[Effect of blockage of the afferent lymphatic vessels on the development of popliteal lymph nodes in the rat].

The effect of blockage of the afferent lymphatic vessels on the development of popliteal lymph nodes in the rat was studied. The afferent lymphatic vessels to each popliteal node were surgically interrupted at the lowest edge of the popliteal fossa at 3, 7 or 28 days after birth and the popliteal nodes were obtained from treated animals at 4, 8 or 16 weeks after the operation. At 7 days after birth, each popliteal node was small and weighed 0.2 mg. Its parenchyma consisted of reticular cells and a small number of dispersed lymphoid elements. Four weeks after birth, each node weighed 4-5 mg, and its parenchyma comprised two layers, an outer continuous layer of peripheral cortex containing 40-50 lymph follicles and an inner discontinuous layer of deep cortex made up of 4-5 deep cortical units. At 10-12 weeks after birth, each node weighed about 10 mg and showed full structural development; the peripheral cortex contained 100-130 lymph follicles and the deep cortex consisted of 4-6 well developed units. The popliteal node was drained by 4-6 afferent lymphatic vessels, which opened into the subcapsular sinus of the node. Each lymphatic opening was topographically associated with a respective deep cortical unit, as previously described by Bélisle and Sainte-Marie (1982). In animals treated at 3 or 7 days after birth, the development of the popliteal nodes was considerably inhibited. Four weeks after surgery, each node showed 10-30 lymph follicles in the peripheral cortex and 1-3 small units in the deep cortex. Sixteen weeks after surgery, the node weighed about 4 mg and its cortex exhibited about 50 lymph follicles in the peripheral cortex and only 2 units in the deep cortex. The popliteal nodes of the treated animals generally received 2 afferent lymphatic vessels. In animals treated at 4 weeks after birth, the popliteal nodes showed no gain in weight for following 16 weeks. Four weeks after surgery, each node usually had 4-5 deep cortical units and 50-60 lymph follicles. Thereafter, some units and their overlaying peripheral cortex underwent atrophy, while others persisted. Sixteen weeks after surgery, the popliteal node showed only 2 deep cortical units and 50-60 lymph follicles, and was drained by 2 afferent lymphatic vessels. Surgical interruption of the afferent lymphatic vessels to the popliteal node at the lowest edge of the popliteal fossa did not obliterate all the draining lymphatic vessels , but reduced the number of vessels opening into the node.(ABSTRACT TRUNCATED AT 400 WORDS)

[Evaluation of mid-ventricular obstruction of hypertrophic cardiomyopathy by real-time two-dimensional Doppler flow imaging: a case report].

Mid-ventricular obstruction was evaluated in a case of hypertrophic cardiomyopathy using a real-time two-dimensional Doppler flow imaging system. A 60-year-old woman was referred to our hospital because of oppressive precordial sensation., I-mode echocardiography showed asymmetric septal hypertrophy: thickness of the end-diastolic left ventricular posterior wall was 9 mm, and that of the interventricular septum was 19 mm. However, there was no systolic anterior motion of the mitral apparatus. Doppler color flow imaging showed a mid-left ventricular narrowing in late-systole and a mosaic pattern was depicted from the mid-ventricle to the outflow tract. Continuous wave Doppler echocardiography disclosed a peak velocity of 2.0 m/sec (pressure gradient (PG) = 16 mmHg). In the right ventricular outflow tract, a mosaic pattern was also seen and a peak velocity of 1.5 m/sec was detected (PG = 9 mmHg). These results were nearly identical with the data measured by cardiac catheterization. Thus, it was concluded that intraventricular obstruction of hypertrophic cardiomyopathy is diagnosed by observing the flow image and flow velocities in the ventricle using a real-time two-dimensional Doppler flow imaging system.

Methicillin-resistant Staphylococcus aureus in nosocomial infections in the surgical ward and operating room.

In this study 214 strains of Staphylococcus aureus were isolated from clinical specimens on the surgical ward from 1983 to 1988 and in addition, 62 airborne strains were collected in the operating room. Highly methicillin-resistant strains of S.aureus (H-MRSA, MIC greater than 100 micrograms/ml) not detected in 1983 showed a significant increase in frequency by 1987 accounting for about 60% of MRSA (MIC greater than or equal to 12.5 micrograms/ml). Countermeasures instituted in 1987 such as the use of disinfectant chlorhexidine alcohol significantly decreased the frequency of MRSA and H-MRSA isolates in 1988. In our study of coagulase type, MRSA type IV strains were predominant until 1984, whereas after 1986 type II was prevalent. All airborne strains collected in the operating room were methicillin-sensitive S.aureus, with type VII currently epidemic. We therefore concluded that cross infection with MRSA took place on the surgical ward rather than in the operating room.

[Correlation between coagulase typing and antibiotic susceptibility in methicillin resistant Staphylococcus aureus].

One hundred and fifty-two strains of Staphylococcus aureus isolated from clinical specimens from 1983 to 1987 were examined in their susceptibility against various antibiotics and in their coagulase typing. The isolation frequency of methicillin resistant strains (MRSA) was 61.6%, and highly methicillin resistant (MIC greater than 100 micrograms/ml) group occupied 81.3% of MRSA in 1987. From the study of coagulase type, it was found that current epidemic strains in our ward was type II. In MRSA, type IV strains were predominantly isolated until 1984, but after 1986 most of all MRSA strains belonged to type II. According to the coagulase type-classification of MRSA, the isolation frequency of type II was 97.5% of highly methicillin resistant group. Type II strains were more sensitive to MINO and OFLX than type IV, although in the susceptibility to DMPPC type II was less sensitive than type IV. In type IV, the percentage of beta-lactamase-producing strains was 79.3%, and type II, 33.3% with the high significance of statistical difference (p less than 0.01). In conclusion, a close correlation was suggested between beta-lactamase producing activity and the mechanism of resistance to non-beta-lactam antibiotics as to MINO, OFLX in type IV strains. On the other hand, in type II strains the mechanism of resistance seemed to be simply explained by changes of penicillin-binding-protein (PBP), and there was little concern with beta-lactamase, and other antibiotic-modifying enzymes.

A monoclonal anti-platelet antibody which recognizes p55 antigen.

A monoclonal antibody (MoAb) H229(IgGl) was obtained by fusion between SP2/0, mouse myeloma cell line and spleen cells from BC3Fl mice immunized with HEL, a erythroleukemia cell line. MoAb H229 precipitated a 55 kilodalton(KD) molecule in reduced condition. It reacted with platelets and megakaryocytes, but not with monocytes, granulocytes, lymphocytes, thymocytes, red blood cells (RBC), colony-forming units-granulocyte/monocyte (CFU-GM), and burst forming units- erythroid (BFU-E). These findings suggest that MoAb H229 reacts with platelets specifically. However, MoAb H229 did not inhibit platelet aggregation induced by ristocetin, ADP, epinephrine and collagen.

[Tricuspid valve replacement in the true annulus of Ebstein's anomaly without involvement of atrioventricular conduction disturbance].

Two female patients with Ebstein's anomaly, aged 12 and 45 years, underwent new procedures of tricuspid valve replacement in the true annulus without involvement of atrioventricular conduction disturbance. The first patient had a relatively small atrialized right ventricle. A Dacron patch was sutured on the atrialized portion from the right ventricular side. Buttress stitches were placed on the whole of the atrialized ventricle except at the part of the His' bundle. A porcine valve was sutured in the true annulus of the tricuspid valve and on the top of the patch. The second patient had a fairly large atrialized ventricle, and plication was made only in the inferior part of the atrialized ventricle form the coronary sinus. A porcine valve was sutured in the true tricuspid annulus except at the part of Koch's triangle, where the porcine valve was placed on the false annulus.

Ornithine decarboxylase antizyme in kidneys of male and female mice.

Antizyme, a protein inhibitor of ornithine decarboxylase (ODC), was shown to be induced in mouse kidney by repeated injection of putrescine. Antizyme was also present as a complex with ODC in the kidney of untreated mouse. The amount of the renal ODC-antizyme complex was 3-fold higher in male mice than in female mice. On the contrary, the proportion of ODC present as a complex with antizyme was 24-fold higher in females than in males, and the decay of renal ODC activity after cycloheximide treatment was about 5-fold more rapid in females than in males. Administration of testosterone to female mice, a procedure known to prolong the half-life of renal ODC, increased both ODC activity and the content of ODC-antizyme complex, but decreased the antizyme/ODC ratio in the kidney. These results are consistent with the previous observation in HTC cells that the decay rate of ODC activity in the presence of cycloheximide correlated well with the proportion of ODC present as a complex with antizyme, suggesting the ubiquitous role of antizyme in ODC degradation.

Succinic acidemia: a new syndrome of organic acidemia associated with congenital lactic acidosis and decreased NADH-cytochrome c reductase activity.

Two siblings with succinic acidemia, a hitherto unreported organic acidemia, were investigated. Succinic acidemia, lactic acidosis and respiratory distress were observed in one of the siblings, who died 37 days after birth, and succinic acidemia was also detected in the next sibling at the fetal stage. NADH-cytochrome c reductase activity was significantly low in both cases and NADH-ferricyanide reductase activity was also low in the fetal case, suggesting a complex I deficiency of the electron transport system in the mitochondrial membrane.

[Bioactivity and heterogeneity of human growth hormone in urine from acromegalic patients].

Bioactivity of hGH in urine from five acromegalic patients was determined by Nb2 rat lymphoma bioassay (Nb2BA) and IM-9 receptor modulation assay (RMA). One urine sample (case 1) was concentrated by dialysis and lyophilization. Bioactivity by IM-9 RMA showed close correlations with immunoactivity by RIA in serial dilutions (RMA/RIA; 0.66-1.77). The estimates of bioactivity by Nb2BA were higher in all diluted samples except undiluted one than those of immunoactivity. Four samples were concentrated by affinity chromatography, dialysis and lyophilization. The ratio of RMA and RIA estimates was between 0.81-1.24, and that of Nb2BA and RIA estimates was between 0.46-2.62. Heterogeneity of urinary hGH was analyzed by gelfiltration using TSK-G-3000SW column by high performance liquid chromatography (HPLC) and by sensitive sandwich enzyme immunoassay. In addition to the main peak of 22K molecular weight (more than 97%), very small peaks of 40-60K (big hGH) and 150K (big-big hGH) were detected in chromatographic profile. Urinary hGH from acromegalic patients was bioactive in IM-9 RMA and Nb2BA. The predominant form of urinary hGH from acromegalic patients was little hGH, but big and big-big hGH were detected by sensitive sandwich enzyme immunoassay.

Changes in ornithine decarboxylase and antizyme activities in developing mouse brain.

A macromolecular inhibitor to ornithine decarboxylase (ODC) present in mouse brain was identified as ODC antizyme [Fong, Heller & Canellakis (1976) Biochim. Biophys. Acta 428, 456-465; Heller, Fong & Canellakis (1976) Proc. Natl. Acad. Sci. U.S.A. 73, 1858-1862] on the basis of kinetic properties, Mr and reversal of its inhibition by antizyme inhibitor. The brain antizyme, however, did not cross-react immunochemically with any of seven monoclonal antibodies to rat liver antizyme. ODC activity in mouse brain rapidly decreased after birth, in parallel with putrescine content, and almost disappeared by 3 weeks of age. Free antizyme activity appeared shortly after birth and increased gradually, whereas ODC-antizyme complex already existed at birth and then gradually decreased. Thus total amount of antizyme remained about the same throughout the developmental period in mouse brain. In addition to ODC-antizyme complex, inactive ODC protein was detected by radioimmunoassay in about the same level as the complex at 3 weeks of age. Upon cycloheximide treatment, both free ODC activity and ODC-antizyme complex rapidly disappeared, although free antizyme and the inactive ODC protein were both quite stable.

Reversal of transformed phenotypes by herbimycin A in src oncogene expressed rat fibroblasts.

We studied the effectiveness of herbimycin A, an inhibitor of the function of the src oncogene, to reverse the various transformed phenotypes in normal rat kidney (NRK) cells integrating temperature-sensitive v-src (ts/NRK). Elevated glucose transport in ts/NRK cells at a permissive temperature (33 degrees C) was decreased by herbimycin in 8 h to near the level that was observed either in ts/NRK grown at a nonpermissive temperature (39 degrees C) or in untransformed NRK cells at either temperature. Herbimycin caused no significant decrease in glucose uptake in ts/NRK cells grown at 39 degrees C. The effects of herbimycin on serum- and anchorage-independent growth properties of ts/NRK cells and of NRK cells integrating K-ras (KNRK) were also examined. With ts/NRK cells grown at 33 degrees C, the inhibition of cell growth by herbimycin became more pronounced when the serum concentration in the medium was lowered. With KNRK cells, in contrast, almost the same extent of cell growth inhibition was exerted by herbimycin irrespective of the serum concentration. Furthermore, with ts/NRK cells grown at 33 degrees C, herbimycin inhibited the colony formation in the soft agar medium more strongly than on a solid support. No such differential effects were observed with KNRK cells under similar conditions. These results suggest that herbimycin specifically acts on cells expressing the src oncogene and reverses various transformed characteristics to the normal ones.

[Measurement of urine human growth hormone levels by ultra-highly sensitive enzyme immunoassay].

A highly sensitive enzyme immunoassay (EIA) for measurement of urine hGH was set up by a modification of the method of Hashida and Ishikawa, which was a sandwich enzyme immunoassay using anti-hGH antibody coated polystyrene balls and anti-hGH antibody-peroxidase conjugate. Anti-hGH serum was obtained in rabbits by subcutaneous injections of hGH emulsified in complete Freund's adjuvant. In order to reduce non-specific binding to the solid phase, anti-hGH IgG was precipitated from rabbit serum followed by digestion to F(ab')2 and affinity purification. Fab'-peroxidase conjugate was produced by maleimide method. The assay procedure was as follows. 1. 100 microliters of urine samples or hGH standard were incubated with anti-hGH IgG coated polystyrene balls. 2. Polystyrene balls were then incubated with Fab'-peroxidase conjugate. Polystyrene balls were carefully washed three times in saline after incubation with Fab'-peroxidase conjugate, which reduced contamination and non-specific binding. 3. Peroxidase activity bound to the balls was assayed by enzyme reaction using 3(p-hydroxyphenyl) propionic acid as a substrate, and fluorescence intensity was measured by a spectrofluorophotometer (Shimadzu RF-540). Reducing the energy of excitation by setting the slit width of the spectrofluorophotometer at 2nm made it possible to gain stable fluorescence. The minimum detectable quantity of hGH was 30fg/tube in the assay, so that the detection limit was 0.3 pg/ml when 100 microliters of urine samples were used. Coefficient of intra- and inter-assay variation was 6.0% and 8.6%, respectively. The recovery was 98.8 +/- 2.8 (+/- SE) on average. Multiple dilution of acromegalic urine and urine after insulin injection produced dose-response curves parallel to those of the standards. Urine hGH levels in acromegalic patients were significantly greater than those in normal subjects. These findings indicate that sensitive EIA of urine hGH is potentially useful for evaluating the pituitary function.

Characterization of DNA polymerase alpha activity from a mouse DNA temperature-sensitive mutant, strain tsFT20, which shows a defect in DNA polymerase alpha activity at restrictive temperatures.

tsFT20 cells derived from mouse FM3A cells are DNA temperature-sensitive mutants, which have heat-labile DNA polymerase alpha activity. When tsFT20 cells were incubated at restrictive temperatures, intracellular levels of DNA polymerase alpha activity changed biphasically, showing an initial fast decrease (phase I) and a subsequent slow decrease (phase II). The activity of DNA polymerase alpha from tsFT20 cells cultured at a permissive temperature (33 degrees C) was greatly increased by the addition of glycerol or ethylene glycol to the reaction mixture, while little increase in enzyme activity was observed at any concentration of glycerol or ethylene glycol tested with the enzyme from the cells cultured at a restrictive temperature (39 degrees C) for 8 h (phase II). The activity of DNA polymerase alpha from wild-type cells was also increased by the addition of glycerol but the increase was much less than that in the tsFT20 cells. An in vitro preincubation experiment showed that DNA polymerase alpha from tsFT20 cells cultured at 33 degrees C very rapidly lost its ability to be stimulated by glycerol. Furthermore, the experiment using the extracts prepared from tsFT20 cells cultured at 39 degrees C for various periods showed that the ability to be stimulated by glycerol decreased with the duration of incubation time at 39 degrees C. DNA polymerase alpha from the revertants, which can grow at 39 degrees C and exhibit a partial recovery in heat stability of DNA polymerase alpha activity, showed an intermediate response to glycerol, between those of DNA polymerase alpha from tsFT20 and from the wild-type cells. Finally, it was observed that the level of enzyme activity that can be stimulated by glycerol correlated well with the DNA synthesizing ability of tsFT20 cells.

Identical catalytic-centre activity for mouse kidney and rat liver ornithine decarboxylases as determined with antizyme and affinity labelling.

Since the catalytic-centre activity of mouse kidney ornithine decarboxylase (ODC) has been assumed to be twice as high as that of rat liver ODC, we compared relative catalytic-centre activity of the two enzymes by titration with antizyme, which inhibits ODC by stoichiometric binding. In either a crude or a purified state, both enzymes were inhibited by rat liver antizyme to the same extent, indicating that they have nearly identical catalytic-centre activities. This conclusion was supported by comparison of affinity labelling of the enzymes with alpha-difluoromethyl[14C]ornithine.

Repair of salivary fistula after reconstruction of pharyngoesophagus.

We describe three surgical methods used to repair salivary fistulas in different situations that are seen after reconstruction of the pharyngoesophagus in patients with hypopharyngeal cancer. The principle of these methods is the same; a cervical skin flap next to the fistula is used as the internal lining and an anterior chest skin flap is rotated as an external lining to cover it. For the primary closure of heavily irradiated fistulas, this combination may not always be effective, and we emphasize that a combined use of a pedicled pectoralis major muscle flap with these two kinds of pedicled skin flap is extremely reliable.

Mechanism of prolongation of pre-ejection period in the hypertrophied left ventricle with normal systolic function in unanesthetized hypertensive dogs.

To determine the mechanism for prolongation of pre-ejection period (PEP) in the hypertensive heart with normal systolic function, cardiac catheterizations, echocardiograms, and electrocardiograms were performed at the baseline period (CS, control stage) and eight weeks (HS, hypertensive stage) after the induction of systemic hypertension by Page's method in unanesthetized dogs. Mean aortic blood pressure increased significantly (96 +/- 12 to 137 +/- 26 mmHg) (p less than 0.05). Diastolic aortic pressure also increased significantly (80 +/- 12 to 118 +/- 24 mmHg) (p less than 0.01). Left ventricular (LV) PEP of HS was significantly prolonged (72 +/- 12 vs. 87 +/- 22 ms, CS vs. HS). The ratio of LV pre-ejection period to ejection time (PEP/ET) also increased significantly (0.33 +/- 0.04 to 0.42 +/- 0.07) (p less than 0.05). LV end-diastolic wall stress as an index of preload decreased significantly (7.7 +/- 2.5 vs. 5.9 +/- 1.8 10(3) dynes/cm2, CS vs. HS). Heart rate, mean velocity of circumferential fiber shortening (VCF) and end-systolic wall stress of HS did not change. That is, myocardial contractility in the hypertensive heart of HS most likely is not depressed. At autopsy, left ventricle to body weight ratio of HS was significantly (p less than 0.01) greater than the sham-operated dogs (5.7 +/- 0.8 vs. 4.3 +/- 0.5 g/kg). From these findings, there are two possible factors responsible for prolongation of PEP of HS. First, elevated diastolic aortic pressure may influence PEP. Second, the prolonged PEP is possibly caused by the decrease of preload.(ABSTRACT TRUNCATED AT 250 WORDS)
