[Therapeutic indications in scoliosis of Duchenne muscular dystrophy. Apropos of 10 cases].

Ten patients suffering from Duchenne muscular dystrophy were followed up for more than 7 years. The diagnosis was determined by biopsy. Progress of the disease was severe and all patients died before the age of 18. The average angle of the scoliosis at the end of this development was 73 degrees. The maximum vital capacity was 1198 ml. The authors conclude that neither the use of a brace nor adaptation of a wheelchair are helpful. Surgical treatment is hazardous when the curve is greater than 23 degrees. The indications for such treatment are based on the importance of the degree of pulmonary restriction and on early signs of severe progress of the scoliosis.

[Electromyography of the leg. Study during various segmental movements].

Using anatomical landmarks as fixed points, the authors have squared the cutaneous surface ot the leg into areas which cover the following superficial muscles: m. gastrocnemius, caput mediales et caput laterales, m. peroneus longus, m. tibialis anterior, m. soleus. The electrical surface activity, collected in these areas for some segmented movements (dorsal and plantar flexions with and without load) was integrated and quantified. The individual and inter-individual variability in electrical activity was obtained from six subjects and discussed. The typical electromyography of the leg was established for the four segmental movements investigated. This analysis had also lead to determine the most suitable electrode site for each of the superficial muscles. This study confirms the functional role classically attributed to these various muscles.

[Electromyocartography of the thigh. Attempt at a functional analysis of the locking of the knee against resistance].

Electromyocartographic analysis of the extension of the leg in different positions has shown that: - on the whole energy developped by all superficial muscles of the thigh is more important in the position of dorsal decubitus than in a seated position; - functionnal activity of the rectus femoris muscle varies according to different subjects. Its activity is lower in the seated position, compared to dorsal decubitus, a classic notion; - in contradiction to classic notions, the most active muscle electrically is the vastus lateralis muscle. The vastus medialis muscle is however also very active; - from these results, it is possible to obtain a smaller variability owing to their relative quantification. One observes a hierarchy of electric activity levels of different surfaces, that constitutes the basis of functionnal and clinical study.

[Significance of echotomography B in the analysis of pronation].

By an ultrasonic technique (B. scan), the authors have analysed dynamically, the wrist during pronation. They could show that when the elbow is bended at 90 degrees, the distal tip of the radius effects a circular movement of great radius but low grade value, while the ulna draws in the opposite direction a nearly complete small circle.

[Functional analysis of the muscles of the anterolateral wall of the abdomen using electromyocartography].

The purpose of this study was to analyze the function of the superficial muscles of the anterolateral wall of the abdomen in dorsal decubitus by an original technique: electromyocartography. This technique attempts to monitor the individual or inter-subjective variations that complicate quantitative analysis of the overall electromyogram with integration of action potentials. It is shown that relative quantification of the areas explored with respect to the most active area considerably improves the consistency of the results. The most dependable electrode locations for the analysis of the abdominal muscles are determined. From a functional point of view: The rectus abdominis is active in flexion of the cranial portion of the trunk, either alone or in combination with right or left rotation of the trunk. Its activity is less, and predominates in the distal portion of the muscle, in flexion of the caudal portion of the trunk in combination with a 15 degree flexion of the hips. The obliquus internus abdominis, which can be explored only in a single area, is especially active in rotation. The obliquus externus abdominis plays a major role in stabilizing the pelvis when the lower limbs are involved in the movement of the trunk.

[Electromyocartography of the posterior and inferior wall of the abdomen during extension of the torso].

Analysis of the functions of the muscles of the inferior and posterior regions of the trunk was carried out by electromyocartography for the basic movements of extension of the trunk in ventral decubitus. For a given subject, the results varied little with time. By contrast, the results were substantially the same from subject to subject in the case of only one movement : extension of the caudal portion of the trunk combined with extension of the hips. A functional balance-sheet can be based only on this movement. The sacro-spinalis (without attempting to determine the respective roles of the various bundles) seems to be the only one involved in certain movements of extension of the trunk: Its action is at a maximum, highly variable, when the extension involves both the caudal and the cranial portions of the trunk; It is considerable in the extension of the caudal portion alone, and Moderate in the extension of the cranial portion alone.
