Pharmacokinetic study of S-1, a novel oral fluorouracil antitumor drug.

S-1 is a novel oral fluorouracil antitumor drug that combines three pharmacological agents: tegafur (FT), which is a prodrug of 5-fluorouracil (5-FU); 5-chloro-2,4-dihydroxypyridine (CDHP), which inhibits dihydropyrimidine dehydrogenase (DPD) activity; and potassium oxonate (Oxo), which reduces gastrointestinal toxicity. Phase I and early Phase II clinical trials have already been completed. On the basis of the results of these trials, 80 mg/m2/day, given daily in two divided doses after breakfast and supper, a 28-day consecutive oral regimen is recommended. In this study, we investigated the pharmacokinetics of 5-FU, intact FT, CDHP, and Oxo, after administration of S-1, at a standard dose of 80 mg/m2/day, in advanced cancer patients. Twelve patients were recruited to the study; 5 patients with gastric cancer, 4 with colorectal cancer, and 3 with breast cancer. Among them, analysis was conducted on 12 patients for single administration and on 10 patients for consecutive administration. The initial dose of S-1 for each patient was determined according to his/her body surface area (BSA) as follows: for BSA < 1.25 m2, 80 mg/body/day; for 1.25 m2 < or = BSA < 1.5 m2, 100 mg/day; and for 1.5 m2 < or = BSA, 120 mg/day. For single administration, half of the standard dose was used. For 28-day consecutive administration, the standard dose was given daily in two divided doses. The average single dose per BSA was 35.9 mg/m2 (31.7-39.7 mg/m2). Pharmacokinetic parameters of plasma 5-FU were as follows: Cmax, 128.5 +/- 41.5 ng/ml; Tmax, 3.5 +/- 1.7 h; AUC(0-14), 723.9 +/- 272.7 ng x h/ml; and T(1/2), 1.9 +/- 0.4 h. In the 28-day consecutive regimen, there were no fluctuations in pharmacokinetics nor any drug accumulation. Because the pharmacokinetics of orally administered S-1 is almost similar to that of continuous i.v. infusion of 5-FU, we concluded that S-1 may improve patients' quality of life.

[Vitamin B1].

Vitamin B1 (thiamin), taken-up into cells, is converted to thiamin diphosphate (TDP), and TDP acts as a cofactor for several enzymes involving in carbohydrate metabolism. CoA-dependent oxidative decarboxylation of pyruvate is catalyzed by pyruvate dehydrogenase multienzyme complex (PDC) with NAD+ as an electron acceptor in most organisms involving mammals and higher plants. PDC consists of three component enzymes, one of which is pyruvate dehydrogenase (lipoamide) which contains TDP as a prosthetic group. Similar multienzyme complex for 2-oxoglutarate or branched chain 2-oxoacids is also found in mammals. In anaerobic bacteria, archaebacteria and anaerobic protozoa, pyruvate:ferredoxin oxidoreductase (PFOR) functions for the oxidative decarboxylation of pyruvate with ferredoxin in place of NAD+. PFOR contains TDP as a cofactor; however its structure is quite different from PDC and 1-3[4Fe-4S] clusters are involved as redox centers. Pyruvate:NADP+ oxidoreductase (PNOR), which catalyzes the oxidative decarboxylation of pyruvate with NADP+ as an electron acceptor, occurs in mitochondria of Euglena gracilis, a protist containing chloroplasts. PNOR consists of two functional domains, one of which contains TDP and 3[4Fe-4S] clusters and resembles PFOR. Another domain involves FMN and FAD as redox centers and its structure is similar to NADPH-cytochrom P450 reductase.

[Vitamin B12].

Vitamin B12 is unique among all the vitamins in that it contains not only a complex organic molecule but also an essential trace element, cobalt. Vitamin B12 is synthesized in some bacteria but not in animals and plants. Intestinal absorption and subsequent plasma transport of vitamin B12 are mediated by specific vitamin B12-binding proteins and their receptors in mammals. Vitamin B12 taken up by the cells is enzymatically converted to coenzyme forms of vitamin B12, methyl- and adenosyl-vitamin B12, which function as coenzymes of methionine synthase (EC 2.1.13), involved in methionine biosynthesis, and methylmalonyl-CoA mutase (EC 5.4.99.2), involved in oxidation of odd-numbered fatty acids and amino acids (valine, isoleucine, and threonine), respectively. Chemical properties, physiological function, and intracellular metabolism of vitamin B12 are summarized in this section.

[A mechanism for the anti-inflammatory effect of nedocromil; inhibition of both adhesion molecule expression on eosinophils and endothelial cells, and eosinophil chemotactic activities].

The accumulation of eosinophils in the airway is one of the characteristics seen in patients with bronchial asthma. One of the newly developed anti-asthma drugs (controller), nedocromil sodium (nedocromil) is known to suppress the influx of eosinophils into allergic lesions. However, little is known about this mechanism. Therefore, in this report we investigated the effects of nedocromil on Mac-1 expression on PAF-stimulated eosinophils, and adhesion molecule expression on endothelial cells stimulated by either IL-1 beta or IL-4. We also investigated the eosinophil chemotaxis. A significant suppression of the Mac-1 expression on PAF-induced eosinophils was observed at both concentrations of 10(-5) and 10(-7) M of nedocromil. The expression of adhesion molecules, particularly ICAM-1 and E-selectin, on IL-1 beta-stimulated human umbilical vascular endothelial cells (HUVEC) was significantly suppressed at these concentrations, whereas the VCAM-1 expression was not changed. No significant suppression of VCAM-1 expression on IL-4-stimulated HUVEC was observed, although there was a tendency of suppression at these concentrations. On the other hand, the expression of the E-selectin molecule was significantly suppressed by nedocromil even under resting (non-stimulated) condition. PAF-induced eosinophil chemotactic activities were also suppressed at these concentrations in a dose-dependent manner. These results suggested that nedocromil suppressed the influx of eosinophils to inflammatory lesions by inhibiting not only the expression of the Mac-1 on eosinophils and of E-selectin and ICAM-1 molecules on HUVEC, but also the eosinophil chemotactic activities.

Lumbar spinal synovial cyst presenting with neurological deficit. A case report and review of the literature.

A spinal synovial cyst is a rare condition. We reported a case of lumbar spinal synovial cyst presenting neurological deficit. A 78-year-old woman was admitted to our hospital with the low back pain radiating to the left buttock. A myelography with computed tomography and magnetic resonance imaging revealed an extradural cystic lesion at the L5 and S1 level. At the time of surgery, a standard posterior approach was used to expose the posterior elements from L5 to S1. An en bloc laminectomy and total removal of the cyst was performed at the L5 to S1 level. The postoperative recovery was uneventful except for a slight pain in the left leg which persisted for some time.

A gene cluster for 6-deoxy-L-talan synthesis in Actinobacillus actinomycetemcomitans.

The serotype c antigen of Actinobacillus actinomycetemcomitans consists of 6-deoxy-l-talose. A gene cluster involved in the synthesis of serotype-specific polysaccharide antigen was cloned from the chromosomal DNA of A. actinomycetemcomitans NCTC 9710 (serotype c). This cluster consisted of 17 open reading frames. Escherichia coli produced the polysaccharide that reacts with the serotype c-specific antibody when transformed with a plasmid containing the cluster. Comparing the structure of the gene cluster with a similar cluster from A. actinomycetemcomitans Y4 (serotype b), which produces a polysaccharide consisting of l-rhamnose and d-fucose, revealed that a 5.7 kb region containing seven genes in the cluster from strain Y4 was replaced by a 3.8 kb region containing three genes in strain NCTC 9710. The results suggest that these region, as well as dTDP-6-deoxy-l-talose-forming dTDP-4-keto-l-rhamnose reductase, is essential to the production of extracellular polysaccharide specific to serotype c.

The intron 5-inserted form of rat erythropoietin receptor is expressed as a membrane-bound form.

The cDNA encoding an intron 5-inserted form of the erythropoietin receptor (I5Epo-R) has been cloned from rat. DNA sequence analysis reveals that the insertion of intron 5, which consists of 79 bp, causes a shift in reading frame and results in termination in the region of exon 7. The deduced amino acid sequence is composed of 316 amino acid residues, which is a molecular weight of 34220. To study the function of rat I5Epo-R, we established a Chinese hamster ovary cell line expressing rat I5Epo-R. Western blot analysis and binding studies with 125I-recombinant human erythropoietin showed that the transfected cells expressed rat I5Epo-R with a molecular size of 36 kDa as a membrane-bound form, but not as a soluble form, and had a single class of binding sites with a Kd of 700 pM.

Genes for sexual behavior.

The mating behavior of Drosophila melanogaster is a stereotyped sequence of fixed action patterns, composed of orientation, tapping, singing, licking, attempted copulation and copulation. Mutations that block a unique aspect of mating behavior were isolated and analyzed at the cellular and molecular levels. The wild-type counterparts of the mutated genes were shown to rescue the phenotypes by their ubiquitous or targeted expression in some of the mutants. This strategy of artificial control of fly behavior opens up an avenue for studies to identify the neural center for individual behavioral actions.

Suppressive effect of insulin on the synthesis of sucrase-isomaltase complex in small intestinal epithelial cells, and abnormal increase in the complex under diabetic conditions.

An abnormally high level of the sucrase-isomaltase (SI) complex in the small intestine of rats with streptozotocin-induced insulin-dependent diabetes mellitus (IDDM) was normalized in 11 h by the administration of insulin, in addition to normalization of the blood glucose level. Phlorizin, an inhibitor of renal glucose reabsorption, also caused normalization of the blood glucose level in the IDDM rats; however, the level of the SI complex was barely changed. When mucosa explants were cultured in a medium, the SI complex synthesized during the cultivation was accumulated as its precursor protein without maturation, owing to the absence of pancreatic proteases, and the amount of the precursor protein that accumulated in the explants was decreased by the addition of insulin into the medium. Further, the mRNA level of the SI complex in the explants incubated with insulin was obviously lower than that in the absence of insulin. These results indicate that insulin has a suppressive effect on the synthesis of the SI complex, presumably by decreasing the transcriptional level of the gene encoding the complex, in small-intestinal epithelial cells. Thus the synthesis of the SI complex might exceed normal levels in the epithelial cells as a direct result of the depletion of insulin under IDDM conditions.

Effects of Microwave Heating on the Loss of Vitamin B(12) in Foods.

To clarify the effects of microwave heating on the loss of vitamin B(12) in foods, raw beef, pork, and milk were treated by microwave heating and then their vitamin B(12) contents were determined according to a chemiluminescent vitamin B(12) assay with hog intrinsic factor. Appreciable loss ( approximately 30-40%) of vitamin B(12) occurred in the foods during microwave heating due to the degradation of vitamin B(12) molecule by microwave heating. When hydroxo vitamin B(12), which predominates in foods, was treated by microwave heating and then analyzed by silica gel 60 thin layer chromatography, two vitamin B(12) degradation products were found. One of the compounds with a R(f)() of 0.16 was purified and partially characterized. The vitamin B(12) degradation product did not show any biological activity in the growth of a vitamin B(12) requiring microorganism, Euglena gracilis Z, and was not bound to hog intrinsic factor, a mammalian vitamin B(12) binding protein. Intravenous administration of the compound (1 µg/day) for 7 days to rats showed that the compound neither has toxicity nor acts as a vitamin B(12) antagonist in mammals. These results indicate that the conversion of vitamin B(12) to the inactive vitamin B(12) degradation products occurs in foods during microwave heating.

Isolation and characteristics of yeasts able to grow at low concentrations of nutrients.

Seven oligotrophic yeasts, which can grow in a 10(4)-fold dilution of malt-yeast-glucose-peptone medium (10(-4) YM), were mainly isolated from soil. These yeasts belong to the Cryptococcaceae. When inoculated at about 10(2) cells/ml in 10(-4) YM, the isolates grew to 1.4 x 10(3)-2.4 x 10(5) cells/ml after 3 days. Some culture collection yeasts fell into three groups according to their growth characteristics in 10(-4) YM, one group showing characteristics of the oligotrophic yeasts. The half-saturation values of uptake by the five isolated oligotrophic yeasts for D-glucose, L-leucine and L-amino acids were 6.0-25.0, 1.7-43.3 and 3.5-21.6 microM, respectively. The oligotrophic yeasts suspended in 10 mM-phosphate buffer (pH 6.0) had high tolerances for starvation, and remained more than 15% viable after 90 days of starvation.

Initial and follow-up results of the ACS multi-link stent: a single center experience.

The Palmaz-Schatz (PS) stent has effectively reduced restenosis; however its rigidity makes it sometimes difficult or impossible to deliver. The initial and follow-up outcomes with the ACS Multi-Link stent (deployed from April to November 1995) were evaluated in 70 patients (79 lesions): unplanned in 34% (abrupt closure 1%; threatened closure 5%; suboptimal results 28%) and planned in 66%. Three to six month follow-up angiograms were analyzable in 67 lesions; 96% procedural (in nine lesions PS stenting had failed) and 95% clinical success were achieved. In-hospital mortality was 1.4%. Myocardial infarction occurred in 2.9%, and subacute stent thrombosis in 1.4%. Stenting improved immediately the minimal luminal diameter (from 0.97+/-0.41 mm to 2.72+/-0.31 mm), but at 6 months it had decreased to 1.89+/-0.44 mm. Angiographic restenosis (<50% diameter stenosis) occurred in 11, a rate of 16.4%; target lesion revascularization (TLR) was required in six (re-PTCA in five or bypass surgery in one; 6/67=8.7%). Actuarial 1-2 year survival rate was 91%, 80% surviving free from major complications or need for TLR. We conclude that the ACS Multi-Link stent can be implanted in lesions unsuited for the PS stent with a high success rate, and an anticipated restenosis rate perhaps comparable to with the PS stent.

Dual radionuclide single-photon emission computed tomography in the prediction of further ischemic risk after acute myocardial infarction.

To evaluate whether the findings of dual single-photon emission computed tomography (SPECT) with technetium-99m pyrophosphate (Tc-99m PPi) and thallium-201 were predictive of further cardiac events in their hospital course, we studied 130 patients recovering from acute myocardial infarction (AMI). Fifty-four patients showed overlapping of Tc-99m PPi and thallium-201 in the same location (overlap positive group), and 76 patients had no overlap (overlap negative group). Of the 130 patients, 36 (28%) had a cardiac event. In patients in the overlap positive group, the incidence of subsequent events was significantly higher than in patients in the overlap negative group (44% versus 16%; p < 0.001). In the overlap positive group, the Tc-99m PPi uptake score and the number of overlap segments in patients with further events was significantly higher than those in patients without further events (10.2 +/- 5.1 versus 6.4 +/- 4.1, p < 0.005 and 5.2 +/- 2.0 versus 3.3 +/- 1.3, p < 0.001, respectively). These results suggest that patients who have a Tc-99m PPi and thallium-201 overlap negative scan can be candidates for early hospital discharge. In contrast, patients who have a greater number of overlap segments may require early catheterization and revascularization, so that simultaneous SPECT imaging Tc-99m PPi and thallium-201 might be useful for identifying patients with further ischemic risk after AMI in their hospital course.

Expression of the mRNA for types I and II interleukin-1 receptors in dental tissues of mice during tooth development.

Interleukin-1 (IL-1) can exert its pleiotropic effects on nearly every tissue by binding to its cognate receptor. Two types of IL-1 receptors have been identified. A large number of cell types have been shown to possess IL-1 receptors in vitro and in vivo, but few studies have addressed the question of expression in dental tissues in vivo. Using in situ hybridization in normal newborn, young and adult mice, we have examined the cellular distribution of both types of IL-1 receptors in dental tissues. In the ameloblast layer of incisors and molars, the mRNA for the type I IL-1 receptor (IL-1RI) and the type II IL-1 receptor (IL-1RII) was detected at the presecretory stage. The expression level markedly increased and remained during amelogenesis at the secretory stage. At the maturation stage, however, the transcripts for both IL-1RI and -II mRNA disappeared. Expression of IL-1RI and -II mRNA was also observed in odontoblasts after crown morphogenesis had been completed, and continued in these cells during dentinogenesis. No transcripts were detected in stratum intermedium cells and other cells in dental follicle, stellate reticulum, dental papilla, or pulp. Additionally, both types of IL-1R mRNA were also detected in osteoclasts on surfaces of alveolar bone. These results demonstrated for the first time that enamel-secreting ameloblasts and dentine-secreting odontoblasts express IL-1RI and -II mRNA, suggesting that IL-1 plays a regulatory role in the function of ameloblasts and odontoblasts during tooth development of mice.

Importance of type IV collagen, laminin, and heparan sulfate proteoglycan in the regulation of labyrinthine fluid in the rat cochlear duct.

The distribution of major components of the basement membrane, such as type IV collagen, laminin, and heparan sulfate proteoglycan (HSPG), was investigated in the rat cochlear duct. Immunofluorescence demonstrated that type IV collagen, laminin and HSPG were distributed along capillaries in the cochlear duct, including the stria vascularis, spiral ligament, spiral prominence and spiral limbus. Additionally, type IV collagen, laminin and HSPG were found to be distributed from the basement membrane of Reissner's membrane to that of the spiral prominence in a linear pattern. The scala media was surrounded by these basement membrane components, demarcating endolymph from perilymph, along epithelial cells except at the stria vascularis. These findings suggest that type IV collagen, laminin and HSPG create the anatomical separation between endolymph and perilymph, thus indicating that they may be involved in the regulation of fluid transport between the endolymph and perilymph.
