Epidemiological analysis of prognosis of 496 Japanese patients with progressive systemic sclerosis (SSc). Scleroderma Research Committee Japan.

For the first time, we performed an epidemiological study of SSc in Japan to study the factors influencing prognosis, survival rate and cause of death of Japanese SSc patients and to compare our data with those from foreign countries. Prognosis of 496 Japanese patients with progressive systemic sclerosis (SSc) was analyzed based on clinical data described in case cards provided by the members of the Scleroderma Research Committee of the Japanese Ministry of Health and Welfare. The essential observation period was from 5 to 20 years, at ending in 1994. Ninety patients died (males 11, females 79). The age of onset of the deceased patients was significantly higher than that of surviving patients (deceased, 45.6 yrs, surviving 41.3 yrs). Statistically significant factors for a poor prognosis were as follows: Barnett type III > type II > type I, positive for anti-Scl-70 antibody, negative for anti-centromere antibody. The survival rate at 5 years after the onset of the disease was 0.937, followed by 0.82 at 10 years, 0.567 at 20 years and 0.40 at 30 years after the onset. Sex was not a predictor for prognosis, although male patients died at an earlier stage of the disease. The most common causes of death were heart failure, pulmonary insufficiency, lung fibrosis, and renal failure. Twenty-four patients had cancer of which 13 were lung cancers. The current status of the survival rate and prognostic factors of 496 Japanese SSc patients is summarized. In future, more well-controlled studies using the same criteria should be performed for the better understanding and management of SSc.

Expression of autocrine motility factor receptor in cutaneous malignant melanoma.

BACKGROUND: The autocrine motility factor receptor (AMFR), a specific receptor for AME, is considered to be related to the metastatic potential of tumor cells. OBJECTIVE: We investigated whether AMFR is expressed on cutaneous malignant melanoma cells and has any relationship to clinical stage and metastatic ability. METHODS: We performed an immunohistochemical study on 20 cases of cutaneous malignant melanoma using anti-AMFR antibody. AMFR was expressed in 9 cases (45%), and its expression had a tendency to correlate with the clinical stage and the presence of lymph node metastasis. CONCLUSIONS: The expression of AMFR may have a possible association with the metastatic potential of malignant melanoma.

X-linked dominant chondrodysplasia punctata with decreased dihydroxyacetone phosphate acyltransferase activity.

BACKGROUND: We examined an 18-month-old girl with typical clinical features of X-linked dominant chondrodysplasia punctata (CDPX2) with decreased activity of dihydroxyacetone phosphate acyltransferase (DHAP-AT), previously reported in CDPX2. On the other hand, steroid sulfatase, whose activity is deficient or decreased in X-linked ichthyosis and X-linked recessive chondrodysplasia punctata, has been reported to be normal in CDPX2, although all of these diseases have ichthyotic skin changes. OBJECTIVE: We measured the activity of DHAP-AT and steroid sulfatase of the patients fibroblasts to confirm the DHAP-AT abnormality in CDPX2. RESULTS: The DHAP-AT activity was remarkably reduced, but steroid sulfatase activity was within normal levels when compared with those of 2 healthy controls. CONCLUSION: The abnormal metabolism in plasmalogen synthesis in CDPX2 was confirmed. It is, however, unknown how the decreased DHAP-AT activity is related to the skin changes.

Enzymatic hydrolysis of copoly(N-hydroxypropyl-L-glutamine/L-leucine) hydrogels in vitro.

Two component random copolypeptide hydrogels consisting of N-hydroxyalkyl L-glutamine and L-leucine were prepared by carrying out aminolysis reactions with 3-amino-1-propanol(P) together with cross-linking reactors with 1,8-octamethylenediamine (OMDA) on hydrogels of the starting copolymers consisting of gamma-methyl-L-glutamate(M) and L-leucine(L). The relation between their bulk structure and properties was investigated with regard to the swelling ration in water, aqueous vapor permeability, tensile properties, and enzymatic degradation behavior in a pseudoextracellular fluid (PECF). The tensile property of the hydrogels was highly dependent on the swelling ratio in PECF, and on the hydrophobicity of the side chains, whose behavior was typical of an elastomer. It was shown that a common relation was obtained between the rate of water vapor permeabilities and the swelling ratio of hydrogels in PECF regardless of the difference of the nature of side chains. Biodegradation of the hydrogels in vitro by bromelain indicated that the degradation took place in bulk rather than on surface, and that the rate of degradation was also highly dependent on the swelling ratio of samples as well as on the hydrophobicity of the side chains of samples.

Clinical and angiographic analysis of congenital coronary artery fistulae in adulthood. Is there any new trend?

Fifteen (2.1%) patients were diagnosed as having congenital coronary artery fistula (CAF) in a consecutive series of 704 adult patients undergoing selective coronary arteriography; the incidence was astonishingly higher than previous observations. The presentation of clinical symptoms and the electrocardiographic changes at rest and/or after exercise testing probably attributable to the CAF were observed unexpectedly often in spite of the fact that the magnitude of the shunt seemed not to be significant. With respect to the anatomy of the CAF, the incidence of origination from plural coronary arteries and that of fistulation into the left ventricle were also unexpectedly high. These observations presented a striking contrast to those of previous reports, but we were unable to determine the reason. In four cases, the CAF was ligated either electively or concurrently with mitral valve surgery, and the results were satisfactory. Taking these circumstances into consideration, we should not minimize the impact of a CAF with a seemingly small shunt.

Generalized morphea with vascular involvement. A case report and disaccharide analysis of the skin glycosaminoglycans.

We report a 69-year-old man with severe generalized morphea, who showed over 80% of skin involvement, while the internal organs were not affected. We performed histological examinations and analysis of skin disaccharides constituting chondroitinase-digestible glycosaminoglycans in the center and periphery of the sclerotic lesions and the clinically uninvolved skin. In both the central and peripheral parts of the sclerotic lesions, sclerotic fibrosis and a dense perivascular cell infiltration, consistent with morphea, were seen in the entire dermis and subcutis. Furthermore, various vascular changes were observed, such as endothelial cell swell, thickened basement membrane and obstruction of vascular lumen in the fat lobules. In the clinically uninvolved skin, interstitial edema was prominent along with a slight perivascular cell infiltration. On disaccharide analysis, the increase in the amount of delta Di-4S(DS), the main disaccharide unit of dermatan sulphate, delta Di-6S and delta Di-6S, the main disaccharide units of chondroitin sulphate, and the decrease in delta Di-HA, which is derived from hyaluronate, were found not only in the sclerotic lesions but also in the clinically uninvolved skin, though less prominent. These alterations were consistent with systemic sclerosis, suggesting a close relationship between severe forms of generalized morphea and systemic sclerosis.

Focal dermal hypoplasia (Goltz syndrome): a decreased accumulation of hyaluronic acid in three-dimensional culture.

We report a 28-year-old female with focal dermal hypoplasia, (Goltz syndrome). We compared the growth kinetics and the production of type I collagen and glycosaminoglycans by fibroblasts from affected and unaffected skin. Fibroblasts were grown in conventional medium supplemented with ascorbic acid 2-phosphate, which makes fibroblasts from a tissue-like structure in vitro. The population doubling time of fibroblasts in affected skin was slightly shorter than that of unaffected skin. There was no difference in the levels of the carboxyterminal propeptide of type I procollagen liberated into the media between affected skin and unaffected skin. However, the cell layer of affected skin fibroblasts revealed a decreased amount of hyaluronic acid-derived disaccharide unit (delta Di-HA). The abnormal metabolism of glycosaminoglycan in the affected dermis might be involved in the development of skin changes in focal dermal hypoplasia.

Antineutrophil cytoplasmic autoantibodies in patients with systemic sclerosis.

Perinuclear type of antineutrophil cytoplasmic antibodies (p-ANCA) have been found in patients with periarteritis nodosa, Churg-Strauss arteritis, or pauci-immune idiopathic crescentic glomerulonephritis. Recently, the association of p-ANCA and normotensive renal failure, in patients with systemic sclerosis (SSc), was reported. We have studied the incidence of p-ANCA in patients with SSc and have investigated its relationship to clinical and laboratory findings. Sera from 77 patients with SSc were examined by the indirect immunofluorescence (IIF) test, employing an ethanol-fixed human neutrophil and enzyme-linked immunosorbent assay, using purified myeloperoxidase (MPO) as an antigen (MPO-ELISA). The sera from seven patients (9.1%) were p-ANCA positive, by both IIF and MPO-ELISA. One patient died from systemic necrotizing angiitis but the remaining six patients have shown no symptoms of systemic vasculitis or of renal involvement. There was a tendency for patients positive for p-ANCA (anti-MPO antibody) to also be positive for other autoantibodies, such as anti-Sc1-70 antibody, anti-centromere antibody, anti-microsome antibody, anti-thyroglobulin antibody and rheumatoid factor. Although the incidence of p-ANCA (anti-MPO antibody) is low in patients with SSc, and its clinical significance in SSc needs further investigations, this could be a serological marker for certain symptoms in SSc.

Membranocystic changes in the panniculitis of dermatomyositis.

Clinically apparent panniculitis is rare in dermatomyositis. The common histopathological findings are infiltration of lymphocytes, epithelioid cells and plasma cells in the fat lobules, along with varying degrees of fat degeneration and fibrosis. We report a 65-year-old woman with dermatomyositis who developed panniculitis with a characteristic histological change known as a membranocystic lesion. Although this change has been observed in various diseases affecting the subcutaneous fat tissue, it has rarely been reported in dermatomyositis. Dermatomyositis should be included in the diseases showing a membranocystic lesion.

Characteristics of histamine release from cultured human mast cells.

BACKGROUND: The mast cell is one of the important cells in the pathogenesis of allergic disorders. However, isolating human mast cells is a laborious procedure. Recently, cultured human mast cells raised from umbilical cord blood cells have become available. It is necessary to examine whether these cells are useful in investigating the role of mast cells in human diseases. OBJECTIVE: The phenotype of mast cells depends on their anatomical sites. To examine which phenotype of mast cells these cultured mast cells most closely resemble, their ability to release was investigated. METHODS: The mast cells were raised from human umbilical cord blood cells in the presence of stem cell factor and interleukin-6. To determine the mast cell subtypes, the mast cells were immunocytochemically stained for tryptase and chymase. The cultured mast cells were then stimulated with various secretagogues, and histamine release was measured by a fluorometric technique using high-performance liquid chromatography. RESULTS: The immunocytochemical staining for mast cell proteases revealed that virtually all cells contained tryptase, the definitive marker of mast cells, and that about a quarter of the cells contained chymase. Anti-IgE effectively stimulated these mast cells to release histamine in a dose-dependent, time-dependent manner. The release was completed in about 30 min. One of the non-specific stimuli, calcium ionophore A23187, also induced histamine release in a dose-dependent, time-dependent manner. In contrast, compound 48/80 and substance P failed to induce histamine release from these cells. CONCLUSION: Cultured human mast cells resemble lung mast cells in their ability to release histamine. They will help in studying the functional properties of human mast cells and may contribute to clarifying the pathophysiology of human allergic diseases.

Flow cytometric DNA content analysis on squamous cell carcinomas according to the preceding lesions.

DNA-ploidy in primary cutaneous squamous cell carcinomas which had developed from different preceding clinical lesions was examined by flow cytometry using paraffin-embedded blocks. DNA-aneuploidy was detected in none of 15 squamous cell carcinomas arising from a burn scar. On the other hand, 12 of 26 squamous cell carcinomas arising from sun-damaged skin in the elderly, and two of four arising from chronic radiodermatitis, showed aneuploid patterns. A significantly higher incidence of DNA-aneuploidy was observed in squamous cell carcinomas arising from sun-damaged skin than those from a burn scar (P < 0.01); the mean diameter size of the former was significantly smaller than that of the latter (P < 0.0001). The mean age of patients with aneuploid tumours (74.2 +/- 9.2: years +/- standard deviation (SD)) was significantly higher than that with diploid tumours (64.1 +/- 13.8) (P < 0.01).

[A proposal of a composite patch for early repair of ventricular septal perforation after acute myocardial infarction].

A 68-year-old female presented ventricular septal perforation (VAP) 3 days after acute myocardial infarction. The patient, supported with intraaortic balloon pumping, underwent surgical repair with success on day 15. At operation, the VAP, measuring 2.5 x 1.5 cm, was repaired using a composite patch consisting of Dacron felt and xenopericardium attached each other with fibrin glue. The composite patch was found really useful for repair of VSP in such early phase as when VSP-sorrounding myocardium was very fragile, particularly in respect to the prevention of residual and/or recurrent shunt. This was because the composite patch was considered to have such excellent characteristics as: 1) toughness with sufficient compressibility and extensibility, 2) high antithrombogenesity and good tissue compatibility, 3) zero porosity, and 4) easy handling because of unification of two materials.

Photoaging from an oxidative standpoint.

The free radical theory proposes that photoaging, which is both qualitatively and quantitatively different from chronological aging, may result from imperfect protection against cumulative stress of free radicals produced by chronic and repeated ultraviolet irradiation. Since the skin is always in contact with oxygen and is occasionally exposed to ultraviolet light, skin is one of the best target organs of environmental photo-oxidative stress. A growing body of evidence suggests that reactive oxygen species are generated by ultraviolet irradiation resulting in the structural and functional alteration of cutaneous components which should affect the photoaging process over a long period. The age-related alteration of cutaneous antioxidant defense capacity against cumulative effects of continual photo-oxidative stress to the skin may also affect the photoaging. Thus the possible use of antioxidants that attenuate photo-oxidative toxicity is believed to be an important strategy modulating photoaging. Several antioxidants have readily been proved to work in the experimental conditions. This paper reviews photoaging from a photo-oxidative standpoint and discusses the possible regulation of photoaging by antioxidants that is an important issue in the photodermatological field.

Disaccharide analysis of the skin glycosaminoglycans in chronically ultraviolet light-irradiated hairless mice.

The alteration of main disaccharide units in the skin of hairless mice (HOS:Hr 1) after chronic and repeated ultraviolet light (UV) radiation was investigated using high performance liquid chromatography after labeling with 1-phenyl-3-methyl-5-pyrazolone. The total amount of main disaccharide units increased by UVA irradiation at the 24th and 36th weeks in comparison with the control. At the 36th week UVA significantly increased hyaluronic acid-derived delta Di-HA (HA). Dermatan sulfate-derived delta Di 4S (DS) and chondroitin sulfate-derived delta Di-4S (CS) increased at the 36th week, although not statistically significant. The total amount of main disaccharide units was increased significantly by UVB irradiation at the 24th week as compared with the control. Concerning the compositional change in main disaccharide units after a 36-week repeated exposure, the decrease in delta Di-HA(HA) and the increase in delta Di-4S (DS) were found in the order of control, UVA- and UVB-irradiated groups. These results, for the first time, indicate the precise alterations of glycosaminoglycans, both in the total amount and in the composition, confirming the previous histochemical findings. This disaccharide analysis should provide a useful method to examine the biochemical changes of skin glycosaminoglycans in photoaging.

Ulcerative colitis associated with preceding systemic lupus erythematosus.

We report a 50-year-old female patient who developed ulcerative colitis 31 after years being diagnosed with systemic lupus erythematosus. The overall clinical evaluation of her SLE activity differentiated ulcerative colitis from lupus colitis. Since the association of idiopathic systemic lupus erythematosus with ulcerative colitis has rarely been reported, the combination of these two diseases may be coincidental.
