Immunopathology of vitiligo vulgaris, Sutton's leukoderma and melanoma-associated vitiligo in relation to steroid effects. I. Circulating antibodies for cultured melanoma cells.

Sera of patients with various types of vitiligo have been found to contain circulating antibodies for the cytoplasmic components of human melanoma cell lines. This incidence is high, especially in generalized vitiligo vulgaris with or without Sutton's phenomenon. There is good correlation between the antibodies and positive microsome test, thyroid test, DNA test, as well as the effect of steroid ointment.

Immunopathology of vitiligo vulgaris, Sutton's leukoderma and melanoma-associated vitiligo in relation to steroid effects. II. The IgG and C3 deposits in the skin.

Microsome, thyroid and DNA tests on patients with generalized vitiligo vulgaris, with or without Sutton's leukoderma, show a high positive rate without thyroid diseases. A small, but not negligible, amount of IgG and C3 deposits in the basement-membrane zone and keratinocytes, has been frequently seen in these diseases by immunohistology and immuno-electron microscopy. This evidence strongly suggests an autoimmune character for these diseases.

[Hemodynamic effects of DBcAMP in patients with low cardiac output syndrome following open-heart surgery].

Hemodynamic effects of DBcAMP given at 0.05 to 0.3 mg/kg/min for 30 minutes to patients with low cardiac output less than 2.21/min/m2, to patients on IABP and on dopamine or dobutamine were investigated after open-heart surgery. Hemodynamic improvements were observed in cardiac index from 1.81 +/- 0.3 (mean +/- SD) to 2.56 +/- 0.401/min/m2 (p less than 0.001), stroke index from 20.5 +/- 5.2 to 26.4 +/- 5.2 ml/best/m2 (p less than 0.001). TRP decreased from 1963.8 +/- 682.8 to 1153.9 +/- 449.0 (p less than 0.001). These changes were similar to those of Groups II (3.0 greater than or equal to C1 greater than or equal to 2.21/min/m2) and of Group III (C1 greater than 3.01/min/m2). Increases were also observed in CI from 2.28 +/- 0.67 to 2.96 +/- 0.671/min/m2 (p less than 0.001) and in stroke index from 24.4 +/- 7.2 to 29.5 +/- 6.4 ml/best/m2 (p less than 0.001) and significant decreases were observed in TPR and PVR in patients receiving dopamine or dobutamine. These results strongly suggest the inotropic action of DBcAMP was independent on the beta receptor activity and could be a powerful adjunct in the treatment of low cardiac output patients on whom the dopamine or dobutamine was ineffective.

[Factor XIII in the treatment of postoperative refractory wound-healing disorders. Results of a controlled study].

The efficacy of factor-XIII concentrate in the improvement of postoperative wound healing disorders (suture dehiscences, fistulae) was examined in 61 patients. In an open, randomised, and controlled trial the results (decrease of wound area, signs of inflammation, wound secretion, drainage volume, contrast x-ray films, and colour photos) were evaluated twice: open judgement by the clinicians themselves and blind judgement by an independent evaluation committee. The blind evaluation of the clinical data showed relevant general improvement in the patient groups treated with factor-XIII concentrate: 61.9% (dosage: 750 IU factor XIII for 3 days) and 76.2% (dosage: 1500 IU factor XIII for 3 days) as compared to 10.5% in the control group without factor XIII substitution. The differences were significant (Mann-Whitney-U-Test, p less than or equal to 0.001). These results would suggest substituting factor XIII activity in plasma above 70% of normal value in case of wound healing disorders.

[Maximum tolerated dose of tegafur in rats].

Maximum Tolerated Dose of Tegafur was estimated by per-oral administration in male Donryu rats. Tegafur was administered for 24 weeks at respective doses of 90 mg, 120 mg and 150 mg/kg/day. Six of 10 (60%) died during the treatment in those groups administered with 120 mg and 150 mg/kg/day, respectively. The cause of death of these rats was severe pneumonia. All 10 of the group given 60 mg/kg/day survived until end of the administration and were sacrificed for histological examination of organs. These rats showed apparent suppression of body weight gain compared with controls (p less than 0.001). Slight inflammation of the lungs was observed in all rats, congestion of the liver and local degeneration and necrosis of the liver cells in a few rats and cellular degeneration of bone marrow in one rat. There was no remarkable change in the digestive tract, kidney, thymus or reproductive organs. Accordingly, the Maximum Tolerated Dose of Tegafur was determined to be 90 mg/kg/day.

[Determination of discriminatory power of prognostic factors for recurrence of breast cancer].

Vascular and lymphatic invasion of cancer cells, type of histology, histopathological TNM classification, level of tumor invasion, menopausal status and age, were reported as prognostic factors of breast cancer. We determined the discriminatory power of above prognostic factors for predicting breast cancer recurrence in 98 patients, with the method of quantification theory type II using a computer, model Hi-TAC M-150. The discriminatory rate of these combined seven factors for recurrence was 82.609%. We calculated the probability of breast cancer recurrence using the method of quantification theory type II. The probability of breast cancer recurrence (P) was given as follows. P = 1/1 + 7.333 e5.468x The score x is given by sum of category weight of seven prognostic factors. Averages of P values of patients with recurrence and without recurrence were statistically different (p less than 0.01). The rate of having more than 30% of P values was 78% in 23 patients with breast cancer recurrence, and its was only 3% in 75 patients without breast cancer recurrence. The difference was statistically significant. (x2 = 61.83, p less than 0.001). These results suggest that we can discriminate a high risk patient with breast cancer recurrence using this method.

Novel repetitive sequence families showing size and frequency polymorphism in the genomes of mice.

A middle repetitive sequence, PR1, originally found in mouse rDNA appeared as satellite-like bands when EcoRI and BglII digests of genomic DNA were subjected to Southern blot hybridization using PR1 as probe. The copy number and sizes of PR1-related satellite-like bands, designated as PR1 families, differed remarkably among the subspecies and laboratory strains of mice when the EcoRI digests of genomic DNAs were compared. These bands were not detected in rat and human DNAs. A unit of PR1 sequence was determined by examining cloned EcoRI 3.5 kb (kb, 10(3) bases) fragment and 6.6 kb rDNA by cross-hybridization and sequence analysis: 3.5 kb and 6.6 kb DNAs are composed of homologous PR1 regions and the flanking non-homologous sequences. The results indicate that amplification of different sequences containing PR1 has occurred in different subspecies and strains of mice, and that the segments of satellite-like bands are likely to have been created by recombination of the PR1 sequence with other DNA segments before amplification. The chromosomal distribution of the 3.5 kb PR1 family was studied by back-crossing the female F1 between BALB/c and DDD/1 to male DDD/1. The segregation data strongly suggest that most, if not all, of this family are located on a single chromosome. The stability of these PR1 families in the genomes of cultured cells of a given strain was also examined. An extra band homologous to PR1 appeared in their genomes, but was not detected in other tissues, indicating that some PR1 families may change even during cell propagation.

Histology of the healing process after total colectomy, mucosal proctectomy and ileo-anostomy.

Total colectomy, mucosal proctectomy and ileo-anostomy were performed on 10 dogs to observe the healing process of the space between the rectal muscle cuff and the pulled through ileum. Adhesion was complete in six cases. The ileal and rectal muscle layers were well preserved, but two weeks after the surgery the healing was not completed. One month after the operation, there was little scar formation between the rectal muscle cuff and the ileum, in the absence of infection. If there was infection between the ileum and the rectal muscle cuff, abscess and scar formation was seen even one month after the operation. This operation is thought to be rational from a histological viewpoint and the most important factor is the prevention of infection in the space between the ileum and rectal muscle cuff.

Selective aberration and pigment loss in melanosomes of malignant melanoma cells in vitro by glycosylation inhibitors: premelanosomes as glycoprotein.

We have found that glucosamine (1 mg/ml) or tunicamycin (0.2-0.4 micrograms/ml), specific inhibitors of lipid carrier-dependent glycosylation of protein, when added to cultured B-16 melanoma cells produce a marked loss of pigmentation, accompanied by distinctive biochemical as well as ultrastructural aberrations in their melanogenic compartments. Electron microscopic analysis shows that these newly induced unpigmented cells form uniquely altered melanosomes containing little or no melanin, although their population is not substantially reduced. Within the melanogenic compartments, selective aberration of melanosomes is seen, that is, deformity, bulging, and segregation of their interior membrane, as well as the intramelanosomal formation of irregularly concentric lamellar structure. No apparent structural deformity of Golgi apparatus, Golgi-associated endoplasmic reticulum of lysosome (GERL), and coated vesicles has been observed. Further, no substantial alteration is seen in mitochondria or other cellular structures. Quantitative analysis of altered and nonaltered melanosomes has revealed that the ratio of altered premelanosomes to the total number increases to 44% in glucosamine-treated cells and to 99.5% in tunicamycin-treated cels. Compared to 13% in the control. Electron microscopic dopa reaction has also revealed that these altered melanosomes seem to exhibit a weakly positive or a negative dopa reaction in both glucosamine- and tunicamycin-treated melanoma cells although a number of dopa-positive altered melanosomes are still seen. However, GERL and coated vesicles show no apparent decrease in dopa reaction. It may be concluded that glycoprotein synthesis is integral to the formation of normal melanosomes and to their specific melanizing function, which could be impaired by inhibition of the synthesis of asparagin-linked mannose-containing sugar chains. This results in retrogressive changes in the premelanosomal tyrosinase during its maturation process.

gamma-Glutamyl transpeptidase, tyrosinase, and 5-S-cysteinyldopa production in melanoma cells.

The distribution of gamma-glutamyl transpeptidase (gamma-GTP), tyrosinase, and 5-S-cysteinyldopa (5-S-CD) within melanoma cells has been studied in vitro as well as in vivo. Sodium periodate treatment of intact B-16 melanoma cells has been found to inhibit gamma-GTP present as an ectoenzyme. However, these periodate-treated cells in the presence of 10(-5) M dopa and glutathione have been found to continue to secrete large quantities of 5-S-CD in their medium. The large-granule fraction of Greene's melanotic melanoma contains substantial amounts of both tyrosinase and gamma-GTP. However, further separation of the large-granule fraction into sub-fractions indicates that tyrosinase and gamma-GTP seem to co-exist with premelanosome. It is suggested that glutathione-dependent t-S-CD genesis proceeds within premelanosomes through the formation of glutathione-dopa. The excess of glutathione-dopa and 5-S-CD, unutilized for pheomelanin formation overglow into the cytoplasm.

[Ifosfamide in the treatment of small cell carcinoma of the lung].

A total of 36 cases with small cell carcinoma of the lung were treated with Ifosfamide. Fourteen cases (38.9%) out of 36 cases showed good or marked clinical response, and 120 mg/kg of Ifosfamide was necessary as a minimum dose to obtain effective response. As side effects of Ifosfamide, gastrointestinal disturbance (66.7%), depilation (66.7%), leukopenia (38.9%) and hematuria (36.1%) were observed. Mean survival time (M.S.T.) was prolonged by using Ifosfamide, comparing with the groups treated by with other anticancer drugs. Ifosfamide, therefore, should be used for the treatment of small cell carcinoma of the lung.
