Psychiatric, somatic, and gastrointestinal disorders, and Helicobacter pylori infection in children with recurrent abdominal pain.

AIMS: To examine the utility of the Rome II criteria in children with recurrent abdominal pain (RAP) and compare them to those who met Apley's criteria and those who met neither criteria. METHODS: Prospective study in general paediatric clinics in Komagane, Japan. Children with abdominal pain were classified into those who met Rome II criteria, those who met Apley's criteria, and those who met neither. RESULTS: A total of 182 children with RAP participated; 45 children met Rome II criteria, 55 met Apley's criteria, and 82 met neither. Children who met Rome II criteria had a significantly higher prevalence of psychiatric and somatic disorders compared to the group met neither (36% v 6%, 22% v 10%, respectively). The overall prevalence of H pylori was 7%; prevalence increased with age from 3% at age < or = 10 to 10% for children >10 years. Children who met Rome II criteria had a significantly higher prevalence of H pylori infection than the reference group (18% v 4%). In a logistic regression model, all the study variables were included in the model specifying first the Rome II criteria group as the independent variable; psychiatric disorders, H pylori infection, and older age group were independent risk factors. CONCLUSIONS: More than half the children suffering from recurrent abdominal pain met neither Apley's nor Rome II criteria. Children who meet Rome II criteria should be evaluated for psychiatric disorders and should be tested for H pylori infection. Despite the overall trend for a fall in the prevalence of H pylori infection among children in Japan, there are subpopulations of sick children where the prevalence of the infection is relatively high.

Anti-prolactin (PRL) autoantibody-binding sites (epitopes) on PRL molecule in macroprolactinemia.

Macroprolactinemia, in which serum prolactin (PRL) mainly consists of PRL with a molecular mass greater than 100 kDa, has been demonstrated to be associated with hyperprolactinemia. We previously reported that anti-PRL autoantibody is the major cause of macroprolactinemia. In this study, the autoantibody-binding sites (epitopes) on the PRL molecule were examined using deletion mutant PRL. The sera from 159 patients with hyperprolactinemia were screened for macroprolactinemia using the polyethylene glycol method and 18 patients (11%) were diagnosed with macroprolactinemia. The sera from these patients were incubated with glutathione S-transferase-human prolactin (hPRL) fragment fusion proteins immobilized on glutathione sepharose and the amounts of bound immunoglobulin G (IgG) were measured using ELISA. IgG was bound to full-length hPRL1-199 in significantly greater amounts in sera from 14 of 18 patients with macroprolactinemia than in controls. hPRL, but not PRL of other species such as bovine, porcine, rat, or human GH, dose-dependently displaced the binding, suggesting that these patients had hPRL-specific autoantibodies. Deletion of 34 amino acid residues from N-and/or C-terminals significantly reduced the binding and N- or C-terminal fragment alone showed partial but significant binding, suggesting that the major epitopes recognized by anti-PRL autoantibodies are located in both N- and C-terminal residues of the PRL molecule.

Development of microporous covered stents: geometrical design of the luminal surface.

To reduce in-stent restenosis rates we have developed newly designed covered stents, in which a stent strut is buried into a microporous elastomeric cover film to provide a physical barrier against tissue ingrowth and a pharmacological reservoir for drug-eluting. The covered stents were prepared by dip-coating balloon expandable stents mounted on a stainless steel rod in a segmented polyurethane (SPU) solution, and were subsequently subjected to laser-processed microporing (pore diameter, 100 microm; interpore distance, 200 microm). The covered stents, which possessed flat luminal surfaces and micropores that were homogeneously arranged on the whole surface of the covering film, were deployed into the bilateral common carotid arteries of normal New Zealand white rabbits. Angiography after one month of implantation showed all stents were patent with little thrombus formation. The mean thickness of the formed neointimal layers was 292 +/- 177 microm (n=8), which was close to the size in non-covered bare stent (231 +/- 58 microm, n=7), but markedly decreased (about 2/3) from that in the previously developed wrapping-type covered stents (415 +/- 173 microm, P<0.01, n=8).

Electrocardiographic Q-waves as a predictor of mortality in patients with cerebral infarction.

OBJECTIVE: To determine whether abnormal EKG findings could be predictive factors for death after a stroke event. METHODS: Patients with acute cerebral infarction who were 35 to 98 years old during April 1996 through November 2000 were analyzed (n = 216). A standard 12-lead EKG was recorded for each patient after acute cerebral infarction. The authors prospectively investigated the association between abnormal EKG findings and the risk for death after stroke over a 1-year period. RESULTS: Using multivariate Cox proportional hazards models, the authors found age (hazard ratio of mortality per year 1.10, 95% CI 1.06 to 1.15, p < 0.001), sex (female; hazard ratio of mortality 3.42, 95% CI 1.43 to 8.19, p = 0.006), and the presence of Q-waves in more than two leads (hazard ratio of mortality 2.75, 95% CI 1.23 to 6.14, p = 0.013) were independently associated with death after stroke. CONCLUSION: The presence of Q-waves in more than two leads could be a predictive factor for death after acute cerebral infarction.

Comparative study on adhesive performance of functional monomers.

Mild self-etch adhesives demineralize dentin only partially, leaving hydroxyapatite around collagen within a submicron hybrid layer. We hypothesized that this residual hydroxyapatite may serve as a receptor for chemical interaction with the functional monomer and, subsequently, contribute to adhesive performance in addition to micro-mechanical hybridization. We therefore chemically characterized the adhesive interaction of 3 functional monomers with synthetic hydroxyapatite, using x-ray photoelectron spectroscopy and atomic absorption spectrophotometry. We further characterized their interaction with dentin ultra-morphologically, using transmission electron microscopy. The monomer 10-methacryloxydecyl dihydrogen phosphate (10-MDP) readily adhered to hydroxyapatite. This bond appeared very stable, as confirmed by the low dissolution rate of its calcium salt in water. The bonding potential of 4-methacryloxyethyl trimellitic acid (4-MET) was substantially lower. The monomer 2-methacryloxyethyl phenyl hydrogen phosphate (phenyl-P) and its bond to hydroxyapatite did not appear to be hydrolytically stable. Besides self-etching dentin, specific functional monomers have additional chemical bonding efficacy that is expected to contribute to their adhesive potential to tooth tissue.

The systems biology markup language (SBML): a medium for representation and exchange of biochemical network models.

MOTIVATION: Molecular biotechnology now makes it possible to build elaborate systems models, but the systems biology community needs information standards if models are to be shared, evaluated and developed cooperatively. RESULTS: We summarize the Systems Biology Markup Language (SBML) Level 1, a free, open, XML-based format for representing biochemical reaction networks. SBML is a software-independent language for describing models common to research in many areas of computational biology, including cell signaling pathways, metabolic pathways, gene regulation, and others. Bonding efficacy of polyalkenoic acids to hydroxyapatite, enamel and dentin.

Previously, we introduced a methodology to determine the chemical bonding potential of polyalkenoic acids to mineralized tissues through quantification of the degree of ionic bond formation between the carboxyl groups of a polyalkenoic acid with calcium of hydroxyapatite. In a continuation of that study, we now investigated in how far the chemical bonding potential to synthetic hydroxyapatite is influenced by the molecular structure of the polyalkenoic acid and if this also may affect the self-adhesiveness to enamel and dentin. X-ray photoelectron spectroscopy and inductively coupled plasma atomic emission spectrometry were used to quantitatively analyze the chemical bonding efficacy of a polyalkenoic acid consisting of acrylic acid units (PAA) to synthetic hydroxyapatite (HAp) in comparison to the chemical bonding potential of the previously investigated synthesized polyalkenoic acid (s-PA) co-polymer consisting of 90w/w% acrylic and 10w/w% maleic acid units. In addition, the analysis was carried out for enamel and dentin samples. PAA revealed a significantly lower bonding effectiveness with only half of its carboxyl groups bonded to HAp versus about two-third of the carboxyl groups of s-PA. The difference in bonding potential was confirmed by the considerably lower adhesiveness of PAA to enamel and dentin as compared to that of s-PA The present findings indicate that the molecular structure of the polyalkenoic acid significantly influences the chemical bonding efficacy to Hap-based substrates.

Acid pretreatment of titanium implants.

The purpose of this study was to evaluate the effectiveness of several methods of cleaning titanium surfaces as pretreatment for surface modifications by analyzing the chemical interaction of three acids, such as Na(2)S(2)O(8),H(2)SO(4) and HCl, followed by rinsing with acetone or ultrapure water. Chemical evaluation, using X-ray photoelectron spectroscopy (XPS), and mechanical evaluation, using nanoindentation, were employed. XPS revealed that an untreated Ti surface consisted of carbon- and nitrogen-containing contaminant and titanium oxide layer on metallic titanium substrate. The method involving the combination of 10 N HCl and acetone was the most effective of all the methods investigated. Such a combination most effectively reduced values of contamination parameters C/Ti and N/Ti, as well as the intensity of the titanium oxide component in Ti 2p spectra. Chlorine was barely detected from the surface treated with HCl in any concentration. Sulfur from the residual S(2)O(8)(2-) or SO(4)(2-), however, was detected from the samples treated with either Na(2)S(2)O(8) or H(2)SO(4). The S/Ti values depended on concentration of the acidic solution. In addition, nanoindentation measurements revealed that Young's modulus of the surface treated with 0.1-10 N HCl was not significantly different from that of an untreated surface (p > 0.05). Consequently, the HCl/acetone treatment is proposed as an excellent decontamination method for the surface preparation process of Ti.

Chronic granulomatous pleuritis caused by nocardia: PCR based diagnosis by nocardial 16S rDNA in pathological specimens.

Nocardiosis is an uncommon infection caused by the aerobic actinomycete nocardia. Identification of the pathogen is essential for the definitive diagnosis and for an effective treatment. This report describes a case of chronic granulomatous pleuritis caused by nocardia. A 59 year old Japanese man had a history of repeated pyothorax. Right pleural decortication and thoracic drainage were performed. Microbiological examinations of the drained fluid failed to identify a pathogen. Pathological examinations revealed Gram positive filamentous and branching bacilli in the granulomatous lesion of the pleura. Sequencing of the 971 bp 16S ribosomal DNA extracted and amplified from paraffin wax embedded sections identified the microorganism as Nocardia sp. IFM 0860. The patient received sulfamethoxazol/trimethoprim and minocycline. Although the presence of a brain abscess was disclosed by systemic examination, the clinical course has been favourable. In this patient, polymerase chain reaction analysis of 16S ribosomal DNA in pathological specimens was useful in making an accurate diagnosis of nocardiosis and in determining the appropriate treatment.

Angiotensin-converting enzyme inhibitor withdrawal and ACE gene polymorphism.

AIMS: Withdrawal of angiotensin-converting enzyme (ACE) inhibitors may affect the progression of chronic renal failure and an insertion/deletion (I/D) polymorphism of the ACE gene may influence it. METHODS: We retrospectively collected patients with chronic glomerulonephritis and benign nephrosclerosis who discontinued ACE inhibitor use. The relationship between the decline of renal function after the withdrawal and the influencing factors such as ACE gene polymorphism, blood pressure and proteinuria were evaluated using multiple regression analysis. RESULTS: Forty-two patients (initial serum creatinine 0.5 - 6.5 mg/dl) had been treated and discontinued ACE inhibitor use. Only patients with the II or DI genotypes of the ACE gene developed the deterioration of renal function, starting at 2 months after the withdrawal. Stepwise regression analysis revealed that the level of proteinuria after the withdrawal, presence of the insertion of ACE gene and serum creatinine level at the time of withdrawal mainly influenced the decline of renal function after the withdrawal (adjusted R2 = 0.48). CONCLUSION: Withdrawal of ACE inhibitor causes the deterioration of renal function in patients with the II or DI genotypes, high proteinuria after the withdrawal, and high serum creatinine level at the withdrawal, which probably causes the rebound increase in serum ACE activity.

Laser-perforated membranous biomaterials induced pore size-dependent bone induction when used as a new BMP carrier.

Previously we found that laser perforation of a collagen membrane (35 microm thickness, Koken Co., Tokyo) produced an effective bone morphogenetic protein (BMP) carrier, if the created pore sizes were larger than 0.5 mm. In this study we applied the same technique to create pores of 0.2 and 1.0 mm in a thicker (1.2 mm thickness) porous biodegradable membrane made of polylactic acid and an epsilon-caprolactone copolymer (PLA-CL) to obtain an effective membranous BMP carrier with higher mechanical strength. Pieces of PLA-CL (0.5 x 1.0 x 0.12 cm) combined with rhBMP-2 (5 microg) were implanted subcutaneously into rats and processed for analyses at 1-3 weeks. The laser-perforated PLA-CL membranes equipped with 1.0 mm pores induced mineralization beginning from the margins of the pores judging from the X-ray patterns, but bone formation seemed to proceed irregularly inside the pores. In the perforated PLA-CL membrane with 1.0-mm pores bone formation did not significantly increase compared with the nonperforated one. This was due to the fact that the PLA-CL membrane was already a porous structure (85% porosity). In contrast with laser-perforated PLA-CL 0.2 mm pores, bone was induced on the collagen fibers and fiber bundles inside the pores. The different patterns of bone formation between the PLA-CL membranes with 1.0 and 0.2 mm pores seemed to be related to the active formation of perpendicular collagen fibers through the 0.2 mm pores.

[Aortic dissection presenting with neurologic sign; report of a case].

A 59-year-old man was admitted to our hospital for neurological deficit and left hemiplegia, presenting remission in the short time. Computed tomography(CT) of the brain was normal. CT of the chest showed a thoracoabdominal aortic dissection from the ascending aorta to the bilateral femoral artery, that involved all cervical arteries. The intimal flap separated the false and true lumens and there was flow in both lumens. That phenomenon can be due to the extension of the dissection along the aorta, occluding the origin of the cervical arteries and the change of the flow by the intimal flap. He was amputated his left leg below knee on the 1 postoperative day (POD), but discharged at 70 POD without neurologic complications.

Biomaterial surface chemistry dictates adherent monocyte/macrophage cytokine expression in vitro.

An in vitro human monocyte culture system was used to determine whether adherent monocyte/macrophage cytokine production was influenced by material surface chemistry. A polyethylene terephthalate (PET) base surface was modified by photograft copolymerization to yield hydrophobic, hydrophilic, anionic and cationic surfaces. Freshly isolated human monocytes were cultured onto the surfaces for periods up to 10 days in the presence or absence of interleukin-4 (IL-4). Semi-quantitative RT-PCR analysis on days 3, 7 and 10 of cell culture revealed that interleukin-10 (IL-10) expression significantly increased in cells adherent to the hydrophilic and anionic surfaces but significantly decreased in the cationic surface adherent monocytes/macrophages. Conversely, interleukin-8 (IL-8) expression was significantly decreased in cells adherent to the hydrophilic and anionic surfaces. Further analysis revealed that the hydrophilic and anionic surfaces inhibited monocyte adhesion and IL-4-mediated macrophage fusion into foreign body giant cells (FBGCs). Therefore, hydrophilic and anionic surfaces promote an anti-inflammatory type of response by dictating selective cytokine production by biomaterial adherent monocytes and macrophages. These studies contribute information necessary to enhance our understanding of biocompatibility to be used to improve the in vivo lifetime of implanted medical devices and prostheses.

In situ hydrogelation of photocurable gelatin and drug release.

We devised an in situ tissue-adhesive, drug-release technology based on a photoreactive gelatin, which allows in situ drug-incorporated gel formation on living tissues and sustained drug release directly on diseased tissues. Styrene-derivatized gelatins, synthesized by condensation reaction of gelatin with 4-vinylbenzoic acid, were photopolymerized in the presence of a water-soluble camphorquinone derivative as a photoinitiator upon visible-light irradiation to form swollen gels. Using albumin as a drug model, gelation characteristics and drug-release characteristics easily were manipulated by material variables, formulation variables, and operation variables. Tissue adhesivity of the gel was superior to that of fibrin glue. The biologic response, which was evaluated by intraperitoneal implantation in rats, showed that the gel was biodegraded and biosorbed, without cytotoxicity, within a few months after implantation. An in situ processable tissue-adhesive local drug release system effectively may be used to help inhibit tumor recurrence.

Performance of the carbon nano-tube assembled tip for surface shape characterization.

The carbon nano-tube (CNT) has ideal properties for atomic force microscope (AFM) tips. We assembled a CNT using 2 three-axial manipulators in a scanning electron microscope (SEM) chamber. In this process, the length and angle of the CNT were adjusted by observing the SEM image, after which the CNT was glued by amorphouscarbon. The results of performance are as follows. The lifetime of the CNT tip proved to be 5 times better than that of the silicon tip when continuously measuring the micro-roughness of a Czochralski (Cz) P-type (100) silicon wafer. The CNT tip is able to trace a narrow space (width less than 1 microm) better than the conventional silicon tip because of its high aspect ratio. The relationship between the observed image and CNT geometry is discussed herein.

Ascending fractional pulse pressure closely relating to large artery function.

It is not known whether ascending fractional pulse pressure (PPf) is related to large artery function. This study was to evaluate whether PPf is associated with large artery function by augmentation index. A total of 190 subjects were enrolled (age range: 50 to 78 years) who had normal contractions, no local asynergy, and no history of myocardial infarction. The ascending aortic pressure was measured using a fluid-filled system. To quantify the relative magnitude of the pulsatile to mean artery pressure, we normalised the pulse pressure to the mean pressure and referred to this value as PPf and the association between the PPf and the augmentation index was investigated. Augmentation index showed significant associations with PPf by univariate analysis (R = 0.690, P < 0.001). The associations between PPf and the factors influencing large artery function were examined by multivariate analysis, and PPf revealed significant associations with age, mean aortic pressure, heart rate and type 2 diabetes (R(2) = 0.477, P < 0.001). The results were that ascending aortic PPf is closely associated with large artery function detected by augmentation index.

Low fractional diastolic pressure in the ascending aorta increased the risk of coronary heart disease.

Although the fractional pulse pressure in the ascending aorta is related to the occurrence of coronary heart disease (CHD) and restenosis after percutaneous transluminal coronary angioplasty, the relative values of diastolic pressure in the ascending aorta at the onset of CHD have not been reported. The purpose of this study was to evaluate whether the relative values of diastolic pressure are associated with the risk of CHD. For this study, we enrolled 406 patients with chest pain, normal contractions, no local asynergy, and no history of myocardial infarction. We measured the ascending aortic pressure using a fluid-filled system. To quantify the relative diastolic pressure, we normalised the diastolic pressures to the mean pressure and referred to this value as the fractional diastolic pressure (FDP). We investigated the association between the FDP and the risk of CHD. Low FDP in the ascending aorta was associated with an increased risk of CHD. The multiple-adjusted odds ratio of CHD was 1.68 (95% CI, 0.67-4.22) in FDP for the middle tertile of the level. The multiple-adjusted odds ratio of CHD was 2.20 (1.16-4.75) in FDP for the lowest tertile compared with the highest tertile. FDP was associated with the risk of CHD.
