[A study of papillary renal cell carcinoma. Clinicopathological, immunohistochemical features and its typing].

BACKGROUND: Papillary renal cell carcinomas (PRCCs) are genotypically distinct from nonpapillary renal cell carcinomas. METHODS: We studied the clinical, pathomorphological, immunohistochemical features in 34 PRCCs and 7 papillary renal adenomas. Immunohistochemical studies were performed using lectins and antibodies against cytokeratins, epithelial membrane antigen and Tamm-Horsfall protein. RESULTS: PRCCs were divided into two types based on the features of tumor cells and vascular stalks. Fifteen PRCCs displayed small cuboidal cells with basophilic cytoplasm and thin, short vascular stalks (type 1, micropapillary). Nineteen PRCCs displayed large columnar cells with eosinophilic cytoplasm and edematous or fibrous thick stalks (type 2, macropapillary). Infiltration of foam cells was more common in type 1. Co-existence of papillary renal adenomas was recognized in three cases among type 1, but in only case among type 2. In type 1, a male to female predominance was evident (13:2), and the majority of tumors in type 1 were in lower nuclear grade and lower stage. The 5-year survival rates of patients in type 1 and 2 were 87% and 46%, respectively. Immunohistochemically, 15 (100%) cases in type 1 were diffusely positive for cytokeratin 7 (CK7), 15 (100%) were positive for cytokeratin 19 (Progen 19) and 11 (73%) were positive for dolichos biflorus agglutinin (DBA). In type 2, only 4 (19%) cases focally stained for CK7, 10 (53%) were stained for Progen 19 and only 2 (10%) were positive for DBA. The staining pattern in papillary renal adenoma was similar to that of type 1, all cases were positive for CK7, four of five cases (80%) were positive for Progen 19 and five of seven cases (71%) were positive for DBA. CONCLUSION: The pathomorphologic and immunohistochemical features suggested that it is possible to divide PRCCs into 2 types and that PRCCs in type 1 confer an favorable prognosis. Furthermore, our results supported the possibility of adenoma-carcinoma sequence.

[Early establishment of bone marrow hypoplasia by antileukemic chemotherapy with concurrent rhG-CSF in a case of acute myelogenous leukemia complicated with intestinal perforation].

We report a case of 53-year-old man with acute myelogenous leukemia (M2) showing a karyotype of t(7;11) (p15;p15), del(10) (q11;q12), who was complicated with perforation of a duodenal ulcer during the antileukemic chemotherapy using behenoyl ara-C, daunorubicin, 6-mercaptopurine and prednisolone. As his bone marrow still showed high cell density and leukemic proliferation at the time of intestinal perforation, the therapeutic regimen was changed to a combination of behenoyl are-C and mitoxantrone, and daily rhG-CSF was concurrently administered for the purpose of early establishment of bone marrow hypoplasia. On the 8th day after the therapeutic regimen had been changed, his bone marrow became nearly aplastic, and complete remission was obtained on the 24th day. This case may indicate that the concurrent administration of cell-cycle specific antileukemic drugs and rhG-CSF is available for AML patients with emergent need of leukemic cell reduction.

Decreased serum apolipoprotein B-100 and A-I concentrations in cows with ketosis and left displacement of the abomasum.

OBJECTIVE: To assess the relevance of hepatic lipidosis (fatty liver) in the development of ketosis and left displacement of the abomasum (LDA). SAMPLE POPULATION: Sera from 22 healthy cows in early lactation, 21 cows with ketosis, and 19 cows with LDA, and serum and liver specimens from 35 slaughtered cows with or without fatty liver, ketosis, and/or LDA. PROCEDURE: Apolipoprotein B-100 and A-I concentrations were measured in sera of healthy farm cows and of farm cows with ketosis and LDA. Serum apolipoprotein concentration, together with liver triglyceride content, also were surveyed in a subset of slaughtered cows. RESULTS: Compared with those in healthy cows or controls, apolipoprotein B-100 and A-I concentrations were decreased in cows with ketosis and LDA. CONCLUSIONS: Decreases in apolipoprotein B-100 and A-I concentrations in cows with ketosis and LDA indicate that the 2 disorders may be intimately associated with fatty liver. CLINICAL RELEVANCE: Monitoring of the apolipoprotein B-100 and A-I concentrations during the stages of nonlactation and early lactation is helpful for detecting cows susceptible to ketosis and LDA.

[Isolation and characterization of Epstein-Barr virus-infected clones of a human T-lymphoblastoid cell line].

Infection of the human T-cell line MT-2 by Epstein-Barr virus(EBV) recombinants with a hygromycin resistance gene and subsequent selection with this drug permitted isolation and long-term maintenance of EBV-infected MT-2 clones. These MT-2 clones harbor monoclonal episomes of EBV DNA and stably express two EBV latent proteins, EBV nuclear antigen 1(EBNA1) and latent membrane protein 1 (LMP1). Some MT-2 clones also express EBNA3C, yet none of the clones express EBNA2. This pattern of EBV gene expression is similar to that observed in most cases of EBV-positive peripheral T-cell lymphoma. In addition to these latent viral functions, expression of some early EBV replicative genes, including BZLF1, was also detected. EBV recombinants with positive selection markers will be a useful tool in establishing experimental systems of persistent EBV infection in certain non-B-cell lineages.

Trigonal splitting is a major complication of endoscopic trigonoplasty at 1-year followup.

PURPOSE: The long-term results of endoscopic trigonoplasty in patients with vesicoureteral reflux were investigated. MATERIALS AND METHODS: We performed endoscopic trigonoplasty in 28 patients with vesicoureteral reflux, including 6 girls 6 to 8 years old. Reflux was grade I in 14 reno-ureteral units, grade II in 15, grade III in 8, grade IV in 1 and grade V in 1. Median followup was 23 months. RESULTS: At a median of 3 months postoperatively vesicoureteral reflux was not apparent in 37 units (95%) and it was downgraded in 1 (3%). At a median of 12 months postoperatively 27 (79%) and 4 (12%) of the 34 ureters had cessation and downgrading, respectively. Of the 13 select patients who underwent cystoscopy 3 had retreating ureteral orifices and 5 had trigonal splitting 6 to 12 months postoperatively. Vesicoureteral reflux was demonstrated in 3 of the 5 patients (4 ureters) with trigonal splitting and in 1 (1 ureter) with retreating ureteral orifices. At a median of 24 months after surgery voiding cystography showed reflux in only 1 of 15 units. No postoperative upper urinary tract dilatation was noted at followup. CONCLUSIONS: Cessation rates of vesicoureteral reflux 12 months after endoscopic trigonoplasty were low with a high incidence of trigonal splitting. These unsatisfactory results mandate technical refinements to avoid trigonal splitting.

[Clinical study of primary central nervous system lymphoma: the role of chemotherapy].

We conducted a retrospective study of 32 patients with histologically confirmed primary central nervous system lymphoma treated in our institute between 1971 and 1995 with an emphasis on the role of chemotherapy. Thirty of the 32 patients underwent tumor resection, whereas 2 patients had biopsies only. Twenty-eight patients received adjuvant therapy, 9 of whom received radiation therapy alone, 2 received chemotherapy alone, and 17 received both radiation therapy and chemotherapy. Chemotherapies performed were CHOP (cyclophosphamide, doxorubicin, vincristine, and prednisolone). VEMP or VENP (vincristine, cyclophosphamide, mercaptopurine [or Natulan], and prednisolone), intravenous ACNU (nimustine), and intravenous or intra-arterial MCNU (ranimustine) and CBDCA (carboplatin). Survival data were available for 30 of the 32 patients. The median survival time of this study was 12.5 months. Twenty-seven patients died from one month through 79 months after the initiation of therapy, and 3 patients were alive for 13 to 69 months. Two patients who received the combination of radiation therapy and chemotherapy survived longer than 5 years. Although radiation therapy and chemotherapy were individually both effective and prolonged the survival time, their combination was more effective. The median survival time was significantly shorter (7.0 vs 16.5 months, p < 0.05) for the patients who received radiation therapy alone than for the patients who received the combination of radiation therapy and chemotherapy. We conclude from our results and review of previous studies that it is important for the chemotherapy of primary central nervous system lymphoma 1) to apply a combination of a variety of effective drugs, similar to that for systemic malignant lymphoma, and 2) to make a sufficient amount of anti-cancer drugs penetrate the whole central nervous system, thereby satisfying the adequate dose intensity for each drug.

[A case of vesical endometriosis associated with Wunderlich syndrome].

A case of vesical endometriosis associated with unilateral renal agenesis and uterus didelphys (Wunderlich syndrome) is reported. A 20-year-old unmarried woman visited the gynecologic department with dysmenorrhea. She was referred to us because a bladder mass was detected on abdominal ultrasonography. She had no episode of gross hematuria. Cystoscopy revealed a finger tip-sized cystic mass dark red to dark blue in color. Computerized tomography and magnetic resonance imaging demonstrated right renal agenesis and uterus didelphys. The mass was located at the ureterovesical pouch and protruded into the bladder. Partial cystectomy, right hysterectomy and right salpingo-oophorectomy were performed. Histopathological diagnosis was endometriosis. The patient was treated with 400 mg danazol for 6 months has had no recurrence and regular menstruation.

[Preoperative evaluation of the right ventricular function using the pulsed Doppler echocardiogram in the patients scheduled for the elective pulmonary resection].

Preoperative evaluation of the both right ventricular (RV) systolic and diastolic function using the pulsed Doppler echocardiogram were examined for 46 patients scheduled for the pulmonary resection of the pulmonary tumorous lesions. The parameters which included the RV inflow pattern at the tricuspid orifice and the RV ejection flow pattern at the RV outflow tract were obtained by the pulsed Doppler echocardiogram. Following results were obtained. 1) The RV afterload shown by the parameters of the acceleration time (AT, time beginning of RV ejection to peak velocity) were higher in the aged patients, the low FEV 1% populations, and the patients having the deteriorated left ventricular contraction. 2) The RV diastolic dysfunction were present preoperatively in aged patients over the 60 years old. This phenomenon was characterized by a high degree of atrial contraction and an increased ratio of the peak velocity in atrial contraction phase to that in early rapid filling phase (A/E). When we evaluate the RV inflow and ejection Doppler flow patterns after the major lung resection, these findings must be considerable thing.

Laparoscopic radical nephrectomy: the Nagoya experience.

PURPOSE: We evaluated the efficacy of laparoscopic radical nephrectomy for removing kidneys with small volume renal cell carcinoma. MATERIALS AND METHODS: 25 patients (19 men and 6 women, mean age 59) who had a kidney with small volume renal cell carcinoma (diameter less than 5 cm.), underwent laparoscopic radical nephrectomy. The kidney was dissected laparoscopically together with the adrenal gland, perirenal fatty tissue and Gerota's fascia. In 11 patients, we used the transperitoneal approach and in the remaining 14 we used the retroperitoneal approach, in which a working space is created by finger and balloon dissection. We maneuvered the kidney into the laparoscopy sack, which was then removed through an additional 5 to 6 cm. incision. RESULTS: All 25 kidneys were removed successfully. Mean operation time was 5.3 hours and mean estimated blood loss was 335 ml. There were 5 complications, including a patient who suffered an injury to the duodenum, which was treated by open duodenojejunostomy. Full convalescence occurred at an average 23 days. No metastatic disease, no local recurrence and no seeding at the port sites occurred during the followup of 7 to 49 months (mean, 22 months). CONCLUSIONS: Laparoscopic radical nephrectomy is recommended as a minimally invasive procedure for removing kidneys with small volume renal cell carcinoma.

[Intravenous neostigmine enhances the analgesic effect of epidural anesthesia].

A single-blind trial of the intravenous neostigmine on epidural anesthesia was carried out on 75 patients undergoing lower limb or lower abdominal surgery. They were allocated to three groups of 25: patients of group C received 2 ml of 0.9% saline, patients of group AN 1 ml (0.5 mg) of atropine and 2 ml (1 mg) of neostigmine, and patients of group N 2 ml (1 mg) of neostigmine, intravenously 5 min before epidural injection of 15 ml of 2% mepivacaine solution without epinephrine. We assessed the onset and spread of cold sensory block and analgesia, and the degree of motor block and sedation. The onset time of analgesia at T 11 was significantly shorter in group N [6.2 +/- 2.0 (SD) min] than in groups C (8.5 +/- 2.6 min) and AN (8.7 +/- 2.4 min). The segmental spread of analgesia 10 and 20 min after epidural injection was significantly larger in group N (10.8 +/- 3.9 and 15.4 +/- 2.7 seg, respectively) than in groups C (7.6 +/- 3.4 and 12.8 +/- 3.5 seg, respectively) and AN (7.8 +/- 3.6 and 13 +/- 3.4 seg, respectively). There were no differences in the onset and spread of cold sensory block, and the degree of motor block and sedation among the three groups. The incidence of bradycardia and fecal incontinence was significantly higher in group N than in groups C and AN. These results demonstrate that intravenous neostigmine potentiates the analgesic effect of epidural anesthesia mediated by a cholinergic muscarinic mechanism. However, in clinical practice, it does not seem to be useful, because of the side effects.

[Regional difference in the etiology of liver cirrhosis in Iwate].

To assess regional differences in the etiology of liver cirrhosis in Iwate, we analyzed 324 patients with liver cirrhosis treated at various hospitals. The etiology was HCV 44.8%, HBV 11.1%, HBV + HCV 4.6%, alcohol 27.5% (including heavy drinkers 17.9%), PBC 1.5% and non-B non-C 10.5% in Iwate. The incidence of alcoholic cirrhosis was higher than that in other prefectures, while that of HCV was lower. Especially in the northern area of Iwate, the rate of alcoholic cirrhosis was very high (39.1%--including heavy drinkers 21.8%) while viral cirrhosis was relatively low. Although the alcohol consumption volume in Iwate was not very high, marked alcohol consumption, especially shochu, was observed in the northern area of Iwate. The volume and kind of alcohol consumed in each area differed, and the etiology of liver cirrhosis differed regionally in Iwate. Thus, we should consider these districts and levels of alcohol consumption when treating patients with liver cirrhosis.

Helicobacter pylori infection in early gastric adenocarcinoma: relationship between histologic subtypes and ulcer-formation.

Early stage of gastric cancers were divided into two subtypes; differentiated and undifferentiated adenocarcinomas, histologically. We examined the involvement of Helicobacter pylori (Hp) infection in the development and progression of cancer, and presence or absence of peptic ulcer (UL+/UL-). From the results, these findings obtained as follows; 1) Hp positive rate of UL+ group was significantly higher than that of UL-group. 2) Neither of gross features nor depth of the tumor did not correlate with Hp positive rates. 3) Hp positive rate of undifferentiated type carcinoma was significantly higher than that of differentiated type, contrarily to our expectation. These findings suggested that Hp infection might relate with ulcer formation in the cancerous lesion. The hypothesis which is "gastritis-intestinal metaplasia-differentiated type carcinoma sequence" was not supported by present study. Hence, Hp infection was suggested as an important factor of the gastric cancer development and progression, not only in differentiated type but also in undifferentiated type.

[A case of Goodpasture's syndrome with myeloperoxidase specific anti-neutrophil cytoplasmic autoantibody (MPO-ANCA) during chronic interstitial pneumonia].

A 55-year-old man was admitted to our hospital with of hemoptysis, progression of anemia and renal failure in February, 1996. Idiopathic interstitial pneumonia had been diagnosed and he had been followed at a regional hospital since 1988. On the third day after admission, he suffered from sudden and massive hemoptysis. Goodpasture's syndrome was diagnosed because anti-GBM antibody was detected in serum. A high titer of MPO-ANCA was also recognized simultaneously. Steroid pulse therapy, immunosuppressive therapy, and plasmapheresis were begun, but he died on the 28th hospital day because of severe hypoxemia and multi-organ failure. Histological examination after autopsy revealed crescentic glomerulonephritis with linear deposition of IgG in the glomerular capillary wall, and interstitial pneumonia accompanied by massive alveolar hemorrhage. It was suggested that in this patient, not only anti-GBM antibody but also circulating MPO-ANCA might have participated in the progression of the crescentic glomerulonephritis and alveolar hemorrhage observed in Goodpasture's syndrome.

Cloning of the polyketide synthase gene atX from Aspergillus terreus and its identification as the 6-methylsalicylic acid synthase gene by heterologous expression.

Southern blot analysis of genomic DNAs of several fungi that produce polyketide compounds with the 6-methylsalicylic acid synthase (MSAS) gene of Penicillium patulum as a probe indicated the presence of an MSAS-homologous gene in the (+)-geodin-producing strain IMI 16,043 of Aspergillus terreus. The gene, designated atX was cloned from an A. terreus genomic DNA library and 7588 bp of the gene together with its flanking regions were sequenced to reveal the presence of a 5.5 kb open reading frame coding for a protein of 1800 amino acids with 190 kDa molecular mass. The presence of a short (70 bp) intron near the N-terminus of the atX gene was predicted that contains the canonical GT and AG dinucleotides at its 5'- and 3'-splicing junctions. The predicted ATX polypeptide showed high homology with P. patulum MSAS along the whole sequence. On the other hand, slight homology was detected only around the beta-ketoacyl synthase regions of Aspergillus nidulans wA, PKSST and Colletotrichum lagenarium PKS1. No transcription of atX was observed throughout the culture period by Northern blotting analysis. To identify the function of the polypeptide encoded by the atX gene, its coding region was introduced into the fungal expression vector pTAex3 under the control of the amyB promoter. The constructed expression plasmid was introduced into A. nidulans. The transformant produced significant amounts of 6-methylsalicylic acid, the structure of which was identified by physicochemical analysis. This result unambiguously demonstrated that the atX gene codes for MSAS of A. terreus.

Differential production of interleukin-6 and its close relation to liver metastasis in clones from murine P815 mastocytoma.

Interleukin-6 (IL-6)-producing abilities of plastic-adherent and plastic-non-adherent P815 clones were investigated in connection with the liver metastasis. Most adherent clones produced IL-6 at high level (over 10 ng/10(5) cells per 48 h), and they coincided with highly liver-metastatic clones. The remaining adherent and all the non-adherent clones tested produced IL-6 at low level (under 1.5 ng/10(5) cells), and they coincided with the low or non-liver-metastatic clones. The IL-6 production was greatly enhanced by IL-1 alpha, but not by IL-6, tumor necrosis factor-alpha or interferon-gamma.

Isolation of the active form of RAC-protein kinase (PKB/Akt) from transfected COS-7 cells treated with heat shock stress and effects of phosphatidylinositol 3,4,5-trisphosphate and phosphatidylinositol 4,5-bisphosphate on its enzyme activity.

RAC-protein kinase (PKB/Akt) has been shown to be activated by growth factor stimulation as a downstream target of phosphatidylinositol 3-kinase and also by heat shock through a pathway independent of phosphatidylinositol 3-kinase. RAC-protein kinase was purified by antibody affinity chromatography from COS-7 cells transfected with the epitope-tagged expression plasmid. The protein kinase activity of RAC-protein kinase purified from heat-treated cells was 9-fold higher than the enzyme isolated from untreated control cells. Phosphatidylinositol 3,4,5-trisphosphate did not enhance the activity of RAC-protein kinase purified from either heat-treated cells or control cells, whereas phosphatidylinositol 4,5-bisphosphate suppressed the enzyme isolated from heat-treated cells. These results indicate that RAC-protein kinase may interact with phosphoinositides, however, it could not be activated by simple association with the product of phosphatidylinositol 3-kinase reaction.

Translocation of PKN from the cytosol to the nucleus induced by stresses.

Effects of environmental stresses on the subcellular localization of PKN were investigated in NIH 3T3, BALB/c 3T3, and Rat-1 cells. The immunofluorescence of PKN resided prominently in the cytoplasmic region in nonstressed cells. When these cells were treated at 42 degrees C, there was a time-dependent decrease of the immunofluorescence of PKN in the cytoplasmic region that correlated with an increase within the nucleus as observed by confocal microscope. After incubation at 37 degrees C following beat shock, the immunofluorescence of PKN returned to the perinuclear and cytoplasmic regions from the nucleus. The nuclear translocation of PKN by heat shock was supported by the biochemical subcellular fractionation and immunoblotting. The nuclear localization of PKN was also observed when the cells were exposed to other stresses such as sodium arsenite and serum starvation. These results raise the possibility that there is a pathway mediating stress signals from the cytosol to the nucleus through PKN.
