Vascular involvement in cutaneous sarcoidosis.

Sarcoidosis is a systemic granulomatous disease with systemic vascular involvement, that is, granulomatous angiitis and microangiopathy. To determine if there is vascular involvement in cutaneous sarcoidosis, we examined 42 skin specimens taken from 32 patients with cutaneous lesions of sarcoidosis. Cutaneous sarcoidosis was prevalent in older females with high serum angiotensin-converting enzyme (ACE) levels. Most skin lesions appeared during the following-up of sarcoidosis. Granulomatous angiitis was present in 12 specimens of sarcoid skin lesions (30.8%). Eight of the 12 specimens showed venous involvement in the dermis. There was no correlation between the incidence of granulomatous angiitis and the gross pattern of cutaneous sarcoidosis. Immunohistochemically, thrombomodulin was negative in the vascular endothelium close to the granuloma or a periphlebitis lesion. Electron microscopy revealed endothelial swelling, luminal narrowing, and basal lamina layering of the basement membrane in the capillaries and venules in the dermis. These findings demonstrated that granulomatous angiitis and microangiopathy coexist in cutaneous sarcoidosis.

Effect of epidermal growth factor on mouse sperm acrosome reaction induced by zona pellucida.

PROBLEM: The effect of epidermal growth factor (EGF) on the acid-solubilized zona pellucida (ZP)-induced acrosome reaction was investigated in mouse sperm. METHOD: Mouse epididymal sperm were capacitated in modified Krebs-Ringer bicarbonate buffer (m-KRB) for 120 min and further treated with acid-solubilized ZP(4 zona/microliters) for an additional 60 min to induce the acrosome reaction. The chlortetracycline fluorescence assay was used to monitor the acrosome reaction. The acrosome reacted sperm featured the acrosome reacted (AR) pattern, which demonstrates the lack of fluorescence on the head and bright midpiece. RESULTS: EGF caused an early increase in the AR pattern in response to the acid-solubilized ZP in a dose-dependent manner. The EGF-dependent stimulation of the ZP-induced acrosome reaction was inhibited by an inhibitor of protein tyrosine kinases, genistein or activators of Ca++ and phospholipid-dependent protein kinase (protein kinase C). Furthermore, the stimulatory effect of EGF was not attenuated when sperm were capacitated in the presence of islet-activating protein, an inactivator of inhibitory guanine nucleotide-binding regulatory protein (Gi protein). CONCLUSION: EGF stimulates the ZP-induced acrosome reaction in a manner that is independent of the Gi protein. The EGF action is regulated by protein tyrosine kinase and protein kinase C.

The accumulation and energy load of exogenous lipids in cirrhotic rat liver after partial hepatectomy.

BACKGROUND: The suitability of three energy substrates, glucose, medium-chain triglycerides (MCT) and long-chain triglycerides (LCT), was studied in cirrhotic rats after a partial hepatectomy. METHODS: Rats with thioacetamide-induced cirrhosis underwent a 70% hepatectomy, and were divided into three groups. Each group was then injected with 14C-labeled glucose, 14C-labeled MCT or 14C-labeled LCT, respectively. The subsequent tissue distribution of 14C and the cumulative amount of expired 14CO2 were determined. In a second experiment, the 70%-hepatectomized cirrhotic rats received total parenteral nutrition (TPN). The source of the nonprotein calories was 100% glucose (glucose group), 60% MCT + 40% glucose (MCT group), and 60% LCT + 40% glucose (LCT group). The adenylate energy charge and the glycogen content in the liver remnant were determined. RESULTS: The tissue distribution of 14C revealed that the fat emulsions accumulated preferentially in the liver. One hour after the partial hepatectomy, the concentration of 14C-labeled MCT in the liver remnants was threefold higher than in sham-operated controls. Similarly, the concentration of 14C-labeled LCT was twofold higher. The adenylate energy charge in the glucose group with TPN recovered to preoperative levels within 1-hour after the partial hepatectomy, whereas the LCT group with TPN showed a 24-hour delay in their recovery. The MCT group with TPN exhibited an intermediate time course. CONCLUSIONS: It is suggested that the specific accumulation of MCT and especially LCT emulsions in the cirrhotic liver remnant acts as an energy load rather than an energy substrate.

A day care program and evaluation of animal-assisted therapy (AAT) for the elderly with senile dementia.

We conducted a survey to clarify the evaluation methods of animal-assisted therapy (AAT) for the elderly with senile dementia in an adult day care center. AAT was implemented for a total of six biweekly sessions. The AAT group consisted of seven subjects and the control group numbered 20 subjects. In a comparison between Mini-Mental State Exam (MMSE) scores at baseline and those measured three months later, the average MMSE score before AAT (baseline) was 11.43 (+/- 9.00), and three months later it was 12.29(+/- 9.69). In the AAT group, the average score on Nishimura's Activities of Daily Living (N-ADL) at baseline was 28.43(+/- 14.00), and after ATT it was 29.57(+/- 14.47). In the AAT group, the average baseline score on behavioral pathology of Alzheimer's disease (Behave-AD) was 11.14(+/- 4.85), and three months after AAT it was 7.29(+/- 7.11) (p < 0.05). In the control group, the average baseline score was 5.45(+/- 3.27) and three months later it was 5.63(+/- 3.59). The evaluation of salivary CgA, as a mental stress index, showed a decreasing tendency in the AAT group. Our findings demonstrate the usefulness of using several methods for evaluation of the changes in patients given AAT.

Visual hand display (VHD) as an introductory procedure for measuring vision in infants and young children with visual impairment.

We developed the Visual Hand Display (VHD) to measure vision in visually impaired infants and young children. The VHD is a circular fabric mitten, which is held easily by inserting the hand between the two surfaces. Black-and-white stripes are attached to one surface, 25, 15, 10, 4, and 2 mm per stripe. The VHD acuity is determined by the shortest test distance and the smallest stripes that the patient can detect. The VHD acuities were compared with preferential looking (PL) staircase acuities in 130 patients (53 males, 77 females; age range, 2 to 13 years; median, 21.0 months). Of these, 107 (82.3%) had various degrees of retinopathy of prematurity. The correlation between the VHD and the PL acuities was high (R2 = 0.849). PL acuities were better than the VHD acuities in 98/130 patients (75.4%), with an average difference of 0.51 (SD = 0.70) octave. The visual acuity differences were more pronounced in subjects with slight visual impairment and much less in subjects with severe visual impairment. The VHD seems to be an effective introductory method to evaluate visual acuity in severely visually impaired infants and young children. This method also would be effective with severely mentally and physically disabled patients who cannot undergo PL testing.

Histomorphometric evaluation of the effect of hyperbaric oxygen treatment on healing around hydroxyapatite implants in irradiated rat bone.

The effect of hyperbaric oxygen (HBO) on the tissue reaction around hydroxyapatite (HA) implants in irradiated bone of rats was investigated. A single dose of 15 Gy was delivered to the right leg of 20 rats. HA implants were placed in the bilateral tibial proximal metaphysis 3 months after irradiation. HBO was administered to half of the rats before and after 15-Gy irradiation. The healing process was examined histologically and histomorphometrically. The results indicated that HBO slightly improved trabecular bone formation in the irradiated bone, accelerated bone remodeling in the nonirradiated bone, and improved HA-bone contact in both the irradiated and nonirradiated bones.

Successful surgical resection of solid cystic tumor of the pancreas with multiple liver metastases and a tumor thrombus in the portal vein.

We report a case of solid cystic tumor of the pancreas with widespread liver metastases and a tumor thrombus in the portal vein. The patient was a 43-year-old woman. She was referred because of an upper abdominal mass and weight loss. Computed tomography disclosed a 10-cm cystic and calcified mass in the body and tail of the pancreas and multiple masses in the liver. She underwent a distal pancreatectomy with splenectomy, extended right lobectomy, and partial resection of the liver. All the tumors were completely resected despite the presence of 20 liver metastases. Histopathological studies showed a tumor thrombus in the intrahepatic portal vein. The patient is well without any signs of recurrence 8 months after the operation. Aggressive surgical resection is considered to yield a good outcome for solid cystic tumor with liver metastases and tumor thrombus of the portal vein.

Benign biliary stricture associated with atherosclerosis.

We report a case of benign bile duct stricture that could not be differentiated from intrahepatic bile duct carcinoma preoperatively. The patient was a 79-year-old man. Computed tomography showed dilatation of the intrahepatic bile duct in the left lobe. Direct cholangiography showed segmental stricture of the left bile duct. Angiography showed narrowing of the left hepatic artery. Although bile cytology did not show malignant cells, we suspected intrahepatic bile duct carcinoma preoperatively. We performed extended left hepatic lobectomy. Histopathologic examination of the resected duct also showed no malignant cells; fibrosis with infiltration by lymphocytes was seen at the bile duct stricture. In addition, the resected liver specimen showed sclerotic change in the intrahepatic arteries. The postoperative course was uneventful for more than 26 months, without recurrence or cholangitis. We encountered a very rare case of benign segmental bile duct stricture, which was difficult to differentiate from bile duct carcinoma. We think the biliary stricture was secondary to atherosclerosis which may have caused bile duct ischemia.

Alternative splicing of fibronectin transcripts in osteochondrogenic tumors.

BACKGROUND: Fibronectin (FN) acts in many fundamental biological processes including cell adhesion, migration, proliferation and apoptosis. Although FN shows an ubiquitous expression pattern, alternative mRNA splicing modulates tissue-specific molecular heterogeneity at three regions: extra domain-B, extra domain-A and variable region. MATERIALS AND METHODS: In the present study, we analyzed the FN mRNA splicing in osteochondrogenic tumors as well as in various normal tissues using the RT-PCR method. RESULTS: Normal cartilage almost exclusively contained short mRNA lacking alternative exons in all regions. However, 14 osteochondrogenic tumors including osteochondroma, enchondroma, chondroblastoma, chondrosarcoma and osteosarcoma uniformly contained long mRNA with various patterns of alternative exons in the three regions. CONCLUSION: These results indicated that the lack of regulation excluding the alternative exons may be associated with tumorigenesis in chondrocytes.

Local hyperthermia combined with radiotherapy, chemotherapy, or arterial chemoembolization.

Local hyperthermia combined with radiotherapy, chemotherapy, or arterial chemoembolization was performed in 31 patients with malignant tumors using an HTM 3000 heater. The response rate was 41.9%. Fifteen patients with superficial tumors had a high rate of response (60%). Response was particularly good (75%) when radiotherapy was also used. In contrast, the response rate was low (25%) in 16 patients with deep seated tumors. However, the criteria for assessment might not be appropriate. If improvement in symptoms and intratumorous changes were also considered, the therapy was judged to be valuable in 56.3% of the patients. No adverse reactions of concern developed except in some special cases. Hyperthermia with the HTM 3000 heater was safe.

Determination of inorganic tin in biological samples by hydride generation-atomic absorption spectrometry after silica gel cleanup.

A method is described for the determination of inorganic tin in biological samples by hydride generation-atomic absorption spectrometry (HG-AAS). A sample is extracted with ethyl acetate after addition of HCl and NaCl. The concentrated extract is passed through a silica gel column. The column is washed with ethanol, water, and 0.2N HCl successively, and then inorganic tin is eluted with 2N HCl and measured by HG-AAS. Recoveries from fish muscle spiked with 0.1 micrograms/g Sn4+ are 78.9 +/- 4.2% (average +/- standard deviation, n = 5). The detection limit is 0.01 micrograms/g as Sn.

Modern trends in the management of aortic aneurysms. Review of 450 cases experienced.

Treatment of various aortic aneurysms in 450 cases experienced during the past 25 years was analysed. One hundred and five patients were treated with survival rate of 80.2% in the thoracic aortic aneurysm group, 65 patients with survival rate of 76.9% in the dissected aneurysm of the aorta group, 175 patients with survival rate of 93.1% in the nonruptured abdominal aortic aneurysm group and 37 patients with survival rate of 75.7% in the ruptured abdominal aortic aneurysm group. From 1981 to the present time, considerable improvement of the operative results has been achieved with refinement of cardiovascular adjunctive techniques and operative procedures with careful management of the patients. Survival rates are now reached to 88.7% in the thoracic aortic aneurysm, 81.0% in the dissected aneurysm, 100% in the nonruptured abdominal aortic aneurysm and 81.2% in the ruptured abdominal aortic aneurysm. From the careful analyses of the results, prompt diagnosis, distinct treatment policy, strict operative indication and better surgical procedure are considered to have contributed to these excellent operative results.

Application of a low-adhesive color-developing chewing gum system to patients with osseointegrated implant-supported prostheses.

Few methods have been described that can quantitatively evaluate masticatory function. This study investigated the usefulness of a low-adhesive color-developing chewing gum system for the evaluation of patients with implant-supported prostheses. Fifteen dentate subjects and 22 patients with dental implants participated in this study. The color developed in the gum after chewing reflects masticatory function and is evaluated quantitatively with a color-analyzing system. The results indicate that this system can be used to evaluate occlusal function, and they confirm the importance of stable prostheses for higher masticatory function.

A computerized method for evaluating distribution of occlusal load on implant-supported fixed cantilever prostheses.

An investigation was conducted to determine whether Dental Prescale, a material that undergoes a graded color-producing chemical reaction when force is applied, could be used to evaluate occlusal load distribution objectively and quantitatively when combined with computer analysis. Ten patients with an implant-supported fixed cantilever prosthesis and a complete conventional maxillary denture were studied. The results indicated that the system can be used to evaluate occlusal load distribution with sufficient reproducibility. However, in its present form, the system is somewhat cumbersome, and its use must now be considered experimental. Further refinement is necessary to allow easier clinical use.

Effects of free latissimus dorsi dynamic cardiomyoplasty on left ventricular function.

In their experimental approach to dynamic cardiomyoplasty (DCMP), the authors hypothesized that a wrapping method using the proximal part of a free latissimus dorsi muscle (LDM) graft might augment ventricular contractility more than the classic Carpentier's wrapping method. The authors divided 12 mongrel dogs into a pedicled LDM graft group (Group 1, n = 6) and a free LDM graft group (Group 2, n = 6) to evaluate the properties of the different wrapping methods. To evaluate the effect of DCMP on left ventricular systolic and diastolic function, the authors used precise indicators that minimize the influences of load conditions. The slope of the linear preload recruitable stroke work relationship (Mw) and an X-intercept (Vo) were utilized as the indicator of left ventricular systolic function. The constants of pressure decay (tau) and peak filling rate (PFR) were measured to determine diastolic function. All experimental animals were evaluated with the acute, non preconditioning model. Mw was significantly increased with LDM stimulation in both groups (postwrap non stimulation 59.1 +/- 6.3; postwrap stimulation 98.6 +/- 9.7 erg.cm-3. 10(3); p < 0.01 in Group 1, postwrap non stimulation 66 +/- 6.7; postwrap stimulation 155 +/- 15.7 erg.cm-3.10(3), p < 0.001 in Group 2). Stimulated free LDM grafts significantly increased the Mw in comparison to pedicled grafts (p = 0.011). Vo was unchanged in both groups and there was no significant difference between the two groups. Tau increased after LDM wrap in both groups (p < 0.05), but there was no difference between the two groups (tau; prewrap 45.8 +/- 6.0; postwrap non stimulation 69.3 +/- 10.3; postwrap stimulation 72.3 +/- 13.9 msec in Group 1, prewrap 50.0 +/- 6.0; post wrap non stimulation 61.8 +/- 5.0; post wrap stimulation in 64.3 +/- 4.7 msec in Group 2). Peak filling rate was unchanged after LDM wrap in both groups. Free LDM grafts significantly increased left ventricular systolic function compared to the pedicled LDM. Although myocardial relaxation was impaired after LDM wrap in both groups, there was no difference between the two groups. The authors' results emphasize the explicit benefit of utilization of a free LDM graft in respect to left ventricular systolic function; the free LDM grafts have no gross detrimental effect on diastolic function when compared with a pedicle graft in the acute model.
