Demonstration of heterogeneity of autoantibodies to insulin receptors in type B insulin resistance by isoelectric focusing.

With isoelectric focusing, we examined heterogeneity of autoantibodies to insulin receptors in serums of two patients with insulin-resistant diabetes and one patient with hypoglycemia. Immunoglobulins were prepared by ammonium sulfate precipitation and ion-exchange chromatography with DEAE-Sepharose and subjected to isoelectric focusing for separation into 30 fractions. The fractions were tested for their ability to inhibit 125I-labeled insulin binding to human placental membranes, immunoprecipitate solubilized insulin receptor cross-linked with 125I-insulin, and mimic or inhibit the action of insulin in rat adipocytes. The results varied among the three patients. In the first patient, inhibition of 125I-insulin-binding activity (IBA) and insulin-receptor-precipitating activity (IPA) were distributed almost identically, but the distribution of insulinlike bioactivity (ILBA) was somewhat different. In the second patient, some fractions exhibited potent IBA without IPA, and these fractions inhibited the action of insulin in rat adipocytes. In the third patient, all of the isoelectric fractions showed IBA without IPA and were insulin antagonists. These observations indicate that some patients have antibodies with pure insulin-antagonist properties and provide further evidence that autoantibodies to insulin receptors are polyclonal and recognize different antigenic sites on insulin-receptor molecules. The findings also suggest that the ability of antibodies to elicit ILBA is linked to the ability to immunoprecipitate 125I-insulin-cross-linked and solubilized receptors, whereas antibodies that only inhibit insulin binding behave as insulin antagonists.

[Recent trends in the treatment of stomach carcinoma and its link with treatment of carcinoma of other organs].

The updated surgical modality for gastric carcinoma tends to be divided into an extended operation for advanced carcinomas and the limited surgery for early carcinomas. Moreover, the value of cancer chemotherapy for inoperable carcinomas and postoperative carcinomas has been recognized. The treatment efficacy of lymph nodal dissection of the para-abdominal aorta area is such that the 5-year survival in the curative resection of N(-)-N3(+) is 34.6%, and that in N4(+) is 19%. Therefore, it is concluded that lymph nodal dissection of the paraabdominal aorta area is significant to achieve better long-term survival. The 5-year survival rate in the adjuvant cancer chemotherapy group in cases of curatively resected Stage III carcinomas of the stomach was significantly superior to that in Stage III cases with surgery only.

Improvement of glucose tolerance in NIDDM by clofibrate. Randomized double-blind study.

A randomized double-blind study was performed to examine the effect of clofibrate on glucose tolerance in subjects with non-insulin-dependent diabetes mellitus (NIDDM). Clofibrate (1.5 g/day) or placebo was administered to 70 patients and an oral glucose tolerance test (OGTT) was performed before and 12 wk after treatment. Blood glucose levels were significantly improved in clofibrate-treated groups at all time points during OGTT, whereas there was no change in insulin levels. Improvement of fasting glucose levels required 8 wk of clofibrate treatment. Insulin binding to erythrocytes demonstrated no significant change in the clofibrate-treated subjects. These results suggest that clofibrate improves glucose tolerance in NIDDM subjects without a change in insulin receptors and that clofibrate increases insulin sensitivity through an unknown postreceptor mechanism.

[Post-treatment changes of skeletal Class III morphology operated by orthognathic two-jaw surgery].

The purpose of this study was to investigate the dentofacial changes following orthognathic Two-jaw surgery in skeletal Class III patients. The materials consisted of cephalometric head films of 20 patients who had simultaneously undergone Le Fort I osteotomy and mandibular ramus osteotomy. All cases received the pre- and post-operative orthodontic treatment and had the observation periods over than one year after postoperative orthodontic treatment. The cephalometric radiographs were taken at following five stages: 1. Before preoperative orthodontic treatment, 2. Immediately before surgery, 3. After intermaxillary fixation, 4. After postoperative orthodontic treatment, and 5. Posttreatment observation period. The results were as follows: 1. The anteroposterior skeletal relapse was minimum after surgery. 2. The surgical manner of the maxillary vertical displacement did not effect the stability of the upper facial height after surgery. The lower facial height tended to decrease slightly during postoperative orthodontic treatment. 3. The upper and lower incisors showed lingual tipping during intermaxillary fixation, but the upper incisors tipped labially during retention period. In conclusions, Two-jaw surgery was considered to be quite effective for the correction of skeletal Class III malocclusions especially with the maxillary deficiency.

Hepatic resection for hepatocellular carcinoma. Clinical features and long-term prognosis.

Ninety-eight hepatic resections for hepatocellular carcinoma were performed on 94 patients from 1963-1985. HBs antigen was positive in 17% of patients, preoperative serum alpha-fetoprotein was more than 20 ng/mL in 70% of patients, and liver cirrhosis was present in 75% of patients. Hospital mortality rate was 19%, and the volume of operative blood loss was the most decisive factor that affected the short-term prognosis. Excluding the 19 hospital deaths, the long-term survival rates of 75 patients were 73%, 42%, and 25% for 1, 3, and 5 years, respectively. Prognostic factors that influenced the long-term prognosis were investigated by comparing the survival curves. Significant differences of survival patterns were noted when analyzed on the basis of preoperative alpha-fetoprotein level (less than or equal to 200 vs. greater than 200 ng/mL), tumor size (less than or equal to 5 vs. greater than 5 cm), and tumor capsule. The recurrence of carcinoma was the main cause of death in 56% (42 patients) who died after discharge from the hospital. The development of effective prevention and treatment against recurrent tumors is necessary to improve long-term prognosis.

Histopathologic observations on the senescence-accelerated mice (SAM) reared under specific pathogen free conditions.

Five to 12 months old senescence-accelerated mice reared under specific pathogen free conditions (SPF SAM) were subjected to histopathologic examination. Senescence-prone SAM-P/1/Ta, P/3/Ta, and P/8/Ta exhibited marked depletion in thymic cortical lymphocytes which were associated with lymph follicle formation in the medulla, and marked infiltration of lymphocytes in the submaxillary gland, kidney, and other peripheral tissues. Morphometrical analysis of the adrenal gland revealed cortical atrophy and medullary hyperplasia in male SAM-P/8/Ta at 5 months of age. Other characteristic changes were marked vacuolation of the renal proximal tubules in male P/3/Ta and P/8/Ta mice, and degeneration of vascular wall in the testis of P/3/Ta mice. The pathogenesis of these changes appeared to be related to the adrenal lesions. Systemic amyloid deposition, one of the characteristic features of SAM housed in conventional facilities was not observed under the SPF condition. Acceleration of senescence in SPF SAM may arise from disorders of the thymus and adrenal.

Urinalysis for detection of chemically induced renal damage (1)--Changes in urinary excretions of enzymes and various components caused by mercuric chloride and gentamicin.

In order to establish sensitive methods of detecting minor renal damage, changes of enzymes, tubular cell counts, and creatinine in the urine were investigated in rats that had been given nephrotoxic chemicals. Daily administration of mercuric chloride (HgCl2) dose-dependently increased urinary excretions of lactate dehydrogenase (LDH), aspartate aminotransferase (GOT), alkaline phosphatase (ALP), leucine aminopeptidase (LAP), lysozyme (LZM), N-acetyl-beta-D-glucosaminidase (NAG), and acid protease together with increased counts of tubular cells in the urine. The increase in tubular cell counts and the change in urinary LDH isoenzyme profile preceded the changes in the other enzymes. Daily administration of gentamicin (GM) increased urinary excretions of LDH, GOT, LZM, NAG, acid protease and tubular cell counts in a dose-dependent manner, but did not increase gamma-glutamyl transpeptidase (gamma-GTP) and ALP excretions. The urinary isoenzyme profiles of LDH in rats treated with GM were different from those with HgCl2. The increase in acid protease excretion outlasted those in LDH and GOT in the high dose group. It was concluded that the severity of renal damage can be readily detected by periodic determinations of the following urinary parameters: tubular cell counts, LDH isoenzyme, acid protease, LZM and NAG, in addition to either LDH or GOT and one of the enzymes ALP, LAP or gamma-GTP. Furthermore, the site of renal damage can be presumed from these results.

Urinalysis for detection of chemically induced renal damage (2)--Changes in urinary excretions of enzymes and various components caused by p-aminophenol, puromycin aminonucleoside and hexadimethrine.

In order to establish sensitive methods of detecting minor renal damage, changes of enzymes, protein, tubular cell counts, and creatinine in the urine were investigated in rats to which nephrotoxic chemicals had been administered. Daily administration of p-aminophenol (PAP) dose-dependently increased urinary excretions of lactate dehydrogenase (LDH) and its isoenzymes (LDH5 = LDH4 greater than LDH3 greater than LDH2 = LDH1), aspartate aminotransferase (GOT), alkaline phosphatase (ALP), gamma-glutamyl transpeptidase (gamma-GTP), leucine aminopeptidase (LAP), lysozyme (LZM), N-acetyl-beta-D-glucosaminidase (NAG) and acid protease together with increased counts of tubular cells in the urine. Tubular cell counts, LDH and GOT were more sensitive indicators in the PAP tubulonephritis. Single i.v. injection of puromycin aminonucleoside (PM) dose-dependently increased urinary excretions of LDH and its isoenzymes (LDH1 = LDH5 greater than LDH2 = LDH4 greater than LDH3), GOT, NAG, acid protease and protein but degree of the increases in these enzymes was lower than those in the rats treated with PAP. PM increased excretions of high molecular weight proteins but did not increase ALP, gamma-GTP, LAP, LZM and tubular cells excretions. Single i.v. injection of hexadimethrine increased urinary excretion of LDH and its isoenzymes (LDH1 = LDH5 greater than LDH2 greater than LDH3 = LDH4), GOT, LZM, NAG and acid protease together with increased counts of tubular cells in the urine but did not increase ALP, gamma-GTP and LAP excretions. It is concluded that tubular cell counts, LDH isoenzymes and battery of these enzymes in urine are useful markers for detecting the severity and the site of renal damage in addition that urinary protein is a useful marker for detecting glomerular damage.

[Long-term follow-up study of renal autotransplantation in reno-vascular hypertension].

Therapeutic effects of heterotopic renal transplantation for renovascular hypertension were evaluated in ten patients with a follow-up period of more than three years. These ten patients consisted of 4 males and 6 females with the range of age from 21 to 55. The causes of renovascular hypertension were fibromuscular dysplasia in five patients, atherosclerosis in 3 and aortitis syndrome in 2. All the ten patients had a unilateral stenosis of the renal artery. At 3 years after operation, the blood pressure fell to normal range without any medication in 9 patients (90%) and it could be controlled with antihypertensive drugs in the remaining one patient. Operative mortality or deterioration of graft function was not experienced in any of them. One of the female patients had three deliveries after operation. The heterotopic renal autotransplantation is a therapeutic method to be selected for renovascular hypertension, since excellent and stable long-term results can be achieved.

Different effects of chemicals on metabolism of N-nitrosamines in rat liver.

The effects of phenobarbital (PB), 3-methylcholanthrene (MC), pyrazole (PY) and ethanol (EtOH) pretreatment on N-nitrosodimethylamine (NDMA), N-nitrosobutylmethylamine (NBMA) and N-nitrosomethylbenzylamine (NMBzA) metabolism were examined in rats. In isolated hepatocytes, PB increased the metabolic decomposition of NBMA and NMBzA, and MC increased that of NBMA; PY and EtOH increased only that of NDMA. In studies of hepatic microsomal dealkylation, PB increased NBMA debutylation and NMBzA debenzylation, and MC increased NBMA debutylation; PY and EtOH increased NDMA demethylation selectively. Several cytochrome P450 (P450) species were active in dealkylating nitrosamines, indicating that the organ-specific carcinogenicity of nitrosamines might be changed by various P450 inducers.

[A successful cross-over femorotibial bypass for severe ischemic leg of thromboangiitis obliterans].

A case of thromboangiitis obliterans (TAO) successfully treated with a modified vascular reconstruction is reported. A 53 year-old man who had undergone lumber sympathectomy 9 years ago visited our hospital with complaints of rest pain and intractable ulcer at the right big toe. Preoperative angiography could not visualize distal arteries from the right common iliac artery and major amputation above knee was intended. Operative angiography, however, revealed collateral pathways from the posterior tibial artery to plantar arteries. Then, a long bypass from the left common femoral artery to the right posterior tibial artery was made using saphenous vein grafts of both extremities, 75 cm in length. Postoperatively, a solution containing urokinase, PGE1 and heparin was infused continuously for two months through a tube inserted into a branch of the graft. The pain disappeared and the ulcer healed. Now one year after the operation, the bypass graft is patent and the patient is fully rehabilitated. This experience indicates that some of ischemic legs with poor run off due to TAO can be salvaged by such modified vascular reconstruction and postoperative local PGE1 infusion therapy.
