Protective effect of diethyldithiocarbamate and carbon disulfide against liver injury induced by various hepatotoxic agents.

Diethyldithiocarbamate (DTC) and carbon disulfide (CS2), at nearly equimolar oral dose levels, protected mice against liver damage induced by carbon tetrachloride, chloroform, bromotrichloromethane, thioacetamide, bromobenzene, furosemide, acetaminophen, dimethylnitrosamine and trichloroethylene, as evidenced by the suppression of elevations in plasma GPT activity and liver calcium content, and of histopathological alterations. Both agents also prolonged hexobarbital sleeping time and zoxazolamine paralysis time in mice. DTC and SC, alone, given orally, decreased microsomal metabolism of several substrates (aniline, p-nitroanisole, hexobarbital, zoxazolamine, aminopyrine and 3,4-benzopyrene), CC14-induced lipid peroxidation, and cytochrome P-450 content. The loss of microsomal drug-metabolizing enzyme activity was also observed in the experiments in vitro using liver slices and isolated microsomes. Since a characteristic common to such diverse hepatotoxins is that they require metabolic activation before exhibiting hepatotoxicity, the protective mechanisms of DTC and CS2 may involve their interference with the process of metabolic activation of these hepatotoxins. The protective action of DTC may be mediated almost entirely through CS2 when administered orally and at least partly with parenteral administration, since, in CCl4-induced liver injury, DTC was most effective when given orally, while the action of CS2 was less dependent on the route of administration. Thus CS2 and CS2-producing agents in vivo such as dithiocarbamate derivatives and disulfiram may modify toxicological and pharmacological effects of foreign compounds by inhibiting microsomal drug-metabolizing enzyme activity in the liver.

Thallium-201-chloride thyroid scintigraphy to evaluate benign and/or malignant nodules: usefulness of the delayed scan.

The purpose of this study is to evaluate benign and/or malignant thyroid tumors with 201TI thyroid scan. We studied 76 cases of histologically verified thyroid tumors, all seen as cold nodules on the 123I thyroid scan. 201TI thyroid scan was performed 5-15 minutes (early scan) and 3-5 hours (delayed scan) after intravenous administration of 1.5-2.0 mCi of 201TI. In 35 (94.6%) of 36 malignant tumors (anaplastic carcinoma, six; papillary carcinoma 23; follicular carcinoma, five; epidermoid carcinoma, one; malignant lymphoma, 1) 201TI accumulated in the cold nodule of the 123I thyroid scan on both early and delayed scans. On the other hand, the delayed 201TI scan was negative in 35 out of 39 (89.7%) benign tumors. Employing early and delayed 201TI scans, we were able to differentiate most malignant thyroid tumors from those which were benign. False-negative and -positive cases are discussed.

Washout curves from myocardial infarctions in dogs, studied by contrast-enhanced computed tomography.

Experimental myocardial infarctions appear on CT scans as areas of mixed high and low densities. As the healing of the infarction proceeds, the low-density areas gradually decrease in size and disappear completely within 5 weeks. Healed infarctions appear as a high-density area only. In order to explore the contrast medium transport in the infarcted area, washout curves were constructed from the infarct components, the normal myocardium, and the left ventricular cavity at regular intervals over a period of 145 days. All regions except the low-density zones showed continuous decrease in density during the washout. The low-density zones showed gain in density during the first 10 to 15 min of washout, probably representing slow diffusion of contrast medium into the necrotic tissue. During the first 5 weeks of the healing process, the washout curve from the low-density zones gradually became similar and ultimately conformed to the other curves, concomitant with the ongoing neovascularization of the lesion.

ECG synchronized computed tomography in clinical evaluation of total and regional cardiac motion: comparison of postmyocardial infarction to normal hearts by rapid sequential imaging.

Computed tomograph (CT) of the heart was performed using the JEOL Dynamic Scanner, which provides CT cardiac images with minimal radiation and within a short period of time. ECG-synchronized CT was undertaken at various phases of the cardiac cycle every 0.04 second. Approximately 30 minutes of scanning was necessary to obtain a series of CT images of one complete cardiac cycle. In contract to 24 normal subjects, 38 patients with recent or remote myocardial infarction (MI) demonstrated hypokinetic, akinetic, or paradoxical movement of the ventricular segment corresponding to the MI sites predicted by ECG. The sequential cardiac area curve was useful in evaluating instantaneous changes of cardiac dimension, extent of ventricular contraction, and regional dyssynergy. ECG-synchronized CT studies using Somatom contrast dye enhancement in selected patients allowed sequential assessment of left ventricular cavity size and wall motion.

Effects of autonomic activity and changes in heart rate on atrioventricular conduction.

The effects of autonomic neural stimulation and changes in atrial pacing frequency on atrioventricular (AV) conduction were determined in anesthetized open-chest dogs. Increases in vagal stimulation frequency and in pacing rate significantly increased the AV interval, whereas increases in sympathetic stimulation frequency reduced the AV interval. Vagal stimulation (1.4 Hz) prolonged the AV interval by 17 ms when the atrial pacing rate was 2 Hz. On the other hand, the same vagal stimulation increased the AV interval by 29 ms when the pacing rate was 2.73 Hz. In addition, sympathetic stimulation (1.2 Hz) reduced the AV interval by 29 ms when the pacing rate was 2 Hz. In contrast, the same sympathetic stimulation reduced the AV interval 54 ms when the pacing rate was 2.73 Hz. However, the increase in vagal stimulation did not significantly alter the positive dromotropic response of the AV node to sympathetic stimulation. Therefore, the response of AV conduction to combined sympathetic and vagal stimulation was essentially the algebraic sum of the responses to the individual stimulations. Furthermore, the level of activity in one autonomic division did not alter appreciably the interaction between the pacing rate and the activity in the other autonomic division; i.e., the interaction between pacing rate, sympathetic stimulation, and vagal stimulation was not significant.

A unique change of steroid metabolism in rat liver microsomes induced with highly toxic polychlorinated biphenyl(PCB) and polychlorinated dibenzofuran(PCDF).

Pretreatments of rats with the highly toxic compounds such as 3,4,5,3',4'-pentachlorobiphenyl(PenCB), and 2,3,7,8-tetrachloro(TCDF) and 2,3,4,7,8-pentachlorodibenzofuran(PenCDF), which are potent 3-methylcholanthrene(MC)-type inducers, increased selectively 7 alpha-hydroxylation, but strongly suppressed 2 alpha-, 6 beta- and 16 alpha-hydroxylations as well as 5 alpha-reduction of progesterone and testosterone in the liver microsomes. This unique change in the metabolic pattern was accompanied by a marked decrease in total metabolism of both steroids and appeared to correlate apparently with their toxic potency. This kind of change was not shown by pretreatments with not only MC itself but also the phenobarbital-type(2,4,5,2',4',5'-hexachlorobiphenyl) and the mixed type PCBs(Kanechlor 400, a PCB mixture with 48% chlorine content), all of which possess only a low acute toxicity. The metabolic change produced by 3,4,5,3',4'-PenCB, 2,3,7,8-TCDF and 2,3,4,7,8-PenCDF might not be due to their stimulatory or inhibitory effects, because when added to the incubation mixture 3,4,5,3',4'-PenCB did not change the metabolic pattern with MC-microsomes to that with 3,4,5,3',4'-PenCB-microsomes. Furthermore, either 2,3,7,8-TCDF or 2,3,4,7,8-PenCDF gave similar metabolic pattern whereas their residual levels in the liver were greatly different from each other at the time of sacrifice. These results suggest that this kind of unique change of the steroid metabolism produced by highly toxic PCBs and PCDFs may be responsible, at least in part, for the endocrine symptoms caused by these compounds via disturbance of steroid homeostasis.

Estimation of pulmonary water distribution and pulmonary congestion by computed tomography.

Computed tomography (CT) of the lung in normal subjects and patients with congestive heart failure was performed in the supine position with deep inspiration to obtain pulmonary CT values and images. The mean CT value in normal subjects was higher in the posterior than anterior lung field, presumably because blood vessels were more dilated in the former than the latter due to the effects of gravity. The mean pulmonary CT value in patients with congestive heart failure was significantly increased possibly due to an increase in blood flow per unit lung volume arising from either pulmonary congestion or pulmonary interstitial and alveolar edema. The mean pulmonary CT value increased parallel to the severity of pulmonary congestion, interstitial or alveolar edema and was well correlated with the pulmonary arterial wedge pressure, indicating that such a correlation was a valuable tool in assessing therapeutic effects. The results of the present study indicate that pulmonary CT is useful for the noninvasive estimation of intrapulmonary water content and its distribution, thereby providing an effective diagnostic clue to various conditions in congestive heart failure.

Ventilatory response and drive due to carbon dioxide stimulation in idiopathic scoliosis.

Ten patients with idiopathic scoliosis and seven controls were studied for the purpose of investigating the ventilatory response and drive due to CO2 inhalation by the steady state CO2 response test. To evaluate the ventilatory drive for the respiratory output mouth occlusion pressure (P0.1) was used. The ventilatory response to CO2 of the patients was not significantly different from that of the controls. P0.1 for a given PCO2 stimulation or tidal volume in the patients was significantly higher than the control. Ventilatory output against P0.1, however, was not different in the two groups. The small tidal volume of the scoliotic patients seemed to be compensated by the high respiratory frequency with a large ventilatory drive in order to maintain adequate ventilation.

Unilateral cystic inner ear anomalies in siblings.

Unilateral cystic inner ear anomalies were diagnosed in two siblings, a 9 year old boy and a 6 year old girl. X-ray examination of the temporal bone was performed, together with audiological examinations and vestibular function tests. The common tomographic X-ray findings consisted of an enlarged solitary sac type deformity of the vestibule with narrowing of the internal auditory canal, severe hypoplasia of the anterior semicircular canal and no visualized cochlea. Pure-tone audiometry revealed severe mixed type of hearing loss in the right ear in both children. The test for vestibular function showed no response to caloric testing.

Prospect of cytologic diagnosis for malignant melanoma in the maxillary sinus.

Two cases of malignant melanoma arising in the maxillary sinus are reported. Cytological examination of the solution obtained by local washing through the sinus puncture identified numerous melanoma cells together with melanophages. The cases were then scheduled for well-planned, preoperative treatment. The cytological criteria for diagnosing malignant melanoma are outlined, and the cytological approach is stressed as a valuable diagnostic procedure for early detection of malignant tumors and surveillance of postoperative recurrence, especially in paranasal sinuses.

A case of monostotic fibrous dysplasia of the temporal bone associated with epileptic seizure.

An 11-year-old male with monostotic fibrous dysplasia of the left temporal bone was reported. At the age of seven years, the patient began having epileptic attacks, and a bony swelling of the left temporal region was noticed by his mother. Roentgenologically, there were almost thorough osseous obstruction and osseous proliferation of the external auditory canal and pars petrosa, respectively. Audiologic examinations indicated gradual functional disturbance based on the affected internal ear. A total of 20 cases with monostotic fibrous dysplasia of the temporal bone reported between 1946 and 1980 was analyzed, and the association of fibrous dysplasia and epilepsy was discussed.

Chemical analysis and toxicity of polychlorinated biphenyls and dibenzofurans in relation to yusho.

A typical lot of Kanemi rice oil ingested by patients with yusho (PCB poisoning) and the blood, liver and adipose tissue of the patients were analyzed for individual congeners of polychlorinated biphenyls (PCBs) and polychlorinated dibenzofurans (PCDFs) by gas chromatography and gas chromatography-mass spectrometry. The individual congeners identified were assayed for biological properties such as accumulation ability in the liver of monkeys and rats, inducing activities of benzo[a]pyrene 3-hydroxylase, benzphetamine demethylase and DT diaphorase in rats, and gravimetric changes of the thymus and liver in rats. Among the seven PCB congeners detected in yusho patients, 2, 3, 4, 5, 3', 4'-hexa-CB seems to be the most related compound to yusho by its strong effects on induction of the liver enzymes, and on atrophy of the thymus and hypertrophy of the liver in rats. PCDF congeners identified in the patients showed severe toxicity in rats than this PCB, exhibiting stronger enzyme induction and gravimetric changes of the tissues even at very low doses of 1-10 micrograms/kg. These PCDFs, especially 2, 3, 4, 7, 8-penta-CDF, were also very accumulative in the liver. Therefore, they are considered as the most important etiologic agents for current symptoms of yusho.

Attenuated ventilatory responses to hypercapnia and hypoxia in assisted breath-hold drivers (Funado).

The steady-state ventilatory responses to hypercapnia and hypoxia in 7 assisted breath-hold divers (Funado) were compared with those in 7 normal sedentary controls. Ventilatory response to hypercapnia was measured from the slope of the hyperoxic VN-PETCO2 line, where VN was normalized minute ventilation using the allometric coefficient and PETCO2 end-Tidal PCO2. The slope of this line in the Funado (1.48 +/- 0.54 liters . min-1 . Torr-1) was significantly less than in the control (2.70 +/- 1.08 liters . min-1 . Torr-1) (p less than 0.025). On the other hand, hypoxic sensitivity estimated by hyperbolic and exponential mathematical equations was not found to be significantly different between the two groups, although estimated increments in ventilation using the hyperbolic equation exhibited significantly lower response in the Funado than in the control only when PETO2 decreased lower than 50 Torr (p less than 0.05). These findings in the Funado were different from our previous observations obtained in unassisted breath-hold divers (Kachido), in whom no obvious attenuations in CO2 sensitivity were seen. This difference was assumed to be derived from more hypercapnic and hypoxic conditions produced in the Funado than in the Kachido during diving activities.

Fine spontaneous contractions of the arterial wall of the rat in vitro.

Fine spontaneous contractions of the vascular wall were recorded from the isolated carotid artery of the rat. The amplitude and/or frequency of these fine contractions decreased in lower temperature and increased in norepinephrine. It was suggested that there is an important relationship between these fine contractions and vascular "tone" of the artery.

[Amyl nitrite test in the evaluation of left ventricular function: application to ischemic heart disease and Duchenne's progressive muscular dystrophy].

The effect of amyl nitrite (AN) inhalation on the left ventricular function was evaluated by mechanocardiography and echocardiography. The patient's group consisted of 110 cases with ischemic heart disease (IHD) and 25 cases of dystrophia musculorum progressiva (DMP) of Duchenne type. The former was a representative of impairment of blood supply and myocardial involvement, and the latter was of predominant myocardial disease. The control was age-matched 32 normals for IHD group and 17 cases for DMP group. Left ventricular function was mainly evaluated by systolic time intervals (STI) and the echocardiographic correlates. Fifty-five cases of IHD group were tested by coronary angiography and left ventriculography and these data were compared with the noninvasive measures. The results were as follows: I. IHD group: The ratio of ejection time (ET) to pre-ejection period (PEP), ET/PEP, did not change so much as in controls after AN inhalation, and this percent change was much smaller in cases with lesions of the left anterior descending artery (LAD) than in cases with lesions of the right coronary artery (RCA). On the other hand, mean posterior wall velocity (mPWV) and posterior wall excursion (PWE) changed greater in patients with LAD lesion than in those with RCA lesion. In cases with LAD stenosis, percent change in ET/PEP was smaller in cases with asynergy than in cases without it, disclosing more significant impairment of the left ventricle in the former. II. DMP group: In severe cases, ET/PEP was small even at rest, and percent change by AN inhalation was smaller than control in mild cases and smallest in severe cases. This seems to be useful in evaluating the severity of the diseased process. The mPWV and PWE showed impairment of left ventricular motion even at rest, but it was clearly showed in severe cases after AN inhalation. These results indicate that impairment of left ventricular function induces the poor response to AN inhalation and this, in turn, results in the lack of hyperactivity of the heart produced by this drug.

[Clinical studies with cefmenoxime in the otorhinolaryngeal field (author's transl)].

We conducted a therapeutic trial with cefmenoxime in the field of otorhinolaryngology on a total of 14 subjects including 7 cases of otitis media, 5 cases of peritonsillar abscess, 1 case of chronic sinusitis and 1 case of congenital aural fistula. 1. The effective rate for peritonsillar abscess was extremely high being judged 'markedly effective' in 3 cases and 'effective' in 2 cases. 2. The effective rate for otitis media was also remarkably high; it was judged effective in 85.7% of the cases. It is thought that this is probably due to the fact that therapy and supervision were carried out on inhospital patients. 3. The 1 case of chronic sinusitis and the 1 case of congenital aural fistula were each judged 'effective' and 'markedly effective'. The only side effect observed was a slight case of skin eruption in 1 of the patients.
