[Alveolar cancer (author's transl)].

Alveolar cancer is the only true cancer of the lung as opposed to the common bronchial cancer. The authors defend this thesis by contesting the excessive usage of the expression bronchiolo-alveolar cancer and by supporting it with 12 original observations together with the literature review. The anatomo-pathological data are reported and discussed in order to analyze the reliability of the observations; diagnosis can vary according to the methods used in comparing the bronchial cytology and the examination of the operation tissue samples; the evolutive back search of observations revealed that out of 9 operated cases with localized involvement, 5 patients were still alive 2 years later. Complementary examinations by scintigraphy with labelled bleomycin were disappointing; this test performed 4 times was only once positive. Data from electron-microscopy and tissue culture seem to point towards a histogenesis from type II pneumocyte.

Activated human monocytes express the c-sis proto-oncogene and release a mediator showing PDGF-like activity.

Current ideas about the mechanism of wound healing and the pathogenesis of atherosclerosis, pulmonary fibrosis and hepatic fibrosis suggest a central role for the mononuclear phagocyte in attracting and/or stimulating the proliferation of mesenchymal cells. We demonstrate here that activated human blood monocytes, but not resting monocytes, release a mediator that attracts smooth muscle cells and cooperates with other mediators to stimulate fibroblast proliferation. This mediator is very similar to platelet-derived growth factor (PDGF): its chromatographic properties and chemical stability are similar to those of PDGF, it competes with 125I-PDGF for binding to fibroblasts and it immunoprecipitates with anti-PDGF antibodies. In parallel, stimulated monocytes, but not resting monocytes, express the c-sis proto-oncogene, a gene coding for one of the PDGF chains, consistent with the concept that expression of the c-sis proto-oncogene may be involved in the ability of mononuclear phagocytes to modulate the accumulation of mesenchymal cells.

Gene Expression Regulation↗

[Eosinophilic lungs].

Eosinophilic pneumonias are characterized by the occurrence, at one moment or another, of radiological opacities in the lungs classically--though inconstantly--associated with an excess of eosinophils in peripheral blood. The syndrome is mostly of diagnostic value, being due to various causes (parasites, fungi, drugs, etc.) and encountered in several and not always well-defined pathological conditions, such as vasculitis, Carrington's chronic eosinophilic pneumonia, malignant hypereosinophilia and others. What is known of the physiology of eosinophilic granular leucocytes has thrown no light, so far, on the pathogenesis of this syndrome, and causes other than those already known cannot be ruled out. Even after well-conducted investigations, the aetiology of the syndrome is sometimes impossible to determine. In such cases, the need for a lung biopsy and, only afterwards, for a test corticosteroid therapy ought to be discussed.
