Clinical studies on the transabdominal resection of esophagocardial cancer and cervical anastomosis with jejunal segment bypass obviating thoracotomy.

Transabdominal resection of esophagocardial cancer and reestablishment of alimentary continuity using a bypass method have been performed in 64 patients. Twelve underwent a bypass with a gastric tube and in 30, a colonic segment was prepared. In the remaining 22, a jejunal segment was used as the bypass organ. Leakage of cervical anastomosis occurred in 4 (18 per cent) but all were limited to minor leakage and oral ingestion was resumed within 10 days after operation. In one, necrosis of the jejunal segment occurred. The history of jejunoesophagoplasty was reviewed and the usefulness of jejunal segment as a substitute of an esophagus was emphasized.

[Clinical use of peplomycin in esophageal cancer].

A preliminary use of Peplomycin (PEP) was investigated in 14 patients with advanced esophageal cancer. PEP was given intermittently with a dose of 10 mg intravenously twice a week. As side effects there were observed fever elevation in 8 cases, stomatitis in 4 cases, erosion of the skin of the scrotum in 1 case and pigmentation in 1 case, respectively. Dyspnea associated with decrease of PaO2 was observed in 4 cases, which recovered promptly after discontinuing of the administration. Out of 10 evaluable cases, partial response was observed in 1, minor response in 1, no change in 3 and progressive disease in 5 cases, respectively. While the effect was only limited in these experiences, the local injection of PEP into or around the tumor using the external fistula of the remaining esophagus which was made at the time of by-pass operation was discussed.

Inhibitory effects of immunopotentiators on the enhancement of lung metastases induced by operative stress in rats.

To investigate the mechanism of tumor growth enhancement induced by operative stress, laparothoracotomy was performed on day 2 after tumor cell inoculation in rats associated with administration of various kinds of immunopotentiators. OK-432 (streptococcal preparation), PS-K (extract from mycelium of Coriolous versicolor), lentinan (extract from Lentinus edodus), levamisole and Corynebacterium parvum were administered intravenously or intraperitoneally in the fractionated form prior to or after inoculation. In general, administration of each immunopotentiator showed an inhibitory effect on the increase of lung metastases induced by laparothoracotomy. In particular, administration of lentinan prior to inoculation and that of levamisole after inoculation caused significant inhibition in the number of metastatic nodules on the lungs. The significance of the inhibitory effects is discussed.

The mechanism of the lipolytic action of theophylline in fat cells.

The lipolytic action of theophylline was examined using both intact fat cells and a fat globule system. Theophylline had similar lipolytic actions in both systems. However theophylline did not activate hormone-sensitive lipase in the fat globule system as measured with added Ediol. Pretreatment of the fat globules with phospholipase C suppressed theophylline-induced lipolysis, but phospholipase D had no effect. A theophylline-sensitive system was reconstituted from endogenous fat and a lipase fraction. Inhibitors of theophylline-induced lipolysis such as quinine and propranolol inhibited theophylline binding to artificial lipid micelles. Purine nucleosides such as adenosine, inosine and guanosine inhibited theophylline-induced lipolysis in the fat globule system. These results suggest that theophylline has a lipolytic action similar to that of adrenaline. Both share a lipolytic mechanism additional to that involving the activation of hormone sensitive lipase through the cyclic-AMP dependent protein kinase. Phospholipids play an important role in this additional mechanism.

Congenital stridor caused by redundant arytenoid mucosa.

Two cases of congenital stridor caused by merely unusual redundancy of the mucosa covering the arytenoids are reported in this paper. The stridor was noted only when the patients were awake, and was not affected by postural changes of patients. The patients who did not reach their full mental and physical development had congenital abnormalities: cleft palate in one case, and hypertrichiosis, short palpebral fissures of camptodactyly in the other. No abnormalities were revealed in the patients on examinations of chromosomes, thyroid hormones and brains using the techniques of computerized axial tomographic scanning and electroencephalography. The diagnosis of these cases should be confirmed by a direct laryngoscopy or a laryngofiberscopy. Although the etiology of redundant arytenoid mucosa is not known, it may be suggested that this condition is incidental to other congenital abnormalities.

Clinical experiences with left gastric vena caval shunt in patients with esophageal varices.

Left gastric vena caval shunt (Inokuchi) was performed in four patients with liver cirrhosis; electively in two and prophylactically in two cases. The overall results have been satisfactory in terms of the effect on esophageal varices and postoperative complications. This procedure appeared to be superior to distal splenorenal shunt or other direct surgical procedures in several aspects. Since left gastric vena caval shunt does not require particularly elaborate surgical techniques and can be performed safely even in patients with considerably impaired hepatic function, it can be recommended as an ideal surgical procedure to be performed in elective and prophylactic surgical candidates.

A case of subglottic hemangioma: a new technique of surgical removal.

Congenital malformation of the larynx are life-threatening anomaly for newborns and infants. Although tumorous lesions in these malformations (especially subglottic hemangioma) are extremely rare, when they do occur some definitive attempts should be made to treat them in infancy. This report concerns a case of subglottic hemangioma with marked stridor and sternal retractions which had begun in the third month of age in a six month-old male. In the excision of the hemangioma a new technique is employed. That is: the excision through the laryngofissure was made under surgical microscope. Assuming that careful removal of the subepithelial hemangioma and closure of the overylaying epithelia would help to maintain the mucosa and thereby avoid excessive scarring, the hemangiomatous tissue was completely removed while preserving the overlaying epithelial layer.

Urinary excretion of LATS in a patient with Graves' disease complicated by nephrotic syndrome.

A case report of a female patient with Graves' disease complicated by nephrotic syndrome with high LATS activity in urinary gamma-globulin is presented. When in the hyperthyroid state with high LATS activity in the serum, she was treated with antithyroid drugs, excess iodine, and finally radioisotopes. Mild hypothyroidism occurred transiently without any significant change in serum LATS activity. Nephrotic syndrome suddenly appeared. Urinary IgG was purified by salting out with ammonium sulfate, DEAE and protein A-Sepharose, and LATS activity in the purified urinary IgG fraction was demonstrated. The specific activity of LATS activity in urinary IgG protein was slightly lower than that of the serum. This case is the first demonstration of LATS activity in urine from a patient with hyperthyroidism and nephrotic syndrome.

Pneumatosis cystoides intestinalis: Report of 3 cases with special reference to its non-surgical treatment.

Three patients with pneumatosis cystoides intestinalis had been reported. All patients are females with lesions in the left and sigmoid colon, grouped as idiopathic etiology. Two patients are in middle age, workers in the manufacturing plant for electric apparatus. One patient received surgical resection of the affected segment. After oral antibiotics treatment, one patient was resected only the intensely affected segment, so that many cysts were still remained, but they disappeared after 3 weeks. One patient was treated with lactobacillus preparations and lactulose. Her subjective symptoms and fecal occult blood disappeared, and X-ray studies and endoscopic examination revealed an almost complete recovery.
