Impaired insulin sensitivity in young, lean normotensive offspring of essential hypertensives: possible role of disturbed calcium metabolism.

OBJECTIVES: To determine whether insulin sensitivity is impaired in normotensive offspring of essential hypertensives and to investigate how it relates to disturbed calcium metabolism. DESIGN: In 10 young lean, normotensive subjects with family histories of essential hypertension and in 10 age-matched controls we investigated insulin sensitivity by the euglycaemic hyperinsulinaemic clamp technique, and 75-g oral glucose tolerance test (GTT). We simultaneously measured calcium-related factors, including the intracellular calcium concentration in platelets. RESULTS: Insulin-induced glucose uptake was significantly impaired in subjects with positive compared with in subjects with negative family history of essential hypertension. Intracellular calcium concentration was significantly higher in subjects with positive than in those with negative family history. Insulin sensitivity was inversely correlated with intracellular calcium concentration (r = -0.56; P < 0.05). Those who showed a higher intracellular calcium concentration had impaired insulin sensitivity. Oral GTT results were normal in both groups. CONCLUSION: Impaired insulin sensitivity and disturbed calcium metabolism exist before the onset of hypertension. Insulin sensitivity correlates inversely with intracellular calcium concentration. These findings suggest that the association of the impaired insulin sensitivity and disturbed calcium metabolism may contribute to the development of hypertension.

Prognostic significance of selected lifestyle factors in urinary bladder cancer.

To examine the prognostic significance of lifestyle factors in urinary bladder cancer, we conducted a follow-up study of 258 incident bladder cancer patients, who were originally recruited in a case-control study in metropolitan Nagoya. Information on individual survivals was obtained from the computer data-file of the tumor registry of the Nagoya Bladder Cancer Research Group. Univariate analyses revealed significant associations of 5-year survivorship with educational attainment, marital status, drinking habits and consumption of green tea in males, and age at first consultation, histological type and grade of tumor, stage and distant metastasis in both sexes. After adjustment for age, stage, histology (histological type and grade) and distant metastasis by means of a proportional hazards model, drinking of alcoholic beverages was significantly associated with the prognosis of bladder cancer in males. Its adjusted hazard ratio was 0.46 (95% confidence interval: 0.26-0.79), favoring patients who had taken alcoholic beverages. In detailed analysis, ex-drinkers and all levels of current drinkers demonstrated hazard ratios smaller than unity, although no clear dose-response relationship was detected. No prognostic significance was found for such lifestyle factors as smoking habit, uses of artificial sweeteners and hairdye, and consumption of coffee, black tea, matcha (powdered green tea) and cola.

Multicenter study with recombinant human erythropoietin.

A multicenter study with recombinant human erythropoietin (rh-EPO) was carried out. Of 172 hemodialysis patients with anemia selected for the study from 20 hospitals and clinics, 77 were males and 95 females (mean age 53.9 years). A starting dose of 1,500 U of rh-EPO (Epoetin beta) was administered intravenously at the end of every dialysis session. If the efficacy was not acceptable, the dose was increased to 3,000 U. When the target hematocrit was achieved (30%), the total dose was decreased. The results of the study were excellent relative to those of other multicenter studies with regard to efficacy, safety, and changes in laboratory data. The incidence of hypertension was lower in our study compared with other reports because we used a low initial dose. The efficacy of rh-EPO therapy was determined earlier and more reliably by reticulocytes than by hematocrit or hemoglobin. Prompt iron supplement therapy is recommended with careful observation of serum iron and ferritin.

Oxysterols inhibit gap junctional communication between rat hepatocytes in primary culture.

Several oxysterols were examined for their effect on gap junctional communication between rat hepatocytes in primary culture. 25-Hydroxycholesterol, 22(S)-hydroxycholesterol and 7 beta-hydroxycholesterol, in decreasing order of potency, markedly inhibited gap junctional communication. In contrast, 7-ketocholesterol showed no inhibitory effect. The inhibition of gap junctional communication by oxysterols was not a consequence of changes in cell viability, as measured by lactate dehydrogenase leakage and 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) reduction activity. The addition of exogenous cholesterol to the culture medium did not abolish the effect of 25-hydroxycholesterol, suggesting that the capacity of oxysterols to inhibit gap junctional communication is independent of their inhibitory effect on cholesterol synthesis. We suppose that inhibition of gap junctional communication may be an early sign of oxysterols-induced toxicity on hepatocytes.

Nitrogen dioxide-induced eosinophilia and mucosal injury in the trachea of the guinea pig.

Nitrogen dioxide exposure-induced mucosal pathology of the guinea pig trachea was studied. Exposure to 3 or 9 ppm of nitrogen dioxide for 6 h a day, 6 times weekly for 2 weeks resulted in decreased ciliary activity as well as a dramatic eosinophil accumulation to the epithelium and submucosal layer. Especially exposure to 9 ppm of nitrogen dioxide induced epithelial injury through the activation of eosinophils accumulated in the tracheal mucosa. The epithelial damage induced by nitrogen dioxide could lead to hyperresponsiveness and prolonged allergic inflammation. Our study suggests that environmental nitrogen dioxide could contribute to the hyperresponsiveness and could most probably be involved in the development and chronicity of airway allergic disorders.

In vitro production of interleukin-1, interleukin-6, and tumor necrosis factor-alpha in insulin-dependent diabetes mellitus.

The in vitro production of interleukin-1 (IL-1), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-alpha) by monocytes was examined in patients with insulin-dependent diabetes mellitus (IDDM), in those with noninsulin-dependent diabetes mellitus (NIDDM), and in healthy volunteers. The production of IL-1 and IL-6 by monocytes was significantly lower in IDDM patients than in NIDDM patients and normal subjects whereas the TNF-alpha production by monocytes did not differ between IDDM patients and normal subjects. On the other hand, the TNF-alpha production was significantly higher in NIDDM patients than in IDDM patients and normal subjects. There was a significant correlation between IL-1 and IL-6 concentrations in culture supernatants of monocytes for IDDM patients but not for NIDDM patients and normal subjects. Neither glucose nor insulin showed any stimulatory effect on in vitro production of these monokines. In the serial observation lasting 3-18 months, the monocyte production of IL-1 was found to be consistently reduced in IDDM patients unrelated to the control state of diabetes, suggesting that the reduction of the IL-1 and IL-6 production by monocytes in IDDM patients may be intrinsically affected by immunological defects.

Effects of SM-9018, a potential atypical neuroleptic, on the central monoaminergic system in rats.

The effects of SM-9018, a potential atypical neuroleptic, on monoamine metabolism were studied in rats. SM-9018 dose-dependently increased the levels of 3,4-dihydroxyphenylacetic acid (DOPAC) and homovanillic acid (HVA) in the cerebral cortex and striatum without affecting the levels of 5-HT, norepinephrine and their metabolites. This action persisted for 8 hr with a maximum effect at 1-2 hr. The SM-9018-induced increase of dopamine (DA) turnover, as indexed by HVA/DA and DOPAC/DA ratios, was comparable in the cerebral cortex and striatum, whereas the typical neuroleptic haloperidol preferentially enhanced the striatal DA turnover. These findings suggest that SM-9018, unlike haloperidol, increases DA turnover to a similar extent both in the cerebral cortex and striatum.

[Lead exposure of workers in the ceramics industry and relevant factors].

To clarify lead exposure and factors relevant to it, an occupational health survey and air lead-concentration survey were conducted among 425 workers in the ceramics industry in the Seto region in Aichi Prefecture. As for biological monitoring, blood lead level was measured according to sex, job, product, factory size and duration of lead exposure. The following major findings were obtained in the present analysis. Workers making dolomite novelties showed higher blood lead levels than those making porcelain novelties, semi-porcelain novelties and tableware. The difference in blood lead level by job was statistically significant, but not that by factory size. In males, the highest blood lead level was found for spray painting work (28.7 micrograms/dl), followed by glazing work (28.0 micrograms/dl), kiln work (23.3 micrograms/dl) and painting work (22.3 micrograms/dl). In females, the blood lead level for painting work (13.5 micrograms/dl) was lower than those for glazing work (26.1 micrograms/dl) and kiln work (31.4 micrograms/dl). The blood lead level was significantly increased with the number of years of lead exposure in females (painting work). It was coincidentally clarified that the environmental lead concentration varied according to the job in which workers were engaged. As protective measures against lead exposure for workers in the ceramics industry, the development of nonlead paint and glaze and the introduction of more effective dust collectors at the workplace should be established.

Toxicity studies of a synthetic antioxidant, 2,2'-methylenebis (4-ethyl-6-tert-butylphenol) in rats. 2. Uncoupling effect on oxidative phosphorylation of liver mitochondria.

Effects of 2,2'-methylenebis (4-ethyl-6-tert-butylphenol) (MBEBP) on hepatic mitochondrial oxidative phosphorylation in vitro, and on hepatic peroxisomal enzymes activities and microsomal mixed-function oxidase activities were studied. 1. A low concentration of MBEBP, less than 50 microM, increased state 4 respiration and decreased state 3 respiration. However, a higher concentration of MBEBP, greater than 100 microM, acted as a respiratory inhibitor. Therefore, MBEBP was found to act as an uncoupler of oxidative phosphorylation in rat liver mitochondria. 2. MBEBP significantly decreased peroxisomal enzymes, cyanide-insensitive palmitoyl-CoA oxidizing activity and catalase activity in the livers of rats fed 0.2, 1.0 or 5.0% MBEBP for 4 weeks. 3. In microsomal enzyme assay, NADPH cytochrome c reductase activity was significantly increased, however, cytochrome P-450, cytochrome b5 levels, aminopyrine N-demethylase and benzo [a] pyrene hydroxylase activities were not significantly increased in the livers of rats fed 1.0 or 5.0% MBEBP for 4 weeks. The weight loss and the decrease of serum triglyceride level observed in the MBEBP-treated rats seemed to be caused by its uncoupling effects, which might also be the cause of the testicular damage induced by MBEBP.

Lymphangioma of the jejunal mesentery in an adult: a case report and a review of the Japanese literature.

A rare case of lymphangioma of the jejunal mesentery in a 34-year-old woman is presented. She was diagnosed as having an ovarian cyst preoperatively, but laparotomy revealed a cystic tumor of the mesentery that was histologically diagnosed as lymphangioma. Thus far, 44 cases of adult mesenteric lymphangioma have been reported in Japan. In nine cases, accurate diagnosis of mesenteric cysts was obtained preoperatively. In 31 cases, complete excision with or without bowel resection was done, whereas two cases were partially resected and one case was drained.

EPO test in hemodialysis patients.

It is well known that red blood cell (RBC) and reticulocyte counts increase when anemic hemodialysis patients receive repeated administrations of recombinant human erythropoietin(rh-EPO). However, the early response of reticulocyte has not been reported precisely. In this study, reticulocyte counts and its fractions were observed in 8 maintenance hemodialysis patients after a single intravenous administration of 60 u/Kg of rh-EPO. The peripheral blood cells taken from the patients were stained with auramine 0, which bound to RNA and caused fluorescence. R-1000 (Sysmex, Kobe, Japan) analyzed the reticulocytes and classified the stained cells into three groups by the intensity of fluorescence. The number of highly fluorescent reticulocyte, which was young and large, increased and reached its highest level on day 4 after a single administration of rh-EPO. The reticulocyte counts paralleled these changes, but RBC counts did not change after administrations of rh-EPO. These data suggest that the observation of reticulocytes and its fractions in hemodialysis patients after a single administration of rh-EPO could tell us whether reticulocytes are responsive to rh-EPO or not, and this could be called as EPO test.

[Sequential hormone and radiation therapy for stage D1 prostate cancer].

From 1980 to 1991, 18 patients with pathologically proven stage D1 prostate cancer were treated with sequential hormone and radiation therapy at Cancer Institute Hospital. The patient mean age was 65.5 year old and mean follow-up period was 3.7 years. Diethylstilbestrol diphosphate or chlormadinone acetate was given prior to radiation therapy and 70 Gy of external radiotherapy using linear accelerator was sequentially delivered to the primary lesion in 35 fractions. Hormone therapy was continued following radiation therapy. Complete flattening of the primary lesion on digital examination was achieved in all cases. Complications of the treatment were minimal and transient. Tumor progression was observed in 4 cases and 2 of them died of cancer. Five-year non-progression rate, 5-year overall survival rate, 5-year disease specific survival rate and cancer death rate were 65%, 68%, 82% and 50% respectively. Prognoses of the patients with poorly differentiated cancer were worse than those with more differentiated cancer. Sequential hormone and radiation therapy for patients with stage D1 prostate cancer improved the patients' survival almost comparable to those of patients with stage C disease.

[A case of testicular choriocarcinoma achieving pathological complete response by "COMPE" chemotherapy, consisting of cisplatin, vincristine, methotrexate, peplomycin, and etoposide].

We report a case of pure choriocarcinoma of the left testis in a 27-year-old male. He consulted a physician with left back pain in August, 1990. Intravenous pyelography (IVP) revealed left hydronephrosis. Abnormal computerized tomography (CT) revealed para-aortic lymph node swelling. The chest X-ray revealed bilateral multiple coin lesions. The genital examination revealed a left testicular nodule 8 mm in diameter. Left high inguinal orchiectomy was performed. Histopathological diagnosis was pure choriocarcinoma. After 4 courses of the PVB (cisplatin, vinblastine, bleomycin) therapy, tumor regression rate of the pulmonary metastases was 41.9%. He was transferred to our hospital on December 3, 1990 and was admitted 3 days later. "COMPE" chemotherapy, consisting of cisplatin, vincristine, methotrexate, peplomycin, and etoposide, was administered. After seven courses of "COMPE" chemotherapy, he achieved a partial response (the regression rate of the pulmonary metastases and the retroperitoneal lymph node metastasis were 78.8% and 69.1%, respectively) and the residual masses in the lungs and the retroperitoneum were removed. Necrosis and xanthogranulomatous fibrosis were found in the resected material. The patient showed no evidence of disease one year after thoracotomy.

[Dose escalation study of high dose etoposide in autologous hematopoietic stem cell transplantation].

Eight cases with poor prognosis hematological malignancies (non-Hodgkin lymphoma, 6 cases; acute non-lymphocytic leukemia, 2 cases) and nine cases with non-hematological malignancies were treated with high dose etoposide (VP16) containing regimen followed by autologous hemopoietic stem cell transplantation. Results were as follows; 1) all of three chemotherapy sensitive relapse patients with hematological malignancies continue complete remission without any cyto-reductive therapy 2) one of four refractory relapse patients continue remission 3) partial anti-tumor effect was noted in non-hematological malignancies, however, only two cases continue complete remission. Remission duration of other responders was not so long. The results disclosed the dose-limiting factor of high-dose VP16 therapy as reversible stomatitis with no related mortality, and maximal tolerated dose appears to be 60 mg/kg over 72 hr with 45 mg/kg as a safe and recommended therapeutic dose in future clinical trial. The clinical effect of dose escalation was not clearly demonstrated.

Effects of extracorporeal circulation on blood ketone body ratio reflecting hepatic energy metabolism during cardiac operation.

To examine the effects of extracorporeal circulation using an artificial heart and lung machine on hepatic energy metabolism in patients with cardiac operation using hypothermia, the arterial blood ketone body ratio (AKBR) reflecting the hepatic mitochondrial redox state was determined in 12 patients who had undergone cardiac operation using extracorporeal circulation from March to August 1991. Changes in AKBR were compared with those before and after extracorporeal circulation. AKBR decreased significantly after the beginning of extracorporeal circulation (p < 0.001) and remained at a lower level throughout extracorporeal circulation. On termination of extracorporeal circulation, the initial level was immediately resumed. The extent of decrease in ketone body ratio at ten minutes before termination of extracorporeal circulation was correlated with short term postoperative hepatic insufficiency. The patients whose ratio decreased below 0.4 showed increased levels in glutamic-pyruvic transaminase at the end of the first and second week after operation. Changes in AKBR were significantly associated with those in blood pressure (r = 0.433; p < 0.005) and body temperature (r = 0.472; p < 0.005). It was concluded that blood pressure and body temperature influence the blood ketone body ratio during extracorporeal circulation.

Effect of S-carboxymethylcysteine on ciliary activity in chronic sinusitis.

This study was designed to investigate the possible pharmacological effect of S-carboxy-methylcysteine (S-CMC) on the ciliary activity, using an in vitro experimental system after removing mucus. Ciliary activity from healthy rabbit maxillary sinus and from healthy human nasal mucosa demonstrated no significant change in RPMI 1640 containing S-CMC. On the other hand, the effect of S-CMC on the reduced ciliary activity from patients with chronic sinusitis was quite varied among the cases examined. S-CMC demonstrated no stimulatory effect on the beating activity of cilia that have a baseline activity of less than 400 beats/min. However, S-CMC was able to enhance the beating activity of cilia that demonstrated a baseline activity of more than 400 beats/min. S-CMC at 0.5% induced a larger ciliostimulatory effect than 0.05% S-CMC. In conclusion, our study has clearly demonstrated that S-CMC could directly enhance ciliary activity of chronic sinusitis in the absence of significant organic change of ciliated cells.

[Determination of intrauterine pressure using catheter-tip transducer inserted outside fetal membranes].

To determining intrauterine pressure outside fetal membranes, we used a catheter-tip transducer to study 20 women before the occurrence membrane rupture. Their mean age was 28.2 +/- 3.4 years and all women were in weeks 37 to 41 of pregnancy when studied. In the first stage of labor, the peak intrauterine pressure was 60.0 +/- 12.5 mmHg (mean +/- SD) when the external os was dilated 4 to 6 cm, 90.0 +/- 14.8 mmHg, at 7 to 8 cm dilation, and 80.0 +/- 11.5 mmHg at 9 cm or greater dilation. Each pressure wave lasted 45 to 55 sec. The highest baseline pressure (28.0 +/- 4.5 mmHg) was obtained when the subjects were sitting. A baseline pressure of 17.0 +/- 4.0 mmHg was obtained in the supine position, as well as a value of 21.0 +/- 3.5 mmHg in the recumbent position. There were no complications related to the catheter-tip transducer. Our findings indicate that this method is both accurate and reliable in determining the amounts of intrauterine pressure to which fetal membranes are subjected.
