Quantification of Aquaporin-CHIP water channel protein in microdissected renal tubules by fluorescence-based ELISA.

Several transporters have been localized along the nephron by physiological methods or immunocytochemistry. However, the actual abundance of these molecules has not been established. To accomplish this goal, we have developed a fluorescence-based ELISA method and have used it to quantitate Aquaporin-CHIP (AQP-CHIP) water channel protein in rat kidney tubules. Microdissected tubules (2 mm/sample, permeabilized with 0.5% Triton X-100) or purified AQP-CHIP standards (0-200 fmol) were utilized in a fluorescence ELISA protocol after covalent immobilization on epoxy-activated Sepharose beads. The lower limit of detection was 2.4 fmol of AQP-CHIP. Preabsorption with excess purified AQP-CHIP or use of nonimmune serum eliminated the signal. In proximal segments, the measured AQP-CHIP was linearly related to tubule length (1-10 mm). The measured AQP-CHIP was (mean +/- SE, fmol/mm): S-1 proximal, 10.8 +/- 2.1; S-2, 10.0 +/- 2.3; S-3, 21.3 +/- 3.1; type 1 thin descending limb (DTL), 12.9 +/- 4.6; type 2 DTL, 86.5 +/- 19.5; type 3 DTL, 43.0 +/- 11.2. In thin ascending limbs, thick ascending limbs, distal convoluted tubules, connecting tubules, and collecting ducts, the AQP-CHIP signal was indistinguishable from zero. Based on the unit water conductance of single CHIP molecules, our calculations show that the content of AQP-CHIP is sufficient to explain water permeability measured in isolated proximal tubules and DTL segments.

[Study of biochemical gene markers in the Mongolian gerbil (Meriones unguiculatus)].

The present study was conducted to analyze biochemical marker genes and to clarify genetic variation in 4 Mongolian gerbil (Meriones unguiculatus) strains (MGS/Sea, MON/Jms Gbs, Kwl: Mongolian Gerbil and Hos: Mongolian gerbil) maintained in Japan. Cellulose acetate, starch gel or agar gel electrophoresis was used. Of the 23 kinds of proteins detected, variation was found only in liver acid phosphatase (Acp2); no variation in other proteins was found within or among strains. The results suggest that these 4 Mongolian gerbil strains are genetically similar. A strain difference was found in the Acp2 locus of liver acid phosphatase. The Kwl: Mongolian Gerbil strain had a band of Acp2 locus, whereas MGS/Sea, MON/Jms and Hos: Mongolian gerbil strains did not.

A proteinous factor mediating intercellular communication during the transition of Dictyostelium cells from growth to differentiation.

In general, cell differentiation and proliferation are mutually exclusive. Transition of the cellular slime mold Dictyostelium discoideum from growth to differentiation is triggered mainly by a secreted factor(s) in addition to nutritional deprivation. To purify and identify the factor required for the growth/differentiation transition, a new assay system was designed. Under low-nutrient conditions, cells could grow to multiply, but never developed. The cellular development including aggregation, however, was induced by the addition of conditioned medium (CM) in which growing or starving Dictyostelium cells had been cultured. The CM inhibited the synthesis of nuclear DNA and induced the cells to acquire chemotactic competence to cAMP, thus suggesting the presence of a secreted factor(s) required for growth/differentiation transition in the CM. The active factor(s) in CM (referred to as CMF450; conditioned medium factor) was found to be sensitive to heat and have a large molecular size. The CMF450 was purified using FPLC through a gel filtration column, and was identified to be a proteinous macromolecule of Mr 450 kDa, which was mainly composed of 94 kDa, 79 kDa, and 49 kDa subunits under a native condition.

[Image guided stereotactic biopsy for brain tumors: experience of 71 cases].

Despite advances in the neuroimaging of the brain, an accurate diagnosis of intrinsic lesion of the brain requires tissue sampling and histological verification. Seventy-one patients with intraparenchymal lesion of the brain underwent CT or MRI-directed stereotactic biopsy at Okayama University Hospital between June, 1987 and March, 1995. There were 32 men and 39 women whose ages ranged between 7 and 78 years (mean 46.1 years). All patients underwent preoperative cerebral angiography, high resolution contrast enhanced CT and MRI. The lesions were located in the hemisphere in 40 cases, the thalamus or basal ganglia in 14, the midline (corpus callosum or ventricle) in 11, the pineal region in 4, the suprasellar in one and multiple sites in one. A Brown-Roberts-Wells (BRW) CT-directed stereotactic system was used for biopsy under CT guidance. For MRI-directed biopsies a prototype modification of the BRW frame was employed. Target localization was performed using either CT or MRI. Usually one or two targets within the lesion were chosen and target coordinates were calculated using the CT or MRI scan soft ware. Positive diagnosis was obtained in 67 cases and the accuracy of the histological diagnosis was 94.4%. There were 53 gliomas, 4 metastasis, 5 germinomas, 3 malignant lymphomas, one pineoblastoma, one infarction and 4 negative biopsies. Bleeding as a complication due to stereotactic intervention occurred in one patient (1.4%). To patients with potentially inoperable lesions or lesions which might be best treated by chemotherapy or irradiation, modern techniques of neurosurgery now offer the option of precise stereotactic biopsy through small twist-drill burr holes as opposed to open biopsy.(ABSTRACT TRUNCATED AT 250 WORDS)

Endoscopic polypectomy for pacemaker patients.

Endoscopic polypectomy using high frequency voltage is contraindicated in patients with cardiac pacemakers. Recently, highly advanced pacemakers have enabled us to perform endoscopic polypectomy on these patients by taking appropriate cautions. We successfully removed 10 colonic polyps and one gastric polyp in patients with pacemakers by endoscopic polypectomy. No complication and dysfunction of the pacemaker occurred before, during or after the polypectomy.

[A case of rhinocerebral phycomycosis].

Rhinocerebral phycomycosis is an uncommon opportunistic infection with ubiquitous fungi of the class Phycomycetes, starting in the nose and extending to the paranasal sinuses and then intracranially. The condition is often characterized by poor prognosis because of occlusion of the internal carotid artery. This disease is commonly associated with predispositions such as uncontrolled diabetes mellitus, which is the most common, immunosuppressive states and metabolic bankruptcy including leukemia, lymphoma, myeloma, malnutrition, uremic or diarrheal acidosis, severe burns, anemia, carcinoma, radiotherapy, liver cirrhosis, hemochromatosis, tuberculosis, septicemia, long-term medication of steroid, antibiotics and antimetabolite, drug addiction, cytotoxic drug administration and AIDS. Cases with unknown predisposition, however, have been infrequently reported in the literature. The authors report a case of rhinocerebral phycomycosis in which concurrence of Candida species instead of the above-mentioned common predispositions was considered a potential predisposition. To our knowledge, only 1 report in which Candida species are referred to as a potential predisposition for this disease has been previously issued. A 85-year-old man was admitted to our hospital on March 2, 1994 because of generalized convulsion. He had received a total extirpation of an ascending colon cancer in July 1993. On admission, physical inspection showed no abnormalities and neurological examination revealed obtunded consciousness without other abnormalities. He had no diabetes mellitus. Hematological and blood chemistry values were normal except for CA19-9 of 45 U/ml.(ABSTRACT TRUNCATED AT 250 WORDS)

[Severe hyperbilirubinemia after cisplatin-based chemotherapy].

Cisplatin is the most effective and widely used anti-cancer drug for ovarian cancer. We report 2 cases with severe hyperbilirubinemia after cisplatin-based chemotherapy. Case 1 was a 67-year-old woman with stage IV ovarian cancer. After operation, she had 2 courses of chemotherapy consisting of cisplatin (90 mg) and cyclophosphamide (550 mg). The regimen was changed to low-dose consecutive cisplatin (10 mg/day, day 1-day 5) because of her ileus and poor performance status. After 2 courses of cisplatin alone, her total bilirubin was elevated to 19.1 mg/dl. She died of respiratory distress. At autopsy, chronic cholangiolitis with intrahepatic bile stasis were noted. Case 2 was a 60-year-old woman with stage IIIc ovarian cancer. After operation she was treated with carboplatin (383 mg/day, day 1) and cisplatin (102 mg/day, day 3). One month after completing the first chemotherapy, her bilirubin elevated to 20.5 mg/dl. It took 3 months to normalize the serum bilirubin with steroid administration. During the second course using the same regimen as in the first course, the bilirubin elevated again. Cisplatin was suspected to be the drug inducing her hyperbilirubinemia. She was consecutively treated with carboplatin alone and the bilirubin did not elevate. Both cases had blood transfusion and intravenous hyperalimentation. It is difficult to disregard the effect of other drugs and therapy. In case 2, her bilirubin elevated repeatedly after cisplatin administration. Cisplatin may thus be the drug which induces the liver dysfunction.

[Consensus and new development in epithelial ovarian cancer chemotherapy in 1995].

Recent clinical trials have confirmed that cisplatinum combination has advantages for advanced ovarian cancer interns of response rate, response duration and disease-free time, but little advantage for overall survival. Although addition of doxorubicin to cisplatin-based chemotherapy is a matter of controversy between European countries and the USA, high-dose epirubicin, which is an analog of doxorubicin, has a potent response in relapsed cases after CDDP combination chemotherapy. Clinical trails of epirubicin + CPA + CDDP for advanced ovarian cancer and underway in Japan. Under standard supportive care, escalated CDDP dose may prolong median survival time, but its advantage for long-time survival is questionable. Trials of high-dose chemotherapy as front line treatment for advanced ovarian cancer supported by PBSCT/G-CSF in combination with assessment of scheduled AUC have been collected from many world-wide institutes. GOG demonstrated that intraperitoneal administration of CDDP with intravenous CPA had obvious superiority to intravenous CDDP and CPA in overall survival and adverse effect for stage III ovarian cancer, under the conditions that 70% of cases had minimum residual tumor (< 0.5 cm). We have to make sure whether this modality has the same effect on early and stage IV ovarian cancer. Taxol plus CDDP has promising benefits for median disease free survival and median survival time, so it is becoming the standard regimen for advanced ovarian cancer in USA. The delay of the clinical introduction of taxanes should be eliminated promptly in Japan.

[Two cases of isolated ambient cistern hematoma after head injury].

This is a report of two cases of isolated ambient cistern hematoma after head injury. Isolated small hematomas in the ambient cistern after head injury are considered indirect evidence of brain stem injury. In such cases the initial neurological signs are often severe, but the ultimate prognosis is almost always good or excellent. Our patients showed hardly any neurological signs, and their head trauma was not very severe. Isolated ambient hematomas are the result of injury to small vessels or of mild contusions of the brain stem produced by the edge of the tentorium. In case 2, MRI revealed small, high signal intensity near the hematoma, and this sign was interpreted as evidence of mild brain stem injury. We think our patients had very mild brain injuries which were unaccompanied by diffuse axonal injury.

Synchronous circadian rhythms of mRNA levels and activities of cholesterol 7 alpha-hydroxylase in the rabbit and rat.

Cholesterol 7 alpha-hydroxylase, the key enzyme in a series of metabolic changes for the production of bile acids from cholesterol, shows circadian rhythms. The mechanism of the circadian rhythms is the subject of controversy; moreover, few pieces of information are presently available. Because the rabbit is a good animal model for the study of human cholesterol-bile acid metabolism, in the present study we isolated a complete cDNA encoding rabbit cholesterol 7 alpha-hydroxylase (3022 base pairs (bp) long and 1503 bp open-reading frame encoding 501 amino acid residues). Using this isolate rabbit cDNA and the rat cDNA as probes, the circadian rhythms of this enzyme were studied in normal male rabbits and rats. The animals were maintained on a 12-h light-dark cycle. Three subjects each were then killed every 3 h for 1 day. The livers were harvested, and the mRNA levels and the activities of the enzyme were measured. Both mRNA levels and activities of the enzyme showed remarkable circadian rhythms, with higher values during the dark phase and lower values reaching minimum during the light phase. The enzyme activities correlated well with the mRNA levels. There was no shift in the cycles of the enzyme activities and the mRNA levels. These findings suggest that pretranslational regulation is the most likely mechanism for the circadian rhythms of cholesterol 7 alpha-hydroxylase. Because the rhythms of mRNA levels and activities of this enzyme are synchronous, the turnover of the enzyme should be as fast as that of the mRNA, and we can hereafter measure the mRNA levels instead of the enzyme.

[Recent advances in ovarian cancer chemotherapy from the survival standpoint].

The following consensus has been reached over the last two decades from clinical research: 1) Single nonplatinum agents or nonplatinum combinations have failed to demonstrate a significantly longer median survival. 2) Platinum combinations are generally better than single-agent platinum when platinum is used at the same dose. 3) Cisplatin-based chemotherapy produced a substantial clinical improvement as indicated by increased response rate duration of response and time to progression, but the overall effect on survival has been modest. The higher doses of cisplatin, ranging from 12.5 to 50 mg/sm/week, have demonstrated a general trend for prolonged median survival. But a more escalated dose of platinum supported by PBSCT is an investigational setting to ensure the advantage for overall survival in the near future. This consensus has been achieved from analysis of clinical trials in Europe and the United States for ovarian cancer chemotherapy, but only a few Japanese clinical trials have showed a similar tendency in the manner of retrospective studies. Six cycles of chemotherapy of cisplatin/cyclophosphamide or carboplatin/cyclophosphamide have become the standard and yield clinical response rates of approximately 60-70% and 5-year survival of 15-25 percent for advanced ovarian cancer. Taxol is thought to be the next promising candidate for cisplatin combination and secondary chemotherapy for ovarian cancer. It augurs to improve the overall survival. We look forward to early use of taxol in Japan at the clinical level.

[Early effects of intravenous steroid therapy on cell recruitment in sputum of chronic asthmatics].

Steroid is the key drug in the asthma therapy but not well known to play the role on the airway inflammatory cells. We examined sputum cells of acute excerbated 11 chronic adult asthmatics before and 2-3 hours after intravenous steroid and aminophylline therapy. There was no changes in the percent count of living cell, epithelial cell, metachlomatic cell, macrophage and neutrophil before and after treatment. Lymphocyte was 3.3 +/- 6.5% before and 2.6 +/- 2.0% after treatment. CD4 and CD25 double positive cell (CD4+/CD25+) was 0.7 +/- 0.5 and 1.2 +/- 0.9% and CD8+/CD25+ cell was 0.4 +/- 0.4 and 0.6 +/- 0.5% before and after treatment respectively. These changes were not significant. Eosinophil percent count did not decrease significantly but EG1+/EG2+ cell decreased from 6.7 +/- 7.8 to 4.3 +/- 5.2% significantly (p < 0.05). In the light of no decrease of activated T lymphocytes (CD25+ cells), we concluded that failure of tissue eosinophil response to lymphokines might result in a decrease in activated eosinophil count.

[Studies on the mechanism of hyperfibrinolysis in liver cirrhosis--changes of plasma t-PA, PAI-1 and active PAI-1 levels in liver cirrhosis].

Liver cirrhosis is known to have abnormal fibrinolysis. In order to investigate the changes of fibrinolytic system in liver cirrhosis, plasma levels of various fibrinolytic molecular markers were measured in patients with different stages of liver cirrhosis, 20 compensated liver cirrhosis (c-LC) and 14 decompensated liver cirrhosis (d-LC) and were compared with those in normal subjects. Both the plasma levels of plasmin alpha 2-plasmin inhibitor complex (PIC) and the plasma levels of total fibrin/fibrinogen degradation products (T-FDP) were significantly elevated in patients with both c-LC and d-LC as compared with normal controls. Moreover, both factors in d-LC were significantly higher than those in c-LC. These findings indicate that patients with liver cirrhosis have hyperfibrinolysis. Then, in order to investigate the mechanisms of hyperfibrinolysis in liver cirrhosis, plasma levels of tissue-type plasminogen activator (t-PA) and plasminogen activator inhibitor type 1 (PAI-1) were determined in these patients. The plasma levels of t-PA significantly increased in both c-LC and d-LC as compared with normal controls. Its levels were higher in patients with LC at their decompensated stage than at their compensated stage. In patients with LC, plasma PAI-1 levels showed significantly higher values than those in normal controls. However, the plasma levels of active PAI-1 in patients with LC were similar to those in normal controls. These results suggest that hyperfibrinolysis in patients with LC is mainly due to increased concentrations of t-PA, without increased active PAI-1 levels.

Differential expression of mRNA for alpha 2,3-sialyltransferase during development of rat retina.

Glycoconjugates, consisting of O- and N-linked types, are present on the surface of photoreceptor cells and in the interphotoreceptor matrix (IPM). This study was undertaken to facilitate understanding of the metabolic features of O-linked sialoglycoconjugates in comparison with those of N-linked glycoconjugates in the rat retina. We examined the developmental change in distribution of Gal beta 1,3GalNAc alpha 2,3-sialyltransferase mRNA in rat retinas using in situ hybridization histochemistry to detect the synthesis period of O-linked sialoglycans. Positive hybridization signal was observed in the ganglion cells throughout the postnatal days (P) examined, whereas strong signals were detected in the photoreceptor inner segments and the inner nuclear layer exclusively at P16 and P18. At P20 and older, weak or sparse signals were detected in these regions. It is likely that O-linked sialoglycoconjugates, which are detected in the mature IPM, are actively synthesized from P16 to P18 in the rat photoreceptor inner segments, suggesting that the O-linked sialoglycoconjugates in the photoreceptor layer may be stable with litter turnover in the mature retina.

[The use of twist drill craniostomy in stereotactic surgery for brain tumors].

A twist drill craniostomy is a rapid and relatively simple technique for perforating the skull in order to gain access to the epidural and subdural spaces and the brain. Fifty-eight patients underwent 173 twist drill craniostomies in the stereotactic procedures for the diagnosis and the treatment of brain tumors (brain tumor biopsy, implanting radioactive sources and placement of the catheters for navigation surgery) from September, 1992, through to May, 1995, at our institute. The technique was compared with standard burr hole craniostomy used in 42 patients with brain tumor. In the twist drill craniostomy, the scalp was penetrated directly by hand twist drill (2.7 mm in diameter) and scalp bleeding was able to be be controlled by injecting local anesthetic with epinephrine subcutaneously. The safety stop on the drill was set in advance based on the expected thickness of the skull at its penetration point to provide control of penetration depth as it passed through the skull hole and dura. A hole in the skull was made as the drill was advanced until a change in resistance indicated that the inner table of the skull had been penetrated and the dura lacerated. The cannula with stylet was then inserted through the guide tube to assure the penetration of the skull and dura. As the cannula penetrated the dura, a loss of resistance was noted as it proceeded through the dura toward the pial surface, The time needed in this procedure was less than 3 minutes. Associated with this procedure, there were no complications such as bleeding, or infection in 173 twist drill craniostomies in the 58 patients. Our experience with this procedure proved it to be simple, efficient, safe and superior to conventional burr hole craniostomy.
