[The changes in tumor marker levels after chemolipiodolization in early phase].

Changes of the serum tumor marker levels were analyzed in 80 patients with hepatic malignancy treated with chemolipiodolization. In 52 cases, the levels of the tumor marker elevated on the 1st day after treatment with an exponential drop afterwards. The levels of the tumor marker on the first day compared with those before treatment were significantly higher in effectively treated cases. The exponentially declining curve dissociated about 10 days before reaching the lowest point, and it gradually rose again in 32 cases. The second treatment of chemolipiodolization should be applied before dissociation of the exponential curve.

[An experimental study on portal infusion of 5-fluorouracil].

We conducted these experiments to investigate the differences between continuous portal and one shot portal infusion of 5-fluorouracil (5-FU) using male Donryu rats. Continuous portal infusion of 5-FU (20 mg/kg/day x 5 days) revealed no remarkable side effects on the rats with significant decrease in the number of liver metastases generated by intraportal inoculation of ascites hepatoma AH60C. Consecutive 5-day administration of 5-FU (20 mg/kg x 5) by one shot method via spleen resulted in no metastases but caused death in 75% of rats because of severe leukopenia and dehydration. The rats receiving one shot infusion of 5-FU (33.3 mg/kg x 3, day 1, 3, 5) via spleen showed a significant decrease in the number of metastases without lethal side effects. Continuous portal infusion of 5-FU would be a safe and effective therapy to prevent liver metastases, whereas one shot portal infusion of 5-FU may give rise to severe side effects while preventing liver metastases.

[In vitro activities of newly developed quinolones, fleroxacin, lomefloxacin and sparfloxacin against Mycobacterium tuberculosis].

In vitro antituberculous activities of three newly developed quinolones, fleroxacin (FLRX, AM-833), lomefloxacin (LFLX) and sparfloxacin (SPFX, AT-4140) were evaluated in comparison to that of ofloxacin (OFLX) using M. tuberculosis strains isolated from patients and the Ogawa egg medium. SPFX was apparently more active than OFLX, but both FLRX and LFLX were less active. SPFX inhibited completely the growth of all 20 strains of M. tuberculosis isolated from patients who were not previously treated with OFLX in a concentration of 1.25 micrograms/ml. However, this agent inhibited the growth of only 4 strains (28.6%) of 14 OFLX-resistant M. tuberculosis in a concentration of 1.25 micrograms/ml, suggesting a partial cross-resistance between SPFX and OFLX.

The relative biological effectiveness of 10B-neutron capture therapy for early skin reaction in the hamster.

The relative biological effectiveness (RBE) of 10B-neutron capture therapy (BNCT) on skin was analyzed using hamsters. The Kyoto University Research Reactor, which has a very low contamination of gamma rays and fast neutrons, was used as a thermal neutron source. Boron-10-para-boronophenylalanine hydrochloride ([10B]BPA.HCl) was administered to the hamsters. The evolution and time course of early skin reactions were assessed. These reactions were compared with those produced by electron beams. The maximum safe skin doses (no more than moist desquamation) of BNCT and electron beams were established to be 11 and 21 Gy, respectively. The RBE at this single dose with BNCT was found to be 1.94, assuming that the RBE of the gamma rays was 1.0 and each component of BNCT (mixed radiations) was simply additive.

Epidermal growth factor suppresses the tumor growth of human breast cancer transplanted in nude mice.

The present work was performed in order to clarify the effects of epidermal growth factor (EGF) and transforming growth factor-beta (TGF-beta) on the tumor growth of human breast cancer transplanted in nude mice. A local injection of 2 micrograms EGF significantly suppressed the tumor growth of human breast cancer MX-1 and primary breast tumor (UM-1) which was first isolated from a human female breast cancer patient in our laboratory. On the other hand, TGF-beta did not suppress the growth of those tumors. Our result indicates that human EGF may be useful as a therapeutic agent for some human breast cancers. Human EGF may lead to a new trend of therapy in the treatment of cancer.

[A case of rectal carcinoma disappeared by the preoperative radiation].

A 64-year-old man was admitted to our department with a diagnosis of rectal carcinoma. Preoperative histological diagnosis was highly differentiated adenocarcinoma of Dukes' A classification (Rb', Is', 20-25 mm in diameter, PM'). Preoperative radiation (2.5 Gray/time, total 30 Gray) and chemotherapy (PSK 3g/day x 3 weeks, ACNU 30 mg/m2/week x 3 weeks) were performed on this lesion. After these therapies, form of the tumor changed to IIa', SM', which was recognized by endo-scopic examination with rectal echography. A reduction of the size was 25%. Low anterior resection, curative operation, was performed 2 weeks after the therapy. The resected rectum was examined in detail, resulting in complete disappearance of the carcinoma cell and the tumor was replaced by the inflammatory cells, fibroblasts and granular cells. These findings were compatible with grade IVB of Ohboshi-Simosatos' classification.

Distribution of immunoglobulins and secretory component in gastric cancer of the aged.

The secretory immune system plays an important role in the local humoral immunity of the gastrointestinal tract. In order to evaluate humoral immunity in gastric cancer, distribution of immunoglobulins (Ig) and secretory component was immunohistochemically studied in 74 early and 15 advanced primary gastric cancers. In non-cancerous gastric mucosa, IgA and IgM, and secretory component were mainly identified in the cytoplasm of the intestinal metaplasia. In early gastric cancer of well-differentiated type, the localization of IgA and IgM, and secretory component was similar to that of intestinal metaplasia. In advanced gastric cancer, they were faintly observed and showed low positivity. The number of Ig-containing cells infiltrating cancerous stroma was enumerated. Immunoglobulin A-containing cells were dominant in the stroma of early gastric cancer. On the other hand, there were few Ig-containing cells in the stroma of advanced gastric cancer, and the number of IgA-containing and IgM-containing cells was also decreased. These results suggest that local humoral immunity is suppressed in gastric cancer, especially in advanced gastric cancer.

Association of congenital hypertrophy of the retinal pigment epithelium with familial adenomatous polyposis.

Ophthalmological examinations were performed in 49 people (43 patients) from 24 families affected by familial adenomatous polyposis (FAP). No features of congenital hypertrophy of the retinal pigmented epithelium (CHRPE) were noted in six first-degree relatives without colonic polyps. Patients with Gardner's syndrome exhibited CHRPE more frequently than those without exostosis or desmoid tumour (91 per cent versus 45 per cent). In addition the prevalence of CHRPE was slightly higher in the patients with gastric or duodenal polyps (68 per cent compared with 50 per cent) in those who had a normal gastroduodenal examination. There were 11 families in which two or more patients underwent ophthalmological examinations. In five of these 11 families, all 12 patients were negative for CHRPE. In four of the remaining six families, all 11 patients had CHRPE. CHRPE is one of the pleiotropy of FAP and ophthalmological examination is an important screening test for the family of patients with CHRPE associated with FAP.

A case of adenomatosis coli complicated with malignant lymphoma of the spleen.

A case of adenomatosis coli complicated with malignant lymphoma of the spleen is described herein. An increase in the serum lactic dehydrogenase level of a 50 year old female was noted 9 years after she underwent a total colorectal resection for adenomatosis coli complicated by cancer. A tumor was observed in the spleen on CT and ultrasonography, and a diagnosis of malignant lymphoma was subsequently made by biopsy. At the time of splenectomy, no extension of the tumor was observed in any other organs and it was found histologically to be large cell type follicular lymphoma. A review of the 903 patients with adenomatosis coli documented in Japan revealed the occurrence of mesodermal malignant tumors to be considerably high. Hence, adenomatosis coli appears likely to be a causative factor in malignant lymphoma of mesodermal origin.

The pathophysiological characteristics of bile from patients with gallstones: the role of prostaglandins and mucin in gallstone formation.

Bile was obtained from 82 patients with various biliary tract diseases and concentrations of prostagloandins, leukotriens, mucin, and a number of lithogenic components were measured in order to evaluate the role of these substances in the pathogenesis of gallstone formation. The characteristics of bile in cases of cholesterol gallstones included high concentrations of prostaglandins and hexosamine and a high cholesterol saturation index. Prostaglandin E2 and prostaglandin F2 alpha concentrations in bile were correlated with hexosamine concentration, and prostaglandins and hexosamine were found to be actively synthesized and secreted in the gallbladder. Prostaglandins E2 and F2 alpha may therefore stimulate mucin secretion in the gallbladder with supersaturated bile. The characteristics of bile in cases of calcium bilirubinate gallstones included a high detection rate for bacteria, high beta-glucuronidase activity, a high percentage of unconjugated bilirubin, a low cholesterol saturation index and high concentrations of prostaglandins and hexosamine. Moreover, the synthesis and secretion of prostaglandins in the biliary tract were accelerated in cases of infected bile. Thus, hypersecretion of mucin, stimulated by prostaglandins, my participate in the onset and development of biliary tract infection or in the formation of calcium bilirubinate gallstones. Regarding the role of prostaglandins and mucin, the hypotheses for gallstone formation previously reported by many authors are supported by the clinical data obtained in the current study.

A proposal of indices to describe the carcinoembryonic antigen time course in in order to assess the effects of treating recurrent colo-rectal carcinoma.

In order to assess the effects of treating recurrent or non-resectable colo-rectal cancer, the following 5 indices for the serum CEA time course were devised: 1. If the serum CEA doubling time is altered by any treatment from D1 to D2 we term D2/D1 as the "CEA doubling time ratio" of the treatment; 2. the per cent decrease in the serum CEA level from CEA1 to CEA2 is calculated by (logCEA1 - logCEA2) X 100/logCEA1; 3. the period when the CEA level has returned to normal after a treatment is termed the "normalization period"; 4. "backward time retrieval" is the period between the lowest point of the posttreatment CEA and the crossing-point of this level by the pretreatment rising slope and; 5. "forward time earning" can also be defined. By using these indices, surgical treatments resulted in a higher per cent decrease in CEA, a longer normalization period but a shorter doubling time when the tumor recurred.

Clinical trials of long-term RF local hyperthermia for advanced gastric cancer.

The effect of local hyperthermotherapy (HT) using 13.56-MHz radiofrequency (RF) capacitive heating was evaluated in 25 patients with unresectable or recurrent gastric cancer. HT was carried out once to 3 times a week for a duration of one hour at each session. Patients who underwent RF-HT frequently showed maintenance of performance status, symptomatic improvement and a reduction in tumor size. Moreover, the survival time of 9 patients who had numerous metastases to the distant peritoneum was significantly high (p less than 0.01), compared with 42 historical control patients who also had massive peritoneal dissemination, but had not received HT. The results of this study therefore indicate RF-HT to be a favorable modality in the palliative treatment of patients with far-advanced gastric cancer.

Ampullary carcinoma associated with familial polyposis coli--a case report.

We report herein a case of a 43 year old female who was found to have developed ampullary carcinoma seven years following a subtotal colectomy for familial polyposis coli. She presented to us with only a light fever and early stage ampullary carcinoma was subsequently diagnosed by endoscopic examination of the upper gastrointestinal tract, including an endoscopic retrograde choledochopancreatography. The patient is presently alive and well without any evidence of recurrence 33 months after undergoing a pancreaticoduodenectomy.
