Endoscopic intramural injection of anti-neoplastic emulsion.

With the aim of establishing a topical chemotherapy against stomach carcinoma, 5-fluorouracil (5-FU) emulsion (oil/water type) for injection has been developed. The drug distribution was analyzed by 5-FU bioassay and radiographic examination of soft parts, for which radiopaque Lipiodol was employed in an oil phase. In order to examine local toxicity, tissue retention, and transfer to lymph nodes of 5-FU emulsion, the drug was administered perorally to rats and injected intramurally through the gastric serosa into laparotomized dogs. Following this series of experiments, which gave satisfactory results, the time courses of drug concentration in the gastric wall and regional lymph nodes were studied as a pre-clinical trial, by applying endoscopic intramural injection of 5-FU emulsion or solution in dogs. The anti-metastatic and anti-neoplastic effects of 5-FU emulsion were investigated in an experimental model of lymph node metastasis in mice. The emulsion was more effective in subduing metastasis and tumor growth than the solution, and the effectiveness of the former was further augmented by the use of repeated injections rather than a single injection. This method of endoscopic injection of 5-FU emulsion should be of great value as a local therapeutic measure against stomach carcinoma itself as well as against metastatic lesions in the lymph nodes.

beta-Adrenoceptor alterations coupled with secretory response in rat parotid tissue.

Simultaneous studies on the secretory response of amylase and the neurotransmitter receptors of rat parotid gland, after brief treatment with agonists, showed selective alteration in beta-adrenoceptors with specific change in amylase secretion, suggesting a regulatory role of the receptors in the secretory response. The beta-adrenergic agonist (+/-)-isoprenaline (IPR) stimulated amylase secretion from rat parotid tissues much more than did the same concentration of an alpha-adrenergic or cholinergic agonist. The stimulatory effects of IPR were studied by pre-treating rat parotid tissues with IPR for 10 min and then incubating the tissue in fresh medium for 10 min. Pre-treatment with 10 microM-IPR for 10 min resulted in increased amylase secretion during further incubation with IPR and also in a lower EC50 value of amylase secretion for IPR. This treatment also resulted in selective changes in the number and affinity of beta-adrenoceptors, assessed by measuring binding of [3H]dihydroalprenolol (DHA): the maximal binding sites increased from 286/357 f-mole to mg protein and the IC50 value (the concentration for 50% inhibition of specific [3H]DHA binding) of beta-agonists, not antagonists, decreased significantly. An increase in the period of pre-treatment with IPR to 30 min resulted in a decrease in the maximal binding sites of beta-adrenoceptors and a decrease in amylase secretion during further incubation with IPR. Experiments with other agonists showed that supersensitivity of the secretory response was induced specifically by beta-agonists. Binding studies with [3H]WB-4101 and [3H]quinuclidinyl benzilate showed that alpha-adrenoceptors and muscarinic ACh receptors in rat parotid did not change under the conditions tested. The alteration in beta-adrenoceptors was parallel with a change in amylase secretion after IPR pre-treatment, but not with a change in cyclic AMP content.

Adenomatous intestinal hyperplasia in guinea pigs associated with Campylobacter-like bacteria.

Adenomatous intestinal hyperplasia was diagnosed in two dams and five young guinea pigs. The pathological condition was characterized by diarrhea and the thickened, rugose jejunum and ileum. Adenomatous proliferation of immature crypt epithelial cells was observed primarily in the ileum and the distal part of the jejunum. Immature epithelial cells contained various numbers of intracytoplasmic, non-membrane bound, curved organisms resembling Campylobacter sp. bacteria.

[Basic and clinical studies on ceftazidime in the field of obstetrics and gynecology].

Ceftazidime (CAZ), a new cephalosporin antibiotic, was studied in the field of obstetrics and gynecology, and the following results were obtained. The absorption and tissue penetration of CAZ into intrapelvic genital organs were good after a single drip infusion of 1.0 g for 30--60 minutes. The maximum level of 76.4 micrograms/ml was obtained in uterine artery serum at 8 minutes after administration. The high concentrations were also obtained in genital tissues; the maximum concentrations ranged from 46.8--62.1 micrograms/g at 20 minutes after administration and the levels were as high as 2.1--7.7 micrograms/g at 5 hours and 40 minutes after administration. The concentration curves in tissues were consistent with those of serum levels. The concentrations of CAZ in retroperitoneal dead space exudate were determined after intravenous drip infusion of 1 g. The peak levels ranged from 26 to 32 micrograms/ml after 30 minutes of administration and the level of 8.53 micrograms/ml was sustained even 6 hours later. Good response was obtained in cases of gyneco-obstetric infections such as intrauterine infection, intrapelvic infection and external genital infection with daily dose of 2--4 g. CAZ was effective in 13 out of 14 cases (the efficacy ratio; 92.9%). As to side effects, gastric discomfort and vomiting were observed in 1 case.

Silicone as coating material for hemoperfusion.

The effectiveness of activated charcoal in direct hemoperfusion (DHP) has been recognized, but there are still problems with a decrease in solid components of blood and of dimishing adsorption capacity by coating layers on the surface of the sorbents in DHP. In the present study, we have developed activated charcoal derived from thermosetting resin, coated with silicone, and have observed changes in blood components after a 3 hr DHP. Adsorption capacity of the activated charcoal was determined in vitro, and was compared with noncoated charcoal and poly(hydroxyethyl methacrylate) (PHEMA) coated charcoal. Reduction in leukocyte and platelet counts was less with the silicone coated and PHEMA coated charcoals than with the noncoated charcoal. Variations in leukocyte counts and platelet counts were less in DHP with silicone-coated charcoal than with the others. Adsorption of the solid components of blood on the silicone-coated charcoal, examined by scanning electron microscopy after DHP, was very slight, showing its excellent applicability to DHP therapy. The degree of adsorption of various amino acids and related compounds on the silicone-coated charcoal was comparable to that on the noncoated charcoal. Thus, there seems only slight reduction in the degree of adsorption on the silicone-coated charcoal.

[Aerosol administration of dibekacin--the sputum concentration].

Serum concentrations of dibekacin (DKB), sisomicin (SISO) and gentamicin (GM) were measured in 3 rabbits after intratracheal administration through the transtracheal teflon tube. The peak serum levels (average) were 106 micrograms/ml (administrated 100 mg DKB for injection), 148 micrograms/ml (administrated 100 mg DKB solution), 166 micrograms/ml (administrated 100 mg SISO solution) and 80 micrograms/ml (administrated 80 mg GM solution). Serum concentrations and urine excretions of DKB were measured in 3 volunteers after aerosol administration using ultrasonic nebulizer. The peak serum levels (average) were 4.6 micrograms/ml (administrated 100 mg DKB for injection) and 3.1 micrograms/ml (administrated 100 mg DKB solution). The urine excretions (average) were 3.7 mg and 4.3 mg respectively during 6 hours. Before and after administration of DKB aerosol the spirogram and flow-volume curve were examined in the volunteers. But the examinations showed no changes. Sputum concentrations were measured in 1 patient with chronic bronchobronchiolitis after administration of DKB aerosols using the ultrasonic nebulizer. The highest sputum concentration was acquired immediately after nebulization and the sputum levels decreased gradually while time passed. Six patients with the lower respiratory tract infections were treated with DKB aerosol therapy and the utility rate was 80%.

Cancer-cell detection system based on multispectral images.

An automatic detection system for gynecologic cervical cancer cells from Papanicolaou-stained smears is proposed. The system is based on multispectral scanning, with smear images provided as three monochromatic images by illumination with light of 610-, 535- and 470-nm wavelengths. Scene segmentation in the system is performed by classifying all two-dimensional pixels defined at all image points in the red and green images into one of three classes (background, cytoplasm and nucleus) by means of a maximum-likelihood classifier. The statistical parameters for the classifier are determined under certain assumptions from a two-dimensional histogram, which indicates the bivariate distribution of the optical densities in the red and green images. The nuclear characteristic parameters extracted from individual nuclei by the system are (1) nuclear area, (2) nuclear diameter, (3) nuclear area coefficient and (4) nuclear density vector, which consists of the average optical densities of the red, green and blue images of the nuclear subregion. The system was tested on 34 scenes containing cellular clumps of different cell types. The results of the segmentation of images and the performance of a primitive logic for the detection of malignant nuclei proved the usefulness of the system and the advantages of using multispectral images in the segmentation and detection procedures. The system can detect not only a free-lying cancer cell but also a malignant cell in a clump of cells.

Effects of betazole hydrochloride and cyclic AMP on the pepsinogen secretion by rabbit gastric mucosa in organ culture.

The effects of betazole hydrochloride, dibutyryl cyclic AMP (DB-cyclic AMP) and betazole hydrochloride plus aminophylline on pepsinogen secretion by rabbit gastric mucosa were studied in organ culture. Betazole hydrochloride alone did not stimulate pepsinogen secretion at the concentrations of 10(-8), 10(-6), 10(-5) and 10(-4) M. However, 10(-3) M DB-cyclic AMP produced a significant stimulation of pepsinogen secretion into the culture medium when compared with the control. In the presence of 3 x 10(-3) M aminophylline, betazole hydrochloride in the concentration of 10(-5) and 10(-4) M stimulated pepsinogen secretion into the culture medium, and the magnitude of this increase was 2.0- and 2.8-fold, respectively, compared with the control. Aminophylline alone could not change pepsinogen secretion into the culture medium. These results suggested that the pepsinogen secretion, stimulated by betazole hydrochloride, was mediated by cyclic AMP in the chief cells.

[Irritation test of dexamethasone valerate (DV-17) and other steroid ointments in rabbits. Skin and eye primary irritation tests, and skin cumulative irritation test].

The skin and eye primary irritation tests, and skin cumulative irritation test were performed on male rabbits, which were topically treated with a volume of 1 g/body of 0.06% and 0.12% dexamethasone valerate (DV-17) ointments as a test drug, and 0.1% hydrocortisone butyrate (HB) and 0.12% betamethasone valerate (BMV) ointments as an active control drug in order to evaluate the topical and systemic effects. No skin and eye primary irritations except minimal skin erythema and slight conjunctival redness were seen in above four corticosteroids. Judging from the results such as skin atrophy, inhibition of body weight gains, and decreases of thymus, spleen and adrenal weights, the potency of toxicities among the four corticosteroid ointments appeared to be as follows: 0.12% DV-17, 0.12% BMV greater than 0.1% HB greater than 0.06% DV-17.

[Comparative toxicity test of dexamethasone valerate (DV-17) and other steroid ointments in rats].

The comparative toxicity test was performed on male rats, which were topically treated with a volume of 0.25 g/kg of 0.12% dexamethasone valerate (DV-17) ointment as a test drug, and 0.1% dexamethasone (DEX), 0.05% fluocinonide (FAA), 0.025% beclomethasone dipropionate (BMD), 0.1% hydrocortisone butyrate (HB), 0.05% clobetasol propionate (CP) and 0.12% betamethasone valerate (BMV) ointments as an active control drug for 30 consecutive days to evaluate on the effects of following items: body weight changes, adrenal functions (organ weight, histological changes of the cortex and serum contents of corticosterone), pathological changes of hypophysis and lymphatic systems (thymus, mesenteric lymph nodes and spleen), and skin changes. Furthermore, the recovery test was also carried out. The severity of topical toxicities was in parallel with that of systemic toxicities. Judging from the results obtained in this study, the potency of toxicities among seven corticosteroid ointments appeared to be as follows: FAA greater than DEX much greater than CP greater than BMV, DV-17 greater than HB greater than BMD. The toxicities of DV-17 were comparable with that of BMV, and these side effects were found to be minimized on the recovery test.
