Heterogeneity of liver/kidney microsomal antibody type 1 in autoimmune hepatitis and hepatitis C virus related liver disease.

Liver/kidney microsomal antibody type 1 (LKM-1), the serological marker of a subset of autoimmune hepatitis, is also present in a proportion of patients with hepatitis C virus (HCV) related chronic liver disease. To characterise further this autoreactivity and to evaluate whether an autoantibody giving an identical immunofluorescence staining, and detected in two different clinical conditions, involves the same antigenic target(s), sera from autoimmune and HCV infected patients were tested with native, recombinant, and synthetic antigens. Sixty five sera were selected on the basis of the typical immunofluorescence pattern: 50 patients had serological markers of HCV infection, the remaining 15 suffered from autoimmune hepatitis. The reactivity of each serum with rat and human microsomal fractions, full length human recombinant CYP2D6, and two synthetic peptides spanning the amino acid regions 257-269 and 373-398 of CYP2D6 was systematically investigated by immunoblotting. Fourteen (93%) sera from autoimmune hepatitis patients and 39 (78%) from HCV infected patients reacted with rat and/or human microsomal polypeptides of 39 kD, 50 kD, 58 kD, and 66 kD in different associations, the 50 kD band being the most frequently observed. Reactivity to CYP2D6 and its amino acid sequence 257-269 was significantly more common in autoimmune hepatitis than in HCV infected patients (p < 0.001 and p < 0.0003, respectively). LKM-1 reactivity is directed against heterogeneous and not entirely defined autoantigens. The main target in autoimmune sera is CYP2D6 and its 257-269 amino acid region, while sera from patients with HCV infection are more likely to recognise other microsomal targets, the molecular identity of which is currently unknown.

Patients with apoE3 deficiency (E2/2, E3/2, and E4/2) who manifest with hyperlipidemia have increased frequency of an Asn 291-->Ser mutation in the human LPL gene.

Approximately 1% to 2% of persons in the general population are homozygous for a lipoprotein receptor-binding defective form of apoE (apoE2/2). However, only a small percentage (2% to 5%) of all apoE2/2 homozygotes develop type III hyperlipoproteinemia. Interaction with other genetic and environmental factors are required for the expression of this lipid abnormality. We sought to investigate the possible role of LPL gene mutations in the development of hyperlipoproteinemia in apoE2/2 homozygotes and in apoE2 heterozygotes. As a first step, we performed DNA sequence analysis of all 10 LPL coding exons in 2 patients with the apoE2/2 genotype who had type III hyperlipoproteinemia and identified a single missense mutation (Asn 291-->Ser) in exon 6 of the LPL gene. The mutation was then found in 5 of 18 patients with type III hyperlipoproteinemia who had the apoE2/2 genotype (allele frequency = 13.9%; P < or = 7.4 x 10(-5)) and 6 of 22 hyperlipidemic E2 heterozygous patients with the apoE3/2 and E4/2 genotype (allele frequency = 13.6%; P = 2.2 x 10(-5)). In contrast, this mutation was found in only 3 of 230 normolipidemic controls (allele frequency = 0.7%). In vitro mutagenesis studies revealed that the Asn 291-->Ser mutant LPL had approximately 60% of LPL catalytic activity and approximately 70% of specific activity compared with wild-type LPL. The heparin-binding affinity of the mutant LPL was not impaired. Our data suggest that the Asn 291-->Ser substitution is likely to be a significant predisposing factor contributing to the expression of different forms of hyperlipidemia when associated with other genetic factors such as the presence of apoE2.

[Protective action of Lycium barbarum L. (LbL) and betaine on lipid peroxidation of erythrocyte membrane induced by H2O2].

H2O2 was used to induce lipid peroxidation of RBC membrane in rats to observe the protective action of different ingredients of LbL and betaine on the membrane. The result shows these ingredients cna inhibit the lipid peroxidation of RBC membrane in the following order of antioxidizing power: Fructus Lycii.LbL.dry(FL/LbL.dry) > Polysaccharide FL/LbL > Residue FL/LbL > betaine.

[Action of Rhodobrum roserm Limpr on changes of red cell aggregation and yield-shear stress in dogs with acute experimental coronary occlusion].

Following acute occlusion of the left anterior descending coronary artery in dogs, significant increases were observed in the red cell aggregation index, yield-shear stress and red cell electrophoretic time in blood drained from ischemic area. When transfusion was performed with Rhodobrum roseum solution from the right femoral vein, significant rises of the above-said items were observed.

[Efficacy of poria-polyporus anti-diarrhea oral liquor in treating infantile rotavirus diarrhea: a controlled study with smicta].

The efficacy of Poria-Polyporus Anti-diarrhea Mixture (PPAM) in infantile rotavirus diarrhea with Smicta was observed. In two groups of patients, altogether 60 patients were enrolled. The effective rate in the PPAM group and Smicta groups were 83.33% and 90.00% (P < 0.05) respectively. No statistically significance in respect of the duration of fever and diarrhea. Human rotavirus RNA in stool was detected by PAGE, which showed positive result in 13 patients of PPAM group and 19 in Smicta group. The duration of virus excretion in both group were 2-7 days, which was much shorter than 9-21 days of previous report.

[Influence of heating degrees on appearance of calcined shell drugs].

The colors, crisp extents and comminution rates of 4 kinds of shell drugs calcined under various conditions were conpared systematically. The appearance alterations of the calcined drugs were closely related to the heating degrees during calcining operation, and the effect of calcining duration was greater than that of calcining temperature.

[Influence of heating degrees on internal quality of calcined shell drugs].

The internal quality of 4 kinds of shell drugs calcined under different conditions was investigated and compared in terms of the yielding ratios and total calcium content of the calcined product, Ca2+ content of the decoction and total water-soluble extractive rate. It was found that at calcining temperatures higher than 800 degrees C the internal quality of the shell drugs would change markedly.

[Biologically active lignins from Magnolia biondii Pamp].

Guided by PAF induced platelet aggregation, six active lignins were isolated from the flower buds of Magnolia biondii and identified as fargesin, demethoxyaschantin, aschantin, pinoresinol dimethyl ether, magnolin, lirioresinol-B dimethyl ether. The 13CNMR and CD data were reported and the configurations were determined.

[Stability of angesin solution].

Chemical kinetics of ethyl acetate extract from the roots of Angelica sinensis in aguerus solution (angesin solution) was studied by isothermal acceleration test. The useful life (t0.9) calculated for angesin solution is one year.

Research of the exploitation of human resources in blind prevention and primary eye care.

PURPOSE: This research studied how to establish a relatively advanced blindness prevention and eye care cause in economically underdeveloped countryside. METHODS: Ophthalmic vocational schools and professional lectures were held to train "practical type" primary eye care workers for the countryside. Further study in high-level (above provincial) hospitals was taken to train blindness prevention & eye care backbones and leaders. RESULTS: In 1986, the ratio of the number of the eye care workers of all levels to the number of the whole population in the prefecture was 1:26,000. In 1992, it rose to 1:17,000. An eye care network of 222 stations had been established in the countryside. Ten in the 13 county hospitals had a seperated ophthalmology department, in which 3 were awarded "National advanced blindness prevention County". Twenty one hospitals were appointed as the Unit of Surgical Vision-Rehabilitation of Cataract. Blindness prevention and eye care covered 1,000,000 population (eye care available within 5 kilometers), 23.5% of the whole population. CONCLUSIONS: In a demographically large but economically underdeveloped countryside area, the key to wide-range blindness prevention and eye care is to exploit human resources effectively. We should train "Practical type" primary eye care workers, and have a number of department leaders who are authoritive, influential in this field and ready to sacrifice to this cause.

[The significance of dynamic change of plasma endothelin level in epidemic hemorrhagic fever].

One hundred and twenty nine hospitalized cases of epidemic hemorrhagic fever (EHF) diagnosed and classified according to the protocol of prevention and treatment of this disease approved by the Ministry of Health in 1987 were studied to ascertain the role of endothelin (ET) in the etiology. 30 healthy adult subjects residing in the same area served as control. Blood samples were collected and plasma ET level was measured with radioimmunoassay and its dynamic change noted. It is shown that plasma ET level of the EHF patients was higher in febrile, hypotensive or shock, oliguric and transitional stages than that of controls. However, the level returned to normal in polyuric stage. Plasma ET level was also highly positively correlated with that of Tamm-Horsfall protein (THP) and beta 2 microglobulin. The higher the level of plasma ET, the poorer the ontcome. It is suggested that ET plays an important role in the epidemiology of EHF and monitoring of plasma ET level will be of help not only in predicting the prognosis, but also in guiding the clinical therapy.

[The change of gut barrier function and gene expression after surgical stress and parenteral nutrition].

Surgical stress and parenteral nutrition (PN) may cause gut mucosal atrophy and alter barrier function. Gene expression of growth factors and enzymes in small intestine may change. The effects of alanyl-glutamine dipeptide (Ala-Gln) on gut barrier and the gene expression of insulin-like factor I (IGF-I) and glutaminase in parenteral infusion rats with massive small intestine resection were investigated. Twenty Wistar rats were catheterized with 60% small bowel resectin. They were divided into two groups. Control group (n = 10) received traditional parenteral nutritional solution, and study group (n = 10) received Ala-Gln enriched nutritional solution (3% Ala-Gln). The rats were maintained with their respective diets for 7 days. The rats in the study group maintained serum glutamine concentration (844.0 +/- 13.2uMol: 640.4 +/- 17.2uMol, P < 0.05), mucosal architecture (mucosal thickness 591 +/- 12uM: 486 +/- 8uM, P < 0.05) and villus height (404 +/- 7uM: 303 +/- 5uM, P < 0.05). Bacterial translocation rate decreased in the study group (70%: 20%, P < 0.05). Ileal mucosal IGF-I mRNA and jejunal mucosal glutaminase mRNA in the study group increased twofold and threefold respectively. The results suggest that Ala-Gln may enhance gut growth and improve gut mucosa integrity and barrier function in part by means of stimulating IGF-I and glutaminase expression in surgical stress.

[Potassium metabolism in warm heart surgery].

The dynamic balance of potassium getting in and out the body during and after operation was surveyed in 26 adult patients who underwent cardiac surgery with normothermic cardiopulmonary bypass (CPB). We found that during CPB, because of the continous perfusion of warm oxygenated blood cardioplegia solution, a large volume of potassium entered the body; meanwhile, only a little potassium removed from the body in urine. At the moment of CPB finish, the volume of residual potassium in the body was as large as 74.5mmol (mean), that was 1.2mmol/kg body weight. The mean plasma potassium level at the points of aorta cross clamp release and CPB finish were 6.8mmol/L and 5.5mmol/L respectively, and all in normal range at several times measured from the end of operation (usually 12-2pm) to the second postoperative day 7am. The volume of potassium replenishment according to urine output in this group was much less than those in hypothermic CPB patients from the end of operation to the first postoperative day 7am. During CPB extracellular potassium shifted into intracellular fluid, but the shifting became opposite around the operative day 6pm, and the final equilibrium of potassium metabolism was achieved at the second postoperative day 7am. The remarkable hyperkalemia at the point of aorta cross clamp release may delay the heart rebeating ( > 5min, 35%), but the automatic rebeating rate was still very high (96%). With regard to the prevention of hyperkalemia during CPB, we also give some suggestions in this paper.
