Associations between anthropometric indices of adiposity and atherogenic risk factors in Japanese working women aged 21-40 years.

This study was conducted to explore whether anthropometric indices of obesity are associated with atherogenic risk factors in young adult working women in Japan. The subjects were 492 women in an occupational setting. Predictor variables were body mass index (BMI), the sum of triceps and subscapular skinfold thickness (SFT), and the waist to hip ratio (WHR). Outcome variables were serum total cholesterol, triglyceride and blood pressures. The average age of the subjects was 26.3 (SD 3.9) years. The upper quartiles of BMI and SFT were significantly associated with all atherogenic risk factors, while the upper quartiles of WHR were not. Multiple comparisons revealed the 4th quartiles of BMI (> 22.25) and SFT (> 39 mm) to have significantly higher values for all atherogenic risk factors. We found that BMI and skinfold thickness were more relevant to the prediction of atherogenic risk factors than WHR in young adult Japanese women.

Dietary fructooligosaccharides increase calcium absorption and levels of mucosal calbindin-D9k in the large intestine of gastrectomized rats.

BACKGROUND: In gastrectomized rats intestinal calcium absorption and bone calcium levels markedly decrease and the levels recover as a result of feeding fructooligosaccharides (FOS). In the present study we examined the effects of gastrectomy and dietary FOS on intestinal calbindin-D9k (CaBP) levels. METHODS: One group of rats was subjected to a sham operation and fed a control diet. Two other groups of rats were gastrectomized, and those in one group were fed the control diet, whereas those in the other group were fed a diet containing 10% FOS. Intestinal calcium, magnesium, and phosphorus absorption levels and intestinal CaBP levels were measured. RESULTS: Gastrectomy increased CaBP levels in the distal small intestine, cecum, and colorectum but markedly decreased calcium absorption. Dietary FOS increased CaBP levels in the cecum and colorectum in the case of gastrectomized rats and improved calcium absorption. CONCLUSIONS: The results suggest that dietary FOS not only improve intestinal calcium absorption but also serve to maintain local calcium homeostasis in the intestine by increasing mucosal CaBP levels in the large intestine of gastrectomized rats.

Dietary fructooligosaccharides change the concentration of calbindin-D9k differently in the mucosa of the small and large intestine of rats.

Previously, we confirmed that dietary fructooligosaccharides (FOS) increase calcium absorption in rats. In this study, we examined the influence of FOS feeding on the concentration of calbindin-D9k of several intestinal segments in rats. Rats in the control group were fed a diet without FOS. Rats in the other two groups were fed the diet containing FOS at either 50 or 100 g/kg for 10 d and subjected to a calcium absorption study. On the final day of feeding, the rats were killed and the entire intestine was removed. The intestinal mucosa was collected from four segments, i.e., the proximal and distal segments of the small intestine, the cecum and the colorectum, respectively. The apparent absorption of calcium increased dose dependently (r = 0.9256, P < 0.0001). Significant positive correlations between apparent calcium absorption and the relative amounts of calbindin in both large intestinal segments were observed (cecum, r = 0.8956, P = 0.0011; colorectum, r = 0.8828, P = 0.0016). Also, significant negative correlations between apparent calcium absorption and the relative amounts of calbindin-D9k in both small intestinal segments were observed (proximal, r = -0.7149, P = 0. 0304; distal, r = -0.8740, P = 0.0021). In conclusion, FOS feeding increases levels of calbindin-D9k in the large intestine, but decreases those in the small intestine. Moreover, these results suggest that part of the stimulatory effect of fructooligosaccharides relates to the transcellular route of calcium absorption in the large intestine of rats.

Increases in body mass index and waist circumference as outcomes of working overtime.

This epidemiologic study was undertaken to determine whether working overtime is associated with anthropometric indices and serum lipids, risk factors for obesity, in white-collar workers. Non-management white-collar male workers were eligible. Body weight and height, waist circumference, skinfold thickness, serum total cholesterol and triglycerides were measured. Weight, height and waist circumference data obtained 3 years previously were also used. Lifestyle information was obtained by means of a self-administered questionnaire. Overtime hours correlated significantly with the 3-year change in body mass index (r = 0.206, p < 0.0017) and waist circumference (r = 0.218, p = 0.0091), but not with either the most recent anthropometric indices or serum lipids. Overtime hours were also intercorrelated (r = 0.436, p < 0.0001) with dinner time. The present study suggested that working overtime is associated with the increases in BMI and waist circumference over a 3-year period although the associations were weak. Additionally, eating habits of those with working overtime might reflect an intervening effect on the anthropometric changes.

Alteration of circadian time structure of blood pressure caused by night shift schedule.

The effects of night shift schedules on circadian time structure of blood pressure were studied in seven healthy young subjects by continuous monitoring of blood pressure every 30 min for 72 h. In the control experiment, subjects were instructed to sleep at regular times with the light off at 00.00 h and the light on at 07.00 h. In the shift experiment, they were instructed to go to bed at 06.00 h and wake up at 11.00 h. The circadian rhythm of blood pressure rapidly phase delayed by 3.5 h in the second night shift day as a group phenomenon. Individual differences in changes in power spectral patterns of blood pressure were found in the night shift schedule. Ultradian rhythmicity of blood pressure was more pronounced in three subjects, whereas the circadian rhythmicity was maintained in four subjects. These findings held when the adaptation to shift work was taken into account.

[The French health care system and its reform].

The French health care system is characterized by its social insurance scheme with universal coverage, freedom of prescription and of location by the doctor, fee-for-service payment by patients, and free choice of doctor by patients, which essentially tends to inflate its health expenditures. Thus, the health expenditures of France has been increasing over the average expenditures of other European countries. Under the principle of autonomy of the social insurance fund, the increase in medical expenditures has been absorbed by a decrease in the reimbursement rate and an increase in the contribution rate of the insured. However, it is no longer possible to cope with this financial crisis by these traditional measures because of the worsening of the unemployment problem. Nowadays, the increase in the contribution rate is regarded as one of the important reasons for the economic stagnation of the French society. In order to cope with this difficult situation, the French government has changed its health policy from the demand side strategy to the supply side strategy. The concrete plan of this policy was presented by Prime Minister Alain Juppé (the Juppé plan). The plan consists of; 1) unification of the medical insurance scheme, 2) establishment of a ceiling on medical expenditures, 3) regionalization of health policy, 4) disclosure of medical information, 5) introduction of medical references, 6) creation of a social protection scheme for the dependent elderly, 7) introduction of an object tax for the social security fund, etc. These subjects have been materializing step by step after many twists and turn. The most important principles of the plan are the transparency of the medical information and the responsibility of each actor within the health system. The French government has conducted a lot of international comparable studies of health systems. According to the results of a series of active and profound discussions, the French government has conducted various social experiments. It is very meaningful for the Japanese government to analyze this process of health reform occurring in France in order to discuss the health system reforms in Japan.

Liver transferase activity in healthy Japanese employees aged 18-39 years.

The aims of the present study were to identify correlates of alanine aminotransferase (ALT or GPT) and aspartate aminotransferase (AST or GOT) activities among a healthy working population aged 18-39, and to discuss liver transferase abnormalities. Subjects included 1,009 employees of a company in Fukushima, Japan. Pregnant women, employees exposed to organic solvents, and employees with a history of liver diseases were excluded. Serum ALT and AST levels were measured by an enzymatic method. Other information including BMI, job type and lifestyles was recorded. Mean ALT and AST levels were significantly higher for males than females (P < 0.001). Multiple linear regression analysis revealed that sex and BMI explained 45% and 31% of the variability in ALT and AST, respectively. The prevalence of abnormal ALT levels (> 40 IU) was 16.3% for males and 0.4% for females. Sex, BMI, and shift work were independently associated with abnormal ALT levels by logistic regression analysis. It is concluded that ALT and AST levels are well-correlated with sex and BMI, and that abnormal liver transferase activity is prevalent in male employees but rare in females, suggesting that liver function tests should be introduced for male employees under 40 years of age.

Transcriptional regulation of rat calbindin expression during development determined by bacterially expressed protein.

Calbindin-D9k expression in intestinal mucosal cells reveals a specific pattern during development in rats. It shows a low basal level in suckling and adult rats, but after weaning at 21 d of age, increases to three times that of the basal level for several days only, around 24 d. We attempted to clarify whether the regulation of developmental change was at the transcriptional or post-transcriptional level. The calbindin-D9k protein and mRNA concentrations during pre- and postweaned development were determined by Western blot and Northern blot analysis, respectively, and compared with calcium binding activity by 45Ca. For Western blot analysis, a corresponding antibody was raised in rabbit using a bacterially expressed fusion protein, glutathione S-transferase (GST, EC 2.5.1.18), and calbindin-D9k. Calbindin-D9k cDNA was linked to a GST gene within a molecule of vector plasmid and a fusion protein was expressed in Escherichia coli. There were significant (p < 0.001) correlations between calbindin-D9k protein, mRNA concentrations and calcium-binding activity: r = 0.90 for protein vs. mRNA, r = 0.93 for protein vs. binding activity and r = 0.95 for mRNA vs. binding activity. These results indicate that calbindin-D9k expression during postnatal development is regulated at the transcriptional level.

Oral temperatures of the elderly in nursing homes in summer and winter in relation to activities of daily living.

This study was conducted to clarify the seasonal difference in body temperature in summer and winter, and to document the thermal environment of the elderly living in nursing homes. The subjects were 57 healthy elderly people aged > or = 63 years living in two nursing homes in Japan. One of the homes was characterized by subjects with low levels of activities of daily living (ADL). Oral temperatures were measured in the morning and afternoon, with simultaneous recording of ambient temperature and relative humidity. Oral temperatures in summer were higher than in winter, with statistically significant differences (P < 0.05) of 0.25 (SD 0.61) degree C in the morning and 0.24 (SD 0.50) degree C in the afternoon. Differences between oral temperatures in summer and winter tended to be greater in subjects with low ADL scores, even when their room temperature was well-controlled. In conclusion, the oral temperatures of the elderly are lower in winter than summer, particularly in physically inactive people. It appears that those with low levels of ADL are more vulnerable to large changes in ambient temperature.

Lactase decline in weaning rats is regulated at the transcriptional level and not caused by termination of milk ingestion.

Lactase activity declines during postnatal development in rats, but little is known about the underlying molecular mechanism of this phenomenon. We attempted to clarify whether the regulation was at the transcriptional or post-transcriptional level and to examine the effects of dietary factors on that regulation. Newborn rats were divided into two groups, prolonged nursing and weaning, at d 21. The prolonged nursing rats were nursed for a further 6 d, whereas weaning rats were separated from their dams and fed nonpurified diet for the same period. The patterns of declining lactase protein and mRNA concentrations during weaning were determined by Western blot and Northern blot analyses, respectively, and compared with lactase activity. There were significant (P < 0.001) correlations between them: r = 0.97 for specific activity vs. protein, r = 0.99 for specific activity vs. mRNA and r = 0.96 for protein vs. mRNA. The lactase activity per milligram DNA showed a pattern similar to that of the specific activity. This result argues against the decline in lactase activity being due to the dilution caused by newly synthesized materials during the weaning period and suggests transcriptional regulation. Furthermore, the prolonged nursing rats showed the same results as weanlings for lactase protein, mRNA, specific activity and activity per milligram DNA. These observations indicate that the regulation of lactase expression is at the transcriptional level and that it is not affected by the termination of milk ingestion.

Shift work and risk factors for coronary heart disease in Japanese blue-collar workers: serum lipids and anthropometric characteristics.

This study was conducted to determine if there is an association between shift work and risk factors for coronary heart disease (CHD) in Japanese male blue-collar shift workers. Health check-up data on serum lipid concentration and anthropometric indices of 33 three-shift workers and 27 two-shift workers were compared with those of day workers. The average years in age of the shift workers and day workers were 34.5 (SD = 7.1) and 32.7 (SD = 7.6), respectively. Serum total cholesterol levels of three-shift, two-shift and day workers were 5.70 (SD = 1.19) mmol/l, 4.81 (SD = 1.01) mmol/l, 4.98 (SD = 0.95) mmol/l, respectively, and the cholesterol levels of three-shift workers were significantly higher than the other workers (p < 0.05). In addition, the abdominal to hip circumference ratios were 0.905 (SD = 0.060) for three-shift workers and 0.877 (SD = 0.054) for day workers, with a significant difference (p < 0.05). In the present Japanese population, three-shift workers had higher risks of CHD than day workers, which was characterized by higher levels of serum total cholesterol and tendency to central obesity. These findings held when lifestyle factors were taken into account.

Interindividual differences in the flexibility of human temporal organization: pertinence to jet lag and shiftwork.

Interindividual variability in the human temporal structure is seldom taken into account, especially in studies devoted to the effects of shiftwork and jet lag. The understated postulate is that humans can be treated as a pure strain species. This paper reviews some facts and concepts with special reference to interindividual changes in the rhythm period tau and the resulting dyschronism. The following points are addressed. (1) Subjects and methods (importance of longitudinal field studies on shift workers). (2) Criteria for tolerance to shiftwork and jet lag. (3) Interindividual differences and shiftwork problems (subject type; the association between good shiftwork tolerance and stable temporal structure; dychronism with tau s differing from 24h and from variable to variable. (4) The genetic background of circadian dyschronism. The Dian-circadian genetic model of biological rhythms. It allows understanding of one's susceptibility to dyschronism, which was actually observed in approximately equal to 30% of subjects studied longitudinally. (5) Practical implications of interindividual differences (dissociate problems of passengers after a transmeridian flight-who have to adjust their temporal structure to local time-from problems of shiftworkers-who need to prevent alteration of their temporal structure; the advantage for the latter of participating in a rapid rotation system rather than a weekly rotation; emphasis that the suitability of a given subject for a given shiftworking condition is likely to be estimated only after a trial span of time including longitudinal study of a set of rhythms.

Seasonality of mortality in Sri Lanka: biometeorological considerations.

The seasonal variation of mortality in Sri Lanka was studied in relation to meteorological variables. Time series data of the total monthly number of deaths in men and women from 1976 to 1980 were analyzed by power spectrum analysis of the fast Fourier transformation method in the areas of Colombo (lowland area, altitude 7 m) and Nuwara-Eliya (highland area, altitude 1890 m), respectively. In the Colombo area, where the mean temperature was always high (mean, 27.5 degrees C), a seasonal variation in mortality of 6 months was validated by power spectrum analysis with peak times in June and November. These mortality peaks corresponded to those of the amounts of rainfall which also showed an identical predominant period of 6 months. In the Nuwara Eliya area, where the mean temperature was always low at about 15-16 degrees C, a seasonal variation of approximately 3 months was found in addition to the 6 months' periodicity. The comparison of the power spectrum pattern of mortality with those of meteorological variables suggested that the amount of rainfall was associated with the seasonal variation of mortality in the Nuwara-Eliya area. In conclusion, the seasonality of mortality in Sri Lanka, a typical tropical zone country, was related to seasonal variation in the amount of rainfall, or a humidity factor, but not to mean temperature.

Sleep habits and psychosomatic health complaints of bank workers in a megacity in Japan.

The relationship between sleep habits and psychosomatic health complaints was investigated by a questionnaire study in 148 bank workers in Tokyo. Daily bedtime was more flexible than daily rising time, and both mean bedtime and rising time tended to be earlier with increasing age. Multiple logistic regression analysis showed that psychosomatic health complaints of sleep disturbances, fatigue, digestive troubles, and depressive mood were significantly associated with sleep habits and perceived mental stress.

Effects of simultaneous administration of ethanol on styrene metabolism under fed and fasted conditions in the perfused rat liver.

The purpose of this study was to clarify the simultaneous administration of ethanol on styrene metabolism in the perfused rat liver under fed and fasted conditions. Styrene uptake rate, production rate of styrene glycol, oxygen consumption rate, and changes in reduced pyridine nucleotide fluorescence were monitored in the perfused rat liver. The effects of ethanol on parameters of styrene metabolism were observed in rat livers under fed and fasted conditions: fed (group I), fasted (group II), and fasted with xylitol in the perfusate (group III). The simultaneous administration of ethanol and styrene significantly decreased styrene uptake rate, production rate of styrene glycol, and oxygen consumption rate, and produced significant reduction of pyridine nucleotide fluorescence as compared with the single administration of styrene in groups I and III. In contrast, the simultaneous administration of ethanol and styrene significantly increased the production of styrene glycol and oxygen consumption as compared with the single administration of styrene in group II. Significant effects on the styrene uptake rate and the reduced pyridine nucleotide fluorescence were observed with regard to factors of both nutritional status and the interaction between ethanol and nutritional status by two-way analysis of variance. These findings showed that the effects of ethanol on styrene metabolism in the liver were dependent upon the nutritional status of the animal. The fed and fasted conditions affected the effect of ethanol on styrene metabolism by changing the supply of NADPH to the mixed-function oxidase system. In conclusion, ethanol suppressed styrene metabolism in the fed condition, but enhanced it in the fasted condition. This phenomenon should be considered in the prevention of occupational hazard of styrene exposure in industrial workers with alcohol drinking habits and nutritional problems.

Changes in immunohistochemical distribution of cytochrome MC-P-448 in the rat liver during acclimation to cold.

Immunohistochemical localization of cytochrome MC-P-448 (form of cytochrome P-450 induced by methylcholanthrene) in rat hepatic lobule and the changes in their distribution pattern in response to cold exposure at 4 degrees C were investigated. The distribution of hepatocytes expressing immunoreactivity to cytochrome MC-P-448 was demonstrated with rabbit anti-MC-P-448 serum using a microphotomeasurment system P1 (Nikon). A duration of cold exposure for 1, 2, 3 or 4 weeks at 4 degrees C was applied to study the effect of cold adaptation of cytochrome MC-P-448. In control rats housed at 24 degrees C, hepatocytes showing high immunoreactivity to cytochrome MC-P-448 were located in the centrilobular areas of the hepatic lobules, whereas they disappeared markedly in the 4-week cold-exposed rats. In 1-week and 2-week cold-exposed rats, only a slight decrease in the expression of MC-P-448 positive hepatocytes was observed. These changes were clearly seen by visual inspection of the distribution topography as determined by a microphotomeasurement technique. In conclusion, cytochrome MC-P-448 forms which were predominantly located in centrilobular areas in the hepatic lobule decreased in 4-week cold-exposed rats. This was in contrast to our early report which showed an increasing tendency of cytochrome PB-P-450 forms in 4-week cold-exposed rats.
