[Abdominal surgery and vascular surgery].

Vascular surgery itself developed for the surgical treatment for the vascular lesions such as occlusion and/or aneurysm. But recently the fine vascular surgical technique has been applied also to the treatment of malignancy affected the abdominal visceral organs. For instance, it is for the curative operation, when the malignant lesion such as bile duct carcinoma involves the main portal system because the resection with reconstruction of the portal vein is nowadays feasible. In this paper I would like to describe the technique of vascular reconstruction for the general surgeons and to report the current status of introduction of vascular surgery into the abdominal surgery in Japan.

Tyrosinase-positive melanocyte distribution and induction of pigmentation in human piebald skin.

White forelock and hypomelanotic macules of piebaldism have been revealed to have almost regularly distributed, dopa-positive melanocytes, though with lower density than normal, on separated epidermis despite previous reports describing few or no melanocytes in piebald spots. The melanocytes observed in piebald hypomelanotic spots seem to be classified into the following two types: (1) strongly dopa-positive and markedly hyperdendritic large cell type and (2) moderately dopa-positive and slightly hyperdendritic, oversized cell type. The former are primarily seen in hypomelanotic lesions, while the latter are seen in transitional lesions. The above difference seems to be associated with compensatory melanogenic function of melanocytes in vivo. Moreover, we have induced new hyperpigmented spots in hypomelanotic lesions, with the exception of white forelock, following therapy with oral methoxsalen plus ultraviolet A light.

[Long-term pathophysiological studies on loop ileostomy and ileoanal anastomosis].

Mucosal proctectomy with ileoanal anastomosis (IAA) had been performed on 37 patients with adenomatosis coli and 16 with ulcerative colitis between 1978 and 1987. These patients were followed up for a mean of 7.5 years. In 38 cases (73%), this procedure was completed by closure of loop ileostomy. The mean number of bowel movements per 24 hours was 6.0. Seven patients had occasional episodes of minor nocturnal soiling. The postoperative maximum resting anal pressure was the most important parameter reflecting clinical results and it rose from 72 to 92 cm of water during 5.3 year follow-up period after IAA. The pathophysiological studies on loop ileostomy and IAA were performed in these patients. Postoperative small bowel transit time evaluated by radioopaque markers was shortened. Daily output of water and sodium, and Na/K ratio in the ileal excreta increased and total counts of anaerobes in feces decreased. On the other hand, daily volume, Na/K ratio and PH of urine fell significantly. These phenomena were remarkable in patients who received loop ileostomy with about 60 cm defunctioning terminal ileum. These results indicate that it is necessary to maintain intestinal continuity in the ileal pouch-anal procedures.

Vascularizing the ischemic jejunum by intra-seromuscular implantation of a gastroepiploic vascular pedicle--an application of experimental esophageal reconstruction.

Many experimental studies have been done in an attempt to feed various ischemic organs. Of these, both direct intraorganic vascular implantation, and vascular anastomosis, appear to show promise for vascularizing the ischemic gastrointestinal tracts. Preliminary experiments were done in 24 Mongrel dogs to establish the blood flow of the jejunum by direct epiploic vascular pedicle implantation. Morphological development of the communicating vascular system, newly formed between the implanted epiploic vessel and the indigenous vascular network of the jejunum, was clarified by microangiography and histology. In 20 dogs, esophageal reconstruction was successfully accomplished, using the jejunal loop vascularized as described above. The total length of the jejunal segment, and the esophagojejunal anastomotic portion, had neither signs of circulatory disturbance nor necrosis, one, three and twelve months post-operatively. Our experimental findings suggest that direct vascular implantation is the simplest way to vascularize the ischemic intestinal tract and may also be applicable widely in the field of abdominal surgery.

Biliary cystadenoma of the extrahepatic bile ducts: report of a case and review of the literature.

Benign neoplasms of the extrahepatic bile ducts are rare and there have only been thirteen reported cases of cystadenoma. One case of cystadenoma of the common hepatic duct and solitary hepatic cyst is presented here and a total of fourteen cases are reviewed. The patient was a 45 year old woman, who complained of abdominal pain with rigidity, but without jaundice. Ultrasonography of the liver showed a 6 cm cyst of the right lobe. Percutaneous transhepatic cholangiography revealed a multilobular cystic mass. At emergency laparotomy, a polypoid multilobular tumor with a stalk was found hanging from the common hepatic duct and was excised. Almost all the patients reported were middle age woman with jaundice. Preoperative diagnosis was made possible by percutaneous transhepatic cholangiography, endoscopic retrograde cholangiography or ultrasonography. The treatment of choice was complete excision. In three cases, a solitary hepatic cyst accompanied the cystadenoma of the common hepatic duct.

The syndrome of inappropriate secretion of antidiuretic hormone by rectal cancer--a case report.

The syndrome of inappropriate secretion of antidiuretic hormone (SIADH) is commonly observed in patients with bronchogenic carcinoma. We report here a rare case of SIADH induced by rectal cancer. Laboratory findings showed that the serum sodium concentration and serum osmolarity were low, and that the ADH level was high. Resection of the rectal cancer resulted in a complete correction of the associated inappropriate ADH. The titer of the rectal cancer tissue proved to be high (80 pg/g).

Pharmacological properties of musk.

The general pharmacological properties of musk were studied. After the application of musk, inhibition of acetic acid-induced writhing, hemolytic effect and excitation of spontaneous uterine motility of rats, inhibition of spontaneous motility in isolated ileum of rabbits, and anti-inflammatory effects and potentiation of beta-adrenergic action in isolated preparations of guinea pig trachea were observed. These effects were not observed after application of muscone or allantoin, constituents of musk.

Pharmacological studies of water extract of the Zizyphus seed and the Zizyphus seed containing drug.

1. The pharmacological properties of a water extract of the Zizyphus seed (HO2) and that of the Zizyphus seed containing drug (HO1) were studied. 2. After oral or intraduodenal administration of 3 g/kg of HO1 or HO2, the hypnotic effects induced by hexobarbital-Na (38 mg/kg, i.p.) and diazepam-induced sleep in mice were potentiated. 3. After oral or intraduodenal administration of 3 g/kg of HO1 or HO2, gastric juice secretion and ulcers induced by water immersion stress, histamine and reserpine in rats were inhibited.

RBE of a thermal neutron beam and the 10B(n, alpha)7Li reaction on cultured B-16 melanoma cells.

The RBE of a thermal neutron beam and the 10B(n, alpha)7Li reaction were determined in cultured B-16 melanoma cells. The Kyoto University Research Reactor (KUR) was used as a thermal neutron source which had a very low contamination of gamma-rays and fast neutrons. The cells were irradiated with the beam in the presence or absence of 10B-boric acid. The absorbed dose from the neutron capture reaction to the cells was calculated by a method of Kitao (1975). Survival curves in both conditions had no shoulder and Do values were 0.506 or 0.604 Gy in the presence or absence of 5 micrograms 10B/ml-medium, respectively. The Do value of the 10B(n, alpha)7Li reaction was also estimated at 0.466 Gy, assuming each component of radiation was additive. The RBEs of the KUR thermal neutron beam and the 10B(n, alpha)7Li reaction relative to 60Co gamma-rays were estimated as 4.62 and 6.01 at 0.1 surviving fraction, respectively. Using these results, we calculated the absorbed dose from the 10B-compound and estimated the specific accumulation in the cultured cells of 10B-compounds which we have previously reported but not quantified (Nakanishi et al. 1980, Ichihashi et al. 1982).

Metabolism and pharmacokinetics of cinobufagin.

1. The metabolic fate of cinobufagin, a major component of the cardiotonic Senso, has been studied after i.v. administration to rats. Two metabolites were detected in rat serum and shown by n.m.r. and mass spectroscopy to be desacetylcinobufagin (I) and 3-epidesacetylcinobufagin (II). 2. Inhibitory activities of I and II on guinea-pig heart Na+/K+-ATPase were less than that of cinobufagin. 3. Serum or plasma levels of cinobufagin, I and II in rats, cats and dogs were determined by h.p.l.c. after i.v. and/or oral administration of cinobufagin. After i.v. administration, cinobufagin, I and II appeared in rat serum, but in dog and cat plasma only unchanged cinobufagin was present. After oral administration, only II was detected in rat serum, but neither cinobufagin nor either of its metabolites (I and II) were found in cat and dog serum.
