Detection of adrenal and retroperitoneal masses in a general health examination system.

OBJECTIVES: To investigate the detection rate of adrenal and retroperitoneal masses other than kidney diseases in a general health examination system. METHODS: From May 1991 through February 1996, 41,357 subjects participated in the general health examination system in our hospital. Approximately 80% of participants were 40 to 59 years old. For all participants, transabdominal ultrasound (US) was performed by five expert examiners using an Aloka SSD-650 with a 3.5-MHz convex-type transducer. When US revealed abnormal lesions on the adrenal gland and in the retroperitoneal space, we recommended that participants be examined with computed tomography (CT), with a slice width of 5 mm. RESULTS: Forty-three participants (0.1%) had abnormal findings on US. Of the 28 of those who underwent CT examination to confirm the lesions, 12 had adrenal and retroperitoneal masses. The detection rate was 0.029% of total participants and 42.9% of those who underwent CT examination. Clinical diagnoses were primary aldosteronism (1), preclinical Cushing syndrome (2), nonfunctioning adrenocortical tumor (5), pheochromocytoma (1), ganglioneuroma (1), adrenal cyst (1), and retroperitoneal neurinoma (1). False-positive results were dominant on the left side of the body (right 3, left 13). A deformed or accessory spleen (3), a cyst on the upper pole of the kidney (2), bowel air (2), and a pancreatic cyst (1) were misdiagnosed as adrenal lesions on US. CONCLUSIONS: The low detection rate of adrenal and retroperitoneal masses by US does not support screening for the diseases in healthy subjects. However, if it is done as part of a general health examination, examiners should attentively observe not only the kidneys but also the adrenal gland and retroperitoneal space because clinically important diseases may be detected.

Prognosis in severe motor and intellectual disabilities syndrome complicated by epilepsy.

We investigated the prognosis of epilepsy in 54 patients with severe motor and intellectual disabilities syndrome (SMIDS). The prevalence of epilepsy was 75.7% in our institution for SMIDS. We assessed activities of daily living (ADL) of them at the onset of epilepsy and at present, according to the scores in locomotion, language, toileting, dressing and feeding. Among them, patients with uncontrolled epilepsy constituted 40.3%. ADL scores worsened in the uncontrolled group, while they improved in other cases. Early seizure control is very important in SMIDS for improving patients' quality of life.

The anticonvulsant zonisamide scavenges free radicals.

Zonisamide (ZNS) is effective in animal models of epilepsy and epileptic patients. The free radical scavenging activities of ZNS were analyzed by using electron spin resonance. ZNS, in the mmolar range, scavenged hydroxyl and nitric oxide radicals in a dose-dependent manner. This suggests that the mechanism of antiepileptic effect of ZNS may involve protection of neurons from free radical damage and stabilization of neuronal membranes.

Kinesin and dynein superfamily proteins in organelle transport and cell division.

Microtubule-associated motor proteins of the kinesin and dynein superfamilies play important roles in cellular mechanisms such as organelle transport and mitosis. Identification and characterization of new family members (in particular KIFC2, 16 new KIFs, XKlp2 and XKCM1 of the kinesin superfamily, and DHC2 and DHC3 of the dynein superfamily) and further characterization of known family members have improved our understanding of these cellular mechanisms. Sophisticated biophysical and structural analyses of monomeric and dimeric motor proteins have contributed to elucidating the mechanisms behind motor protein motility and polarity.

Cytotoxicity of 1-amino-4-phenyl-1,2,3,6-tetrahydropyridine and 1-amino-4-phenylpyridinium ion, 1-amino analogues of MPTP and MPP+, to clonal pheochromocytoma PC12 cells.

1-Methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) induces parkinsonism in humans after its oxidation into 1-methyl-4-phenylpyridinium ion (MPP+) by type B monoamine oxidase. The 1-amino analogues of MPTP and MPP+, 1-amino-4-phenyl-1,2,3, 6-tetrahydropyridine (APTP) and 1-amino-4-phenylpyridinium ion (APP+), were synthesized, and their cytotoxicity to clonal pheochromocytoma PC12 cells was examined using a tetrazolium formazan assay. After incubation for 48 and 72 h, both APP+ and APTP were found to be cytotoxic to PC12 cells, whereas with the N-methyl analogues, only MPP+, but not MPTP, was cytotoxic. The cytotoxicity of APTP increased with incubation time and equaled that of MPP+ after 72 h. It was found that APTP was oxidized to APP+ by type A monoamine oxidase in PC12 cells, suggesting that APP+ itself may damage the cells. In addition to APTP and APP+, N-amino analogues of N-methylisoquinolines and related derivatives were also synthesized and examined for their cytotoxicity to PC12 cells.

Site-specific localization of Epstein-Barr virus in pharyngeal carcinomas.

In this study, the correlations of factors with Epstein-Barr virus (EBV)-association were investigated in 50 patients with nasopharyngeal carcinoma (NPC), 61 with oropharyngeal carcinoma (OPC), and 55 with hypopharyngeal carcinoma (HPC) in Okinawa and Osaka prefectures in Japan. The incidence of pharyngeal carcinoma in Okinawa was previously found to be higher than that in Osaka; the incidence of OPC was approximately 6 times higher and that of HPC was two times higher. The EBV genome was detected in the tumor cells of the present patients; 83% of the Okinawa and 92% of the Osaka NPC patients. The EBV genome was not detected in OPC or HPC. A univariate analysis showed that sex, the location of the tumor, histology, and the degree of lymphocytic infiltration correlated with the EBV-positive rate. A multivariate analysis revealed that only the location of the tumor was independently correlated with the EBV-positive rate. Histology and tumor size were factors affecting the prognosis of the patients with NPC. The NPC of poorly differentiated type frequently showed the EBV genome, and NPC with lymphocytic infiltration showed a more favorable prognosis compared to the other NPC types. These findings suggest that latent genes of EBV expressed in cancer cells might trigger a cytotoxic T cell reaction against the cancer.

Aino virus antigen in brain lesions of a naturally aborted bovine fetus.

A bovine fetus aborted at 187 days of gestation was serologically and immunohistopathologically examined. Serum and cerebrospinal fluid samples had high titers of virus-neutralizing antibody for Aino virus. A severe necrotizing encephalopathy was noted. Aino virus antigen was demonstrated in neuroglial cells within the brain lesion. The destruction of developing neuronal cells appeared to be a significant feature of the pathogenesis of lesions due to Aino virus infection in the central nervous system.

Effects of treatment with nimustine (ACNU), a bifunctional alkylating anticancer agent, on cultured resting L1210 cells.

Resting L1210 cells were treated with nimustine (ACNU), a bifunctional alkylating anticancer agent, for 2 h in a nutrient-depleted medium. The cells were then transferred to a fresh medium and incubated for a further 48 h. Functions of the cells thus prepared were examined in terms of the dye-exclusion of the membrane, 2,3-bis(2-methoxy-4-nitro-5-sulfophenyl)-5-[(phenylamino)carbonyl] -2H-tetrazolium hydroxide, inner salt, sodium salt (XTT)-reducing ability of the mitochondria, and heat generation due to vital metabolism as the measure of cell viability. The cells treated with ACNU were functioning normally in all the cell functions examined but were completely devoid of proliferating capacity. These results suggest the possibility that ACNU might impair the proliferative capacity of the resting cell population inside a solid tumor without causing such impairment to the cells of normal organs and tissues composed of intrinsically non-proliferative cells.

Isolation of Aino virus from an aborted bovine fetus.

A male fetus of gestation day 187 was aborted from a Holstein-Friesian cow in an epizootic of the Aino virus (AINOV) in September 1995. Neutralizing antibody titers against AINOV were 1:128, 1:16 and 1:64 in the dam serum, fetal ascites and cerebrospinal fluid, respectively. A 10% brain suspension of the aborted fetus was prepared immediately after autopsy, rinsed three times and sonicated before centrifugation. The supernatant was then inoculated into HmLu-1 cell cultures. A cytopathic effect was noted on post-inoculation day 7. The isolated virus was identified as the AINOV based on the physicochemical properties and cross neutralization test. This is the first report on the isolation of AINOV from an aborted bovine fetus.

Abnormal spermatogenesis in the common liver fluke (Fasciola sp.) from Japan and Korea.

Diploid and triploid specimens of Japanese and Korean Fasciola sp. showed abnormality in their spermatogenesis. Live germ cells obtained from the testes were observed under a differential interference contrast microscope. In the stages from spermatogonium to spermatid, the cells combined together at the central cytoplasmic bridge during a series of divisions. One spermatogonium becomes a cell group of 8 primary spermatocytes through 3 mitoses. Until the primary spermatocyte stage, cells are divided in a uniform manner. In most of the diploid specimens, the primary spermatocytes are irregularly divided into non-uniform secondary spermatocytes, however, some specimens perform a regular division. In the majority of triploid flukes, the primary spermatocytes are divided in a regular pattern, but some of the specimens perform an irregular division. The non-uniform spermatids do not perform a spermiogenesis. In the diploid specimens, no spermatozoa were found that were produced by spermiogenesis. Whereas in the triploid specimens, some spermatids distributed uniformly on the surface went through a spermiogenesis. We observed some moving spermatozoa in one triploid specimen. The spermatozoa possibly retain their normal reproductive function.

Neuronal mechanisms of phencyclidine-induced place aversion and preference in the conditioned place preference task.

Place conditioning paradigms are widely used for determining the motivational properties of drugs. Phencyclidine (PCP) has been a common drug of abuse during the past two decades and has a rewarding effect in animals. However, PCP produces place aversion in the conditioned place preference (CPP) task in animals. Here, we report the possible neuronal mechanisms of PCP-induced place aversion and preference in the CPP task in rodents. In naive rats and mice, PCP dose-dependently produced place aversion and PCP had a significant effect at the doses of 4 and 8 mg/kg in rats and mice, respectively. The aversive effect of PCP (4 mg/kg) in rats was significantly attenuated by ritanserin (3 and 10 mg/kg), a serotonin 15-HT2) receptor antagonist whereas the lesion of serotonergic (5-HTergic) neurons by 5,7-dihydroxytryptamine (100 micrograms i.c.v.) and alpha-methyl-rho-tyrosine (AMPT; 100 mg/kg), a tyrosine hydroxylase inhibitor, did not affect the aversive effect of PCP. In rats pretreated with PCP (10 mg/kg/day) for 14 days, tolerance was developed to PCP (4 mg/kg)-induced place aversion. In rats and mice pretreated with PCP (10 mg/kg/day) for 28 days, however, PCP dose-dependently produced place preference but not aversion. The preferred effect of PCP (8 mg/kg) in mice preteated with PCP (10 mg/kg/day for 28 days) was significantly attenuated by AMPT (100 mg/kg) and 6-hydroxydopamine (100 micrograms i.c.v.) a dopaminergic (DAergic) neurotoxin, but not by DSP-4 (30 mg/kg), a noradrenergic neurotoxin and ritanserin. In mice pretreated with methamphetamine (1 mg/kg/day) for 14 days, PCP (8 mg/kg) produced place preference. These findings suggest that 5-HTergic and DAergic systems are involved in the PCP-induced place aversion and preference, respectively, and some changes in the neuronal systems including DAergic systems, induced by repeated PCP treatment play a critical role in the addiction of PCP.

A therapeutic strategy to prevent morphine dependence and tolerance by coadministration of cAMP-related reagents with morphine.

Morphine is the most potent opioid analgesic currently available and its use is increasing for treatment of severe pain, however, long-term morphine exposure induces physical dependence/tolerance. Although the mechanisms underlying this phenomenon have not been established, several biochemical changes including intracellular cAMP systems and Ca2+ mobilization have been suggested. To evaluate the contribution of cAMP, we investigated the effects of nefiracetam [N-(2,6-dimethyl-phenyl)-2(2-oxo-1-pyrrolidinyl)acetamide] and phosphodiesterase inhibitors (theophylline, enprofylline and rolipram) on the development of morphine dependence/tolerance. Mice administered morphine (6 or 10 mg/kg, s.c.) twice daily for 5 days, showed withdrawal signs (jumping, diarrhea and body weight loss) after naloxone challenge (5 mg/kg, i.p.), indicating the physical dependence to morphine. Further, the tolerance to antinociceptive effect of morphine was observed in these mice on the tail-flick test. However, coadministration of nefiracetam (5 or 10 mg/kg, p.o.), enprofylline (30 mg/kg, p.o.) and rolipram (0.3 or 1 mg/kg, i.p.) with morphine during the pretreatment period, significantly reduced the withdrawal signs, moreover, the tolerance was significantly attenuated. Acute administration of nefiracetam failed to reduce the withdrawal signs and did not affect the antinociceptive effect of morphine in morphine-naive mice. Theophylline (3 or 10 mg/kg, p.o.) tended to attenuate the development of morphine dependence/tolerance. The present findings suggest that coadministration of compounds which increase cAMP level with morphine may be a useful strategy to attenuate the development of morphine dependence/tolerance in the clinic.

Recurrent malignant proliferating trichilemmal tumour: local management with ethanol injection.

We report a 59-year-old woman who exhibited a recurrent malignant proliferating trichilemmal tumour on the scalp for 1 5 years. The tumour was recalcitrant to conventional treatments such as chemotherapy, radiation or hyperthermia and we performed intratumoral ethanol injection as an alternative means of reducing tumour mass and obtaining haemostasis. Biopsy specimens obtained after the ethanol injection revealed oedema, haemorrhage in the dermis and degeneration of the tumour cells, showing vacuolization with pyknotic nuclei. For cases of recurrent skin tumours and for patients in poor clinical condition, intratumoral ethanol injection is likely to be a therapeutic alternative to surgery or other conventional treatments.

Antioxidant activity of nasunin, an anthocyanin in eggplant.

Delphinidine-3-(p-coumaroylrutinoside)-5-glucoside (nasunin), an anthocyanin was isolated as purple colored crystals from eggplant peels, Solanum melongena L. 'Chouja'. Using an electron spin resonance spectrometer and 5,5-dimethyl-1-pyrroline-N-oxide (DMPO), spin trapping, hydroxyl (.OH) or superoxide anion radicals (02*-) generated by the Fenton reaction or the hypoxanthine-xanthine oxidase system were measured as DMPO-OH or DMPO-OOH spin adducts. L-Ascorbic acid 2-[3,4-dihydro-2,5,7,8-tetra-methyl-2-(4,8,12-trimethyltridecyl)-2 H-1-benzopyran-6yl-hydrogen phosphate] potassium salt (EPC-K1) and bovine erythrocyte superoxide dismutase (SOD) were used as standards for .OH and O2*-, respectively. Nasunin directly scavenged O2*- with a potency of 143+/-8 SOD-equivalent units/mg), and inhibited formation of DMPO-OH (0.65+/-0.07 EPC-K1 micromol/mg). A spectrophotometric study showed that nasunin formed an iron complex with a molar ratio of nasunin : Fe3+ of 2 : 1. Therefore, hydroxyl radical scavenging by nasunin is not due to direct radical scavenging but inhibition of .OH generation by chelating iron. Nasunin (1 microM) significantly protected against lipid peroxidation of brain homogenates (p<0.001) as measured by malonaldehyde and 4-hydroxyalkenals. These findings demonstrate that nasunin is a potent O2*- scavenger and iron chelator which can protect against lipid peroxidation.

Event-related potentials during a selective attention task with short interstimulus intervals in patients with schizophrenia.

OBJECTIVE: To study selective attention in schizophrenia by examining event-related potentials during a dichotic listening task with short interstimulus intervals (ISIs). DESIGN: Prospective study. PARTICIPANTS: Twelve patients with schizophrenia in remission and 12 age-matched controls with no history of psychiatric or neurological illness. INTERVENTIONS: Participants were asked to push a button in response to target stimuli in either ear. OUTCOME MEASURES: Reaction time, correct response rate and results of electroencephalography recorded at 3 regions: mean segmental amplitudes between 0 and 200 ms and between 200 and 400 ms after stimuli and processing negativity (Nd), measured by the negative area during these periods. RESULTS: Distinct slow-positive potentials for unattended stimuli, which were elicited in a task with long ISIs in a previous report, did not emerge in either group in this study. Although the 2 groups did not differ significantly in terms of Nd area, in the controls the mean segmental amplitude for attended standard stimuli was significantly greater than that in the patients with schizophrenia. CONCLUSIONS: Impairment of selective attention in patients with schizophrenia is related to a lack of ability to focus attention on attended task-relevant stimuli during a selective attention task with short ISI.

An Assessment of Amniotic Fluid Index Among Japanese (A Longitudinal Study)

>Objective: To obtain a longitudinal gestational reference range for the amniotic fluid index (AFI) among Japanese women and to compare the study with those of previous reports. Methods: A total of 739 measurements of the AFI of 96 Japanese women with normal pregnancies were analyzed. The criteria were singleton pregnancies between 20 and 40 weeks, without fetal anomalies, diabetes mellitus, hypertension, and other maternal complications. Logarithmic transformation was used to obtain the predicted mean AFI values with 95% confidence intervals at each gestational week. Results: The AFI rose from 20 weeks reaching its peak at 30 weeks. After the peak, it declined toward 40 weeks. Comparison of this study with previously published reports revealed differences in the mean AFI values. Conclusions: We obtained the gestational age-specific value of AFI in normal pregnancy for Japanese women.
