasticity of nasal mucus.

The effects of nasal provocation on the rheologic properties of nasal mucus were investigated in patients with allergic rhinitis provoked by house dust. The elastic modulus (G') and the dynamic viscosity (eta') of nasal mucus were determined by an oscillating sphere magnetic rheometer. Before and after the allergen challenge, G' increased, whereas eta' decreased with increasing oscillatory frequency; these findings indicate that the nasal mucus under these conditions is a non-newtonian fluid and has the cross-linked gel-like nature typical of mucus. Both G' and eta' values after nasal provocation were significantly lower than before. The values of G' and eta' after allergen challenge were in the optimal viscoelasticity range for mucociliary transport.

EGF abrogation-induced fusilli-form dysmorphogenesis of Meckel's cartilage during embryonic mouse mandibular morphogenesis in vitro.

Mutations associated with genes of the EGF superfamily are implicated in facial malformations arising from abnormal development of the first branchial arch. EGF and EGF receptor (EGFr) transcripts are expressed in the mouse embryonic first branchial arch and derivatives from E9 through E15. EGF transcripts are localized to ectomesenchymal cells associated with precartilage, cartilage, bone and tooth-forming cells. EGF and EGFr proteins co-localize to the same cells suggesting an autocrine regulation. To test whether EGF effects the timing and positional information required for Meckel's cartilage (MC) and tooth development, we cultured E10 mandibular explants in serumless, chemically defined medium with either antisense or sense EGF oligodeoxynucleotides. Antisense inhibition of EGF expression produces bilaterally symmetrical Fusilli-form dysmorphogenesis of MC and decreases tooth bud size; these effects are reversed by the addition of exogenous EGF to the culture medium. Tyrphostin RG 50864, which inhibits EGF receptor kinase activity, inhibits EGF stimulation of tyrosine phosphorylation in a concentration-dependent manner and severely retards mandibular development yet increases tooth size. These findings support the hypothesis that endogenous EGF and EGF-like proteins provide signalling to regulate the size and shape both of cartilage and tooth formation during craniofacial morphogenesis.

Quantitative cytology of nasal secretions under various conditions.

Quantitative cytology was performed in nasal secretions of normal control (NC), seasonal allergic rhinitis in season (SAR), perennial allergic rhinitis (PAR), chronic sinusitis with mucoid secretion (MS), and chronic sinusitis with mucopurulent secretion (MPS). The majority of inflammatory cells were neutrophils in NC, MS, and MPS; the majority were eosinophils in SAR and PAR. The concomitant appearance of inflammatory cells in nasal secretions was found, i.e., there were significant correlations between neutrophil and eosinophil counts in MPS, and between eosinophil and basophil counts in SAR. The eosinophil/neutrophil ratio was more than 0.1 in SAR and PAR, but the ratio was less than 0.1 in all NC, all MPS, and in 93% of MS; this indicates that 0.1 in eosinophil/neutrophil ratio is the critical value between allergic and nonallergic nasal diseases.

Immunohistochemical and electron microscopic studies of substance P in guinea pig nasal glands.

The distribution of substance P-like immunoreactive (SP-IR) nerve endings in the nasal gland of the guinea pig was studied by using histochemical techniques to detect mucous glycoprotein and immunohistochemical techniques for SP, in combination with electron microscopy. Most nasal glands were negative for both AB and periodic acid-Schiff (PAS) staining. The SP-IR nerve fibers were found to form a network around these glands. The SP-IR nerve endings were located in the region between interdigitated cytoplasmic folds of acinar cells and along the cell surface, as well as in the intercellular spaces of proximal ducts. The acini which closely contacted with SP-IR nerve endings were serous in type. Our results suggest that substance P may contribute to the regulation of serous gland secretion in the guinea pig nasal mucosa.

[Maxillary prosthesis for better QOL--analysis of maxillary prosthesis stability].

We report here an analysis of 100 cases of maxillary prosthesis, experienced from July, 1981 through June, 1989, with special reference to stability. Sixty-four percent of prostheses were partial denture type and complete denture type comprised thirty-two percent. Eighty-six percent of cases were estimated to have defects exceeding 2/6 and 3/6 of the surface width of the palate. Ninety percent of prostheses weighed less than 15 gm. The stability of these prostheses was estimated to be good in 85% of cases, the stability of the denture type being good in 95% of partial denture type cases, and 62% of complete denture type cases. According to type distinctions among maxillary defects as a whole, in the smaller 1/6 and 2/6 defect cases, good stability can be expected, and in 3/6 and 4/6 defect cases, 80% achieve good stability. According to prosthesis weight, 80% of those weighing less than 15 gm were considered to have shown good stability. Thus, these maxillary prostheses can remain in place and functional ameliorating disability and contributing to the patient's quality of life.

Nasal mucociliary transport of chronic sinusitis in children.

Nasal mucociliary function is one of the most important and indispensable mechanisms of the respiratory tract, providing protection against the atmospheric environment. We previously found mucociliary dysfunction in the noses of adult patients suffering from chronic sinusitis. In this study, using the saccharin method, we determined nasal mucociliary function in normal children and in children with chronic sinusitis. The mean (+/- SD) value of saccharin transit time in the nose was 28.2 +/- 19.9 minutes in patients with chronic sinusitis, this being significantly slower than that in the control group of children. The incidence of abnormally slow nasal mucociliary transport time (> 30 minutes) in patients was significantly higher than in controls of the same age. Mucociliary dysfunction may initiate a vicious cycle of self-mediated inflammation and may be important in recovery from chronic respiratory inflammation.

Neutrophil elastase and its complex with alpha 1-antitrypsin in the pathogenesis of chronic suppurative otitis media.

Neutrophil elastase (NE) and its complex with alpha 1-antitrypsin were quantified in ear discharges from 15 patients with chronic suppurative otitis media (CSOM), and their levels were compared to those in middle ear effusions from 10 pediatric patients with chronic otitis media with effusion (OME). The localization of immunoreactive NE was also examined by immunocytologic study. The mean value of total NE was 161.8 +/- 29.5 micrograms/mL in CSOM, which was significantly higher than that in OME (16.5 +/- 6.7 micrograms/mL). The mean value of NE-alpha 1-antitrypsin complex was 13.7 +/- 8.8 micrograms/mL in CSOM and 7.8 +/- 4.0 micrograms/mL in OME. There was no significant difference between culture-positive and culture-negative samples in CSOM. Immunoreactive NE could be observed in the neutrophils of ear discharges, and extracellular release of NE was also observed. Ear discharges in CSOM contain a considerable amount of uncomplexed NE, of which the level does not depend on active bacterial infections. Uncomplexed NE seems to play a role in the chronicity of CSOM.

Newcastle disease viral infection in chicken nasal turbinate and maxillary sinus.

Newcastle disease virus (NDV) B strain was inoculated intranasally and intrasinusly into unanesthetized chicken which were maintained on a standard commercial mash chicken diet until the age of 21 days after hatch. The lesions induced by NDV following intranasal inoculation usually produced a selective destruction of significant portions of the inner surface of the turbinate scroll, but sinus lesions, though sometimes present in NDV infection, were not common. The chicken sinus was infected by intrasinus inoculation. These results were supported by the amount of virus and mucociliary transport time in the turbinate and sinus. It is suggested that mucus hypersecretion of ostial gland and/or mucociliary activity in the sinus be one of the most important factors in protecting against chicken sinus infection.

[Surgical treatments of adult chronic sinusitis--a study by questionnaire from 434 hospitals in Japan].

Information concerning surgical treatment of adult patients with chronic sinusitis was obtained through questionnaires from 434 hospitals in Japan. The hospitals were authorized for an in-service otorhinolaryngological training institute by the Oto-Rhino-Laryngological Society of Japan. The questionnaires were sent to the hospitals in December, 1989, and answers were obtained from 370 hospitals within 2 months, an 85.3% response rate. The following results were obtained. In 96.2% of the hospitals, permaxillary sinus surgery was performed on patients whose maxillary and ethmoid lesions were severe. With moderate lesions of both the maxillary and ethmoid sinuses, permaxillary sinus surgery and endonasal sinus surgery were performed in 76.0% and 24.9%, respectively. Application of an endoscope for sinus surgery was reported by 89 hospitals. In January, 1990, a further survey was sent to the 89 hospitals where endoscopic sinus surgery was performed. Questionnaire responses were obtained from 75 hospitals. Rigid endoscope was used most frequently and operative microscope was also used. Endoscopy was applied to all of the sinus surgery procedures in 21.6%, and to a portion of the procedures in 74.3% of the hospitals. About 50% of the hospitals performed endoscopic surgery in fewer than 30% of sinus surgery cases. When questioned as whether endoscopic surgery had been successful, 42.6% said yes, 10.7% said no, and 42.6% were unsure.

[The quantitation of Pseudomonas aeruginosa elastase in suppurative chronic otitis media using a sensitive ELISA method].

A sensitive sandwich ELISA method has been developed in order to quantitate the Pseudomonas aeruginosa elastase (PE) of ear discharge from chronic suppurative otitis media (CSOM) patients. Samples were incubated with EDTA-2Na before ELISA in order to inhibit the PE activity which hydrolyzes anti-PE IgG antibody into smaller molecular fragments. Quantitation of PE in middle ear effusions (MEE) from 10 patients with chronic otitis media with effusion (OME) were also performed. In CSOM, 12 of 14 samples revealed a significant amount of PE from 0.6 microliter/ml to 62.1 microliters/ml, which was significantly higher than those in MEE (p less than 0.05). In MEE, 8 of 10 samples were under the detection limit. Two samples in CSOM with Pseudomonas aeruginosa infection had high levels of PE. The quantitation was linear, with a concentration from 5 ng PE/sample to 500 ng PE/sample. This ELISA system is a sensitive method for quantitation of PE requiring only very small samples.
