[A phase II study of oral VP-16 in primary lung cancer].

A clinical trial of a new semi-synthetic podophyllotoxin, VP-16, was undertaken in patients with primary lung cancer; 56 of the 81 evaluable patients had small cell carcinoma, 9 adenocarcinoma, 8 epidermoid carcinoma, 7 large cell carcinoma, and 1 adenosquamous carcinoma. A dose of 200 mg/body/day orally for 5 consecutive days was administered every 3 to 4 weeks. Partial response (PR) was attained in 19 out of 81 (23%) and PR + MR was 35 out of 81 (43%). PR and minor response (MR) were seen as follows; small cell carcinoma, 17 PR (30%), 13 MR; epidermoid carcinoma, 2 PR (25%), 1 MR; adenocarcinoma, 1 MR; adenosquamous carcinoma, 1 MR. The dose-limiting factor was leukopenia, while thrombocytopenia was experienced in 2 cases. Clinical toxicities noted were anorexia, nausea, vomiting, stomatitis, diarrhea and alopecia, but these were well tolerated in all cases. The result indicated that VP-16 has considerable efficacy in small cell carcinoma and epidermoid carcinoma of the lung and hence its usefulness in combination chemotherapy was suggested.

[Effects of long-term high doses of sex steroid hormones on human endometrial adenocarcinoma transplanted into nude mice].

To clarify the minute reactions of endometrial adenocarcinoma to sex steroid hormones, especially gestagen, an in vivo model (AD-30) transplanted into nude mice was established. The AD-30, well differentiated papillo-tubular type, was given 500 micrograms/kg/day of estradiol benzoate (E2) for 50 days or 0.1-100mg/kg/day of medroxyprogesterone acetate (MPA) for 120 days. Thereafter sequential histopathological changes, tumor growth curve and volume doubling time (VDT) were investigated as compared to the castrated control group. Histopathological changes: Following E2 administration, the cells increased in number and were packed in the tubular glands, and following a large dose of MPA, while secretory producing fluid was accumulated in the lumen, cells were shrunk, damaged and finally necrosed, but almost un affected cells were also observed. Tumor growth curve and VDT: E2 somewhat lifted the curve and shortened the VDT to 184 hr (control: 250 hr), while MPA produced various curves, some were diminished, some suppressed and others equal to the control group-with additional secretory fluid. Also MPA elongated the VDT to 412 hr.

[Tissue penetration and clinical experiences of aztreonam in obstetrics and gynecology].

Tissue penetration and clinical efficacy were studied on aztreonam (SQ 26,776, AZT) in obstetrics and gynecology with the following results. Number of cases was too small to sufficiently review the penetration into each uterine tissue, the ovary and the tube after the intravenous injection of AZT 1 g. Overall clinical effect for all the 6 cases reviewed was more than "good". Also, neither side effect nor abnormal laboratory findings were reported. From the above results, AZT was considered to be a highly useful antibiotic in obstetrics and gynecology.

Migration inhibition and stimulation factors produced from peripheral blood lymphocyte cultures of sensitised guinea pigs.

The supernatants of peripheral blood lymphocytes of tuberculin-sensitive guinea pigs incubated with PPD were investigated using macrophage and leukocyte migration inhibition tests. Inhibition as well as stimulation of cell migration were observed. The effect upon migration cultures seemed to be dependent upon the immunological state of the host; animals of the V-group (vaccinated once without challenge) showed inhibitory activity to both macrophages and leukocytes, while those in the VC-group (vaccinated and challenged) had stimulatory activity only to leukocytes. The addition of antithymocyte serum stopped all activity (macrophage and leukocyte inhibition and leukocyte stimulation), suggesting that thymus-dependent lymphocytes are necessary for such activity.

Immunosuppressive acidic protein in patients with gynecologic cancer.

Immunosuppressive acidic protein (IAP) was determined in sera of patients with gynecologic tumors using the single radial immunodiffusion method. The normal limit of IAP of 490 micrograms/ml was derived from the mean value + 2 SD of IAP in 150 healthy females. Among 141 patients with gynecologic cancers, serum IAP was elevated in 87 patients (62%). Among 190 patients with benign tumors (98 uterine myoma, 92 benign ovarian tumors) serum IAP was elevated in 14 patients (7%). Elevated levels of IAP were recognized in 43% of 77 patients with cervical cancer, in 55% of 11 endometrial cancer patients, and in 91% of 53 ovarian cancer patients. The frequency of elevated levels showed a tendency to increase with advancing stage of disease. In ovarian cancer elevation of IAP was observed even in early stages. All of 13 patients with recurrent cervical cancer had elevated IAP while only 6 of 35 (17%) previously treated patients without evidence of recurrence had elevated IAP. Immunosuppressive acidic protein determinations may be useful in monitoring the recurrence of cervical cancer. The measurement of serum IAP as a marker for gynecologic cancer is recommended as an addition to diagnostic procedures.

Linkage disequilibrium of HLA-SB1 with the HLA-A1, B8, DR3, SCO1 and of HLA-SB4 with the HLA-A26, Bw38, Dw10, DR4, SC21 extended haplotypes.

Homozygous typing cells from 13 normal HLA-A1, B8, Dw3, DR3 and five normal HLA-A26, Bw38, Dw10, DR4 individuals were typed for the following markers: HLA-SB, MB, MT; complement proteins BF, C2, C4A, C4B; and GLO. Ninety-one percent of A1, B8, Dw3, DR3 homozygous individuals (HI) tested were homozygous for BF*S, C2*C, C4A*QO, and C4B*1 (SCO1 complotype), which indicates that the SCO1 complotype is in linkage disequilibrium with the A1, B8, DR3 haplotype in randomly selected normal populations. Sixty-seven percent of HLA-A1, B8, Dw3, DR3, SCO1 positive HI also expressed SB1; since the frequency of SB1 in random Caucasian populations is 11.2%, this finding indicates that SB1 is in linkage disequilibrium with the A1, B8, DR3, SCO1 extended haplotype. All HI with the A26, Bw38, Dw10, DR4 haplotype were homozygous for both SC21 and SB4, suggesting that SC21 and SB4 should be included in the A26, Bw38, Dw10, DR4 extended haplotype. On the other hand, neither of the GLO markers were found in association with either haplotype. The results of this study indicate that HLA-SB is included in some extended haplotypes and may be important in these markers for diseases such as insulin-dependent diabetes mellitus. This study also demonstrated an apparent influence of HLA-SB on primary mixed lymphocyte culture (MLC) responses. The mean relative response of primary MLCs between individuals matched for HLA-A, B, D, DR, MB and MT but not SB was 40% of that for the MLCs with mismatched HLA-D, significantly higher than the MLCs matched for all HLA and complotypes.

Spontaneous remission of hypercalcemia in a functioning parathyroid cyst.

We treated a patient with a hyperfunctioning parathyroid cyst detected incidentally and which disappeared spontaneously. High levels of plasma PTH disappeared after removal of the cystic lesion of the parathyroid gland. Histologically, a secondary pseudocyst resulting from a cystic degeneration of an adenoma was observed. Minor hemorrhage of an unknown cause, within the adenoma, led to the large cystic lesion of the parathyroid gland.

Beating activity of heterokaryons between myocardial and non-myocardial cells in culture.

Cultured mouse myocardial cells grown as monolayers fused upon treatment with HVJ (Sendai virus). The myocardial cells also fused with quail myocardial cells, neuroblastoma cells and non-excitable cells, such as KB cells. The beating activity of these heterokaryons was studied in the present work. Heterokaryons composed of myocardial cells from different species maintained spontaneous beating activity for 2 days or more. Those of one myocardial and one neuroblastoma cell maintained the activity for 22-26 h, while those of one myocardial and one non-excitable cell, such as KB cell, lost the activity within 2-4 h after addition of HVJ. Heterokaryons that had stopped spontaneous beating did not contract on application of electrical-field stimulation. The ration of non-myocardial cells in the heterokaryons increased in inverse proportion to the decrease in beating activity of the heterokaryons. Study of the rapid disappearance of beating activity in heterokaryons composed of one myocardial and one KB cell showed that both excitability of the cell membrane and myofibril organization were rapidly lost.

[Experience with sulbactam/cefoperazone in the field of obstetrics and gynecology].

Sulbactam/cefoperazone (SBT/CPZ) was studied for clinical efficacy in the field of obstetrics and gynecology, and following results were obtained: In the treatment of 6 cases of gynecological infection, the clinical efficacy of SBT/CPZ was assessed as excellent in 1 case and effective in 5 cases. Neither adverse effects nor abnormalities in laboratory findings due to SBT/CPZ were observed. Based on these results, SBT/CPZ is considered to be a highly effective antibiotic with clinical efficacy in obstetric and gynecological infections in the daily dosage of 2 g given in two divided doses.

[Study on tissue transfer of ceftriaxone in the field of obstetrics and gynecology].

Ceftriaxone (Ro 13-9904, CTRX) was studied about the tissue transfer in the gyneco-obstetric field and the following results were obtained. The transfer of CTRX into the uterine tissues and adnexa was favorable following an intravenous injection with 1 g. The mean serum level 1 hour after administration was 123.3 micrograms/ml while the tissue level ranged from 26 to 48 micrograms/g. The level in the pelvic dead space exudate reached a peak 3 to 6 hours after administration and got higher than the serum level at 5 to 6 hours.

HLA antigens of insulin-dependent diabetics. I. PLT colonies detecting Dw10 and a new class II determinant distinct from HLA-D, DR, MB(DC), MT, and SB.

We describe three human proliferating T cell colonies, derived from mixed leukocyte culture with a non-diabetic individual (DR3 + 4) as the source of responding cells and an insulin-dependent diabetic patient (also DR3 + 4) as the source of stimulating cells. One colony detects HLA-Dw10 or a closely related antigen, and two detect an antigen that we call BO1 (Boston 1). BO1 is found so far on cells of all persons with DR5, about half of those with DRw6, and a particular subset of those with DR3. Among DR3-positive subjects, BO1 is positively correlated with HLA-B18 and BfF1, and negatively correlated with HLA-B8. These findings suggest that BO1 occurs in linkage disequilibrium with DR5, DRw6, and the haplotype B18, BfF1, DR3, the latter being common in southern Europe and reported previously to be a marker for insulin-dependent diabetes. In limited testing (21 subjects), BO1 was completely included in the supertypic specificity MT2, BO1 is a Class II HLA antigen, as demonstrated by blocking with monoclonal antibodies, but is distinct from all known antigens of the DR, MB(DC), MT, and SB series. It could be located on the same polypeptide chain as one or more of these antigen groups, however, particularly DR and/or MT.

[Diagnosis and treatment of germ cell tumors of the ovary].

Germ cell tumors represent a relatively small proportion of all ovarian tumors but are becoming important in terms of their clinical management because most of these tumors occur in young women. This group of tumors is composed of several different types of histological feature and prognosis of patients with this group of tumors is varied. Determinations of serum markers has been recommended as a useful diagnostic procedure. A chemotherapeutic regimen of VAC or PVB has markedly improved patient survival.
