Steroid regulation of human placental integrins: suppression of alpha2 integrin expression in cytotrophoblasts by glucocorticoids.

Maintenance of uterine-placental attachment during human pregnancy may depend at least partly on adhesive interactions between cytotrophoblasts and their extracellular matrix (ECM). Such interactions are often mediated by integrins, signal-transducing heterodimeric transmembrane glycoproteins. We previously showed that glucocorticoid (GC) suppressed the expression of collagen and laminin in human placenta; here we show that GC also modulates the expression by human cytotrophoblasts of the integrin subunits alpha2 and beta1, components of a known receptor for these ECM ligands. Cytotrophoblasts were isolated from human term placentas, cultured up to 4 days in the presence of 0-1000 nM dexamethasone (DEX), and assayed for 1) integrin messenger RNA (mRNA) levels by Northern hybridization, 2) integrin subunit synthesis after [35S]methionine labeling, or 3) cell surface integrin levels after 125I labeling by lactoperoxidase. In four independent experiments, 100 nM DEX reduced mRNA levels for integrin alpha2 to 6+/-1% of the control value. This effect was similar between 1-4 days of treatment and was dose dependent between 1-1000 nM DEX. Cortisol treatment (100 nM) inhibited levels of integrin alpha2 mRNA, but 100 nM testosterone, estradiol, and progesterone were less effective, suggesting that this response was specific to GC. In immunoprecipitation studies, treatment of cytotrophoblasts with 100 nM DEX for 2 days reduced the rates of synthesis of the alpha2 integrin subunit as well as its expression on the cell surface to 1-10% of control levels. DEX effects on the beta1 integrin subunit were less dramatic. DEX reduced beta1 mRNA levels to only 69+/-8% of control levels, a smaller reduction compared with effects on alpha2 integrin mRNA. DEX inhibited beta1 protein synthesis and cell surface expression to 60-70% of control levels. In all experiments, DEX had no effect on total protein synthesis. Thus, our results demonstrate that GC treatment specifically and markedly down-regulates expression of alpha2 integrin subunit by human cytotrophoblasts. This finding is consistent with the concept that uterine-placental adherence across gestation may be regulated by coordinate effects on ECM ligands and cellular adhesion receptors.

Parathyroid hormone and mechanical usage have a synergistic effect in rat tibial diaphyseal cortical bone.

Previous reports showed that bone mass and architecture only partially recovered by remobilization (RM) after immobilization (IM)-induced osteopenia, and that parathyroid hormone (PTH) had an anabolic effect on the skeleton. The aim of this study was to determine whether low doses of PTH could restore IM-induced cortical bone loss and whether a combination of PTH plus loading (RM) treatment would be more effective than the PTH in unloaded (IM) limbs. One hundred and sixty 6-month-old rats were divided into aging and IM groups. The right hindlimb of the rat was immobilized by elastic bandage for 18 weeks, and then groups of rats were either kept IM or RM and treated with 30 microgram or 80 microgram of hPTH(1-38)/kg/day for 2, 10, and 20 weeks. Fluorescent-labeled, undecalcified cross-sections of right tibial shafts were studied. We found that RM for 20 weeks after 18 weeks of IM only partially recovered IM-induced muscle weight loss and PTH had no effect on muscle weight in either IM or RM limbs; that RM for 20 weeks after 18 weeks of IM partially restored some minimal cortical width by stimulating periosteal and endocortical bone formation and decreasing endocortical resorption; that PTH treatment of IM limbs completely restored IM-induced cortical bone loss and added extra bone by stimulating bone formation indices on all bone surfaces and depressing bone resorption on endocortical surface; that PTH treatment of RM limbs produced similar anabolic effects as in IM limbs with 30 microgram/kg/day dose but the 80 microgram/kg/day dose-treated limbs had a higher periosteal bone formation rate, which created a larger cross-sectional area, more cortical bone area, and a thicker cortex than the same dose treated IM limbs; and that PTH 80 microgram/kg/day treatment produced more anabolic effect than the 30 microgram/kg/day in both IM and RM limbs. We concluded that reloading the hindlimb by RM after long-term IM could not recover the cortical bone mass. PTH at employed doses was able to completely restore IM-induced cortical bone loss, and this effect was independent of mechanical stimulation. However, when PTH was combined with mechanical loading (RM), a synergistic anabolic effect on periosteal bone formation occurred which increased the cross sectional area that can increase bone strength.

Measurement of murine IgE antibody responses to dust mite allergens by in vitro assay.

In an analysis of murine immune responses to the dust mite allergen Der p 1, treatment with purified allergen induced a significant increase in the level of circulating IgE immunoglobulin (from less than 100 ng/ml in normal mice to 1,350 ng/ml in mice receiving the allergen). Even so, specific IgE antibodies binding to purified Der p 1 were not detected in a conventional ELISA, and the major response appeared to be the induction of high titre IgG antibodies. Specific circulating murine IgE antibodies were however detected using the following assay format: murine IgE was captured to anti-murine IgE antibody coated wells; Der p 1 was added and bound by immobilized anti-Der p 1 IgE antibodies; the captured Der p 1 was then detected by the addition of monoclonal IgG antibodies against Der p 1 and these antibodies were measured by the addition of anti-murine IgG antibody-enzyme conjugate with which colour development is produced after substrate addition. This assay establishes a procedure to measure circulating anti-Der p 1 IgE antibodies which are present together with competing high titre IgG anti-Der p 1 antibodies.

Separation of distinct MUC2 mucin glycoforms using two anti-peptide monoclonal antibodies.

Two monoclonal antibodies (MAb996 and MAb994) were produced by immunisation with a synthetic peptide with a sequence based upon that of the protein core of the gastrointestinal MUC2 mucin. The epitopes were identified as T G T Q for MAb996 and P T G T Q for MAb994. Antibody competition tests also confirmed the overlapping nature of the epitopes for the two antibodies. MAb994 and MAb996 were employed in immunoadsorbent columns for the fractionation of human colorectal carcinoma tissue extracts. While the two antibodies displayed only relatively minor differences in immunological specificity and affinity for the immunising synthetic MUC2 mucin core related peptide, they had the capacity to separate antigenically distinct molecules when used as immunoadsorbents. The findings indicated that subfractions of MUC2 antibody-defined mucins exist in human carcinomas and that these may be distinguished by the differential exposure of determinants in the mucin protein core. The results are in accord with the view that aberrant patterns of glycosylation of mucins in human intestinal tumours produces a spectrum of variably glycosylated macromolecules.

[Expressions of Bcl-2, Bax and Bak proteins in liver tissues of hepatitis B patients and their significance].

OBJECTIVE: To investigate the expressions of Bcl-2, Bax and Bak proteins in liver tissues of hepatitis B patients and their relations to apoptosis and necrosis of hepatocytes. METHODS: TUNEL assay and immunohistochemistry were used to detect hepatic apoptosis, Bcl-2, Bax and Bak proteins expressions on hepatocytes respectively. RESULTS: In liver cirrhosis group(10 cases), acute hepatitis group(8 cases), chronic hepatitis group (55 cases) and hepatic failure group(16 cases), the moderate positive rates of TUNEL assay were 30.0%, 25.0%, 61.8%, and 93.8% respectively; the moderate positive rates of Bcl-2 oncoprotein were 30.0%, 25.0%, 38.2% and 12.5% respectively; the moderate positive rates of Bax protein were 20.0%, 25.0%, 47.3% and 87.5% respectively; the moderate positive rates of Bak protein were 10.0%, 12.5%, 25.5% and 75.0% respectively. There was a positive correlation between degrees of the expression of Bax, Bak and hepatic apoptosis (P < 0.05). The expression of Bcl-2 protein on hepatocytes in proliferation zones was stronger than in necrosis zones. CONCLUSIONS: The enhanced degrees of the expression of Bax and Bak proteins were accompanied with the enhanced degrees of the necrosis.

Animal models of immobilization osteopenia.

There are numerous animal and human models to study the effects upon the skeleton of eliminating mechanical usage by immobilization (IM). Care must be taken in employing the various models. They all have the same pattern of bone loss, but they respond slightly differently in the location and the amount of bone loss, depending upon the degree of unloading. Experimental immobilization studies involving local disuse-induced bone loss models can lose up a baseline level of 60% of their trabecular bone mass. This baseline level suggests a physiological threshold where basal hormonal and cellular activity determined by genetics is reached in the absence of mechanical stimuli. During the transient, immediate, or acute phase of immobilization, remodeling-dependent bone loss occurs in bone adjacent to marrow and from an increase in bone resorption and a decrease in formation. At steady state, or chronic phase, bone mass has plateaued at baseline with cellular activities back to normal levels. Further lowering of immobilization-induced bone loss from immobilization baseline levels can occur with nonmechanical factors like estrogen deficiency. A reasonable working model of immobilization-induced bone loss uses Frost's mechanostat theory and skeletal adaptation to mechanical usage highway to show the IM-induced bone loss, the mechanical usage feedback from the overloaded osteopenic bone and the modification of the IM-induced baseline response with non-mechanical agents. Lastly, an investigator should employ the combination of IM plus OVX model for evaluating agents in the prevention and healing of osteoporosis because it combines the skeletal response of estrogen deficiency in postmenopausal osteoporosis and age-related bone loss due to decreased physical activity.

Antagonistic effects of shikimic acid against focal cerebral ischemia injury in rats subjected to middle cerebral artery thrombosis.

AIM: To study the effects of shikimic acid (SA) on focal cerebral ischemic injury after middle cerebral artery thrombosis (MCAT). METHODS: Thrombosis was induced by FeCl3 in middle cerebral artery of rats. The influences of SA on neurologic deficit (ND), infarct size (IS), brain edema, and cerebral blood flow (CBF) in ischemic region were observed. RESULTS: SA 25 and 50 mg.kg-1 i.p. for 3 d before MCAT attenuated ND, and reduced IS by 51% and 42%; and decreased brain water content from 80.7% to 79.8% and 79.9%; and increased CBF after ischemia from 50.2% of the preischemic level to 75.5% and 73.3%, respectively. In pathologic examination, there was much less thrombosis in MCA in the rat with the pretreatment by SA 25 mg.kg-1. The extent of brain ischemia was much less than that of control. CONCLUSIONS: SA reduced focal cerebral ischemic injury induced by middle cerebral artery thrombosis.

[Repair of the skin defect on the remote part of the lower limb with the distally-based sural nerve nutritional artery island flap].

OBJECTIVE: The study was to verify the existence of the nutritional artery accompanying the sural nerve and to design a distally based island skin flap nourished by this artery for repairing skin defects on the remote part of the lower limb. METHODS: Anatomical study on 15 cadaver lower limbs and clinical applications in 7 cases were performed. RESULTS: The sural nerve nutritional artery is plexiform, forming anastomoses with the vascular network in the superficial and deep fascia. All 7 skin flaps depending on this arterial supply survived in clinical practice. CONCLUSION: This flap has high success rate. The operation is less traumatic and easy to perform. For the repair of the lower limb, it is a good option.

Clinicopathologic, immunophenotypic and ultrastructrual analyses of ATLL patients with cutaneous involvement.

OBJECTIVE: To study 4 cases of adult T-cell leukemia/lymphoma (ATLL) associated with cutaneous lesions for clinicopathology, immunophenotype, human T-cell leukemia/lymphoma virus type I (HTLV-I) provirus DNA and their ultrastructure. At the same time, HTLV-I provirus DNA of ATLL patients were also compared with 18 cases of cutaneous lymphoma (CL), two cases of actinic reticuloid as well as two cases of lymphocytic infiltration. METHODS: Immunohistochemistry studies were carried out on the infiltrating cells using monoclonal antibodies against CD45-RO, CD20, CD68 on paraffin-embedded sections by ABC method and using monoclonal antibodies against CD3, CD4 and CD8 with indirect immunofluorescence (IIF) on frozen sections. Skin biopsies were examined by electron microscope. Serum and bone marrow cells were tested for antibodies against HTLV-I-associated antigen by IIF, and HTLV-I provirus DNA was examined by PCR method. RESULTS: The research showed four patients with ATLL manifesting cutaneous lesions, were subsequently found with additional systemic symptoms, as extensively enlarged superficial lymph node, abnormal increased IL-2 receptor, flower-like cells in their peripheral blood and marrow. The HTLV-I provirus DNA was positive in the peripheral blood, bone marrow, cutaneous lesions and lymph node biopsy specimens by using PCR amplification of specific HTLV-I fragment while 18 cases of the CL were negative for HTLV-I. The special ultrastructure of skin lesions was also found in ATLL patients. CONCLUSIONS: The cutaneous involvement in ATLL is a type of cutaneous T cell lymphoma (CTCL) but shows some differential immunological markers for differential diagnosis. The examination of HTLV-I antibodies or HTLV-I provirus DNA is necessary for diagnosis of ATLL. The ultrastructural characteristics in skin lesions of ATLL were of atypical lymphocytes and mononuclear cells invading the epidermis, and the mononuclear cells are possessing the phagocytic function and phagocytizing the degenerated epidermic cells or lymphocytes.

Evaluation of cardiac beta-adrenergic receptor function in children by dobutamine stress echocardiography.

OBJECTIVE: To investigate the role of dobutamine stress echocardiography in evaluating cardiac beta-adrenergic receptor (beta-AR) function and responsiveness in children. METHODS: Left ventricular ejection fraction (EF), fractional shortening (FS), left ventricular end systolic volume index (ESVI), the ratio of systolic blood pressure and ESVI (SP/ESVI) were measured by dobutamine stress echocardiography (DSE) in 30 children with beta-AR hypersensitivity, 15 children with dilated cardiomyopathy and 30 normal children respectively. RESULTS: Before pharmacological stress, EF and FS were 0.72 and 0.39 respectively in beta-AR hypersensitivity group versus 0.70 and 0.35 respectively in control group. There was no difference of the indices between the two groups (P > 0.05). SP/ESVI was 0.76, higher than the value of 0.66 in control group (P < 0.05); EF, FS and SP/ESVI were 0.41, 0.15 and 0.10 respectively, which were significantly lower than those in control group. After dobutamine stress of 5 micrograms.kg-1.min-1 and 10 micrograms.kg-1.min-1, EF, FS and SP/ESVI were significantly increased in patients with beta-AR hypersensitivity and there were no changes in children with dilated cardiomyopathy compared with values of baseline. CONCLUSION: Cardiac beta-AR function and responsiveness can be evaluated by dobutamine stress echocardiography.

[The diagnostic values of CA242 combining other tumor markers for lung cancer].

OBJECTIVE: To investigate the diagnostic values of CA242, TPA, NSE and CEA tumor markers for lung cancer. METHODS: Using enzyme-linked immunosorbent assay(ELISA). The serum levels of four markers in lung cancer, TB patients and normal subjects (NOR) were measured. RESULTS: Serum concentrations of CA242, TPA, NSE and CEA were significantly higher in patients with lung cancer than patients with TB and NOR (P < 0.01). The sensitivities of CA242 were 65% for adenocarcinoma (AC), 41% for squamous cancer (Sq) and 32% for SCLC, respectively; the specificity was 93%. The sensitivities of NSE were 33%(AC), 41%(Sq) and 71%(SCLC), respectively; the specificity was 97%. The sensitivities of TPA were 65%(AC), 69%(Sq) and 61%(SCLC), respectively; the specificity was 93%. The sensitivities of CEA were 72%(AC), 54%(Sq) and 55%(SCLC), respectively; the specificity was 93%. Using two tumor markers the positive rates were 79% (AC), 54%(Sq) and 63%(SCLC), respectively; the false positive rates in TB and NOR were 7% and 0%, respectively. CONCLUSIONS: Assessing several markers can help differentiating various histological type of lung cancer and can increase significantly the sensitivity and specificity. It is clinically more useful than single marker evaluation. CA242 can be used in diagnosis of lung cancer.

[The use of intervention chemotherapy in comprehensive treatment of advanced nasopharyngeal carcinoma].

OBJECTIVE: To study the application of intervention chemotherapy in the comprehensive treatment of advanced nasopharyngeal carcinoma (NPC). METHODS: In 30 patients with advanced NPC, combined intervention chemotherapy with cisplatin (100 mg), 5-fluoro-uracil (1,000 mg) and bleomycin (16 mg) was given by Seldinger's selective unilateral catheterization of the ascending pharyngeal artery or superior maxillary artery. If the lesion had extended over the midline, the intervention chemotherapy encompassed both sides: 2/3 of the dose infused to the side with the major lesion and 1/3 to the other side. Radiotherapy started one week after intervention chemotherapy. A total dose of 60-70 Gy was given to the primary tumor or 50-60 Gy to the cervical metastases in 8 weeks. Fifty patients with advanced NPC treated in the same period of time with radiotherapy alone served as the control. Both groups of patients were treated with 60Co, the duration of treatment and dose splitting were the same. RESULTS: After intervention chemotherapy, the primary tumor mass reduced in size in 16 patients and the enlarged cervical lymph nodes shrank or became loosened in 12 patients. The radiation dose required to achieve complete response of the primary and metastatic lesions was significantly less as compared to that of patients receiving radiotherapy alone (P < 0.05). At four weeks following completion of radiotherapy, complete response rate in the two groups of patients was 83.3% and 72.0%, respectively (P < 0.05). CONCLUSION: Intervention chemotherapy combined with radiotherapy is an effective treatment for patients with advanced NPC.

[The effect of inhaled nitric oxide and helium-oxygen mixture on the pulmonary function of asthmatics].

OBJECTIVE: To investigate the effect of inhaled nitric oxide (NO) combined with helium-oxygen mixture on the pulmonary function of asthmatics. METHODS: 18 asthmatic patients were included in this study. The patients were divided into two groups randomly. One group of patients inhaled 100 ppm NO combined with helium-oxygen mixture, the other group inhaled 40 ppm NO-containing helium-oxygen mixture. Pulmonary function was examined intermittently during the study process and compared with that of the patients treated with salbutamol. RESULTS: It is shown that inhalation of helium-oxygen mixture improved the forced vital capacity, forced expiratory volume during the first 1.0 second (FEV(1)), peak expiratory flow rate (PEFR) and maximal midexpiratory flow rate (MMEF) significantly as compared with inhalation of air. Although salbutamol inhalation improved FEV(1) and MMEF, the effect was weaker than helium-oxygen mixture inhalation. However the effect of helium-oxygen mixture lasted only one minute. Inhalation of 100 ppm NO with helium-oxygen mixture only slightly improved FEV(1) and PEFR as compared with inhalation of helium-oxygen mixture. 40 ppm NO with helium-oxygen mixture did not influence the pulmonary function in the asthmatic patient as compared with helium-oxygen mixture alone. CONCLUSION: It is suggested that inhalation of helium-oxygen mixture can improve the pulmonary function of asthmatic patients significantly. However, inhalation of NO exerts a weak bronchodilatory effect.

[Clinical analysis of 12 cases of Chinese patients with adult T-cell leukemia/lymphoma].

OBJECTIVE: To define the clinical characteristics and differential diagnosis in Chinese patients with adult T-cell leukemia/lymphoma (ATLL) and its relation to human T lymphotropic virus type-1 (HTLV-1) infection. METHODS: Lymphocyte morphology in peripheral blood was examined in smears by Cytospin II. Lymphocyte immunophenotyping was carried out by indirect immunofluorescence method. HTLV-1 antibody was detected by indirect immunofluorescence method, proviral DNA detected by PCR method and PCR products demonstrated by Southern Blot analysis. RESULTS: The clinical manifestations in these 12 patients were fever (10/12), lymphadenopathy (8/12), skin changes (7/12), spleen and liver enlargement (7/12), jaundice (3/12), pleural effusion (1/12) and lung invasion (1/12). Patul-like cells were not easily found in routine blood smears, but easily found with Cytospin II (10/10). There was increase in lactic dehydrogenase (8/9) and soluble interleukin 2 receptor (9/9). Anemia (7/12), thrombocytopenia (6/12), pancytopenia (2/12), myelodysplasia (4/12) and eosinophilia (8/12) can be found, but no hypercalcemia. HTLV-1 antibody and proviral DNA were positive in 6/10 cases and 9/12 cases respectively. Nine cases were classified as acute type and three as lymphoma type. All patients were misdiagnosed as other diseases at the beginning. The initial presentations were skin changes (4/12), lymphadenopathy (3/12), myelodysplasia (3/12), progressive liver enlargement (1/12) and refractory pleural effusion (1/12). The complications were infections by undetermined pathogens (6/12), Streptococcal infection (1/12), gastrointestinal bleeding (3/12), acute renal failure (3/12), acute respiratory failure (2/12) and cerebral hemorrhage (1/12). Seven patients died, three still survive, and two are out of contact. CONCLUSION: ATLL may not be very rare in China. It is necessary to deepen the recognition of ATLL in Chinese.

[A study on the mechanism of the resistance of Shigellae to fluoroquinolones].

OBJECTIVE: To study the mechanism of the resistance of Shigellae to fluoroquinolones. METHODS: A total of 157 clinical isolates were collected. Antibiotic susceptibility testing, reserpine reverse reaction and plasmid conjugation experiment were performed. Accumulation of norfloxacin by shigellae was determined. The N-terminal coding region of gyrA gene was amplified by polymerase chain reaction (PCR), cloned and sequenced in eight fluoroquinolone-resistant isolates and one fluoroquinolone-sensitive isolate as well as in Shigella 51573 (the standard strain). RESULTS: The positive rate of norfloxacin-resistant isolates was 51.2% in the R plasmid conjugation experiment. R plasmid could not mediate resistance to fluoroquinolones. Reserpine could not decrease the minimum inhibition concentration of fluoroquinolones to Shigellae. Norfloxacin uptake by fluoroquinolone-resistant isolates was the same as that by the sensitive one. DNA sequence analysis of gyrA gene from fluoroquinolone-resistant isolates revealed five point mutations that resulted in amino acid change: Gly-81-->Ser, Ser-83-->Leu, Asp-87-->Asn, Asp-87-->Gly, Met-92-->Lys. CONCLUSION: The mutations of gyrA gene were implicated in the resistance of Shigellae to fluoroquinolones.

[Methodology of DNA fingerprinting and its application in identification of M. tuberculosis].

OBJECTIVE: To explore methodology of DNA fingerprinting technique and its application in identification of strains of M. tuberculosis. METHODS: This research was based on different copy numbers and location of chromosome DNA IS6110 in M. tuberculosis genome. After DNA of M. tuberculosis were cut by endonuclease PvuII, the products were transferred to nylon membrane, then detected and hybridized by enhanced chemiluminescent (ECL) labeling techniques. RESULTS: Fifty clinical isolates from different TB patients had unidentical DNA fingerprinting patterns. DNA fingerprinting patterns showed a great difference from sputum and pus specimens of the same patient. H(37) R(v) ofloxacin resistant and sensitive strains had an identical DNA fingerprinting pattern. CONCLUSIONS: Identification of M. tuberculosis strains is feasible by DNA fingerprinting technique.

[Experimental studies on antioxidative effect of tongcao and xiao-tongcao polysaccharides].

OBJECTIVE: To observe the antioxidative effect of four kinds of Tongcao and Xiao-Tongcao polysaccharides. METHOD: Aged mice (nine months) were abdominal administrated with four kinds of polysaccharides (Tongtuomu, Ximashanjingjiehua, Xinanxiuqiu, Ditanghua) at dosages of 160 mg/kg and 80 mg/kg for 45 days. RESULT: Four kinds of polysaccharides could obviously decrease the content of serum and liver lipoperoxide(LPO) as well as the content of lipofuscin(LF) in brain and cardiac muscle of aged mice, and also increase the activity of superoxide dismutase(SOD) of red cells in aged mice. CONCLUSION: The polysaccharide components of Tongtuomu and three kinds of Xiao-Tongcao have antioxidative effect, suggesting that these substances have antiageing function.

[Determination of the cutaneous viability of skin following incomplete avulsion and its treatment].

OBJECTIVE: In the determination of the viability of skin following incomplete avulsion, subjective criteria such as color, skin temperature, pressure reaction and the stab bleeding would often give a high rate of failure. In order to resolve this problem, a retrospective study was carried out. METHODS: In 27 patients, there were 18 males and 9 females, the age ranged from 7 to 41 years old. In operation, the blood supply of the skin was determined by above subjective criteria carefully. RESULTS: After operation, 5 cases had total survival, 7 cases had peripheral or small area necrosis and 15 cases resulted in large area of necrosis. CONCLUSION: The conclusion was that if the incompletely avulsed skin showed sign of being compressed and squeezed, or the incompletely avulsed skin had uncertain or unstable circulatory status, even though the circulatory status being good, active attitude should be given to debridement in complete removal of the avulsed skin in order to improve the successful rate. For the other 2 cases with degloving injuries of large area of the limbs, the avulsed skin was made into a subdermal vascular network skin flap and several axial incisions were made to save the blood circulation of flap. The result was satisfactory and the vital tissues were preserved and used to the greatest extent.
