Prognostic factors of primary generalized epilepsy: a reappraisal of 96 cases in terminal remission.

Ninety-six patients whose diagnosis was definitively confirmed as primary generalized epilepsy (PGE) were divided into two groups according to the duration of recurrent seizures--A group for less than 7 years and B group for more than 7 years. The patients of both groups were completely free of seizures for more than 3 years at the time of the survey. A retrospective comparison was made between these two groups in terms of clinical and EEG features. Emphasis was placed on the B group which was characterized by an unfavorable seizure outcome. From a prognostic point of view, the risk factors indicative of an unfavorable outcome of seizures were found as follows: a) the presence of generalized tonic-clonic convulsions (GTCs) prior to the occurrence of minor seizures and persisting GTCs following the subsidence of minor seizures, b) scanty epileptic EEG findings in spite of the sporadic occurrence of GTCs, and c) GTCs tended to associate with various seizure-inducing factors. As a conclusion, along with appropriate drug treatment, the necessity of comprehensive approaches to the patients' everyday life was emphasized in order to achieve a favorable seizure outcome.

Comparison of protein sulfation in control and virus-transformed baby hamster kidney cells.

Protein sulfation in baby hamster kidney cells (BHK) and their polyoma virus transformants (PY-BHK) was studied comparatively. On in vivo labeling, [35S]-sulfate was incorporated into the 50K protein and proteins in the 100-180K range, represented by the 155K protein. The incorporation into both the 50K and 155K protein was elevated 2-3 fold in PY-BHK cells compared to in BHK cells. Tyrosine-O-sulfate was the only identifiable sulfated amino acid in both proteins. On in vitro labeling with [35S]3'-phosphoadenosine 5'-phosphosulfate (PAPS), at least 6 radioactive protein bands were discernible on gel electrophoresis. Of these, sulfation of the 57K and 60K proteins was elevated in PY-BHK cells compared to in BHK cells, whereas sulfation of the 39K protein was depressed in PY-BHK cells. Tyrosine-O-sulfate was the only identifiable sulfated amino acid in these proteins.

Gastroscopic biopsy of the stomach for the diagnosis of amyloidosis.

1. Amyloid deposition was investigated in the biopsied specimens obtained from the stomachs of suspected cases of amyloidosis. The subjects were seven cases of suggestive amyloidosis with clinical symptoms of muscle rigidity, masked face, macroglossia and B-J proteinuria. The stomach of an autopsied amyloidosis patient was also investigated. 2. Mucosal tissue was obtained from fundus, corpus and antrum of the stomach using gastrofiberscope, Olympus GFB2. And amyloid in the tissue was detected by H & E Congo red staining and polarization microscopy. Rectal biopsy was also done in the majority of the cases to compare with stomach biopsy. 3. Positive results were obtained with gastric biopsy in all seven cases and five of the 5 cases by rectal biopsy. Thus, gastric and rectal biopsy yielded the same results so far as detection of amyloid deposition was concerned. 4. Biopsy specimens from several sites of the stomach gave similar results in regard to the amyloid deposition. Tissue deposition of amyloid in the stomach wall showed a somewhat high incidence in the walls of intramucosal capillaries and in the walls of blood vessels of lamina muscularies mucosa. Gastroscopic biopsy of the stomach proved to be useful as a routine technique for the definite diagnosis of the disease.

Effects of amoxapine, a new antidepressant, on pseudoneurotic schizophrenia.

During a four-week open study, amoxapine (AX), a new antidepressant agent, was administered to seven patients with pseudoneurotic schizophrenia, seven with neurosis and another seven with schizophrenia, all having similar symptoms. The improvement ratio was 71.4% in the pseudoneurotic schizophrenia group, 57.1% in the neurosis group and 42.8% in the schizophrenia group. Through the application of rating instruments, improvements were observed in the pseudoneurotic schizophrenia group in such items as psychotic and psychoneurotic symptoms in the assessment through the Springfield Outpatient Symptom and Adjustment Index, somatic concern and blunted affect through the Brief Psychiatric Rating Scale, and depression and depersonalization through the Clinical Rating Scale. On the other hand, overall improvements were less seen in the items of the neurosis group and the schizophrenia group. Effective doses of AX were 30 to 75 mg/day in the three groups. Side effects were observed in four cases which included insomnia, tachycardia, palpitation and hypomanic state. There were no cases in which AX was discontinued because of the side effects as these symptoms were slight. AX is remarkable and characteristically efficacious in the pseudoneurotic schizophrenia, and this effectiveness is presumed due to its antidepressant and antipsychotic actions.

A new acidic protein in porcine brain.

An extremely acidic protein has been isolated in a purified form from porcine rain extract, by (NH4)2SO4 fractionation followed by column chromatography on DEAE-Sephadex A-50 and on Sephadex G-75. The purified protein was tentatively named as glutamic acid-rich protein because it was characterized by its remarkably high content of glutamic acid which accounted for 49% of the total amino acid composition. The protein appeared to be a single polypeptide chain with a molecular weight of 56 000-58 000, and had an isoelectric point of 4.6. The N-terminal amino acid sequence was Asp-Glu-Pro-Pro-Ser-Glu-Gly. The immunochemical analysis using rabbit antiserum prepared to the porcine protein has suggested that it is present in the brain of human, cow, cat, dog and goat as well as in various goat organs including liver, kidney, heart, small intestine and spleen.

Plasma cyclic AMP level in psychiatric diseases of childhood.

Determinations were made of the plasma cyclic AM level to examine its relationship with hyperkinesis (Werry-Weiss-Peters Activity Scale, WWPAS) and other features of mental disorders in 80 children, of whom 21 had early infantile autism, 15 hyperkinetic mental retardation, 12 minimal brain dysfunction and 32 Down's syndrome. In autistic and hyperkinetic mentally retarded children, the plasma cyclic AMP levels were higher than in normal children and were positively correlated with the WWPAS score. In children with minimal brain dysfunction, the plasma cyclic AMP level was significantly lower than in normal children and was not correlated with the WWPAS score. In children with Down's syndrome, the plasma cyclic AMP level was somewhat higher than in normal children.
