[Simulation of bone resorption in denture patients using finite element analysis. 1. Application of optimization method].

The purpose of this study was to simulate the process of bone resorption under removable denture bases. Two dimensional finite element models were prepared for the frontal section of a maxillary complete denture with its supporting mucosa and bone in the first molar region. Simulations were performed with the optimization program which could proportionally move the nodal points according to the stress level difference between the actual and the reference stress level. By exerting loads on occlusal table of the model, the bone resorption progressed in the similar manner to reported clinical observations. The location and magnitude of the resorption were highly related to the location and direction of the loads.

[An elderly case of Crohn's disease complicated with internal fistula and mass formation with a review of the Japanese literature].

A case of Crohn's disease in the elderly complicated with internal fistula and mass formation was reported. A 62-year-old woman was admitted to National Toyohashi Hospital on August 16, 1986, with complaints of abdominal pain, pyrexia and diarrhea for two months. Physical examination revealed a goose-egg sized mass in the left lower quadrant of the abdomen. Barium meal examination of the small intestine showed strictures and fistula formation of the jejunum. Angiography showed narrowing and irregularity of the vessels. Malignant tumor of the small intestine could not be ruled out, so that an exploratory laparotomy was done on October 14, 1986. At operation, the jejunum, 70 cm from the ligament of Treitz, formed a 5 x 7 cm mass with adhesion and fistula formation. There was a skip lesion at the terminal ileum. Forty cm of the jejunum and 60 cm of the ileum were resected, and both of the resected specimens showed longitudinal ulceration, cobblestone appearance and thickening of the wall. Histological examination showed noncaseating granulomas with epitheliod cells and giant cells in the tunica muscularis. Since the report by Komi et al. in 1970, 13 cases of Crohn's disease in the elderly have been reported in Japan up to 1988. We summarized the characteristic findings of Crohn's disease in the elderly in Japan as follows; 1) Clinical symptoms and radiographic findings were similar to those in younger patients. 2) Small bowel involvement was reported in 54%. 3) Surgery was performed in 46%. 4) Preoperative diagnosis was only made in 46% of the elderly patients.(ABSTRACT TRUNCATED AT 250 WORDS)

Lectin-binding patterns in epithelial lining of jaw cysts.

Lectin-binding patterns were examined in epithelial walls of 65 jaw cysts (30 post-operative maxillary cysts: POMCs, 20 radicular and 15 follicular cysts), and characteristic lectin staining for each kind of jaw cysts is presented. Between squamous and columnar epithelia, the staining intensity of WGA, Con A and UEA-I was not different, but SBA bound more remarkably to squamous than to columnar epithelia. In both epithelia the outer layers did react more strongly with the lectins examined. Concerning odontogenic cysts, the lectin-binding affinities of outer and intermediate layer cells were nearly the same in both follicular and radicular cysts. Basal cells of radicular cyst walls were however, more markedly positive for lectin binding than of follicular cysts. Furthermore, basal cells of keratinized (RKSE 60 keratin-positive) epithelium were inferior to those of non-keratinized linings in the bindings. Lectin-binding patterns of metaplastic squamose epithelia of POMCs which were positive for RGE53-keratin (principally columnar epithelium-specific keratin) were similar to originally squamous linings of odontogenic cysts. Columnar linings of unusual radicular cysts were positively stained with SBA. By these results, lectin-binding sugar residues of the epithelium seem to be related to the epithelial morphology.

[Inference engines of expert systems for designing removable partial dentures].

The expert system for designing removable partial dentures has a great potentiality for clinical and educational use. The system sorts out several solutions for clinical problems with the aid of logic and data bases stored in the memory bank. The most important part of this system is the inference engine. Production system and frame system are often used as the inference engine. The purpose of this report was to compare different types of inference engine in practical situations. Following conclusion were obtained. 1) Since each type of inference engine has its own advantages and disadvantages, one has to select most suitable one for the purpose of the system. 2) One must select the inference engine type which will help potential user's understanding.

[Clinical application of T-scan System. 1. Sensitivity and reproducibility and its application].

Analyzing occlusal contacts is significant for establishing the occlusal scheme as well as for the diagnosis on dysfunction of stomatognathic system. We tested a new occlusal contact analyzing device (T-Scan: Tekscan Inc. Boston, U.S.A.) for its sensitivity and reproducibility in relation to its clinical application. Following results were obtained. 1) As the modeling of dental arch of this system was not always correct, much care should be needed for identifying the contact location. 2) The minimum amount of load needed for displaying each five step occlusal force increased linear fashion (step 1: 250 g and step 5: 1750 g) when the sensor was vertically pressed on the intersection of force sensitive line. The minimum amount of force changed with the loading condition such as area and direction. Distortion of the sensor could lead the measurement error. 3) Since the time measurement data by this system were shorter than actual length of time and were measured over about 250 g load, light and early occlusal contacts could not be detected.

[Construction of data base for designing removable partial dentures. 1. Retainer and abutment selection].

In designing removable partial dentures (R.P.D.) for edentulous patients, variety of designs are selected with considering the condition of residual teeth, alveolar ridge, occlusion and other factors. We have been trying to develop the computer consultation system for designing R.P.D. (so called "expert system") to rationalize the processes of R.P.D. design. In developing and expanding this system, the construction and enlargement of the rulebase and database are indispensable. In this report, we analyzed R.P.D. designs in student's clinic ('83-'85), in order to find the logic for design (rulebase) and to construct databases. Following results were obtained. 1) Wire clasps were most frequently used in Kennedy classification I, II in maxilla. Double clasps were mostly used in Kennedy class I, II in mandible. Akers' clasps were mostly used in class III. 2) Canine, first and second premolar were more frequently selected for abutment than other teeth.

Clinical application of hair protein glycation in the assessment of blood glucose control and diabetic neuropathy.

Glycation of hair protein was assessed in diabetic patients by the measurement of furosine, which is derived from fructose-lysine, a glycated lysine residue in protein. The level of furosine in 12-cm-long hair which grew over the course of one year was significantly better correlated with the mean values of four determinations of fasting plasma glucose (FPG) and four determinations of hemoglobin A1c, respectively, at the time of hair sampling. The level of glycation in hair, which corresponds to the time taken for hair growth, may represent the mean level of blood glucose during the time corresponding to the growth period. The values of motor nerve conduction velocity and sensory nerve conduction velocity were better correlated with the level of furosine in hair corresponding in the length to 1 year's growth than the levels of FPG and hemoglobin A1c at the time of the determination of nerve conduction velocity. These results suggest that hair glycation may serve as a valuable indicator both of long-term blood glucose trends and of the relationship between diabetic complications and blood glucose.

[Reference value of glutamic transaminase to prevent non-A non-B post-transfusion hepatitis].

GPT is currently used as a surrogate test for detection of the non-A, non-B carrier state. Among the patients who received the blood at least one blood unit from donor with relatively high level of GPT (26-35 KU), incidence of post-transfusion NANB hepatitis was higher than that of the patients who received only blood with low level of GPT (less than or equal to 25 KU). It is expected that introduction of lower GPT cut-off value will substantially reduce the incidence of post-transfusion NANB hepatitis. We reviewed several factors, such as gender, age and body weight that could potentially affect GPT activity, using the data of 40,967 regular blood donors for GPT who donated at Fukuoka Red Cross Blood Center from October to November, 1987. The upper limit of normal GPT for males is significantly higher than that of females. GPT values in both males and females progressively increase with the age and body weight. It would therefore seem that gender, age and body weight should be taken into account in resetting of GPT cut-off values to prevent post-transfusion NANB hepatitis.

Skin blood flow in diabetic patients during cold loading.

We measured periflux blood flow (PBF) in people's fingers and its response to cold loading with a laser Doppler flowmeter. As an index in the cold loading test we used cold recovery time (CRT). Among normal people, CRT increased with age. Diabetics had significantly longer CRTs than did age-matched controls. CRT seems to be useful for evaluating the skin microcirculation in diabetics and the aged.

[Clinical analysis of patients with hepatocellular carcinoma with non-A, non-B hepatitis virus].

An analysis of 242 patients with a hepatocellular carcinoma (HCC) has shown that 65 patients without a hepatitis B virus (HBV), alcohol abuse, or a transfusion history and 17 posttransfusion patients had a mean death age of 65.5 +/- 10.2 and 62.1 +/- 8.4 years respectively. The average age of 65 patients was 8.5 years older than those with a HBV and without a alcohol abuse history. Of 17 patients, 3 with a HBV had a mean death age of 55.3 +/- 10.8 years and the interval time from transfusion to a HCC was significantly shorter than in other post-transfusion patients.

Prevention of limb ischemia in surgical patients by intra-aortic balloon pumping.

Intra-aortic balloon pumping (IABP) was recently performed during the perioperative period in seven patients in whom balloon insertion was difficult because of vascular complication. Prophylactic measures for the prevention of limb ischemia were taken when the balloon was inserted. These measures included (1) balloon insertion through the femoral artery and continuous infusion of heparin into the femoral artery in three patients, (2) balloon insertion through the left femoral artery and femoro-femoral arterial crossover bypass in two patients, and (3) balloon insertion through the left common iliac artery and axillo-femoral bypass on the same side in two patients. Most patients recovered satisfactorily without any evidence of ischemia of the limbs either clinically or in serum biochemistry examination. One patient died of prolonged cardiac failure.

[A case of polymyalgia rheumatica with cytoplasmic bodies].

A 63-year-old man with polymyalgia rheumatica (PMR) with cytoplasmic bodies (CB) in biopsied muscle was reported. He was referred to our hospital in June 1988 because of myalgia of four extremities. The ESR was 114 mm/hr and the serum CPK level was normal. Needle EMG and nerve conduction velocities showed normal findings. On light microscopic examination in biopsied muscle, the inclusion bodies which appeared on HE stained sections as deep reddish masses were present in 10% of all fibers. By electron microscopy the core of the bodies consisted of a dense tangle of thin filaments which were intermixed with granular material. Around the core a network of irregularly or radially oriented filaments was observed. These bodies were identified with cytoplasmic bodies. Since there has been no previous report of PMR with CB, our case is important to discuss the relationship between the pathogenesis of PMR and CB.

[Clinical study on the serum carcinoembryonic antigen, immunosuppressive acidic protein and C-reactive protein levels in patients with adult T cell leukemia].

Serum carcinoembryonic antigen (CEA), immunosuppressive acidic protein (IAP) and C-reactive protein (CRP) levels were measured in patients with adult T cell leukemia (ATL) in order to clarify their significance in this disease. Mean (+/- SD) serum CEA levels in 11 patients with acute ATL (3.1 +/- 1.3 ng/ml) and 7 patients with smoldering ATL (3.1 +/- 0.5 ng/ml) were significantly higher than in sera of 222 healthy controls (2.4 +/- 1.3 ng/ml). However, the levels in 7 patients with chronic ATL and healthy controls showed no differences. On the other hand, mean (+/- SD) serum IAP levels in patients with acute ATL (928 +/- 395 micrograms/ml), chronic ATL (487 +/- 125 micrograms/ml) and smoldering ATL (429 +/- 90 micrograms/ml) were significantly higher than in sera of healthy controls (359 +/- 103 micrograms/ml). However, the levels in patients with smoldering ATL and healthy controls showed no differences. Serum IAP levels in crisis in chronic and smoldering ATL were similar to those in patients with acute ATL. 85% of ATL patients with IAP levels above 500 micrograms/ml had CRP levels above 1+. Serum CEA, IAP and CRP levels were serially measured in a number of patients. Serum IAP and CRP levels reflected each patient's clinical course more than serum CEA levels. Overall the simultaneous measurements of serum CEA, IAP and CRP levels revealed a potential usefulness for determination of ATL subtype, and serum IAP and CRP levels may provide a way to evaluate the effectiveness of chemotherapy.

Genetic studies on pancreatic proteinase in Japanese quail.

A genetic variation was found in pancreatic proteinase of Japanese quail. In eight bands of proteinase isozymes, the variation of band 5 (presence or absence) was detected among quails. Band 5 was identified as a chymotrypsin. The presence and absence of band 5 are controlled by a pair of allelic genes (Prt-5A and Prt-5a) on an autosomal locus, and gene Prt-5A, causing expression of band 5, is dominant to gene Prt-5a, a null allele for band 5. Zymograms of proteinases and their zymogens were also compared; no variation of chymotrypsinogen; corresponding to chymotrypsin Prt 5 were detected. It is suggested that the Prt-5 variant of chymotrypsin may be formed during activation of chymotrypsinogen.

[A case of myokymia-hyperhidrosis syndrome with muscle involvement].

The present report deals with an unique case of myokymia-hyperhidrosis syndrome. A 46-year-old man developed generalized relatively slow, undulating movement of the muscles, excessive sweating, muscle cramps and easy fatigability since three years ago. On admission, he had generalized myokymia, abnormal movements consisting of slow, undulating and worm-like muscular contractions. There was no muscle atrophy or weakness, or sensory deficits. Deep tendon reflexes were diminished and no Babinski sign or other pathological reflexes were present. Muscle tone was in normal range. Neither fasciculation nor myotonia was found. Laboratory examinations revealed elevation of BMR with normal thyroid function. The value of anti-Ach receptor antibody was below 0.6nmol/l. EMG recordings showed (1) continuous spontaneous repetitive discharges of NMU at a frequency of 30-50Hz, with duration of 1-2 seconds and amplitude of about 1 mV, were demonstrated at rest and the findings were consistent with myokymia, (2) waxing phenomenon was observed on repetitive peripheral nerve stimuli, which didn't improve by the tension test, and (3) high amplitude, long duration and polyphasic NMU, a sign of denervation, were observed on voluntary contraction. Muscle biopsy specimens from the left quadriceps showed a number of internal nuclei. On electron microscopy, clustered mitochondria and filamentous bodies were very often found in subsarcolemma. From these observations, damage to peripheral nerves, neuromuscular junctions and muscles may be responsible for the generalized myokymia in our patient.
