Brain-associated small-cell lung cancer antigen (BASCA) is expressed in developing lung: an immunohistochemical and immunoelectron microscopic study.

Expression of brain-associated small-cell lung cancer antigen (BASCA) in developing lung and in lung tumors was investigated immunohistochemically and immunoelectron microscopically with monoclonal antibodies recognizing different epitopes of BASCA. In fetal lung, epithelial and mesenchymal cells had different spatial and temporal expression patterns, in contrast to the consistent pattern in neural cells. The cell membranes of epithelial cells of the proximal bronchial tubes were diffusely positive at the pseudoglandular stage. Ciliated cells lost immunoreactivity shortly after their emergence, but non-ciliated cells, including endocrine cells, lost it at the alveolar stage. The immunoreactivity in mesenchymal cells was reduced in the proximal airway, but positivity remained in the distal lung later during the postnatal period. All endocrine tumors of the lung, defined by diffuse synaptophysin immunoreactivity, expressed BASCA, but some non-endocrine carcinomas which also lacked densely cored granules ultrastructurally, showed BASCA positivity. The temporal and spatial pattern of BASCA expression in the developing lung suggests that BASCA plays an active role in lung morphogenesis. BASCA may be expressed as an oncofetal substance in some non-endocrine carcinomas of the lung.

Epidermoid of the quadrigeminal cistern--case report.

The authors report a case of an epidermoid in the quadrigeminal cistern in a 40-year-old female who presented with headache and nausea. According to the literature, epidermoids are rarely found in the quadrigeminal cistern. Additional unusual features in this case were the absence of a capsule and T1-weighted magnetic resonance images suggestive of both solid (less intense) and fluid (more intense) components.

[Teeth arrangement of anterior artificial teeth. 1. Age and teeth exposure].

In selecting and arranging anterior artificial teeth for edentulous patients, it is necessary to consider their sex, personality, age and other factors. The relation of upper and lower lip lines to incisal edge has been one of the standards for the selection and the arrangement of anterior artificial teeth. The purpose of this report was to establish and to evaluate decision bases for the selection and arrangement. We recorded and measured the exposure rate of anterior natural teeth (48 men, 46 women) during function, such as smile, pronunciation, by means of the image analysis. Following results were obtained. 1. As the age of patients increased, upper anterior teeth appeared less than lower's. 2. Differences in exposure ratio between upper and lower teeth were found more remarkable in smiling rather than in pronunciation.

[Histopathological analysis of incidental prostatic carcinoma by step section technique].

In 244 cases treated by subcapsular prostatectomy during the period from April 1975 to December 1985, whole tissue specimens were prepared by a step section technique to examine them histopathologically in accordance with the handling agreement for prostatic cancer presented by the Urologic Society of Japan. The degree of differentiation, clinical stage of the disease, the size of the lesion and the distance to the margin were determined. Incidental carcinoma was found in 35 cases (14.3%), which increased with age. There were 21 cases of stage A1 and 14 cases of A2. Diffuse proliferation was found in 4 cases. Isolated carcinomatous focus was observed in 40 sites, and well, moderately, and poorly differentiated adenocarcinoma in 33, 3 and 4 sites respectively. The maximum diameter of cancer was below 5 mm in 80%, while the distance from the center of the carcinomatous focus to the surgical margin was within 3.5 mm in 905. Seven cases had plural lesions. These lines of evidence mentioned above may support carcinogenesis from the subcapsular region as well as multicentric origin of cancer.

[A case of embryonal carcinoma arising in the basal ganglia of the cerebrum].

The occurrence sites of intracranial primary germ cell tumors are most often the pineal and suprasellar regions. The histological type observed most frequently in these tumors is germinoma. Cases of embryonal carcinoma arising in the basal ganglia are rarely reported. To our knowledge, only 4 such cases have been previously reported in the literature. A case of an embryonal carcinoma arising in the basal ganglia is reported. A 17-year-old boy was admitted to our hospital on July 30, 1988 because of headache and vomiting, and a right hemiparesis. On admission, physical inspection showed no abnormalities and neurological examination revealed obtunded consciousness, a right central facial paresis and a right hemiparesis with Hoffman and Babinski reflexes. Noncontrast CT scan showed a large mass of low-to slightly high-density in the region of the left basal ganglia accompanied with midline shift and ventricular dilatation. Enhancement of the lesion was made by contrast CT scan. It was not homogeneous. Cerebral angiogram displayed a contralateral shift and an unrolling of the anterior cerebral artery, a lateral stretch of middle cerebral artery, a downward stretch of anterior choroidal artery and a tumor stain fed by the Heubner artery. On August 3, left frontotemporal craniotomy was performed. The tumor was totally removed in a piecemeal manner using microsurgical techniques. Histopathological diagnosis was mixed-type of germ cell tumor comprising embryonal carcinoma and teratoma. Postoperative CT scan showed complete disappearance of the tumor. A course of radiation of 4950 rads and two courses of a combination chemotherapy with cis-platinum, vinblastine and bleomycin were given within 3 months after the operation.(ABSTRACT TRUNCATED AT 250 WORDS)

Degeneration and ossification of the yellow ligament in unstable spine.

Lumbar yellow ligaments were obtained from 20 cases of lumbar spinal canal stenosis and 20 cases of degenerative olisthesis with slippage of more than 3 mm. The ligaments were stained with safranin-O, von Kossa, and immunohistochemical staining methods (S-100 protein). In the unstable group the safranin-O staining was more intense, microscopic ossification and chondroblasts were noted more frequently, and S-100 protein was more abundant. These findings show that instability of the lumbar spine accelerates degeneration and chondrometaplasia of the yellow ligament, which may lead to the enchondral ossification of the ligament.

Problems of denture wearers regarding denture cleaning.

With the increase of elderly population, the number of patients who need removable prostheses increase rapidly. Denture wearers have different problems from non-denture wearers in terms of maintaining oral health. In this study, we attempted to find out specific difficulties of denture wearers with questionnaires regarding social life problems as well as oral hygiene problems. Our results indicated that most of the denture wearers considered the cleaning of denture was essential to maintain their oral health, though methods they employed showed a great deal of difference. Also denture wearers felt difficulties in cleaning their dentures when they went out.

Mouth guard and occlusal force distribution.

Mouth guards are effective to prevent traumatic injury in maxillofacial region during sports activities. This study was designed to find out the influence of wearing mouth guards on occlusal force distribution using an occlusal force transducer system (T-Scan system). Occlusal forces were more evenly distributed when mouth guards were in place comparing with natural dentition and with wearing conventional full arch bite planes.

[Clinical application of T-Scan System. 2. Evaluation of soft mouth guards].

Soft mouth guards are effective to prevent traumatic injury in the maxillofacial region. In this study, we tried to find out the effect of soft mouth guards on the distribution and the magnitude of occlusal force using the T-Scan system. Ten adult subjects volunteered to exert maximum biting force wearing three different types of soft mouth guards. With the limitation of the T-scan system reported in our previous article, following results were obtained. 1) Occlusal forces were more evenly distributed with soft mouth guards than without them. 2) Softer mouth guards were more effective for occlusal force distribution. 3) Since occlusal relationship between mouth guards and mandibular arch could alter the distribution pattern a great deal, occlusion of mouth guards should be examined more carefully.

[Mouth guard and occlusal force distribution].

Mouth guards are effective to prevent traumatic injury in maxillofacial region during sports activities. This study was conducted to investigate the influence of wearing mouth guards on occlusal force distribution with the T-scan system. With comparison to the value of average occlusal force level per occlusal point (OFL) on natural dentition and wearing conventional full arch bite plane, following results were obtained. 1) Difference of OFL within the same dental arch became smaller when mouth guards were on. 2) Statistically significant difference in OFL was not found between two mouth guards with different hardness. 3) During three seconds clenching, OFL in posterior region changed great deal at the initiation but stayed at about the same level after that.

The risk of development of HTLV-I-associated myelopathy/tropical spastic paraparesis among persons infected with HTLV-I.

Using data obtained in national surveys of human T-lymphotropic virus type I (HTLV-I)-associated myelopathy/tropical spastic paraparesis (HAM/TSP) conducted in Japan in 1987 and 1988, we estimated the yearly and lifetime risk that HAM/TSP will develop in an HTLV-I-infected person. "Definite" HAM/TSP was defined as slowly progressive myelopathy with antibodies to HTLV-I in both serum and cerebrospinal fluid. Estimates of HTLV-I infection rates in eight endemic prefectures, by age group and sex, were obtained from serologic studies of blood donors; population figures, by age group, sex, and prefecture, were obtained from the census. Of 589 definite cases of HAM/TSP reported nationally, 397 occurred in residents of the eight endemic prefectures; of these, 170 reported onset of illness during the years 1982-1988 (average incidence, 24.3 cases/year). Using the estimated HTLV-I infection rates and the 1985 census figures, we estimated the number of HTLV-I-infected persons in the eight prefectures in 1985 at 794,800. We therefore estimated the incidence of HAM/TSP among HTLV-I-infected persons at 3.1 x 10(-5) cases/year; assuming a lifetime of 75 years, the lifetime incidence is approximately one quarter of 1%. This estimate is important in counseling persons such as blood donors found to be infected with HTLV-I.

[Study of acute gastric mucosal lesion induced by endotoxemia].

Acute gastric mucosal lesion (AGML) was induced six hours after the administration of endotoxin. The decrease of gastric mucosal blood flow, used to be supposed as an important factor of the formation of AGML, was not found, but thiobarbituric acid (TBA) reactants in the gastric mucosa were increased three hours after endotoxin injection, 198 +/- 18.2 (vs control 130 +/- 18.2). The administration of platelet activating factor (PAF) inhibitor, CV3988, reduced the formation of AGML and increase of the TBA reactants. These results suggested that the chemical mediator like PAF and free radicals may play an important role in the pathogenesis of gastric mucosal injury induced by endotoxemia, without the decrease of mucosal blood flow.

[A case of double cancer: renal cell carcinoma and transitional cell carcinoma of urinary bladder].

This is a case report on a patient with double cancer of kidney and urinary bladder. The patient was a 65-year-old female, and she was admitted with the chief complaint of gross hematuria and right flank pain. After careful examinations, she was diagnosed with right nonfunctioning kidney caused by invasive bladder cancer. Computerized tomography incidentally revealed a mass in the upper pole of the left kidney. Selective left renal arteriography showed stretched arteries and irregularity and tortuosity of the smaller vessels. She was diagnosed with double cancer of bladder and left kidney. Owing to the damage of the right renal function, left partial nephrectomy, total cystectomy, right nephroureterectomy and left ureterocutaneostomy were performed. According to DMSA scintigraphy measured 15 days later, the uptake value of the left kidney was 13.25%, compared to the preoperative value of 25.62%. To date, this case is 36th reported case in Japan.

[Role of PMN elastase in fibrinolytic activity in patients with acute promyelocytic leukemia].

In order to define a specific acceleration of fibrinolytic activity in acute promyelocytic leukemia (APL), we determined fibrinolytic factors in APL and acute myeloblastic leukemia (AML). An increase in plasma levels of D-dimer was observed in both APL and AML, indicating that there is an acceleration of fibrinolysis in both types of leukemia. The levels of D-dimer/FDP ratio were significantly lower in APL than AML. These findings suggest that fibrinogenolytic activities were higher in APL that in AML. The relationship between the plasma levels of plasmin alpha 2PI complex (PIC) and FDP was investigated to study whether fibrinolysis was induced by plasmin. PIC levels were linearly correlated with FDP levels in AML, while in APL there was no close correlation between the plasma levels of PIC and FDP. Then, we measured PMN elastase-alpha 1 proteinase inhibitor complex (E-alpha 1 PI). There was a correlation between the plasma levels of E-alpha 1PI and FDP in APL but not in AML. Furthermore, PMN elastase activity was detected in leukemic cell lysate in patients with APL but not in AML. These findings suggest that PMN elastase may be an important factor in the induction of fibrinolysis in APL.

[Esophageal carcinoma with hypercalcemia that appeared to be caused by prostaglandin E2 produced by the tumor cells].

Discussed is the case of a 50-year-old man with a well advanced esophageal carcinoma who, during his final clinical course, suddenly developed hypercalcemia (max: 15.0 mg/ml). His serum parathyroid hormone level, however, remained within normal limits. On autopsy, an extensive metastasis to many organs and lymph nodes was noted but no evidence of a bone metastasis. Nude mice bearing the same tumoral tissue were found, on autopsy, to have similarly developed hypercalcemia and cells that were cultured were found to produce an excessive amount of Prostaglandin E2 (PGE2). These findings suggest that this humoral hypercalcemia of malignancy (HHM) was caused by excessive PGE2 produced by the tumor cells, although other possible factors should be investigated.

Lymphatics, intraepithelial lymphocytes and endometrial lymphoid tissues in the rabbit uterus: an electron microscopic and immunohistological study.

Rabbit uterine intraepithelial lymphocytes, endometrial lymphoid aggregates and lymphatic capillaries were examined electron-microscopically and immunohistochemically at well-defined intervals after the injection of human chorionic gonadotropin (hCG). Lymphatic capillaries originated near the bases of the glands in the uterine cervix and in the border zone between the lamina propria mucosae and myometrium in the uterine body. The lymphatic capillaries were maximally dilated, and their endothelial cells were thinnest 8 hours after hCG injection. Patent junctions between the adjacent endothelial cells of lymphatics in the uterine body were observed in good accordance with the appearance of stromal edema and lymphatic dilatation. The numbers of intraepithelial lymphocytes and macrophages changed cyclically after the induction of ovulation. They were highest 11 hours after hCG injection when lymphocytes were seen occasionally in the lumens of lymphatics located in the lamina propria mucosae of the uterine body. Most of the intraepithelial and interstitial lymphocytes were cells labeled with T cell serum but some were labeled with IgA serum and were occasionally seen beneath the epithelium. Lymphoid aggregates were uniformly present in the stratum basalis and consisted of lymphocytes and macrophages. They had no germinal centers, surrounding lymphatics or high endothelial venules (HEV). The results suggest that lymphatic capillaries are the main route for the removal of edema fluid and for migratory lymphoid cells in the rabbit uterus.

[Dural arteriovenous malformation of the spine].

A case of dural arteriovenous malformation of the spine was reported. The patient was admitted to our hospital because of paraparesis, and selective angiography revealed dural AVM of spine. Myelo-CT and MR imaging showed abnormal vessels in the spinal canal. After embolization, dural AVM was disappeared.
