A case of chronic veno-occlusive disease of the liver manifested by ascites in a 59-year-old Japanese male is described. The patient had not been exposed to pyrrolizidine alkaloids, but had been receiving allopurinol for hyperuricemia, the only drug taken throughout the period in which the hepatic lesion developed. The hepatic veins and inferior vena cava were patent according to angiography. The first biopsy showed histological characteristics of the disease and the following two biopsies demonstrated the progression of the hepatic lesion from central fibrosis to non-portal cirrhosis. Morphometric analysis of the sublobular and central veins in the three serial biopsy specimens demonstrated stepwise decrease of the ratio of the veins per portal triad and stepwise increase of severely obliterative veins.

A 54-yr-old man was admitted to Hokkaido University Hospital, complaining of fever, multiple arthralgia, edematous erythema and face and muscular weakness of extremities during the last 2 months. He was diagnosed as dermatomyositis by acceleration of ESR, elevation of GOT, GPT, CPK, aldolase, moderate increases of collagen fibers in biopsy specimen of skin and his clinical signs. Although stools were positive for occult blood, the routine radiographic examination failed to detect the bleeding site in the upper GI. tract. However, in the double contrast picture of the stomach, a very fine abnormal linear shadow was observed at the upper corpus of the lesser curvature. This linear shadow was a margin of the tumor, retrospectively. About 4 months later, abnormal pain occurred and a mass was palpable in the left lumbar region, suggesting a pancreatic tumor. He was operated on excising the tumor, but was performed only exploratory laparotomy because of the presence of intra-abdominal metastases. Death occurred 40 days after the operation and necropsy was done. The gross anatomical findings of the abdomen showed a stomach tumor as large as an infant's head and its metastases to pancreas, lymph nodes, and greater and lesser omentum. Esophageal mucosa including esophagocardiac junction was intact. Histological examination of the intragastric tumor revealed a typical squamous cell carcinoma with keratinization. According to the absence of the components of adenocarcinoma and squamous metaplastic gastric mucosa of non-cancerous areas in the stomach, it seemed likely to be a heterotopic squamous cell carcinoma. It was unknown about the precedence between the stomach cancer and dermatomyositis. There have been 11 cases of primary pure squamous cell carcinoma in the world literature since 1968, but this is the first case report of coexistence of these two diseases.
A 11-month-old boy with a right ventricular diverticulum was reported. To our knowledge, this is the 18th case in the literature and the first case observed by two-dimensional echocardiography. Using our systematic two-dimensional echocardiographic approach, the three dimensional structure of this anomaly was reconstructed correctly. On the short-axis view of the ventricle, an abnormal septal echo was revealed in the right anterior region of the interventricular septum, forming an another cavity from the right ventricle, and was observed to have proper own motion by real-time two-dimensional echocardiography. From apical four chamber view, the abnormal cavity was detected on the right side of the small right ventricle. By contrast echocardiography, a contrast echo appears in the right ventricle and then in the abnormal cavity. The diagnosis was confirmed by cardiac catheterization, angiography and operation.

Investigation was made on the therapeutic effect of a new antibiotic, cefatrizine(CFT), in oral infections according to the following procedure. Results are summarized as follows. Determination was made on the MIC of CFT against 8 clinical isolates of S. epidermidis, 5 of S. aureus and 7 of E. coli in comparison with those of CEX and PBPC. MICs of CFT against S aures were demonstrated 3.13 to 6.25 mcg/ml which were superior to those of CEX and ABPC. CFT was administered orally in a dose of 500 mg at an hour and a half before the extraction of the impacted wisdom tooth in the mandibula. During the operation, gingival and blood specimens were collected and each CFT level was determined. The mean CFT level in the gingiva reached to 0.95 mcg/ml and that in the blood to 5.77 mcg/ml. According to these experimental results, CFT was administered to patients with moderate oral infection at a dose of 500 mg and clinical assessment was made according to the criteria established by Japanese Society of Oral Surgeons. As the results, the effectiveness rate of CFT was 85%. No serious side effect was observed. From the results of the present study, CFT may be effective for moderate oral surgery infections.

A high-performance liquid chromatographic method was developed for the simultaneous determination of phenylbutazone and its metabolites, oxyphenbutazone and gamma-hydroxy-phenylbutazone, in plasma and urine. Samples were acidified with hydrochloric acid and extracted with benzene--cyclohexane (1:1, v/v). The extract was redissolved in methanol and chromatographed on a muBondapak C18 column using a mobile phase of methanol--0.01 M sodium acetate buffer (pH 4.0) in a linear gradient (50 to 100% methanol at 5% min; flow-rate 2.0 ml/min) in a high-performance liquid chromatograph equipped with an ultraviolet absorbance detector (254 nm). The detection limit for phenylbutazone, oxyphenbutazone and for gamma-hydroxyphenylbutazone was 0.05 microgram/ml. A precise and sensitive assay for the determination of phenylbutazone and its metabolites was established.
Simultaneous detection by a combination of gas chromatography-mass fragmentography and gas chromatography-mass spectrometry for total ion monitoring was developed for determination of uracil, thymine and cytosine present in biological materials as pyrimidine bases, as their silylated derivatives. The detection limits for uracil, thymine and cytosine in the first method were 0.001 microgram/ml for plasma and 0.001-0.005 microgram/g wet weight for tissues. Those for uracil and thymine in plasma and tissues and for cytosine in plasma in the second method were 0.2 microgram/ml or g wet weight, and that for cytosine in tissues was 2.5 microgram/g wet weight. An accurate and sensitive assay for determination of pyrimidine bases was established.

A high-performance liquid chromatographic method is described for the simultaneous determination of the anti-inflammatory agent suxibuzone and its metabolites, 4-hydroxymethylphenylbutazone, phenylbutazone, oxyphenbutazone, and gamma-hydroxyphenylbutazone, in plasma and urine. Acidified plasma or urine is extracted with benzenecyclohexane (1:1). The organic extract is reduced to dryness and the resulting residue is redissolved in methanol. Aliquots of this solution are chromatographed on a reversed-phase column using a mobile phase of methanol--0.5 M KH2PO4 (linear gradient from 0 to 100% methanol at 8% min with a flow rate of 2.0 ml/min) on a high-performance liquid chromatograph equipped with a UV absorbance detector (254 nm). Detection is limited to 0.10 microgram/ml for suxibuzone and 4-hydroxymethylphenylbutazone and to 0.05 microgram/ml for the other metabolites.
Oral administration of uracil plus ftorafur [1-(tetrahydro-2-furanyl)-5-fluorouracil] has a greater antitumor effect than that of ftorafur alone. A high-pressure liquid chromatographic methods was developed for the determination of ftorafur in plasma and visceral tissues with a sensitivity of 0.025 microgram/ml or g of wet weight. A combination of GLC--mass fragmentography and GLC--mass spectrometry with total-ion monitoring was developed for the specific, simultaneous determination of 5-fluorouracil, and active metabolite, and uracil as their trimethylsilylated derivatives. The detection limits for 5-fluorouracil and uracil in the first method were 0.001 microgram/ml for plasma and 0.001--0.005 microgram/ml for visceral tissues, and those in the second method were 0.2 microgram/ml or g of wet weight. The precision and sensitivity of the assay appear to be satisfactory for determination of the levels of these compounds in plasma and visceral tissues.
In order to clarify morphological changes of the liver in the uremic state, 16 uremic patients treated with chronic hemodialysis were studied. Biopsy was performed in 14 cases under laparoscopic observation and in two on the occasion of renal transplantation. One uremic patients not being treated with dialysis was also studied for comparison. All biopsy specimens were examined by light and electron microscopy. The liver usually appeared mildly or moderately swollen under laparoscopic observations, which was considered at least partially due to the enlargement of the hepatocytes. All patients had hepatocytes with an Orcein-negative "ground glass" appearance, in which marked proliferation of smooth endoplasmic reticulum (SER) was found by electron microscopy. Since the patient not being on dialysis also had such hepatocytes, this finding may be characteristic of uremia. With electron microscopy, in addition to proliferation of SER, alteration of mitochondria and rough endoplasmic reticulum (RER) and an increase in cytoplasmic lipid droplets were observed. Hypertrophy of the Golgi apparatus containing electron-dense particles (VLDL) was often found in patients associated with hypertriglyceridemia. Amorphous electron-dense inclusions in microbodies were occasionally observed. Siderosis was observed in nine patients including three having parenchymal siderosis. With electron microscopy, various siderosomes were seen in the cytoplasm of hepatocytes in patients with parencymal siderosis. Conclusively, these histological and ultrastructral features of hepatocytes are rather associated with several metabolic abnormalities in uremia.
