The electronic states of hydrochlorothiazide and its related molecules were obtained by CNDO/2 (CNDO = complete neglect of differential overlap), and van der Waals volume and hydrophobic parameters of the substituent of the 6th position in the benzothiadiazine were estimated. The results were discussed from the viewpoint of the structure-activity relationship analysis. Rather lower LUMO (lowest unoccupied molecular orbital level) of hydrochlorothiazide which had been predicted by the iterated Hückel's Molecular Orbital Method was confirmed by CNDO/2 calculation. The introduction of the sulfamoyl group to the 7th position in the benzothiadiazine ring brought out a negative formal charge for the 7th position. The diuretic effect of the substituent of the 6th position in the benzothiadiazine ring was analysed with respect to its van der Waals volume and its hydrophobic parameter. Van der Waals volume seemed to have a close relationship to diuretic activity. The highest correlated coefficient of the regression equation for the structure-activity relationship was obtained using the formal charge of the 7th position in the benzothiadiazine ring, van der Waals volume and the hydrophobic parameter of the substituent of the 6th position. On the basis of result obtained, the model for the action site of hydrochlorothiazide was proposed, consisting of a rather larger, lipophilic hole and an electrostatic interaction site in the tubular membrane.

Cardiac effects of aprindine, a relatively new antiarrhythmic agent, were investigated by means of echocardiography in nine patients with ventricular arrhythmias. Three patients had normal cardiac function, and six patients had dilated cardiomyopathy. Aprindine was administered orally in a dosage of 50 to 75 mg/d. The plasma concentrations were 0.86 +/- 0.12 micrograms/ml. No worsening of cardiac signs and symptoms was noted within four weeks. An antiarrhythmic effect was noted in six of the nine patients. Significant changes in end-diastolic dimension or ejection fraction were not observed. Changes in contractile state were also assessed using the peak systolic blood pressure-end-systolic dimension relationship in three patients; none of them showed a decrease in cardiac contractility. This study suggests that aprindine, in a dose sufficient to suppress arrhythmias, does not make cardiac function deteriorate, as evaluated echocardiographically, even in patients with cardiac dysfunction.

This report describes the findings in a young woman with Takayasu's disease, resulting in angina pectoris due to severe narrowing of the left coronary ostium. The patient underwent aortocoronary bypass surgery. The disappearance of angina, with reversal of positive findings on the treadmill exercise test, was noted postoperatively. Selective coronary angiography should be performed in patients with Takayasu's disease with a positive exercise test and symptoms of chest pain.
A 60-year-old woman with multiple coronary aneurysms, complicated with acute infero-posterior myocardial infarction is presented. The cine-coronary arteriogram disclosed a diffuse dilatation of epicardial coronary arteries associated with multiple aneurysms, fusiform ones in the right coronary and the left circumflex coronary arteries and saccular ones in the left anterior descending coronary artery. Distal run-off was poor. Ventriculography revealed a decrease in the ejection fraction (36%) and segmental abnormal wall motion at the apical and infero-posterior regions. The cause of the aneurysms appeared to be atherosclerotic. We suggested that coronary embolization was the cause of acute myocardial infarction in this case.
The time course of the concentration of cefmetazole (CMZ) in the serum and in skin and intestinal tissues was determined after a single intravenous injection of 2 g of the drug. CMZ moved into them well. Furthermore, 41 patients with postoperative wound infection (superficial in 29 and deep in 12) were treated with CMZ 2-4 g daily. Bacteriological examination of the lesions with simultaneously carried out. As a result, 101 strains of bacteria were isolated and identified. Mixed infection was found in 27 cases (65.9%). Fifteen strains (14.9%) of E. coli, 15 (14.9%) of B. fragilis, 7 (6.9%) of Klebsiella sp. and 7 (6.9%) of Proteus sp., were the main bacteria isolated. Eight cases (19.5%) had mixed infection of E. coli and B. fragilis. The committee (3 members) evaluated CMZ to be effective in 75.6% (31 of 41 cases) and bacteria disappeared in 60.5% (23 of 38 cases). The side effects observed were pyrosis and feeling of gastric malaise in 1 case. The results suggest that CMZ is useful, which exerts an excellent effect on postoperative wound infections.
