[Study of the interrelation between variation of blood pressure and pulse rate under various respiratory conditions].

By means of non-invasive continuous blood pressure measurement (Finapres method), coefficient of variation for pulse rate (CV-PR) and coefficient of variation for blood pressure (CV-BP) under various respiratory conditions were calculated simultaneously to examine the interrelation between them. The subjects were 148 healthy normal adults and 75 patients with diabetes mellitus (DM). Both coefficients of variation for healthy normal adults showed significantly (p < 0.01) positive correlations: correlation coefficients tended to be greater during deep breathing than that during resting breathing. The case was the same for the ratio of CV-PR to CV-BP (CV-PR/CV-BP): the ratio for deep breathing was approximately 1.0. The ratio showed a significant decrease for DM patients compared with that for healthy normal group. The examination of the interrelation between these coefficients of variation under various respiratory conditions, which reflected the sensitivity of cardiovascular vagal reflex was considered to be also useful for the judgment of the degree of progress of DM-complications.

[Small airway obstruction in patients with idiopathic interstitial pneumonia (IIP)--comparison between acute and chronic types].

Small airway obstruction in patients with acute and chronic interstitial pneumonia was studied. Patients with acute interstitial pneumonia (IP, n = 3), and with acute exacerbation of chronic IP (n = 4) were studied as a group, and were compared to patients with chronic idiopathic interstitial pneumonia (IIP, n = 41). Small airway obstruction was defined on a logarithmic %VC-V25/VC plane, because the V25/VC (an index of both small airway obstruction and decreased lung compliance) depended on %VC. Six of seven patients (86%) with acute IP and 8 of 42 patients (19.5%) with chronic IIP had small airway obstruction (p < 0.01). Small airway obstruction was often seen in acute IP, but was not often seen in chronic IIP.

[Long-term study of patients with sequelae of pulmonary tuberculosis after pneumonectomy--obstructive impairment and its causes].

Changes in pulmonary function, chest X-ray films, and CT were retrospectively studied in patients with sequeae of pulmonary tuberculosis 30 years after pneumonectomy (PNX). Over more than 10 years, VC and FEV1% changed at a rate of 23.2 +/- 8.8 ml and 0.683 +/- 0.688% per year, respectively. In the patients studied 30 years after PNX (N = 30), VC was 900 +/- 210 ml, %VC 31.6 +/- 8.1%, and FEV1% was 67.2 +/- 6.8%. About half of these patients had mild airway obstruction. FEV1% was negatively related to the distance from the hilum to the diaphragm on the mid-clavicular line (on the chest films) divided by body height (r = -0.607). Therefore, we think that the airway obstruction may be due to overextension of the bronchi of middle and lower lobes. Emphysema was not found in 18 patients who had no sign of hyperinflation or herniation of the remaining lung to the PNX side on their CT, but it was found in one patient with these signs.

[Long-term study of patients with pulmonary tuberculosis sequelae after bilateral thoracoplasty-airway obstruction and its causes].

Chest roentgenograms and results of pulmonary-function tests in patients with pulmonary tuberculosis sequelae 30 years after bilateral thoracoplasty were studied retrospectively to detect airway obstruction in these patients and to determine its causes. For periods of more than 10 years, vital capacity (VC) changed at a rate of 15.5 +/- 5.0 ml/year, and forced expiratory volume in one second as a percent of VC (FEV1%) changed at a rate of 0.546% +/- 0.380% per year (n = 13). Thirty years after thoracoplasty, the VC was 920 +/- 180 ml (%VC = 28.4% +/- 5.3%), and the FEV1% was 66.2% +/- 13.7% (n = 21). Thus, mild airway obstruction was found in about half of the cases. For each patient, the distance from the hilum to the diaphragm was measured along the mid-clavicular line on the side with fewer ribs resected, and this distance was divided by the patient's height. The results of that computation was found to be significantly and negatively related to FEV1% (r = -0.681, which suggests that longer bronchi in the lower and middle lobes on that side were associated with lower values of FEV1%. These findings are similar to those in patients with pulmonary tuberculous sequelae after total pneumonectomy. Over an average of 26 years, scoliosis, the vertebra showing the most bending, the intrapulmonary lesion, and the position of the diaphragm did not change, but the cardio-thoracic ratio increased.

Temperature shift-up leads to simultaneous and continuous plasmid DNA relaxation and induction of DnaK and GroEL proteins in anaerobically growing Escherichia coli cells.

We previously reported that plasmid DNA in Escherichia coli cells growing under aerobic conditions relaxed immediately and transiently after heat shock (Mizushima, T., Natori, S. and Sekimizu, K., Mol. Gen. Genet. (1993) 238, 1-5). We next examined DNA relaxation and induction of heat shock proteins after heat shock in cells growing under anaerobic conditions. DNA in these cells relaxed rapidly (2 min) after heat shock (42 degrees C), as was the case with aerobically growing cells, but full superhelicity was not recovered. The relaxed state of DNA topology was maintained for 60 min after heat shock. Induction of DnaK and GroEL proteins, which was transient in aerobically growing cells, was continuous in anaerobically growing cells. Therefore, induction of heat shock proteins correlated with DNA relaxation in both aerobic and anaerobic conditions.

Effect of thermal stress on glutathione metabolism in human erythrocytes.

This is the first experiment to investigate the effect of heat and cold stress on glutathione metabolism in human erythrocytes. We immersed men at three different water temperatures for 10 min. At 39 degrees C, no remarkable changes were observed. Levels of glutathione (GSH) decreased from 2.44 (0.14) to 1.80 (0.10) mumol.ml red blood cells-1 [mumol.ml RBC-1; mean (SEM); P < 0.0005] and those of lipid peroxides increased from 1.87 (0.03) to 2.06 (0.04) nmol.ml RBC-1 (P < 0.01) after the immersion at 42 degrees C. In contrast, levels of GSH increased from 2.46 (0.17) to 2.91 (0.17) mumol.ml RBC-1 (P < 0.05) and those of lipid peroxides did not change after the immersion at 25 degrees C. The activities of glutathione peroxidase decreased from 35.90 (1.83) to 34.33 (1.66) IU.g Hb-1 (P < 0.01) after the immersion at 42 degrees C; however, these activities did not change after the immersion at 25 degrees C. The activities of glutathione reductase (both active and inactive forms) showed no changes at any temperatures. These changes indicate that heat stress causes oxidative stress in the human body; however, cold stress is thought to augment the activity of the antioxidative defence system. It is suggested that body exposure to hot environmental conditions should not be recommended for patients suffering from a damaged antioxidative defence system.

Docosahexaenoic acid status of patients with extrahepatic biliary atresia.

Docosahexaenoic acid (DHA) is believed to be an important long-chain polyunsaturated fatty acid (LCPUFA), which may be essential for neurofunction in infants. Patients with extrahepatic biliary atresia (EBA) may have DHA deficiency secondary to fat malabsorption. The authors investigated DHA and other LCPUFA levels in plasma and red blood cell (RBC) phospholipids of patients after the Kasai portoenterostomy and after supplementation with essential fatty acids. Ten children aged 8 to 17 months (mean, 12.6 months) comprised the study group. Five were jaundiced and five had a normal bilirubin level. The patients received 1 mL/kg of fat emulsions (10% Intralipid, containing 50% linoleic acid and 9% alpha-linolenic acid) in addition to an age-appropriate diet. Additional supplements were ursodeoxycholic acid (UDCA) (15 mg/kg/d) and taurine (100 mg/kg/d). The percentages of DHA in both plasma and RBC phospholipids of patients in the jaundiced group were significantly lower than those of normal children. Patients in the jaundice-free group had significantly lower levels of DHA and higher levels of linoleic acid in both plasma and RBC phospholipids in comparison to the normal group. This study shows that postoperative EBA patient become DHA-deficient even when supplemented with fat emulsions (largely composed of linoleic acid) that contain DHA's precursor, alpha-linolenic acid. This demonstrates a deficiency in the long-chain acid desaturase activity of these patients. It is recommended that excessive/linoleic acid intake be avoided and that all EBA patients have small amounts of DHA added to their lipid supplementation.

Effects of ursodeoxycholic acid treatment on essential fatty acid deficiency in patients with biliary atresia.

To assess whether ursodeoxycholic acid (UDCA) treatment has any beneficial effect on essential fatty acid (EFA) deficiency in patients who have had a Kasai operation for extrahepatic atresia (EBA), responses of serum fatty acids to UDCA administration (15 mg/kg/d) were investigated in eight jaundice-free patients and in eight patients with jaundice (serum total bilirubin > or = 1.0 mg/dL). All patients were also given taurine supplementation (100 mg/kg/d). Serum fatty acid composition was determined before and 6 months after UDCA treatment. Serum total bile acid concentration and serum total bilirubin value, as a part of conventional liver function tests, were measured before and during UDCA therapy. Before UDCA treatment, the concentrations of linoleic acid and arachidonic acid were significantly lower (P > .05 for the former; P > .01 for the latter) in both the jaundice and jaundice-free groups than in the controls. After 6 months of treatment, the linoleic acid concentration significantly increased (P > .05), to the normal range, in the jaundice-free group, but not in the jaundice group. The arachidonic acid concentration did not increase significantly in either group. The serum total bile acid concentration was lower in six of the eight jaundice-free patients and in four of the eight jaundice patients. The serum total bilirubin value decreased in six of the eight jaundice-free patients and in four of the eight jaundice patients; however, the degree of improvement was not statistically significant in either group. No side effects developed, and there were no changes in blood chemistry values unrelated to liver disease.(ABSTRACT TRUNCATED AT 250 WORDS)

Thin hydroxyapatite coating produced by the ion beam dynamic mixing method.

An ion beam dynamic mixing (IBDM) method was used to produce thin hydroxyapatite coatings on titanium substrates. Structure, solubility and bond strength of coatings were evaluated. The hydroxyapatite-type structure was obtained from heat-treated coatings. As-coated coatings seemed to be almost dissolved within 1 d in Hanks' solution. The thin layer of films, however, was recognized with a firm intermediate compound (such as Ti3P4) to the substrate by X-ray photoelectron spectroscopy, indicating the high tensile bond strengths from the as-coated films. Although slight reductions of the bond strength were observed in heat-treated specimens, the strengths were not reduced by immersion in Hanks' solution for 30 d.

The regulation of intestinal hypersensitivity reactions to ovalbumin by omega-3 fatty acid enriched diet: studies of IEL and LPL in mucosal damage.

In order to clarify the mechanisms of food-sensitive enteropathy, a food hypersensitive model was generated by feeding ovalbumin to female BALB/c mice after intraperitoneal injection of cyclophosphamide and morphological and immunological changes in the gut mucosa were investigated. Villus atrophy, crypt hyperplasia and increased numbers of intra-epithelial lymphocytes (IEL) were confirmed in this model, as seen in food-sensitive enteropathy in humans. Subpopulations of IEL and lamina propria lymphocytes were enumerated by immunohistochemical observation. CD8-positive cells were increased both in epithelium and lamina propria, whereas CD4-positive cells were decreased in lamina propria. We document here that orally administered food antigen actually induces food-sensitive enteropathy and mucosal damage is generated by lymphocytes that infiltrate the intestinal mucosa. We also investigated the effect of feeding an omega-3 fatty acid-enriched diet in this model and found that it was efficient in attenuating mucosal damage.

Clinical characteristics of idiopathic interstitial pneumonia (IIP) with bullae.

Multiple bullae have often been observed in the lungs of patients with idiopathic interstitial pneumonia (IIP). The etiology of bullae has been considered to be distinct from that of interstitial pneumonia and honeycomb lesions seen in IIP. The incidence of bullae was evaluated in 27 IIP patients (male; 23 cases, female; 4 cases) and clinical factors which are related to the development of bulla were analyzed. Of the 27 patients, 16 (59%) had bullae. All 16 were male smokers, and had higher smoking indices (p < 0.01) than patients without bullae. Eleven of the 16 IIP patients with bullae had clubbed fingers (69%, p < 0.05). Roentgenologically, cysts (75%, p < 0.05), low attenuation area (56%, p = 0.06), and honeycomb appearance (94%, p = 0.07) were observed more often in IIP patients with bullae. These results suggest that smoking habits may have a close relationship to the development of bullae seen in patients with IIP.

Scoliosis associated with syringomyelia.

From January 1986 to December 1991, 1,059 patients with scoliosis of 20 degrees or more were reviewed. By magnetic resonance imaging, 43 patients were found to have syringomyelia. Thirty-eight patients had Chiari type 1 malformation. Twenty-two of the patients were younger than 10 years. Scoliosis was the only presenting symptom in 32 patients. All patients had some neurologic abnormalities. Fifteen patients had left thoracic curves. The incidence of scoliosis with syringomyelia in our outpatient clinic was 4.0%; 18.4% in boys, and 2.6% in girls. Magnetic resonance imaging should be performed in patients who have abnormal neurologic findings.

Myoclonus epilepsy with ragged-red fibers: a clinical and electrophysiologic follow-up study on two sibling cases.

We performed a 5-year clinical and electrophysiologic follow-up study on two sibling cases with myoclonus epilepsy with ragged-red fibers. Both had myoclonus, intention tremor, slight muscle weakness, slight mental disturbance, hearing impairment, and optic atrophy. Neither had epileptic attacks or truncal or gait ataxia. Biochemical activity of cytochrome c oxidase was at the lower limit of the normal range of values, and an adenine to guanine transition mutation at nucleotide 8344 in the transfer RNA specific for lysine of mitochondrial DNA was detected in both cases. The electroencephalograms showed slowing of basic patterns, diffuse spike-and-wave complexes, occipital dominant wave-and-spike phantoms, 6- and 14-Hz positive spikes, and photosensitivity. No definite deterioration of basic patterns was seen, and diffuse spike-and-wave complexes and photosensitivity gradually disappeared during the slowly progressive clinical course. P2 latencies of pattern-reversal visual evoked potentials throughout the clinical course and III through V interpeak latencies of auditory brainstem responses at follow-up were prolonged without giant sensory evoked potentials in both cases.

Immunohistochemical studies on small intestinal mucosa in Kawasaki disease.

To investigate the etiology of Kawasaki disease (KD), the cell surface phenotypes of mononuclear cells and enterocytes in the jejunal mucosa of KD were investigated in a case-control study. Sixteen Japanese patients with KD were enrolled in the study. As disease controls, jejunal tissues from 10 patients with diarrhea due to cow's milk protein intolerance were used. The numbers of cells stained by an immunofluorescent technique were counted and analyzed statistically by t test. Both HLA-DR+CD3+ and DR+CD4+ cells were significantly increased in the lamina propria of KD patients in the acute phase compared with numbers in controls and patients with cow's milk protein intolerance (p < or = 0.01). CD8+ cells were significantly reduced in both the epithelium and the lamina propria of KD patients in the acute phase in comparison with numbers in both controls (p < or = 0.05) and patients with cow's milk protein intolerance (p < or = 0.01). HLA-DR+ cells were significantly increased in both the enterocytes and the lamina propria of KD patients in the acute phase compared with numbers in controls (p < or = 0.01). These cell patterns returned to normal in the convalescent phase of KD. Differences between these cell patterns in KD patients with and without diarrhea were not significant, although these immunohistochemical features tended to be more marked in patients with diarrhea than in those who lacked diarrhea. These results are consistent with what one would expect to find if a delayed-type hypersensitivity reaction had occurred in the small intestinal mucosa of KD patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Squamous cell carcinoma showing lobar consolidation without collapse.

Most squamous cell carcinoma of the lung arises in the large bronchi and causes bronchial obstruction. We report a 74-year-old man with a squamous cell carcinoma of the lung whose roentgenogram revealed a lobar consolidation without collapse. A postmortem examination showed that the tumor cells filled the alveolar spaces completely without destroying the original architecture of the lung. The finding that tumor cells replacing alveolar epithelium, as in bronchioloalveolar cell carcinoma, was not seen throughout the lung. Airway obstruction by the tumor was not observed at any bronchus. These pathological findings seemed to explain the radiological findings.

[Bronchial asthma--airway inflammation and asthma--role of airway epithelium].

The importance of airway inflammation in the pathophysiology of bronchial asthma has been recognized recently. This inflammation is organized by the interaction between inflammatory cells and the cells which compose airway structures. We studied the role of airway epithelium in airway hyperresponsiveness and airway inflammation, which are the major characteristics of bronchial asthma, and demonstrated a close relationship between them. On the bases of these data and the data reported by many other researchers, and from the point of view of airway inflammation, we discussed the possible roles of airway epithelium in bronchial asthma.
