Angiotropic lymphoma with histologic features of neoplastic angioendotheliomatosis presenting with predominant respiratory and hematologic manifestations. Report of a case and review of the literature [corrected].

Neoplastic angioendotheliomatosis (NAE) is a rare fatal disease characterized by widespread intravascular proliferations of neoplastic mononuclear cells. Clinically, dermatologic and bizarre neurologic manifestations usually predominate. The origin of the neoplastic cells remains still undetermined. The authors report a patient with NAE peculiar with respect to the following points: (1) the patient predominantly manifested respiratory symptoms and hematologic findings and lacked cutaneous or neurologic manifestations; and (2) immunohistochemical and molecular genetic studies showed the B-cell nature of the neoplastic cells, although previous cases with predominant respiratory or hematologic manifestations were reported to be of endothelial origin. Despite the rarity, this type of NAE or angiotropic [corrected] lymphoma should be recognized because it is easily confused with other disorders, particularly vasculitis or thrombotic thrombocytopenic purpura.

A histopathological analysis of five cases of adenomatous hyperplasia containing minute hepatocellular carcinoma.

We present five cases of adenomatous hyperplasia (AH) containing minute hepatocellular carcinoma (HCC) in cirrhotic liver. All the patients were Japanese, four males and one female, ranging in age from 60 to 80 years. Two of the specimens were obtained at surgery and the others at autopsy. The AH specimens ranged from 2.0 to 3.0 cm in diameter, and the maximum diameter of HCC foci in the AH was 2.0 cm. Histologically, apart from the HCC foci, the AH specimens showed intrinsic atypia, suggesting preneoplastic change. These features included an increase of both cellularity and the nucleo-cytoplasmic ratio, distortion of cord structure and pseudoacinar formation. All of the AH specimens contained typical portal triads. Details of diagnostic imaging were also obtained in four cases. The findings of the present study support the possibility that AH with intrinsic atypia is a preneoplastic lesion of HCC. The sequence of "adenomatous hyperplasia with intrinsic atypia-HCC foci" would thus represent part of the early phase of hepatocarcinogenesis in humans.

Studies on the activity of phospholipase D with alkyl phosphorylcholines.

The activity of phospholipase D from the Savoy cabbage on alkyl phosphorylcholines containing alkyl groups of C14 to C22 was examined. Apparent Km and maximal velocity values in the presence of diethyl ester were 3.6-8.3 mM and 0.14-0.29 mumol/(min.mg protein), respectively. The maximal velocities for alkyl phosphorylcholine homologues were lower than the value for pig phosphatidylcholines, but higher than those for lysophosphatidylcholines and synthetic phosphatidylcholine with saturated acyl chains. Results suggest that solubilities of the substrates in the organic solvent play an important role in activity observed with phospholipase D.

Medroxyprogesterone acetate binding sites in human endometrium and endometrial cancer.

Medroxyprogesterone acetate (MPA) binding sites in both human normal endometrium and endometrial carcinoma were identified and characterized by sucrose gradient centrifugation. These binding components were divided into two classes by saturation analysis, one with high affinity and low capacity and the other with low affinity and high capacity. The concentrations of low-affinity binding sites for MPA in endometrial carcinoma were higher than those in normal endometrium (p less than 0.01). By sucrose gradient centrifugation, 4S and 8S components were observed in both high- and low-affinity binding sites of normal endometrium. These components were moved to 4S by the addition of salt. However, in endometrial carcinoma, low-affinity binding sites were displayed at about 4S under either low- or high-salt conditions. High-affinity binding sites in endometrial carcinoma had the same sedimentation patterns as in normal endometrium. An obvious difference between normal endometrium and endometrial cancer was observed in low-affinity binding sites. Our results on the binding sites for MPA suggest that low-affinity binding sites may be related to the response of endometrial cancer to high-dose MPA treatment.

Phytogrowth-inhibitory and antibacterial activities of 2,5-dihydroxy-1,4-dithiane and its derivatives.

2,5-Dihydroxy-1,4-dithiane (I) and its derivatives (II-IV) showed rather marked inhibitory activities on the growth of the roots of two plant species. All compounds tested had phytogrowth-inhibitory activities. These compounds markedly inhibited the growth of the two plant species at the concentration of 1.0 x 10(-3) M. Seeds of Brassica rapa treated with 2,5-dihydroxy-2,5-dimethyl-1,4-dithiane (III) and its diacetate (IV) at the same concentration failed to germinate. Among these compounds, IV showed the most potent inhibitory activity on the two plant species. The radicles of both plant species treated with these compounds at concentrations higher than 1.0 x 10(-4) M showed negative geotropism, even though germination occurred. The compounds except for 2,5-diacetoxy-1,4-dithiane (II) also had antibacterial activities. In particular, III had rather marked antibacterial activity and its minimal inhibitory concentration (MIC) for Staphylococcus aureus IFO-3060 and Escherichia coli IFO-12734 was 4.0 micrograms/ml.

Pharmacological actions of SM-9018, a new neuroleptic drug with both potent 5-hydroxytryptamine2 and dopamine2 antagonistic actions.

Pharmacological studies were undertaken to clarify the profile of cis-2-(4-(4-(1,2-benzisothiazol-3-yl)-1-piperazinyl) butyl) hexahydro-1H-isoindole-1,3-(2H)-dione hydrochloride (SM-9018), a new neuroleptic drug. SM-9018 had very high binding affinities for both 5-hydroxytryptamine2 (5-HT2) and dopamine2 (D2) receptors, unlike many other neuroleptics. SM-9018 also strongly inhibited 5-HT2 receptor-mediated behavior such as tryptamine-induced clonic seizure and D2 receptor-mediated behavior such as methamphetamine-induced hyperactivity, apomorphine-induced stereotypy and climbing behavior. SM-9018 possessed only a weak cataleptogenic activity, which may be clinically related to extrapyramidal side effects, despite its potent D2 antagonistic activity. Moreover, SM-9018 induced weak central depressant effects such as inhibition of spontaneous locomotor activity and motor coordination, as compared with classical neuroleptics (haloperidol and chlorpromazine). These results suggest that SM-9018 is a new neuroleptic drug with both potent 5-HT2 and D2 antagonistic activities and with low cataleptogenic and central depressant activities.

Binding profile of SM-9018, a novel antipsychotic candidate.

The present study employed various receptor-binding assays to clarify the biochemical characteristics of SM-9018. SM-9018 possessed very high affinity for 5-HT2, D2 and 5-HT1A receptors (Ki = 0.61, 1.4 and 2.9 nM, respectively), and it had moderate affinity for alpha 1 and D1 receptors (Ki = 17 and 41 nM, respectively). However, SM-9018 had only negligible affinity for alpha 2, opiate, glutamate, phencyclidine, benzodiazepine and GABAA receptors. These results suggest that SM-9018 may be a novel antipsychotic agent with binding affinity for 5-HT2 and 5-HT1A receptors.

Loss of viability after disulfiram treatment without preceding depletion of intracellular GSH.

Effects of disulfiram (DSF) on freshly isolated hepatocytes were examined. Its effects on the cellular reduced form of glutathione (GSH) were triphasic; GSH decreased instantly after the addition of DSF, returned to subnormal levels within 30 min, and then declined gradually. The initial decrease in GSH after DSF treatment and the subsequent recovery of GSH were accompanied by an increase and decrease in the oxidized form of glutathione (GSSG), respectively. Decreases in cell viability brought about by 0.4 mM of DSF were correlated with the later gradual decrease in GSH. The loss of viability by DSF treatment seemed to appear when the initial GSH levels became lower than approximately 5 nmole/10(6) cells. Hepatocyte toxicity of DSF was potentiated by diethylmaleate (GSH depletor) and inhibited by N-acetylcysteine (GSH biosynthesis precursor). 1,3-bis(2-chloroethyl)-1-nitrosourea (BCNU), an inhibitor of GSH reductase, inhibited the GSH recovery and potentiated the toxicity. Respiration of hepatocytes was also inhibited by DSF. Free sulfhydryl groups other than GSH showed similar changes to those of GSH. From these results, it seemed that DSF reacted with cellular GSH and other free sulfhydryl groups to form diethyldithiocarbamate and GSSG, GSSG was reduced back to GSH by glutathione reductase, and the decrease in the viability was dependent on the initial loss of GSH.

Selective depression of the segmental polysynaptic reflex by phencyclidine and its analogs in the rat in vitro: interaction with N-methyl-D-aspartate receptors.

The differential sensitivity of monosynaptic and polysynaptic reflexes to phencyclidine (PCP) and its analogs was examined in a Mg+(+)-free physiological solution using an in vitro spinal cord preparation of neonatal rats. Whereas the monosynaptic reflex was relatively resistant to N-methyl-D-aspartate antagonists [Mg++, 2-amino-5-phosphonovalerate (APV) and 2-amino-7-phosphonoheptanoate (AP7)], the polysynaptic reflex was markedly reduced in a concentration-dependent manner. The magnitude of the monosynaptic reflex only decreased 20 to 30% at concentrations of Mg++ (1.3 mM), APV (10 microM) and AP (10 microM), which completely depressed the polysynaptic reflex. PCP and its analogs also selectively depressed the polysynaptic reflex in a concentration-dependent manner and had relative potencies consistent with those for the PCP receptor [i.e., 1-(1-m-amino-phenylcyclohexyl)piperidine = MK-801 [(+)-5-methyl-10,11-dihydro-5H-dibenzo[a,d]-cyclohepten-5,10-imine maleate] greater than 1-[1-(2-thienyl)cyclohexyl]piperidine greater than or equal to PCP much greater than (+)-N-allylnormetazocine much greater than 1-(1-m-nitrophenylcyclohexyl)piperidine. The latter compounds depressed the monosynaptic reflex to the same extent as Mg++, APV and AP7 at concentrations which completely depressed the polysynaptic reflex. Furthermore, the depression of the reflexes by PCP was unaffected by haloperidol and methiothepin precluding the involvement of sigma and serotonin receptors in PCP-induced depression of the polysynaptic reflex. Our results suggest that PCP and its analogs selectively depressed the polysynaptic reflex through PCP receptors associated with the N-methyl-D-aspartate receptor complex.

[A note on methods for estimating the number of patients in the nationwide epidemiological survey on intractable diseases].

To find the most adequate method for estimating the number of patients in nationwide epidemiological surveys of intractable diseases, we examined six existing methods using reported data on 13 selected diseases. Estimated numbers of patients by the methods, except for methods with theoretical inadequacy or other problems, were almost equal to each other. The simple method, by which the number of patients is estimated as the reported one divided by the response rate, is recommended.

[The relation among the life style and the clinical laboratory data].

The concept of risk pattern is introduced as the new point of view for the cancer prevention. The risk pattern should be got by drawing out the feature of the human information composed of dietary habit, life style, family history, laboratory data. Using the large scale health screening data in Nagano (total population = 93, 403, 1,140 byte/person), the risk pattern is analysed and the relation among the variables are discussed.

[Three cases of malignant lymphoma developing from chronic empyema wall].

Three patients with non-Hodgkin's lymphoma (NHL) involving the lung or chest wall are reported. All patients had tuberculous pleuritis or received artificial pneumothorax for pulmonary tuberculosis 30 years or more previously. The NHL of these patients developed in or close to the thickened pleura. Histologic examination showed diffuse large cell type (LSG classification) in all cases. Lymphomas of all cases were considered to have originated from B-cell lineage because their neoplastic cells expressed B1 (CD20) antigens. In no case the tumors were resectable, and chemotherapy or radiotherapy were performed. Two died of local NHL at 5 and 6 months respectively after diagnosis. One patient is still alive at 8 months after the first treatment for NHL. Early diagnosis is essential to improve the prognosis of these NHLs although it is difficult because of the co-existent pyothorax. Following features seemed to be useful for diagnosis: 1) chest pain or shoulder pain, 2) elevation of LDH level, 3) mass shadow in computed tomography, and 4) abnormal uptake of gallium 67. As precise assessment of tumor size is difficult because of the co-existent pyothorax and necrosis, follow-up studies by gallium scan or magnetic resonance imaging might be useful for proper treatment.

Acute transformation of chronic large granular lymphocyte leukemia associated with additional chromosome abnormality.

A patient with large granular lymphocyte (LGL) leukemia that transformed into an acute or aggressive form after 20 months of the chronic phase is reported. The patient's leukemic cells were mature, medium-sized lymphocytes with sparse azurophil granules and the surface phenotypes of the cells were CD2+, CD3-, CD11+, and CD16+. Molecular analysis showed a germ line configuration in both T-cell receptor beta-chain genes and T-cell receptor tau-chain genes. A clonal anomaly of chromosome (trisomy 8) was demonstrated in peripheral blood cells. LGL after acute transformation of the disease displayed large blastic morphology with prominent nucleoli, intense basophilic cytoplasm, and numerous granules. Karyotypic analysis demonstrated a mosaic of trisomy 8 and trisomy 8 with an additional marker chromosome. Thus, transformation of chronic LGL leukemia into an acute or aggressive form in this patient was associated with morphologic and karyotypic changes of the leukemic cells. Patients with a stable form of chronic LGL leukemia should be examined carefully for the possible acute crisis associated with a clonal evolution.

Sequence-specific inhibition of DNA strand elongation by incorporation of 9-beta-D-arabinofuranosyladenine.

9-beta-D-Arabinofuranosyladenine (ara-A) is an inhibitor of DNA replication with antitumor and antiviral activity. The molecular basis for this inhibitory effect has remained unclear. The present work has examined the effects of 9-beta-D-arabinofuranosyladenine-triphosphate on DNA polymerase-beta. These studies were performed on different M13 phage DNA templates. The findings demonstrate that 9-beta-D-arabinofuranosyladenine is incorporated into the elongating DNA strand by DNA polymerase-beta. The findings also demonstrate that the incorporated 9-beta-D-arabinofuranosyladenine residue acts as a relative chain terminator. Furthermore, the relative chain-terminating effects of this agent are sequence specific and reversed by competition with deoxyadenosine-triphosphate for incorporation into the DNA strand. These findings are in concert with hydrogen bonding differences of the incorporated arabinosyl moiety which alters reactivity of the chain terminus and thereby inhibits elongation. These findings are also in agreement with recent studies of 1-beta-D-arabinofuranosylcytosine and provide insights into the sequence specific effects of these agents.
