Renal urate excretion in five cases of hypouricemia with an isolated renal defect of urate transport.

Renal urate excretion was studied in two familial, one suspected familial, and two isolated cases of hypouricemia due to a renal defect. All had very low plasma urate concentrations. In four cases, the urate clearances were approximately the same as the creatinine clearances, and in one case reduced to about one-third. In all cases the urate clearances were minimally diminished by both pyrazinamide and probenecid. This renal response to the drugs is probably due to and isolated tubular defect in the reabsorptive transport mechanism of urate. Following intravenous administration of uric acid, one patient excreted uric acid in the urine more rapidly than a normal subject. In this patient, uric acid secretion from renal tubules was clearly demonstrated during infusion of uric acid. In de novo synthesis of purine, no definite abnormalities were found by incorporation of glycine-15N to uric acid.

Repair of DNA damaged by methyl methanesulfonate in bacteriophage T4.

A highly purified preparation of T4 endonuclease V does not degrade DNA alkylated with methyl methanesulfonate, and the methyl methanesulfonate sensitivity of T4 wild type and x mutant is not affected by the v mutation. Thus, T4 endonuclease V, the v gene product, does not seem to be involved in a repair or an abortive repair of methyl methanesulfonate-damaged T4 DNA. The x and y genes of T4 and the polA and the uvrD genes of Escherichia coli are concerned with the repair of methyl methanesulfonate-induced damages in T4 DNA. Since effects of the polA and the x or y mutations are additive, it is supposed that there are at least two pathways for the repair of methyl meth-anesulfonate-damaged T4 DNA, one controlled by the x and the y genes and the other in which E. coli DNA olymerase I is involved. The partial suppression of the x gene mutation at high temerature was also demonstrated.

Erythrocyte adenosine kinase activity in gout.

Erythrocyte adenosine kinase (AK) (EC 2.7.1.20) and guanosine monophosphate (GMP) reductase (EC 1.6.6.8) were measured in healthy male controls and primary gout subjects. Adenosine kinase activity in 19 controls and 26 gouty subjects was 0.717 +/- 0.176 and 0.615 +/- 0.128 nmol/mg protein/h, respectively. The difference was statistically significant (p less than 0.05). GMP reductase activity in 39 controls and 46 gouty subjects was 30.90 +/- 6.28 and 33.43 +/- 7.97 mumol/mg protein/h, respectively, without statistically significant difference.

Some factors related to obesity in the Japanese sumo wrestler.

Sumo is an ancient sport in Japan and there are at present over 800 professional sumo wrestlers (rikishis). After entrance into the wrestler society a wrestler takes strenuous daily training together with a very high calorie diet (more than 5,000 cal). Frequency of food intake is twice a day. The average diet of Japanese people contains of 2,279 calories and the meal frequency is generally three times a day. In 96 wrestlers average actual body weight and modified Broca index was 100.4 kg and 143.5%, respectively. In this group the prevalence of overweight with obesity, overweight without obesity, nonoverweight with obesity, and nonoverweight without obesity was 53.4, 39.1, 1.0, and 6.5%, respectively. Also mean serum levels of triglyceride, phospholipid, uric acid, and total protein were significantly higher than those obtained in 89 age-matched healthy males. The incidence of diabetes mellitus, gout, and hypertension in wrestlers was 5.2, 6.3, and 8.3%, respectively, all values being considerably higher than in controls. Weight correlated significantly with skinfold thickness, diastolic blood pressure, total cholesterol, and uric acid in each group. Multiple regression analyses were made treating weight or uric acid as dependent variables in both groups. Obesity, hyperlipidemia, and hyperuricemia in wrestlers were presumed to be caused chiefly by the high calorie diet and partially by the infrequent meal intake.

A case of gouty arthritis associated with Down's syndrome.

Gouty arthritis developed in a twenty-one-year-old male whose karyotyping showed Trisomy 21. His serum urate level was 13.1 mg per 100 ml and his twenty-four-hour urinary uric acid excretion was 431 mg on a low purine diet. Uric acid clearance was decreased to 3.9 ml per minute. The cumulative incorporation of N15-glycine into urinary uric acid during successive seven days was 0.144 per cent, which was within the normal range. Diminished uric acid clearance may be attributed to this hyperuricaemia. The cause of gouty arthritis and Down's syndrome in the patient is discussed.

Protective effect of allopurinol on bronchiolar constriction induced by histamine.

In order to examine the protective effect of allopurinol on experimental asthma produced by histamine aerosol in guinea pigs, allopurinol was administered intraperitoneally resulting in a significant prolongation of survival times. Significant elevation of blood ATP levels was shown in guinea pigs pretreated with allopurinol compared to the control guinea pigs. It is thought that this beneficial effect prevents the loss of functional purine base.

Effect of sex hormones on uric acid metabolism in rats.

The effects of sex hormones on purine metabolism were investigated in rats. No influence on purine synthesis was shown by the injection of estrogen and androgen. The plasma urate levels were significantly lowered from 2.43 +/- 1.04 mg/100 ml to 1.53 +/- 0.57 mg/100 ml by the injection of progesterone. Urinary excretion of uric acid plus allantoin was slightly reduced. These results suggested that progesterone may influence age and sex differences in human plasma urate levels.

Hyperlipidaemia in gout.

Significant elevations of plasma triglyceride and free fatty acids levels were shown in 107 gouty patients, but no significant difference was found in plasma cholesterol and phospholipid levels as compared with control subjects. A positive correlation was found between plasma triglyceride and free fatty acids levels (r = 0.249, P less than 0.05) in gouty patients. The heavy drinkers with gout (15.9% of the patients) had significantly higher plasma triglyceride, free fatty acids and gamma-glutamyltranspeptidase levels than the moderate or non-drinking gouty subjects. These results suggested that excessive intake of alcohol may play an important role in inducing hyperlipidaemia in gout.

Erythrocyte adenosine deaminase and purine nucleoside phosphorylase activity in gout.

1. Erythrocyte adenosine deaminase (EC 3.5.4.4) and purine nucleoside (inosine) phosphorylase (EC 2.4.1.1) were measured in 33 healthy controls and 43 primary gouty subjects. Adenosine deaminase activity in controls and gouty subjects was 0.373 plus or minus 0.108 and 0.457 plus or minus 0.140 A unit per 5-10-3 ml packed red cells per h, respectively. The difference was statistically significant (P less than 0.01). Mean adenosine deaminase: inosine phosphorylase (X10) in primary gout was also significantly higher than in controls (P less than 0.05). Inosine phosphorylase activities in the two groups were not significantly different. 2. When gouty patients were divided into two groups according to weight, normal weight gouty subjects had a higher adenosine deaminase activity and an increased ration of adenosine deaminase to inosine phosphorylase when compared with overweight patients (P less than 0.10). In two control groups divided according to the percentage overweight, such differences were not found. In the case of two gouty groups divided according to the existence of gouty heredity, tophi or renal impairment, adenosine deaminase and inosine phosphorylase activity in the two groups were not significantly different. The possible biochemical role of adenosine deaminase activity in primary gout is discussed.

An experimental study of intraoral carcinogenesis in rats.

A new method has been devised for preparing artificial cecal pouches lined with mucous epithelium in the lower lips of Wistar and Sprague-Dawley rats in order to make carcinogens act continuously for a long time in the oral mucosa. When a 0.5 per cent mineral oil solution of DMBA, a crystal of MC, and a crystal of NG were administered, squamous-cell carcinoma, carcinoma in situ, papilloma, adenoma sebaceum. neurofibroma, fibroma, hemangiosarcoma, hemangiosarcoma, hemangioma, and lymphangioma were successfully produced in the oral mucosa of rats. In addition, interesting findings were obtained concerning tissue changes in the process of carcinogensis in the mucous epithelium.

Seven-year follow-up study of rubella syndrome in Ryukyu with special reference to persistence of rubella hemagglutination inhibition antibodies.

Rubella hemagglutination inhibition (HI) antibodies in 266 children with rubella syndrome born in 1965 in the Ryukyu Islands and their mothers were followed for seven years. Titers of rubella HI antibody in the mothers declined slowly, while those in the children declined rapidly up to 40 months of age. Thereafter decline of titers became extremely slow and only seven cases (three per cent) became seronegative for rubella HI antibody. Rubella HI antibody titers seemed to have no particular correlation to the severity of clinical manifestations.

Familial hypouricaemia due to renal tubular defect of urate transport.

A 28-year-old man was found to have hypouricaemia (plasma uric acid, 0.40 to 1.25 mg/100 ml). The 24-hour urinary urate excretion on a low purine diet was 690 mg, a value higher than the mean value of 419 mg for gouty Japanese patients. Urate clearance was 88.5 ml/min--approximately the same as the endogeneous creatinine clearance. The ratio of urate clearance to creatinine clearance was scarcely altered by pyrazinamide, but diminished by probenecid (from 69.2% to 52.4%). No other renal tubular abnormalities were detected. The findings in this subject may be accounted for by a nearly complete tubular defect in reabsorptive transport mechanisms of urate. A survey of his family revealed at least three similarly affected persons, who were all from consanguineous marriages. The hypouricaemia was transmitted as an autosomal trait.
