Inhibitory effect of aerosol WP871 on SRS-A mediated bronchoconstriction in the guinea pig in vivo.

Slow-reacting substance of anaphylaxis (SRS-A) is an important factor mediating bronchoconstriction in asthma. We developed a guinea pig model for SRS-A mediated bronchoconstriction induced by antigen inhalation. Using this model, we investigated the effect of inhaled WP871, a new anti-allergic drug, on bronchoconstriction. Aerosol WP871 (0.01 and 0.033%) to some extent inhibited the antigen-induced bronchoconstriction in a dose-dependent fashion, but high-dose WP871 (0.1%) inhalation itself produced a non-specific bronchoconstriction. However, aerosol WP871 (0.033%) showed no inhibitory effect on bronchoconstriction caused by direct inhalation of leukotriene C4, a component of SRS-A. These findings indicate that aerosol WP871 does not antagonize SRS-A, but inhibits synthesis and/or release of SRS-A and has some non-specific bronchoconstrictive effect in high concentration.

The reliability and validity of the Tokyo Autistic Behaviour Scale.

The Tokyo Autistic Behavior Scale (TABS) consisting of 39 items provisionally grouped in four areas--interpersonal-social relationship, language-communication, habit-mannerism and others--is an instrument used by a child's caretaker to rate the child's autistic behaviors on a 3-point scale. Test-retest reliability was satisfactory (i.e., an r for a total score was .94). Among six DSM-III diagnostic groups, infantile autism showed a significantly higher total TABS score than the other five groups, and a taxonomic validity coefficient was .54. An r between total scores of the TABS and the Childhood Autism Rating Scale--Tokyo Version was .59. The area scores showed a lower validity than the total score. The TABS appears to be a useful instrument to assess autistic behavior.

Binding specificity of Fusobacterium nucleatum to human erythrocytes, polymorphonuclear leukocytes, fibroblasts, and HeLa cells.

Eighteen strains of Fusobacterium nucleatum were studied with respect to their adherence to human erythrocytes, polymorphonuclear leukocytes, fibroblasts, and HeLa cells. Four types of adherence were found: 1) sugar-sensitive adherence 2) amino acid-sensitive adherence 3) unknown adherence 4) weak adherence. There was no obvious correlation between the patterns of hemagglutination and adherence to other cells. The results indicate not only a complex network of bacterial receptors on human cells, but also a variety of types of bacterial adherence as expressed by the selectivity of the bacterial binding to specific cell surfaces.

In vitro susceptibility of Campylobacter jejuni to rokitamycin.

The susceptibilities of 100 clinical isolates of Campylobacter jejuni to rokitamycin, a new macrolide, and eight other oral antibiotics were tested by using a broth microdilution method. Rokitamycin demonstrated antibacterial activity that was higher than those of fosfomycin and the tetracyclines and comparable to those of erythromycin and clindamycin, whereas it was slightly less active than the quinolones.

Characterization of sodium dodecyl sulfate-stable Staphylococcus aureus bacteriolytic enzymes by polyacrylamide gel electrophoresis.

Profiles of the bacteriolytic activities of Staphylococcus aureus culture supernatants, sodium dodecyl sulfate cell extracts, LiCl cell extracts, cell wall extracts, and cell membranes were analyzed in sodium dodecyl sulfate-polyacrylamide gels containing Micrococcus luteus or S. aureus. A total of 20 distinct bands of bacteriolytic activity could be detected in gels containing M. luteus, 8 of these bands were found in culture supernatants. The sodium dodecyl sulfate cell extracts, the LiCl cell extracts, and the cell membranes each contained 20 bands (P1 to P20), but no activity was found in cell wall extracts. Less bacteriolytic activity could be detected in gels containing S. aureus, although three bands were found in culture supernatants and LiCl extracts and cell membranes contained one major band, P13. Crude cell extracts showed five bacteriolytic bands of which the major bacteriolytic bands were distributed in an identical manner in all 10 strains of S. aureus studied. The effects of chemical and physical factors were determined, and it was shown that iodoacetic acid, Hg2+, and Cibacron Blue 3G-A reduced activity, and an optimum pH for enzyme detection was between 7 and 8. Preincubation at 100 degrees C for 30 min reduced the activity of P1 and P2 bands.

Production of highly specific anti-testosterone antiserum by an immunotolerance method.

Anti-testosterone antisera were produced by pretreatment of rabbits with 15 alpha-carboxymethyl-5 alpha-dihydrotestosterone linked to a copolymer of D-glutamic acid and D-lysine (D-GL) before immunization with the bovine serum albumin conjugate of 15 alpha- and 15 beta-carboxymethyltestosterone. The specificity for 5 alpha-dihydrotestosterone of the antitestosterone antisera was considerably improved.

Characteristics of anti-testosterone antisera produced by bovine serum albumin conjugates of 15 alpha- and 15 beta-carboxymethyltestosterone: use of [125I]iodinated tracers.

Each testosterone [125I]iodinated histamine derivative where [125I]iodinated histamines were linked to respective 15 alpha- and 15 beta-carboxymethyltestosterone (15 alpha- and 15 beta-CMT), testosterone-3-(O-carboxymethyl)oxime (T-3-CMO) and testosterone-17 beta-hemisuccinate (T-17-HS) were tested for their usefulness as radiotracers in testosterone immunoassay. In the use of anti-15 alpha- and 15 beta-CMT antisera produced in rabbits against 15 alpha- and 15 beta-CMT-bovine serum albumin (BSA) conjugates, the antisera with 15 alpha- and 15 beta-CMT-[125I]iodinated tracers showed low sensitivity and somewhat low specificity in comparison with those of the antisera with tritiated testosterone (T-3H). On the other hand, the antisera with T-3-CMO-[125I]iodinated tracer showed high sensitivity but low specificity for 5 alpha-dihydrotestosterone (5 alpha-DHT) in comparison with T-3H. The T-17-HS-[125I]iodinated tracer was not bound to the antisera. In the use of anti-15 alpha- and 15 beta-CMT antisera produced in rabbits by pretreatment with 15 alpha-carboxymethyl-5 alpha-DHT linked to a copolymer of D-glutamic acid and D-lysine followed by immunization with 15 alpha- and 15 beta-CMT-BSA, the antisera with homologous [125I]iodinated tracer showed high sensitivity and specificity.

[Solid-state stability and preformulation study of a new parenteral cephalosporin antibiotics (E1040)].

In designing the dosage form, one major factor controlling their physicochemical properties is solid forms of the drug powder. The method for improving the physicochemical stability of unstable beta-lactam antibiotics is very important. E1040 is a novel parenteral 3-betaine type cephalosporin which has a broad antibacterial spectrum and potent activities against Citrobacter, freundii, Enterobacter cloacase, and glucose-non-fermentative bacteria, including P. aeruginosa. The present study was intended to provide the solid-state chemical stability of perenteral steril dry dosage form of E1040. The chemical stability differences among the various solid forms, dry amorphous, additive freeze dried amorphous solid and crystalline powder, were evaluated as a function of temperature by thermo stress tests. Freeze dried anhydrous amorphous form was the first steril dry dosage form investigated during the preformulation study. However, this compound is chemically unstable, in the titer of them, reduction are observed in the freeze dried amorphous solid. In order to select the most suitable solid form of E1040, two methods were used. One was crystalline solid and the other was NaCl additive freeze-dried formulation. Through our experiments, the solid-state chemical stabilization can be achieved by these two methods (effect of crystal structure and effect of NaCl additive).

Hyperlipoproteinemia as a risk factor for ischemic heart disease.

We analyzed serum lipoproteins and apolipoprotein E (apo E) from 199 patients in CCU, having ischemic heart disease, and from 211 healthy subjects. It was suggested that serum lipoprotein abnormalities, especially elevated low density lipoprotein (LDL) levels, are closely related to atherogenesis in relatively young patients and subjects with severe coronary lesions. The frequency of apo E-4 was higher and that of E-2 was lower in the CCU group than in the control group. Apo E mutants, E-7 (Glu244----Lys, Glu245----Lys) and E-5 (Glu3 (Glu3----Lys), were also frequent in the CCU group. Apo E isoproteins have higher pI in the order E-2, E-3, E-4, and we observed that LDL-cholesterol levels increased in the same order. The apo E mutants, E-5 and E-7, are also more basic than E-4. These findings suggest that basic apo Es were associated with the development of atherosclerosis. The prevalence of familial hypercholesterolemia (FH) in the CCU group was more than 10 times higher than the reported frequency of FH heterozygotes in normal population. The persistence of marked hypercholesterolemia from infancy probably makes FH patients susceptible to atherosclerosis. Based on the analysis of LDL-receptor protein synthesis, various types of mutations were observed even in phenotypically homozygous FH patients. FH homozygotes were divided into 2 groups, a receptor-negative group and a receptor-defective group. We found a great difference in the frequency of coronary heart disease depending on whether even a small number of receptors were present or not.

A new open reduction treatment for congenital hip dislocation: long-term follow-up of the extensive anterolateral approach.

Congenital hip dislocation, which is conservatively unmanageable, has usually been treated using open reduction. However, a long-term follow-up study of the results suggests that this procedure is unsatisfactory. Since 1973, Tanabe has used a new open reduction procedure that circumferentially dissects the joint capsule and produces sufficient concentric reduction of the femoral head in the acetabulum immediately after the surgery. Fifty-six children (65 hips) from the age of 1 to 3 years were treated by this procedure, and fifty-one of them were clinically and roentgenographically followed up from 6.3 to 12.4 years after the surgery. At the final follow-up session, all children had grown to be over 9 years of age, and no patient had clinically significant symptoms. According to Severin's classification, 33 hips were rated in Group I, and 14 hips in Group II. Another 10 hips were in Group III, and one hip was in Group IV. The incidence of avascular necrosis was 5.2 per cent. These data suggest that our procedure is more useful than the previous ones.

Oscillatory potentials of local macular ERG in diabetic retinopathy.

The oscillatory potentials (OPs) in electroretinogram (ERG) have clinical values in measuring retinal functions of the early stage of diabetic retinopathy. However, OPs have, until now, been evaluated as components of total ERG recorded with a full field stimulus over the entire retina. Therefore, little information could be obtained about the macular condition (diabetic retinopathy). Using focal stimuli, we successfully recorded OPs in the human macular region. We modified our previously reported system for recording local macular ERG under the fundus monitor with an infrared television fundus camera. We evaluated macular OPs with simultaneously recorded a- and b-waves in many patients with diabetic retinopathy. In some kinds of early diabetic maculopathy, the macular OPs were selectively reduced, leaving the a- and b-waves intact. The macular OPs can be a valuable indicator in assessing the macular function in diabetic maculopathy.

[Focal macular electroretinogram in high myopia].

To investigate early changes of the macula in eyes with myopia, we compared 66 eyes with high myopia with 76 normal eyes by recording focal macular electroretinogram (ERG). The eyes with high myopia were divided into two groups; eyes showing only tigroid fundus (group 1) and those associated with posterior staphyloma involving the macula (group 2). The visual acuity of groups 1 and 2 was 1, 0 or better and 0.8 or better, respectively. The amplitude of the a-wave, b-wave in group 1 was significantly smaller than that of normal eyes, however the peak latency was within the normal range. The amplitude of a-wave, b-wave and oscillatory potentials in group 2 was significantly smaller and the peak latency was significantly longer than those in group 1. The abnormal amplitude with normal peak latency in group 1 suggests some reduction of numbers of cones, which is considered to be an early pathologic change in the macula with high myopia. The more reduced amplitude with delayed peak latency in group 2 suggests that, in the advanced stage, the macular pathologic finding includes some quantitative damage as well as further reduction of macular cones.

[Analysis of focal macular ERG in idiopathic central serous chorioretinopathy].

Focal macular electroretinogram (MERG), was analyzed in 33 patients with unilateral central serous chorioretinopathy (CSC) of recent onset. The stimulus spots were 5 degrees, 10 degrees and 15 degrees in diameter. Nonaffected fellow eyes served as controls. When macular detachment was present, the MERG showed significantly reduced amplitude and delayed peak latency in a-wave, b-wave and oscillatory potentials (OPs) in all stimulus spots. The reduction of amplitude in 5 degree and 10 degree spots was more significant in b-wave and OPs than in a-wave. In the convalescent stage, the a-wave and b-wave recovered to nearly normal levels, however OPs showed selective delay of recovery. These abnormalities shown by MERG strongly suggest that CSC may involve functional disturbances in the inner retinal layer as well as photoreceptors. It has been assumed that the pathogenic properties of CSC are receptor disorientation and a disturbance in the rate of photopigment regeneration. The present study showed that the abnormal pattern of MERG in CSC cannot be explained simply by the assumption of receptor disorientation or a disturbance of photopigment regeneration by analyzing similar conditions in normal eyes.

[Analysis of electrically evoked response (EER) in relation to the central visual pathway of the cat (1). Wave shape of the cat EER].

To analyze the Electrically evoked response (EER) in relation to the central visual pathway, the authors studied the properties of wave patterns and peak latencies of EER in 35 anesthetized adult cats. The cat EER showed two early positive waves on outward current (cornea cathode) stimulus and three or four early positive waves on inward current (cornea anode) stimulus. These waves were recorded within 50 ms after stimulus onset, and were the most consistent components in cat EER. The stimulus threshold for EER showed a less individual variation than amplitude. The difference of stimulus threshold between outward and inward current stimulus was also essentially negligible. The stimulus threshold was higher in early components than in late components. The peak latency of EER became shorter and the amplitude became higher, as the stimulus intensity was increased. However, this tendency was reversed and some wavelets started to appear when the stimulus was extremely strong. The recording using short stimulus duration and bipolar electrodes enabled us to reduce the electrical artifact of EER. These results obtained from cats were compared with those of humans and rabbits.

Removal of adherent bacteria from catheter materials in vitro by N-acylated amino acids.

Indwelling urinary catheters may act as a reservoir of bacteria and cause urinary tract infections. Removal of the bacteria adherent to a urinary catheter should reduce the incidence of catheter-associated urinary tract infections. Using several N-acylated amino acids with combinations of four different acyl residues, we investigated their efficacy in removing adherent bacteria from catheter materials in vitro. Among them, acyl phenylalanine and acyl glycine (with acyl lengths of eight or ten) exhibited the highest ability to remove adherent bacteria.

Non-invasive 31P NMR study on the development of brain membrane of gerbils and jimpy mice, a myelin-deficient mutant.

The broad background resonance observed in the in vivo 31P NMR spectra of adult murine heads was investigated in terms of phosphorus atoms in bone and membrane phospholipids. The broad background resonance was found to be weak in a juvenile and increase with advance of age. Fractionation of adult gerbil heads showed that the broad signal was derived from bone and membrane, of which myelin is the major component. The two origins of the broad background resonance exhibited considerably different line shapes in spectra, which enabled us to extract the membrane component from an intact murine head spectrum (sequential subtraction method). By the use of this method, the development of membrane in gerbil brain at various age grades could be estimated. The membrane component was shown to be suppressed in a mutant mouse, jimpy, which has a deficiency in myelin formation ability. Furthermore, the value of T1 of the membrane component was estimated to be 0.9 sec, which was in good agreement with previously reported values for excised brain.

[Requirements for the randomized trial of i.a. vs. i.v. administration to reveal the impact of hepatic arterial infusion on survival in the treatment of liver metastases].

In spite of its high response rate, the impact of hepatic arterial infusion (HAI) chemotherapy on survival is still uncertain in the treatment of liver metastases. There were no significant differences in survival between i.a. and i.v. in western randomized trials for liver metastases from colorectal cancer in the 1980s. The requirements for randomized trials between i.v. and i.a. administration for liver metastases to reveal the impact of HAI on survival are as follows. 1) Investigators have standardized the technical level for HAI. 2) The natural history of the disease is not so long for continuing HAI over living period. 3) The cases have no extra-hepatic lesions. 4) The number of patients suffices for the evaluation. Therefore, the patients with liver metastases from gastric cancer and without extra-hepatic lesions are better materials for the randomized trial of i.a. vs i.v. in Japan, and the multiinstitutional trial with standard technical level is recommended.
