Effects of lecithinized SOD on sequential change in SOD activity after cerebral contusion in rats.

To analyze the effect of lecithinized superoxide dismutase (SOD) on superoxide accumulation after traumatic brain injury (TBI) in rats, we studied the SOD activity by NBT-reducing method and the expression of Cu,Zn-SOD mRNA by Northern blot analysis. As determined by the specific gravity method, the administration of lecithinized SOD decreased brain edema in the periphery of the lesion at 6 hr after contusion. SOD activity, without lecithinized SOD administration, increased at the peripheral portion at 30 min after contusion, but decreased to normal level at 6 hr after TBI. By administration of lecithinized SOD, the increase of SOD activity was preserved until 6 hr after TBI. The expression of Cu,Zn-SOD mRNA increased in the core lesion, peripheral portion, and contralateral hemisphere until 6 hr after TBI, then was suppressed in all three areas by lecithinized SOD. These results support the hypothesis that superoxide anions may play an important role in the development of brain edema after TBI, and that leciyhinized SOD appears to prevent brain edema through a protective effect against superoxide anions.

Noncancerous pulmonary lesions in patients with metastatic testicular germ cell cancer.

By thoracoscopic surgery, we confirmed pulmonary noncancerous lesions in two patients with metastatic nonseminomatous testicular germ cell cancer. The purpose of thoracoscopic surgery was complete resection of postchemotherapeutic residual tumour in one patient or a biopsy for new lesions during chemotherapy. Histological findings were normal lymph node and pulmonary fibrosis. We demonstrated CT findings of these lesions and made some discussion on management of disseminated testicular cancer.

Estimation of canine prostatic volume: nomogram based on prostatic cubic volume.

To estimate prostate volume in canines, we demonstrated the nomogram based on prostatic cubic volume. Transrectal ultrasonography (TRUS) was performed in twenty beagle dogs. Prostatic length (L) and height (H) were determined on sagittal image, and prostatic width (W) was determined on transaxial image. Data of one dog, which had asymmetrical shaped prostate, were excluded from the analysis. Prostate volume was estimated by the following nomogram: Gravimetric prostatic volume (g) = 0.642 x prostatic cubic volume (cm3) + 1.84 (prostatic cubic volume = L x H x W cm3, r = 0.997, p<0.001). By substituting the prostatic height to the average of height, prostatic volume of one dog showing asymmetrical shape was successfully estimated. We also showed technical points in evaluating morphology of canine prostate on TRUS. In conclusion, this nomogram is of great use for directly estimating prostatic volume.

Effect of high glucose concentrations on prostacyclin-stimulating factor mRNA expression in cultured aortic smooth muscle cells.

Prostacyclin (PGI2) is a potent vasoactive prostanoid regulating vascular tone. We recently purified and cloned a PGI2-stimulating factor (PSF), which stimulates PGI2 production by vascular endothelial cells (ECs). Previous study demonstrated that PSF is predominantly located in vascular smooth muscle cells (SMCs) and present in serum. PSF may act on vascular ECs to regulate PGI2 synthesis for maintaining vessel wall homeostasis. Decreased PSF production in the vessel wall may result in an imbalance of prostanoid synthesis, leading to the development of vascular lesions such as diabetic angiopathy. In the present study, to investigate the regulatory mechanisms of PSF gene expression, we examined the effect of high glucose concentrations on PSF mRNA expression in cultured bovine aortic SMCs. Expression of PSF mRNA was significantly decreased to 66+/-6% of control value (p < 0.01), when the glucose level was raised from 5.5 to 27.8 mmol/l. We also examined the effect of osmolarity on PSF mRNA expression by addition of an appropriate dose of mannitol to the culture medium. We confirmed that high glucose concentration itself reduced the expression of PSF mRNA and glucose had much more effect than the osmolarity control. The expression of PSF mRNA was significantly decreased to 72+/-5% of control value (p < 0.05) by a protein kinase C (PKC) activator, phorbol-12-myristate-13-acetate (PMA). The decreased expression of PSF mRNA in the presence of high glucose or PMA was restored by co-incubation with a PKC-specific inhibitor (GF109203X). These results suggest that PSF gene expression in vascular SMCs may be decreased via a specific effect of high glucose concentrations. High glucose-induced activation of PKC is suggested to participate partly in the regulation of PSF gene expression.

A comparative study of physical work load in Japanese and Swedish nursery school teachers.

Comparisons were made of general and local physical work loads between two groups of 58 Japanese and 15 Swedish nursery school teachers. Heart rate, number of steps, rating of perceived exertion (RPE), frequency and time expended with respect to trunk flexion, sitting/kneeling, and lifting/carrying loads of 1 kg or more were monitored during working hours. The average percentage heart rate increase in the maximal heart rate range was lower in the Japanese than in the Swedish teachers. However, all parameters of local physical work load indicating musculoskeletal stress were higher in the Japanese than in the Swedish teachers. The RPE was slightly higher in the Japanese than in the Swedish teachers. These features of general and local work load in the Japanese teachers were typically observed among the teachers in charge of very young children (0-2 years). The Japanese teachers in charge of children aged 3-5 years, on the other hand, had similar levels of both general and local work load as the Swedish teachers in charge of classes comprising children of various ages (1-5 years). The physical work load measured in the present study was not sufficient to explain the difference in the magnitude of musculoskeletal problems for nursery school teachers in the two countries, and other factors should now be examined.

Increased mRNA expression of a novel prostacyclin-stimulating factor in human colon cancer.

We recently cloned a prostacyclin (PGl2)-stimulating factor (PSF), which stimulates PGl2 production by cultured vascular endothelial cells. Immunohistochemistry and Northern blot analysis demonstrated that PSF was highly expressed in colon cancer sites compared with normal colon mucosa obtained from the same patient, as well as in cultured adenocarcinoma cell lines compared with cultured normal colon mucosal cell lines. Increased levels of the PSF protein were detected in the culture media of these adenocarcinoma cells compared with levels in the culture media of normal mucosal cells. These results suggest that PSF is closely associated with carcinogenesis of colon mucosa.

Cerebellopontine angle lymphoma: a case report and review of the literature.

BACKGROUND: Although malignant lymphomas of the central nervous system have been reported to be increasing in frequency, cerebellopontine (CP) angle lymphoma is rare and only 13 cases have been reported previously in the literature. CASE PRESENTATION: A 63-year-old woman had progressive dizziness and nausea for 2 months. Computed tomography scanning and magnetic resonance imaging (MRI) revealed a mass lesion in the left CP angle, that was compressing the lateral-dorsal aspect of the pons and the fourth ventricle. This tumor was avascular on angiography. The tumor was surgically removed through a left lateral suboccipital approach. It was considered to arise from the subarachnoid space of the CP angle cistern. For some reason, the histologic diagnosis was not definitively made, and therefore radiation therapy was not planned. The tumor recurred within 50 days after the tumor excision. Surgical excision of the recurrent tumor was performed again. The histologic diagnosis was B-cell type malignant lymphoma. Radiation therapy was performed. In the 27 months since irradiation, a recurrent tumor has not been detected on MRI. CONCLUSIONS: Although erosion and expansion of the internal auditory canal suggest an acoustic neurinoma, CP angle lymphoma can, in rare circumstances, erode the internal auditory canal. There are three distinct patterns in which malignant lymphomas occupy the CP angle: (1) an extra-axial CP angle lymphoma, (2) an intra-axial lymphoma extending to the CP angle, and (3) a leptomeningeal lymphoma presenting as a CP angle lesion. Although malignant lymphomas rarely occupy the CP angle, it should be considered in the differential diagnosis of CP angle tumors. It is desirable to obtain a frozen section in all CP angle tumors during surgery to identify the tumor, because aggressive removal is not necessary, but radiation therapy should additionally be performed for malignant lymphomas.

Radiation-related adverse effects observed on neuro-imaging several years after radiosurgery for cerebral arteriovenous malformations.

BACKGROUND: To our knowledge, there are no reported arteriovenous malformation (AVM) series in which detailed long-term follow-up results after radiosurgery were described based on the whole patient group. METHOD: We performed a detailed long-term follow-up study of 53 patients with cerebral AVMs treated with gamma knife (GK) radiosurgery, with emphasis on radiation-related adverse effects detected on neuro-imaging after a long post-irradiation latency period (3-10 years). The post-GK follow-up period was 40-232 months excluding two mortalities, the mean being 112 and the median being 111 months. RESULTS: Three patients (5.6%) have, as yet, refused all neuro-imaging follow-up studies. Complete nidus obliteration was confirmed angiographically in 32 patients (60.4%) between 1 and 5 post-GK years. In the other 18 patients (34%), despite significant nidus shrinkage being angiographically demonstrated, complete obliteration was not achieved during a 2-7 year follow-up period. There were two mortalities, one AVM-related (massive re-bleeding during the latency period) and the other angiography-related. There were five radiation-related morbidities (9.4%), three of which-hemi-Parkinson syndrome, hemiparesis, and visual field disturbances attributable to delayed cyst formation-manifested at 5.5, 7 and 7 post-GK years, respectively. We also experienced five patients (9.4%) in whom, despite remaining asymptomatic to date, radiation-related adverse effects were seen on neuro-imaging: middle cerebral artery stenosis at 3 post-GK years in one patient; dural arteriovenous fistula at 7 post GK-years in one; delayed cyst formation in two, at 5 and 10 post-GK years; and a small cavitation at 9 post-GK years. CONCLUSION: Long-term follow-up, particularly with neuro-imaging modalities, is essential even after the "treatment goal" has been attained.

Laparoscopic nephrectomy for renal cell cancer: evaluation of efficacy and safety: a multicenter experience.

OBJECTIVES: Although laparoscopic radical nephrectomy is a safe and minimally invasive alternative to open surgery, the long-term disease-free outcome of this procedure has not been reported. We evaluated our experience with the laparoscopic management of renal cell carcinoma to assess the clinical efficacy of this surgical modality. METHODS: Between February 1991 and June 1997, 157 patients at five institutions were retrospectively identified who had clinically localized, pathologically confirmed, renal cell carcinoma and had undergone laparoscopic radical nephrectomy. Operative and clinical records were reviewed to determine morbidity, disease-free status, and cancer-specific survival. Of the patients followed up for at least 12 months (n = 101), 75% had an abdominal computed tomography scan at their last visit. RESULTS: The mean age at surgery was 61 years (range 27 to 92) and all patients were clinical Stage T1-2,NO,MO. Fifteen patients (9.6%) had perioperative complications. During a mean follow-up of 19.2 months (range 1 to 72; 51 patients with 2 years or more of follow-up), no patient developed a laparoscopic port site or renal fossa tumor recurrence. Four patients developed metastatic disease, and 1 patient developed a local recurrence. The 5-year actuarial disease-free rate was 91%+/-4.8 (SE). At last follow-up, there were no cancer-specific mortalities. CONCLUSIONS: The laparoscopic surgical management of localized renal cell carcinoma is feasible. Short-term results indicate that laparoscopic radical nephrectomy is not associated with an increased risk of port site or retroperitoneal recurrence. Longer follow-up is necessary to compare long-term survival and disease-free rates with those of open surgery.

Increased serum levels of advanced glycation end-products and diabetic complications.

We determined serum advanced glycation end-products (AGE) levels in patients with NIDDM and evaluated the relationship between these levels and diabetic complications. The subjects consisted of 125 patients (mean age, 59.2 +/- 11.1 years, duration of diabetes 11.6 +/- 8.9 years, mean HbA1c, 6.8 +/- 1.0%) with stable blood sugar control. Sixty-three healthy volunteers (mean age, 58.3 +/- 12.7 years) served as controls. Serum AGE were measured by a newly developed ELISA method. Serum AGE levels were significantly higher in the diabetic group compared with the normal control group (7.2 +/- 14.6 vs. 3.3 +/- 1.0 mU/ml, P < 0.05). Significant correlations were seen between serum AGE and the degree of diabetic nephropathy. Serum AGE levels of diabetic patients with proliferative retinopathy were significantly higher than those of patients without proliferative retinopathy (5.7 +/- 1.8 vs. 3.1 +/- 1.0 mU/ml, P < 0.025) in the patient groups whose serum creatinine levels were between 2.0 and 3.9 mg/dl, although serum creatinine levels of both groups were not significantly different. Serum AGE levels reflected the severity of diabetic complications, including nephropathy and retinopathy.

Effects of the Turkish subspecies of Viscum album on macrophage-derived cytokines.

Viscum album L. herbs are used for the treatment of various diseases. Apart from their immunostimulatory, antitumor, and hypotension inducing activities, anti-inflammatory effects has also been recorded in the literature. Since the immunostimulatory activity of mistletoe is mainly attributed to a stimulation of the mononuclear phagocytic system and to an induction of inflammation by macrophage-derived cytokines, utilization based on anti-inflammatory activity seems in contrast with its use as immunostimulant. The inhibitory effects of 80% ethanol extracts and subfractions obtained by petroleum ether, diethylether, ethylacetate and n-butanol from three Turkish subspecies of V. album L. (ssp. album, ssp. abietis, ssp. austriacum) against interleukins (IL-1alpha and IL-1beta) and tumor necrosis factor-alpha were studied. Ethanolic (80%) extracts of all three subspecies exhibited almost no significant inhibitory activity on the proinflammatory cytokines, at least in the concentrations applied in this study. In contrast, they induce a concentration dependent stimulation of these cytokines. On the other hand, moderate to weak, dose dependent inhibitory effects were observed on fractionation, which may be attributed to the existence of less polar components with anti-inflammatory activity. Based on our results, it was concluded that the utilization of ethanol extracts for anti-inflammatory purposes would not be beneficial probably due to the partial extraction of the low-molecular-weight polypeptides, i.e. viscotoxins, which induce an inflammatory response. Moreover, for anti-inflammatory effects, extracts of Viscum album ssp. austriacum may be preferred, since higher inhibitory ratios were obtained against inflammatory cytokines than with the other two subspecies.
