[A case of bilateral testicular tumor].

A case of bilateral germ cell tumor of the testis is reported. In 1988, a 36-year-old male presented with painless swelling of left scrotal contents. Right orchiectomy and retroperitoneal lymph node dissection for embryonal carcinoma had been performed 5 years earlier. Left orchiectomy was performed and its histological finding was seminoma and embryonal carcinoma. Evaluations including CT scan and Ga scintigraphy revealed no metastasis. Postoperatively, the patient was treated with PVB therapy. Previous reports of bilateral germ cell testicular tumor were reviewed, and the age distribution, interval, and histological classification of these cases are discussed.

A case of neoplastic angioendotheliomatosis: angiotropic lymphoma.

An autopsy case of neoplastic angioendotheliomatosis (NAE) in a 65-year-old male is reported. At autopsy, the paraaortic lymph node was slightly enlarged, and bilateral swelling of adrenal glands was found. Histologically, multifocal proliferation of atypical cells was seen in the sinusoid or in the lumen of blood vessels throughout many organs, but there was little invasion of the parenchyma by these cells. From these findings, it was diagnosed as NAE. Immunohistochemically, these atypical cells showed positive reaction to the antileukocyte common antigen, anti-MB-1, LN-1, and LN-2, and negative reaction to the anti-Ki-1, anti-MT-1, anti-kappa, anti-lambda, and antifactor-VIII-related antigen. These findings showed that the atypical cells of NAE were of B lymphocyte origin.

[Blind-ending bifid ureter: 3 case reports].

Blind-ending bifid ureter is one of the most rare anomalies of the upper urinary tract. Three cases of blind-ending bifid ureter are reported. Case 1: A 53-year-old man was admitted with right lumbal colic pain. Kidney-ureter-bladder X-ray revealed the right ureteral stone and drip intravenous pyelography (DIP) revealed the left blind-ending bifid ureter. Case 2: A 61-year-old woman admitted with the complaint of asymptomatic microhematuria. DIP revealed extension of the right middle ureter and the left incomplete duplication of ureter. Retrograde pyelography revealed the right blind-ending bifid ureter. Case 3: A 57-year-old woman was admitted with asymptomatic microhematuria. DIP revealed the left blind-ending bifid ureter. We collected 68 cases of blind-ending bifid ureter reported in Japan including our own according to the definition of Culp.

[Giant liposarcoma of the spermatic cord: a case report].

A case of giant liposarcoma of the spermatic cord was reported. A 75-year-old man was admitted with a 25-year history of a slowly growing left scrotal solid mass without trans-illumination. The removed huge mass measuring 26.0 x 16.5 x 20.0 cm and weighing 4,500 g was confirmed to be a well-differentiated liposarcoma according to WHO classification. The patient died of acute pneumonia 5 months later, although the postoperative course was uneventful and he had no sign of recurrence.

[Nonfunctioning adrenocortical adenoma: a report of two cases].

Two cases of nonfunctioning adrenocortical adenoma are herein reported. The adrenal tumor was found incidentally by computed tomography in both patients. Preoperative evaluation revealed that both patients had neither abnormal values in a variety of adrenal function studies nor any recognizable clinical signs associated with adrenal tumor. Following a definitive diagnosis of nonfunctioning adrenal tumors, surgical extirpation was performed in both cases. Histopathological finding was adrenocortical adenoma. Nonfunctioning adrenocortical tumor will be found more frequently with the increasing use of computed tomography in ordinary clinical practice. The management of nonfunctioning adrenocortical tumor is briefly discussed.

[Adenocarcinoma of the urinary bladder with a bladder stone: report of a case].

A case of primary adenocarcinoma of the bladder is reported. The patient was a 83-year-old man admitted to our hospital in February, 1988 with the complaint of gross hematuria that had lasted for about 1 month. Cystoscopic examination showed a solid tumor in the lateral wall of the bladder with a bladder stone. Papillary adenocarcinoma was found in the biopsy specimen from the bladder tumor. The gastrointestinal, respiratory and genitourinary tracts were examined but no other tumor lesions could be found. Therefore, primary adenocarcinoma of the bladder with a bladder stone was suspected. Transurethral resection of the bladder tumor and cystolithotripsy were performed; histological study showed that the tumor was limited to submucosa, that is to pTlb. The literature is reviewed.

Swallowing-induced arrhythmia.

Deglutition tachyarrhythmia, first reported in 1926 by Sakai and Mori, is a rare clinical entity. Vagal reflex has been supposed to be responsible for the induction of the arrhythmia. We have recently studied a patient with swallowing-induced premature atrial contractions. Atropine sulfate and catecholamine infusion increased the frequency of the arrhythmia, while beta blockade suppressed it. It is suggested that the mechanism of the swallowing-induced arrhythmia might be related to a sympathetic reflex. As far as we know, there have been few reports in which the sympathetic reflex was proposed to be responsible for the arrhythmia.

Urinary trehalase activity as an indicator of kidney injury due to environmental cadmium exposure.

One hundred and seventy-eight subjects, patients with Itai-itai disease and their family members, aged 12-87 years living in a cadmium (Cd)-polluted area in the Jinzu River basin (Cd-exposed group) and 176 controls (control group) were examined. In the Cd-exposed group urinary trehalase increased with increasing age, urinary beta 2-microglobulin (beta 2-m) and retinol-binding protein. Although urinary cadmium was higher in the Cd-exposed group, no particular correlation was found between urinary trehalase and urinary cadmium. Seventeen men and 11 women showed raised urinary trehalase activities despite normal values of urinary beta 2-m (less than 300 micrograms/g.creatinine), suggesting that urinary trehalase increases earlier than urinary beta 2-m. In 19 patients with Itai-itai disease included in the Cd-exposed group, urinary trehalase decreased with decreasing reciprocal of serum creatinine, suggesting that urinary trehalase decreases in the most advanced cases of chronic cadmium nephropathy due to reduced tubular cell mass.

Ki-1 lymphomas in childhood: immunohistochemical analysis and the significance of epithelial membrane antigen (EMA) as a new marker.

Two cases of Ki-1 lymphomas in childhood were analyzed immunohistochemically and immunoelectron microscopically. They expressed Hodgkin's disease associated antigen, Ki-1, interleukin-2 receptor (IL2R), OKT9, and HLA-DR. Histologically, the tumour cells were large in size with abundant cytoplasm and atypical nuclei. Lymph node involvement was characterized by parafollicular and marginal sinus infiltration. These features were identical to those reported in Ki-1 lymphomas. Electron microscopically tumour cells had abundant cytoplasmic organelles with pleomorphic nuclei but had no specific granules. Some tumour cells had marked interdigitation of cell membrane. Immunoelectron microscopically Ki-1 was positive on cell membrane. Tumour cells had no T-cell or B-cell antigens except for Leu-3 (T4). Unexpectedly they expressed epithelial membrane antigen (EMA) strongly. EMA was positive on cell membrane and in the cytoplasm. EMA was detected effectively in paraffin-embedded sections. Among the malignant lymphomas in childhood tested, two cases were EMA-positive. The pattern of EMA-reactivity and the histology were very similar to Ki-1 lymphomas. These results strongly suggest that Ki-1 lymphomas in childhood may arise from non-lymphoid haematopoietic cells and that EMA can be used as a new marker to distinguish certain type of Ki-1 lymphomas in childhood.

Characterization of canine distemper viruses adapted to human neural cells.

The biochemical characteristics of canine distemper virus (CDV) adapted to three human neural cells (glioblastoma, oligodendroglioma, and neuroblastoma cells) were compared with those of the unadapted original virus. The specific gravity of the virions and nucleocapsids of the original and the three adapted viruses were not different. The molecular weights of genomic RNA and messenger RNAs encoding H, F, P, and NP proteins of the adapted viruses as estimated by Northern blot hybridization were similar to those of the original virus. By T1-resistant oligonucleotide analysis of the genomic RNA, the glioblastoma- and the neuroblastoma-adapted viruses gave two more spots than the original virus; the oligodendroglioma-adapted virus had a pattern identical to that of the original virus. By two-dimensional gel electrophoresis of virion proteins, we found a difference in the isoelectric point of the viral envelope proteins H and F between the original and the adapted viruses. These results suggest that viral genomic changes occurred during adaptation, resulting in the alteration of viral envelope proteins.

Reversion of neurovirulence and in vitro phenotype of neural cell-adapted canine distemper virus after passage in non-neural cells.

The stability of neurovirulence and in vitro phenotypes of canine distemper viruses adapted to neural cells was examined. Neurovirulence was estimated by the morbidity, mortality, and histopathological changes in the central nervous system of mice. After a single passage of the adapted viruses in Vero cells in which the unadapted virus had been passed, the neurovirulence of glioblastoma-adapted and oligodendroglioma-adapted viruses reverted completely to that of the unadapted virus. However, the neurovirulence of a neuroblastoma-adapted virus reverted partially. In vitro phenotypes such as the two-dimensional electrophoretic patterns of viral proteins and the cross-neutralization patterns also reverted to those of the unadapted virus. However, plaque sizes remained similar to those of the viruses adapted to neural cells.

Studies on hepatic hemopoiesis induced by immunization with syngeneic liver extracts in mice.

After a 9-week immunization period, all the livers in inbred C57BL 6 mice immunized with syngeneic crude liver protein (LSN) in Freund's complete adjuvant showed histological changes mainly in the portal areas. Morphological features and the subsequent in vitro and in vivo data suggested that the principal histological changes were due to extramedullary hemopoiesis and not to autoimmune hepatitis. This is the first description of extramedullary hemopoiesis in studies concerning experimental autoimmune hepatitis. The significance of our findings is, first, that the occurrence of hepatic hemopoiesis induced by the usual immunization method necessitates morphologically distinct evidence of inflammation to be a prerequisite in studies of experimental autoimmune hepatitis. Without this proof, subsequent investigations would be confusing. Second, immunization can be added to a variety of conditions to induce hepatic hemopoiesis. This would provide a convenient and interesting tool for application to further studies of hemopoiesis.
